Who was caring for Mary? 1993.

An academic physician gives a personal account of his wife's progressive deterioration during her hospitalization at a university medical center. He encounters physicians who are too distracted by other academic pursuits to care adequately for his wife. Her hospital course is complicated by bilateral pulmonary emboli which occur during inadequate heparin therapy and are followed by myocardial infarction, shock, and the adult respiratory distress syndrome. His experience exemplifies the importance of closely supervised care. He calls on the leaders of academic medical centers to make excellence in patient care a top priority and recommends that clinical as well as research skills be rewarded.

Giant millipede 'burns' and the eye.

A retrospective review of 8 cases of millipede 'burns' (caused by Polyconoceras sp. [= Salpidobolus sp.]) of the eye and periorbital tissues seen in a specialist ophthalmology unit over 6 years at Madang General Hospital, Papua New Guinea, was conducted. Such cases comprised 0.06% of the 14,000 patients seen in the same period. All cases were seen in the rainy season. Apart from one adult, all cases were children (age range 9 months-7 years). Clinical manifestations included a 'burn' of periorbital skin (all 8 cases), marked periorbital oedema (3 cases), conjunctivitis (2 cases), and keratitis (one case). All patients recovered fully with standard topical ophthalmic therapy. Despite anecdotal reports that blindness is a likely sequela of millipede 'burns' of the eye, it did not occur in this, the only published series of the condition.

Psychosocial issues affecting crews during long-duration international space missions.

Psychosocial issues can negatively impact on crew performance and morale during long-duration international space missions. Major psychosocial factors that have been described in anecdotal reports from space and in studies from analog situations on Earth include: 1) crew heterogeneity due to gender differences, cultural issues, and work experiences and motivations; 2) language and dialect variations; and 3) task versus supportive leadership roles. All of these factors can lead to negative sequelae, such as intra-crew tension and cohesion disruptions. Specific sequelae that can result from single factors include subgrouping and scapegoating due to crew heterogeneity; miscommunication due to major or subtle language differences; and role confusion, competition, and status leveling due to inappropriate leadership role definition. It is time to conduct research exploring the impact of these psychosocial factors and their sequelae on space crews during actual long-duration international space missions.

Communicating in the real world: accounts from people who stammer.

UNLABELLED: This qualitative study used in-depth interviews to explore the communicative experiences and coping strategies of 14 adults who stammer. Their accounts revealed that those participants with persistent developmental dysfluency felt that stammering had limited their lives especially in the areas of employment, education and self-esteem. All participants shared key styles of communicative management with avoidance and confrontation developing during childhood. The participants who had undertaken therapy during adulthood, relied upon a range of speech management techniques including self-evolved and therapeutic strategies. Many of the strategies regularly used by the respondents contributed to the limiting experience of stammering. This study highlights the experience of stammering on the individual, discusses the variety of strategies used to manage dysfluency and indicates the complexities of day-to-day communication. These findings are discussed in relation to speech and language therapy for adults who stammer. EDUCATIONAL OBJECTIVES: The reader will learn about and be able to give examples of: (1) the lived experience of stammering; (2) the communicative strategies used by adults; and (3) the situational management of stammering.
