Sixtysix patients were submitted to color Doppler ultrasonography, with flowmetric analysis, in the preoperative work up of their thyroid nodules. Three different groups (and two subsets) were so identified, corresponding to the different pathological nodule types: hyperplasia, adenoma, and cancer. The data obtained were then compared to the final pathological results. The color Doppler ultrasonography showed a diagnostic accuracy rate of 87.8%. An evaluation of the peak systolic velocity proved to be very useful in the differential diagnosis between adenomas and cancers. The color Doppler ultrasonography represents a sound method for evaluating thyroid nodules.

Radioiodine ((131)I) treatment for multinodular toxic and non-toxic goiter is therapeutic procedure especially used in patients with contraindication for surgery. (131)I treatment diminishes the size of the goiter and treats hyperthyroidism. Sometimes, due to low radio-iodine uptake (RAIU), this procedure should not be used. In patients with goiter, the elimination of medication or substances rich in iodine is the first step to increase RAIU. Recombinant TSH (rhTSH) enhances RAIU in metastases distant from thyroid cancer. Studies were performed in recent years using rhTSH to increase the radio-iodine uptake in multinodular toxic and non-toxic goiter. These methods led to enhanced radio-iodine uptake, decreased the level of activity of the (131)I administered, changed the distribution of (131)I in the thyroid, lowered the absorption dose, and increased the number of patients with hypothyroidism. The uses of rhTSH can lead to exacerbation of the signs and symptoms of hyperthyroidism, so patients should be hospitalized. Until now there has been no evidence that the adverse effects outweigh the positive results of using rhTSH. The use of rhTSH in benign goiter disease is not yet approved, but its positive activity in multinodular goiter is very interesting and promising.

The purpose of this study was to measure the nuclear DNA content of cells in three different areas of multinodular goiter from 15 patients. Diploid DNA was found in the cell samples from 12 patients; the biopsies from one patient contained a clear ipodiploid peak (61%, D.I. 0.75) and those from the remaining 2 had dubious ploidy. The calculation of the mitotic index, including cells in S + G2M phases, evidentiated a large variability between different samples from the same goiter, thus correlating with the cellular heterogeneous composition of the thyroid.
Psammoma bodies present in fine needle aspiration (FNA) smears with typical cytologic features of colloid goiter from five patients raised the possibility of coexistent papillary carcinoma. However, subsequent careful histologic examination of the entire thyroidectomy specimens did not show occult carcinoma in any of the cases. The psammoma bodies were identified in the stroma next to the foci of papillary hyperplasia in each case. They were small, often equal in size to or slightly larger than the follicular epithelial cell nuclei, and could be misinterpreted as epithelial cell nuclei. These findings suggest that the presence of psammoma bodies in an FNA smear cytologically consistent with a colloid goiter should raise the possibility of coexistent papillary hyperplasia or neoplasia. However, because of the strong statistical association of psammoma bodies with malignant thyroid neoplasms, histologic confirmation is mandatory in such cases.

A study was made of changes of esophageal transport function in 10 patients with thyroid adenoma and in 27 patients after surgical and radiotherapeutic management for thyroid cancer. The results were compared with those of a control group. Significant data on changes on esophageal transport function in patients with thyroid adenoma were unnoticed. Esophageal dysfunction after operation and radioactive iodine therapy for cancer was detected in 55%. Dysfunction was thought to be caused by dysphagia which showed direct correlation with the severity of hypothyroidism.
Graves' disease is characterized by a diffuse and homogeneously hyperfunctioning goiter, presumably caused by thyroid stimulating, TSH-receptor directed antibodies (TRAB). However, in many patients the serum concentration of TRAB is in no way parallel to the severity of the clinical course of Graves' hyperthyroidism. In particular, hyperthyroidism may persist or repeatedly relapse over many years despite the absence of high TRAB titers. The present study summarizes the existing evidence that this course of Graves' disease may be due to gradually evolving autonomously growing and functioning micro- or macronodules within the originally diffuse goiter.
