Trends in laryngeal cancer incidence in Lithuania: a future perspective.

OBJECTIVES: The aim of the present study was to assess the incidence of larynx cancer in Lithuania in the years 1978-2001 and to outline possible future trends. MATERIALS AND METHODS: The number of new laryngeal cancer cases in 1978-2001 was obtained from the Lithuanian Cancer Registry. The Lithuanian Department of Statistics provided data on the population being at the same age in the same years. The data were adjusted for age, using the direct method in accordance with the European standard; a linear regression analysis of trends in the larynx cancer incidence was performed. RESULTS: After standardization of data for the period of 1978-2001, an upward trend was registered for both men and women: in 1978 the incidence was 10.73 for men and 0.26 for women per 100000 population, in 2001 the corresponding data were 11.6 and 0.7. Throughout the study period the incidence was higher in men than in women and the mean age of male and female larynx cancer patients was increasing: mean age for men was annually increasing by 0.1566 years and for women by 0.0602 years. The forecast for men in 2006 is 13.88 and for women 0.54 cases per 100000 population. CONCLUSIONS: The increase in larynx cancer incidence is growing more rapidly among women than among men, and the mean age of the patients is also increasing. The forecast is that in 2006 the incidence rate will be increasing, and both men and women will get ill at an older age.

[Hemolytic-uremic syndrome in Lithuania over the period 1990-2004].

More than 80 cases of the hemolytic-uremic syndrome were registered in Lithuania over the period 1990-2004. The incidence of this syndrome fluctuated annually; higher incidence rates were observed in 1990-1993, 1998-1999 and 2003-2004, however, there was no overall increase in morbidity over the 15-year period. Most cases occurred in the first two years of life; besides, girls suffered slightly more often than boys. At least in 80% of cases the illness was typical, i.e. was associated with diarrhea. In general, the data correspond to the findings of other scientists.

Mutations in the rpoB gene of rifampicin-resistant Mycobacterium tuberculosis clinical isolates from Lithuania.

A total of 196 rifampicin (RMP) resistant Mycobacterium tuberculosis isolates were searched for mutations in the cluster I region of the rpoB gene. Seventeen different types of mutations were observed in 184 (93.9%) isolates. The remaining isolates searched also had no mutations in the N-terminal region of this gene. Mutations were most frequently detected at single codons Ser531 (47.4%), His526 (20.9%), and Asp516 (20.4%). Our data indicate that genotypic drug susceptibility assays based on detection of mutations at the specific codons of the rpoB gene should be effective for predicting resistance to RMP and rifabutin and multidrug resistance in Lithuania.

[Selenium intake by the population of Lithuania].

The level of selenium was assayed in the blood and food products to study the providing of the Lithuanian population with this trace element. It was shown that imported wheat containing high levels of selenium played a significant role in providing with this trace element of the population of Lithuania, Estonia and Latvia where home-produced foods are deficient in selenium. The mean level of selenium in the blood serum of the Lithuanian population comprised 90 micrograms/l (96 micrograms/l was most frequent). Selenium consumption was 100 micrograms/man.

[Sulfate-reducing bacteria in gypsum karst lakes of northern Lithuania].

Microbiological studies were performed in three small gypsum karst lakes in northern Lithuania, most typical of the region. Samples were taken in different seasons of 2001. The conditions for microbial growth in the lakes are determined by elevated content of salts (from 0.5 to 2.0 g/l), dominated by SO(2-)4 and Ca2+ ions (up to 1.4 and 0.6 g/l, respectively). The elevated sulfate concentration is favorable for sulfate-reducing bacteria (SRBs). Summer and winter stratification gives rise to anaerobic water layers enriched in products of anaerobic degradation: H2S and CH4. The lakes under study contain abundant SRBs not only in bottom sediments (from 10(3) to 10(7) cells/dm3) but also in the water column (from 10(2) to 10(6) cells/ml). The characteristic spatial and temporal variations in the rate of sulfate reduction were noted. The highest rates of this process were recorded in summer: 0.95-2.60 mg S(2-)/dm3 per day in bottom sediments and up to 0.49 mg S(2-)/l per day in the water column. The maximum values (up to 11.36 mg S(2-)/dm3) were noted in areas where bottom sediments were enriched in plankton debris. Molecular analysis of conservative sequences of the gene for 16S RNA in sulfate-reducing microorganisms grown on lactate allowed them to be identified as Desulfovibrio desulfuricans.

[Stroke care profile: a study of Lithuania population in Kaunas].

An aim of the study was to evaluate volume of medical care for stroke patients in Kaunas, Lithuania. Patients with first-ever stroke admitted to 2 Kaunas municipal hospitals have been registered. Data have been collected according to the requirements of the EC BIOMED-2 Program "Towards cost-effective stroke care" using standardized questionnaires. In total 331 patients (121 men and 210 women, mean age 69,8 +/- 0,64 years) were studied, 52% being admitted to the hospital during the first 6 h of the illness and over two thirds during the first 24 h. Most of patients (85%) were hospitalized into general neurological wards. Computed tomography of the brain was performed in one third of the patients. More than two thirds did not undergo any of the possible neurodiagnostic procedures. During staying in the hospital, rehabilitation was launched for 53,2% of patients. Out of all patients survived (83,5%), 29,9% were discharged to the rehabilitation unit. The results revealed that the level of medical care for a stroke patient appeared to be rather moderate: delay time for hospital medical care was too long, proportion of diagnostic examinations was too low and the volume of rehabilitation and social services were not sufficient.

The influence of meteorological factors on growth and vegetation process of Perilla frutescens (L.) Britton in Lithuania.

The results of a study on the dependence of growth and vegetation process of Perilla frutescens (L.) Britton upon meteorological factors are presented in the article. The investigations were conducted at Kaunas Botanical Garden of Vytautas Magnus University during vegetation periods in 2001-2005. The object of investigations was Perilla frutescens (L.) Britton, a medicinal annual herbaceous plant of Lamiaceae Lindl. family, native of Eastern Asia, which passes the whole development cycle under climatic conditions of Central Lithuania. The duration of vegetation period of Perilla frutescens on the average is 167 days. The earliest beginning of vegetation and optimal climatic conditions for growth are when hydrothermic coefficient reaches 1.60-1.80 (conditions of excessive humidity). Optimal climatic conditions for massive flowering and seed maturation are observed when hydrothermic coefficient decreases to 1.20 - then massive flowering starts 10 days earlier and lasts 25 days longer. A strong correlation was found between massive flowering and hydrothermic coefficient (r(2)=0.9408). Using mathematical-statistical methods, the consistent patterns of growth and vegetation process of Perilla frutescens (L.) Britton were determined, specifying the time for raw material preparation what determines its quality and quantity.

Healthcare associated infections in university hospitals in Latvia, Lithuania and Sweden: a simple protocol for quality assessment.

Surveillance of healthcare associated infections is an overlooked parameter of good clinical practice in most healthcare institutions, due to the workload demanded in the absence of adequate IT-systems. The aim of the present study was to investigate whether a simple protocol could be used to estimate the burden of healthcare associated infections in three university hospitals in Huddinge in Sweden, Riga in Latvia and Vilnius in Lithuania and form the basis for initiating a long term follow up system. The medical records of all patients receiving antibiotics were reviewed according to a standardised protocol, focusing on the indications for the drugs and on the frequency of hospital acquired infection (HAI) in a point-prevalence survey. Only comparable specialties were included. The proportion of patients treated with antibiotics (prophylaxis not included) were 63/280 (22%) in Huddinge, 73/649 (11%) in Riga and 99/682 (15%) in Vilnius. The proportion of admitted patients treated for a HAI were 15%, 3% and 4%, respectively, (both comparisons Huddinge versus other centres P <0.001). Surgical site infections were most common, followed by infections with an onset more than 2 days after admission without any of the other registered risk factors present. Our inexpensive and simple method showed that healthcare associated infections were a significant problem among patients admitted to Huddinge. The figures obtained can be used for further discussion and form a baseline for follow up at the local level. The comparison of figures between centres was far less relevant than the process the study created.

Architecture for national e-health infrastructure in Lithuania.

Building a patient centered National e-Health system has been a challenging task for the Health Ministry of Lithuania in 2005-2006. The first and the main task is to design and to build the infrastructure of the National e-health system. The paper presents the architecture of the system, which aims: (a) to integrate already designed and implemented information systems, (b) to communicate with other national information systems, (c) to provide services (basic ones at the initial stage) for users and (d) to support further e-health systems development in long-term perspective. The system design is based on the HISA approach. The core of the system consists of on-line data storage (including classifiers and registries), of the automation and integration layers for health care processes, of a security layer, a system control layer and a client layer. After having defined the concepts of processes and activities, of the security, of the registries and classifiers within the system and the Electronic Health Records System, then follows the implementation stage. Until the end of 2006 it will include the core of the national e-Health infrastructure and the part of the information system providing basic services for the pilot Lithuanian healthcare institutions.

Twins rate in the black-and-white cattle population in Lithuania.

Obtaining the maximal number of calves from each cow according to its natural genetic reproduction ability is the main condition of intensification of reproduction and increase of milk production. Twins rate in the black-and-white cattle population in Lithuania is 2.4%. Cows whose mothers have calved twins at least 1 time during reproductive life have twins in 5.6% of all calvings, the others--in 1.9%. Twins rate in separate bulls daughters range from 1% to 6.1%. Twins rate in cows range from 0.4% to 5% by separate bulls-getters. Productivity of 305 days lactation after having twins is better (551.2+/-68.1 kg of milk, 18.4 +/-2.9 kg of fat and 21.5+/-2.4 kg of protein, P < 0.001). The rate of twin births increases when lactation is increased. The influence of lactation on rate of twin births is 0.6% in total dispersion (P < 0.001). The influence of calving season on twins rate is statistically insignificant but the parts of twins by calving seasons are distributed differently.

[The practical aspects of the enterobiasis problem in Lithuania].

The invasion with enterobiasis in individual groups and antienterobiasis measures performed in Lithuania over the 1980-1989 period have been analysed. An overall average 10-year infectivity of children with Oxyuris vermicularis was 9.4%. The infectivity averaged 17.2% in organized groups of preschool children and 8.1% in nonorganized groups of preschool children. According to the data of 1984-1989 the infectivity in the pupils of the first-fourth forms averaged 18.4%. The infectivity of adults with enterobiasis was considerably lower than in children (1.6-2.3% on the average). Sanitary helminthological surveys have been conducted to assess the degree of contamination of articles in children preschool institutions with invaded matter. An overall index of contamination of the articles under study with Oxyuris vermicularis oocysts was 1.1%. So far enterobiasis control in the Republic has been focussed on the isolation and treatment of the invaded persons, as well as on the sanitary helminthological control over the environment. However, the above measures proved insufficient in combatting the epidemic process. It has been shown that the morbidity might be considerably lowered, provided rules of personal hygiene are strictly and constantly observed, the most essential of them being washing of children in the evening and in the morning. The struggle against enterobiasis may be effective in case all the antiepidemic measures are strictly observed with participation of parents and the personnel of the children institutions.

[Lyme borreliosis in Lithuania: its prevalence, clinical picture and treatment efficacy].

In 1988 Lyme borreliosis (LA) was diagnosed in 376 patients in Lithuania, including 101 patients who had the first stage of circular migrating erythema. Of this latter group, in 40 patients (39.6%) specific antibodies to the causative agent of LB were detected. All 101 patients were intensively treated with antibiotics for 10-14 days. The effectiveness of treatment was evaluated 4-5 months later. Information was collected only on 90 patients, including 82 patients who had undergone treatment. Complete convalescence was achieved in 52 cases (63.4%). 20 more persons considered themselves to be practically healthy. But in 10 patients (12.2%) the development of the disease continued, among them in 6 patients who stopped the prescribed course of treatment on their own accord. Out of 8 patients who had not yet even started to undergo treatment, 5 patients had signs of the manifest progress of the disease. Besides, information on the incubation period of the disease, its seasonal character, the characteristic features of erythema and concomitant symptoms in presented.

Perinatal care in Lithuania.

The Preventive Program of Perinatal, Neonatal and Congenital Abnormalities in Lithuania was launched in 1992. That was the beginning of the reorganization of the Soviet maternal and child health care system. The first stage of the Program provided for the years 1992 through 1996 and aimed to create a system of maternal and neonatal care; to create a system of diagnosis and prevention of congenital abnormalities; to collect, process, and analyze maternal and neonatal data (to establish a new database); to evaluate, distribute, and use available resources efficiently; to plan financial and human resources for a perinatal care infrastructure; and to train medical personnel and control the level and quality of their knowledge. The reorganization was based on a three-tiered maternal and neonatal care system. By the end of 1996 the major goal of the Program was achieved successfully (i.e., perinatal, neonatal, and infant mortality rates decreased significantly. During the next 5 years the Program will focus mainly on qualitative aspects of perinatal care.

Investigations of pneumocystosis among humans and rodents in Lithuania.

Pneumocystosis (Pneumocystis carinii pneumonia) is a parasitic disease which in Lithuania used to be diagnosed only with the use of pathohistological methods. The aim of our study was to determine prevalence of P. carinii antibodies among children and women, using immunochemical method (ELISA) and to find out which groups of patients have higher antibody titres. We also addressed the question of infection with P. carinii among rodents. After investigating 9 species of rodents it was found that mostly infected is brown rat (Rattus norvegicus)--42.9% of them were found infected. The least infected was bank vole (Clethrionomys glareolus)--5.9%. The brown rat lives on hatch of domestic birds, rabbits or drinks eggs, while rodents in nature live on insects, molluscs, amphibians etc. European water voles (Arvicola terrestris) and field voles (Microtus arvalis) are vegetable feeders. Our data shows that those species of rodents were not infected with pneumocysts.
