Models for forecasting chronic disease processes in adult and elderly populations: effects of stochasticity.

BACKGROUND: Forecasting the population health burden of chronic diseases requires models consistent with the relation, over time and in an uncertain environment, of risk factors and diseases at the individual level. There is now sufficient longitudinal data, and scientific understanding, of some chronic diseases to construct detailed process-models to better predict their population health burden and more realistically describe the effects of interventions. A crucial clement in constructing models is the way in which stochastic influences are described, e.g. are they allowed to interact over time with deterministic model features? METHODS: A review of statistical and forecasting models aimed to establish what ancillary data and scientific insights are necessary to describe multivariate stochastic health processes and their response to interventions. For circulatory diseases and cancer there exists sufficient longitudinal data and biological insight to construct stochastic multivariate process models. For other diseases, biological knowledge is less complete and there are fewer data sets where multiple risk factors are assessed longitudinally. Forecasting models for those diseases will then rely more heavily on theoretical assumptions about disease behaviour.

Forecasting rehabilitation outcomes for degraded New Zealand pastoral streams.

To understand the timescales and magnitude of responses that can be expected following catchment and riparian rehabilitation, we forecast changes to selected stream ecosystem attributes following tree planting in a pastoral catchment. All planting scenarios were predicted to lead to decreases in daily maximum water temperature after 15-20 years to levels that would be suitable for sensitive invertebrate species. Cooling and reheating were rapid so that most benefits to water temperature along the mainstem were forecast to accrue from shading all of the stream channel network. All planting scenarios were predicted to increase sediment yields over the status quo over the 25-year timeframe examined, with maximal sediment yield occurring about 15 years after planting due to expected erosion of the streambanks under the developing forest shade. Sediment yield was greatest for full catchment planting over 25 years, although sediment yield would be lowest with this scenario over longer timescales. A macroinvertebrate biotic index was predicted to increase by 25% over 15 years if whole catchment afforestation were implemented, compared to 9% if only the 4th order mainstem were planted with riparian trees. The use of ecological forecasting to predict likely outcomes for a range of scenarios should prove useful for prioritising rehabilitation actions.

[A preliminary study on dental-manpower forecasting model of Miyun County in Beijing].

OBJECTIVE: To explore the dental-manpower forecasting model of Chinese rural region and provide references for Chinese dental-manpower researches. METHODS: Chose rural Miyun County in Beijing as a sample, according to the need-based and demand-weighted forecasting method, a protocol WHO-CH model and corresponding JWG-6-M package developed by authors were used to calculate the present and future need and demand of dental-manpower in Miyun County. Further predications were also calculated on the effects of four modeling parameters to the demand of dental manpower. RESULTS: The present need and demand of oral care personnel for Miyun were 114.5 and 29.1 respectively. At present, Miyun has 43 oral care providers who can satisfy the demand but not the need. The change of oral health demand had a major effect on the forecast of the manpower. CONCLUSION: Dental-manpower planning should consider the need as a prime factor but must be modified by the demand. It was suggested that corresponding factors of oral care personnel need to be discussed further.

Short-term risk forecasts of heavy rainfall.

Methodologies for risk forecasts of severe weather hardly exist on the scale of nowcasting (0-3 hours). Here we discuss short-term risk forecasts of heavy precipitation associated with local thunderstorms. We use COTREC/RainCast: a procedure to extrapolate radar images into the near future. An error density function is defined using the estimated error of location of the extrapolated radar patterns. The radar forecast is folded ("smeared") with the density function, leading to a probability distribution of radar intensities. An algorithm to convert the radar intensities into values of precipitation intensity provides the desired probability (or risk) of heavy rainfall at any position within the considered window in space and time. We discuss, as an example, a flood event from summer 2000.

Selective inspection planning with ageing forecast for sewer types.

Investments in sewer rehabilitation must be based on inspection and evaluation of sewer conditions with respect to the severity of sewer damage and to environmental risks. This paper deals with the problems of forecasting the condition of sewers in a network from a small sample of inspected sewers. Transition functions from one into the next poorer condition class, which were empirically derived from this sample, are used to forecast the condition of sewers. By the same procedure, transition functions were subsequently calibrated for sub-samples of different types of sewers. With these transition functions, the most probable date of entering a critical condition class can be forecast from sewer characteristics, such as material, period of construction, location, use for waste and/or storm water, profile, diameter and gradient. Results are shown for the estimates about the actual condition of the Dresden sewer network and its deterioration in case of doing nothing about it. A procedure is proposed for scheduling the inspection dates for sewers which have not yet been inspected and for those which have been inspected before.

The perils of healthcare workforce forecasting: a case study of the Philadelphia metropolitan area.

In 1996, a widely circulated and influential forecast for the Philadelphia Metropolitan Area stated that a decline in hospital and healthcare employment in the region would occur over the next five years. It also suggested that this decline would exacerbate the problem of an oversupply of nurses seeking hospital employment. The forecast reflected a regional leadership and expert consensus on the impact of the managed care transformation on workforce needs and was supported by short-term statistical trends in regional utilization and employment. Confounding these predictions was the fact that hospital and healthcare employment actually grew. By the end of 2001, hospitals in the region were experiencing problems in recruiting sufficient numbers of nurses, pharmacists, and technicians. The forecast failed to anticipate the impact of a strong regional economy on supply and underestimated the resilience of underlying forces that have driven the long-term growth in healthcare workforce demand. More effective ongoing monitoring can help moderate the fluctuation of workforce shortages and surpluses.

[Health workforce forecasting carried out by the C.S.D.M. (1964-2004)].

During the last 4 decades, the Centre has performed 16 pieces of forecast on the various health professions in France, the majority being devoted to medical doctors. During the 60's, information were scarce, and calculations on both the inflows of future new graduates and the outflows of future deaths and retirements could not reach an acceptable level of relevance. In 1972, a numerus clausus system was established at the beginning of medical training. Moreover, efforts were deployed to collect better data and improve projection techniques. All these factors contributed to raise strongly the quality of the forecasting work. The discrepancies between the projected and actual trends were narrowing, as showed below: [table: see text] The forecasts were based on the demographic projection techniques and the results obtained showed not only the total number of active physicians in the country but equally the distribution by age groups of this total number. The latter element allows the predictions of the aging process of the medical profession as an impact of the present-day policies concerning medical student intake (numerus clausus).

On the money: making forecasts what they should be.

A forecast is an extrapolation from data describing what has happened in the past. Most healthcare organizations can perform all of the forecasting they require on a spread-sheet without use of special software. To ensure objectivity, managers should create the forecasts, and executives should evaluate them.

Forecasting the medical care costs of the HIV epidemic: 1991-1994.

This is the first paper to forecast the cost of medical care for all people with the human immunodeficiency virus (HIV). Previous studies have concentrated on the cost of treating people with AIDS (PWAs). It is estimated that the average cost of treating a person with HIV without AIDS is $5,150 per year, and that the average cost of treating a PWA is $32,000 per year ($24,000 for inpatient hospital care and $8,000 for other services). The lifetime cost of medical care for a PWA is calculated to be $85,333. This is higher than many previous estimates of lifetime costs due to increased longevity and the diffusion of costly outpatient drugs. The cost of treating all people with HIV in 1991 is forecast to be $5.8 billion. Of this amount, it is estimated that $1.4 billion will be spent on people with HIV without AIDS, and the remaining $4.4 billion on PWAs. It is forecast that the cost of treating all people with HIV will increase 21% each year between 1991 and 1994, and that $10.4 billion will be spent on treating all people with HIV in 1994.

Forecasting client transitions in British Columbia's Long-Term Care Program.

This article presents a model for the annual transitions of clients through various home and facility placements in a long-term care program. The model, an application of Markov chain analysis, is developed, tested, and applied to over 9,000 clients (N = 9,483) in British Columbia's Long Term Care Program (LTC) over the period 1978-1983. Results show that the model gives accurate forecasts of the progress of groups of clients from state to state in the long-term care system from time of admission until eventual death. Statistical methods are used to test the modeling hypothesis that clients' year-over-year transitions occur in constant proportions from state to state within the long-term care system. Tests are carried out by examining actual year-over-year transitions of each year's new admission cohort (1978-1983). Various subsets of the available data are analyzed and, after accounting for clear differences among annual cohorts, the most acceptable model of the actual client transition data occurred when clients were separated into male and female groups, i.e., the transition behavior of each group is describable by a different Markov model. To validate the model, we develop model estimates for the numbers of existing clients in each state of the long-term care system for the period (1981-1983) for which actual data are available. When these estimates are compared with the actual data, total weighted absolute deviations do not exceed 10 percent of actuals. Finally, we use the properties of the Markov chain probability transition matrix and simulation methods to develop three-year forecasts with prediction intervals for the distribution of the existing total clients into each state of the system. The tests, forecasts, and Markov model supplemental information are contained in a mechanized procedure suitable for a microcomputer. The procedure provides a powerful, efficient tool for decision makers planning facilities and services in response to the needs of long-term care clients.

Forecasting for nursing policy: a news-based image approach.

A prototype of a news-based forecasting model was developed as an aid to policymaking in nursing and to assist in setting priorities for promoting an effective image of nursing. Newspaper articles about nursing were analyzed each month from 1978 to 1981. A model, constructed to show the effects of key nursing tissues on the image of nursing, was estimated using Ridge Regression to determine the direction and magnitude of the effect that each of the key issues had on the image of nursing. It was determined that newspaper articles that show nurses in clinical settings and articles that show nurses as playing a major role are the most important factors in projecting a positive image of nursing. The model was used to construct forecasts for the image of nursing from 1982 to 1984. The image of nursing was forecasted to improve over this period, primarily because of a projected increase in the number of articles that show nurses in this positive image.

Forecasting epidemic pathways for measles in Iceland: the use of simultaneous equation and logit models.

Six measles transmission chains between pairs and triplets of medical districts in Iceland are identified using monthly data for the 26 years from 1945 to 1970. The years studied are divided into two halves, a calibration period (1945-1957) and a forecast period (1958-1970). Some simultaneous equation models of the chains are developed and fitted using three-stage least squares. The resulting one month ahead forecasts are presented in terms of the expected case levels and as the probability of epidemics occurring. A single equation probability model using a logistic transformation is then formulated and compared with the simultaneous equation approach. The results obtained from the Icelandic study confirm in practice the advantages theoretically expected from setting up forecasting models containing geographically based chain transmission components.

A national econometric forecasting model of the dental sector.

The Econometric Model of the the Dental Sector forecasts a broad range of dental sector variables, including dental care prices; the amount of care produced and consumed; employment of hygienists, dental assistants, and clericals; hours worked by dentists; dental incomes; and number of dentists. These forecasts are based upon values specified by the user for the various factors which help determine the supply an demand for dental care, such as the size of the population, per capita income, the proportion of the population covered by private dental insurance, the cost of hiring clericals and dental assistants, and relevant government policies. In a test of its reliability, the model forecast dental sector behavior quite accurately for the period 1971 through 1977.

[Forecast of the beginning day of Japanese cedar pollen release using variation pattern in air temperature].

It is important to forecast the beginning day of Japanese cedar pollen release for taking the preventive medical measure. We analyzed the relation between the beginning day of pollen release and the changing pattern of daily air temperature. A simple and practical method for forecasting the beginning day of pollen release is suggested. Polynomial of degree three was applied to represent the general variation pattern of air temperature. Multiple correlation coefficients between parameters of the polynomial and the beginning days of pollen release were 0.88 for the difference by year, and 0.81 for the difference by place. It was shown that the minimum value and the date of the fitted polynomial curve were related closely to the difference in the beginning day of pollen release by year. The minimum value and the date of the original series of air temperature showed high correlation with the difference in the beginning day of pollen release by place. We propose to use the minimum value and the date of original series and fitted curve of air temperature for forecasting the beginning day of cedar pollen release.

Cost-effectiveness of 3-hydroxy-3-methylglutaryl-coenzyme A reductase inhibitors in the secondary prevention of cardiovascular disease: forecasting the incremental benefits of preventing coronary and cerebrovascular events.

OBJECTIVE: To forecast the long-term benefits and cost-effectiveness of lipid modification in the secondary prevention of cardiovascular disease. METHODS: A validated model based on data from the Lipid Research Clinics cohort was used to estimate the benefits and cost-effectiveness of lipid modification with 3-hydroxy-3-methylglutaryl-coenzyme A reductase inhibitors (statins) based on results from the Scandinavian Simvastatin Survival Study (4S), including a 35% decrease in low-density-lipoprotein (LDL)-cholesterol levels and an 8% increase in high-density-lipoprotein (HDL)-cholesterol levels. After comparing the short-term outcomes predicted for the 4S with the results actually observed, we forecast the long-term risk of recurrent myocardial infarction, congestive heart failure, transient ischemic attacks, arrhythmias, and strokes and the need for surgical procedures such as coronary artery bypass grafting, catheterization, angioplasty, and pacemaker insertions. Outpatient follow-up care costs were estimated, as were the costs of hospital care and drug therapy. All costs were expressed in 1996 US dollars. RESULTS: The short-term outcomes predicted for the 4S were consistent with the observed results. The long-term benefits of lipid modification among low-risk subjects (normotensive nonsmokers) with a baseline LDL/ HDL ratio of 5 but no other risk factors ranged from $5424 to $9548 per year of life saved for men and $8389 to $13747 per year of life saved for women. In high-risk subjects (hypertensive smokers) with an LDL/HDL ratio of 5, the estimated costs ranged from $4487 to $8532 per year of life saved in men and $5138 to $8389 per year of life saved in women. Assuming that lipid modification has no effect on the risk of stroke, cost-effectiveness increased by as much as 100%. CONCLUSIONS: These long-term cost estimates are consistent with the short-term economic analyses of the published 4S results. The long-term treatment of hyperlipidemia in secondary prevention is forecasted to be cost-effective across a broad range of patients between 40 and 70 years of age. Recognizing the additional effects of lipid changes on cerebrovascular events can substantially improve the cost-effectiveness of treating hyperlipidemia.

Forecasting epilepsy from the heart rate signal.

Information contained in the R-R interval series, specific to the pre-ictal period, was sought by applying an unsupervised fuzzy clustering algorithm to the N-dimensional phase space of N consecutive interval durations or the absolute value of duration differences. Data sources were individual, complex partial seizures of temporal-lobe epileptics and generalised seizures of rats rendered epileptic with hyperbaric oxygen. Forecasting success was 86% and 82% (zero false positives in resistant rats), respectively, at times ranging from 10 min to 30 s prior to seizure onset Although certain forecasting clusters predominated in the patient group and different ones predominated in the animal group, forecasting on the whole was seizure-specific. The high prediction sensitivity of this method, which matches that of EEG-based methods, seems promising. It is believed that an on-line version of the algorithm, trained on each patient's peri-ictal ECG, could serve as a basis for a simple seizure alarm system.

Determining the degree of chaos from analysis of ISI time series in the nervous system: a comparison between correlation dimension and nonlinear forecasting methods.

Two different chaotic time series analysis methods--the correlation dimension and nonlinear forecasting--are introduced and then used to process the interspike intervals (ISI) of the action potential trains propagated along a single nerve fiber of the anesthetized rat. From the results, the conclusion is drawn that compared with the correlation dimension, nonlinear forecasting is more efficient and robust for chaotic ISI time series analysis in a noisy environment. Moreover, the evolution of the correlation coefficient curves calculated from nonlinear forecasting can qualitatively give a better reflection of the unpredictability of the system's future behavior and is in good agreement with the values of the largest Lyapunov exponent that quantitatively measures the degree of chaos.
