[Sanitation measures in evidence of Legionella].

Appearance of Legionella disease generally is underrated. Conclusions as to disease frequency from Legionella antibody investigations are not reliable. The number of microbes necessary to produce illness depends from individual preliminary conditions. In special areas of the hospital the rate of diseases caused by Legionella can be reduced by protection. In bathing areas generally the hitherto existing regulations of the Federal Health Authority are sufficient: Continual warm water temperature increases from 60 degrees C, chlorination if necessary, regular stepwise controls. Insufficient results are produced by intermittent temperature increases. Filters that are impermeable for microbes appear uneconomical for bathing areas.

Incorporating problem-based learning exercises into an environmental health curriculum.

Environmental health professionals are often confronted with difficult conflicts involving multiple stakeholders. A problem-based-learning approach was used to engage students in a mock conflict resolution exercise involving the heavily publicized dispute over industrial contamination of an aquifer supplying drinking water to neighborhoods in Woburn, Massachusetts. This dispute provided the basis for the bestselling novel A Civil Action, by Jonathan Harr, as well as a major motion picture bearing the same name. Students were assigned to one of three stakeholder groups: the affected families, an industry consortium, or state and federal regulatory authorities. These stakeholder groups were then directed to participate in alternative-dispute-resolution mock negotiations, with the course instructor serving as a neutral facilitator. The "problem" the students were assigned was to reach a consent agreement that was acceptable to all three stakeholder groups. Two groups of undergraduate students (from different semesters) successfully completed all five phases of this endeavor, with nearly unanimous agreement that the approach was more rapid and potentially less expensive than the civil-litigation process portrayed in the book and movie. The alternative-dispute-resolution approach also provided a less adversarial environment, thereby allowing the industry and community groups to work together to forge a more creative, long-term agreement than that which resulted from the actual civil litigation.

[The value of interferons, interleukin-2 and tumor necrosis factor in the therapy of renal cell cancer].

The treatment of patients with metastasized renal cell carcinoma by biological response modifiers such as interferon (IFN), interleukin-2 (IL-2) and tumor necrosis factor (TNF) should at present only be carried out in prospective studies since there are still no generally accepted treatment regimens for these substances. In addition, one must remember that only interleukin-2 has been approved for the treatment of renal cell cancer by the Bundesgesundheitsamt (Federal Health Authority) in Berlin. Regarding interferons, IFN-alpha seems to be the most suitable substance for the treatment of renal cell cancer. However, objective response rates (almost exclusively partial responses) can only be expected in about 15% of the cases. By combining IL-2 with lymphokine-activated killer cells or IFN-alpha, objectively assessed remissions can be found in 35%. The approximate complete-response rate using this form of treatment, however, is in the range of 10%.

Union dynamics in nursing.

Predictions of union growth among registered nurses may very well depend on hospital and nursing management's response to nurses' dissatisfaction with conditions of employment. The authors discuss federal regulations and other factors which are shaping unionization in nursing.

Report calls for fundamental health care reform.

The Joint Economic Committee in early October issued a report advocating reform of the financing and delivery of health care. Although this committee does not have authority over federal or state health programs, release of the committee's report adds impetus to calls for basic reforms.

[Sugar-free, tooth-protecting chewing gum and candy. Results of a 7-year study].

Experiences of 7 years with sugarless chewing gums and lozenges (tab. I and II) regarding their tooth protective properties are reported. Telemetry of interproximal plaque pH allows to assess acid formation from carbohydrates by plaque bacteria under almost natural conditions. Altogether, 5 chewing gums and 8 lozenges containing sorbitol or mixtures of sorbitol and hydrogenated oligosaccharides were investigated. Lowest pH values during and after chewing sugarless gums varied between pH 6.0 and 7.3. When sucking sugarless lozenges the recorded pH values were between 5.8 and 7.0. In contrast to lozenges, the consumption of sugarless chewing gums becomes particularly important due to their greater stimulation of saliva and buffering capacity of oral fluid. All products tested did not acidify interproximal plaque below the critical pH and therefore comply with the regulations of the Swiss Federal Health Authorities with respect to the labeling or marketed sweets with "safe for teeth". New non-fermentable sugar-replacing substrates are being developed. Their utilization in foodstuffs and sweets is being discussed.

Mechanical ventilation of critically ill newborns on scheduled flights of commercial passenger aircraft.

Transport of critically sick neonates of any gestation on scheduled commercial passenger aircraft is practical, safe, and cost effective. There is no disruption to boarding or egress of passengers and no seats need be removed or rearranged. Civil Aviation and Federal Aviation Authority regulations are obeyed. Power supply modifications and strengthening of the transport incubator are necessary. Other standard neonatal intensive care equipment can be used in battery mode. Replacement of an endotracheal tube in-flight is not difficult.

[Catastrophe medicine: experiences with the employment of German Federal Forces in South Italy (author's transl)].

The earthquake catastrophe in south Italy on November 2rd 1980 brought death or homelessness to thousand of people. Timely preparatory measures could have limited the loss of life. This paper reports experiences from the catastrophe area. The duties of a mobile team of doctors, the problems of hygiene and laboratory medicine which confront a doctor in the catastrophe area and the demands which are made on a field hospital with surgical and internist facilities are dealt with.

Antitrust enforcement in the medical services industry: what does it all mean?

Before 1975, federal antitrust authorities neglected the health care sector, thereby allowing the entrenchment of many anticompetitive practices and institutions that seemed not only "natural," but also beneficial to the quality and professionalism of doctor-patient relations. While antitrust enforcement shares the usual physician preference for free enterprise, prosecutorial discretion is often feared as eroding professional discretion and even well-intentioned reform. However, strengthened competition in the health services industry offers a way to bring some stability to health policy, and to resolve some of the conflicts between advocates of institutional status quo and enthusiasts for increased government regulatory power.

Alcoholism and the courts: experience with a traffic safety project.

The authors describe a federally funded Alcohol Safety Action Project in Texas that attempted to change the drinking behavior of individuals arrested for driving while intoxicated. The project was one of more than 35 around the nation designed to deal with the problem of the drinking driver; unlike many others, its primary emphasis was on rehabilitation. The project functioned as a self-supported ancillary agency to the probation department; however, because of problems in coordination with the legal system, it had access to only about a fourth of the DWI offenders. Despite that drawback, and the inability to prove the value of the projects by the federal evaluation criteria initially set, the authors believe that the experience in Texas has demonstrated the advantages of combining the efforts of the judicial and the health-care systems to deal with alcohol abuse.

High-risk infant case management and assistive technology: funding and family enabling perspectives.

Nursing personnel who are high-risk infant case managers need to be aware of potential funding resources in order to provide needed technology and services to families of high-risk infants and young children. The authors review federal funding streams for the provision of assistive technology and services for this population. Emphasis is placed on encouraging parents to function as independently as possible and assume active rather than passive roles in the case management process. Practical strategies for assisting families to secure needed funding are presented, including the development of a funding portfolio, obtaining technology evaluations, gathering documentation, and becoming involved in appeal processes.

[Risk of adverse effects of drugs].

With regard to the demonstrated adverse effects of some vegetable drugs (Rubia tinctorum L., pyrrolizidine alkaloids), or serious suspicions of adverse effects (drugs containing hydroxyanthracene derivatives), the German Federal Health Authority (BGA) has taken the following measures: the drug Rubiae tinctorum radix and the preparations manufactured from it have been withdrawn from circulation, and in the drugs and preparations containing pyrrolizidine alkaloids and hydroxyanthracene derivatives the manner of use, dosing and information for the customer have been modified.

[Disabled children in the federal republic of Germany in 1974 (author's transl)].

Within the scope of an additional micro-census inquiry--carried out in 1974--the Federal Statistical Office in Wiesbaden assessed the number of physically, mentally and psychologically impaired children. The results are differentiated according to the types of disability and therapy, age, in-patient treatment and special school attendance. Furthermore, the estimated results are compared with earlier surveys of the Federal Statistical Office and other estimates.
