Colored illustrations of obstetrics manipulations and instrumentation techniques of a Turkish surgeon Serafeddin Sabuncuoglu in the 15th century.

Serafeddin Sabuncuoglu (1385-1470) was the author of the first illustrated surgical textbook Cerrahiyyet'ül Haniyye (Imperial Surgery) in the Turkish-Islamic literature. In the obstetric section he gave detailed descriptions of normal and abnormal presentation, surgical manipulation of intrauterine foetal death and retained placenta. He wrote about previously described procedures and the obstetric culture of his time. Some authors have claimed that he only translated Abu Kasim-al Zahrawa (Albucasis)'s Al-Tasrif (Textbook of Surgery) and added the illustrations of the surgical techniques. Even if this is accepted, the illustrations are enough to entitle this work as a milestone. In this paper, we describe the contributions of this pioneer in obstetrics and compare his recommendations with the current practice.

The use of oxytocin for the reduction of cow placental retention, and subsequent endometritis.

Oxytocin was administered to reduce incidence of retained placenta and uterine infections that could delay subsequent conception. Three hundred and fifty multiparous Friesian cows, each with spontaneous delivery of a single calf were divided randomly into two groups. Some (n = 175) were injected with 30 IU of oxytocin immediately after delivery and again 2-4 h later, while the remainder formed an untreated control group. The placental retention 24 h after parturition was 24.6% and 10.9% in control and treated animals respectively (P < 0.01). Endometritis occurred in 51.6% of the animals following placental retention as compared to 10.4% of those with normal expulsion of the fetal membranes (P < 0.001). A comparison of reproductive indices showed a statistically significant improvement of fertility in treated cows with the average interval from calving to conception being reduced from 124.4 d to 93.7 d (P < 0.0001).

Intravenous nitroglycerin for rapid uterine relaxation.

BACKGROUND: To investigate the effect of nitroglycerin in vitro and in vivo on human uterine contractile activity. METHODS: In vitro myometrial strips were obtained from six pregnant women at term who underwent elective cesarean section. The biopsies were mounted in tissue baths. After spontaneous or oxytocin-induced activity had been accomplished, nitroglycerin in various concentrations was added to the baths and the effects were continuously registered. In vivo, in an open study nitroglycerin was administered as a bolus injection of 100-200 micrograms intravenously to 32 women at cesarean section when uterine relaxation was urgently needed; to 22 other women after vaginal delivery for facilitation of manual removal of retained placentas, and to one patient at vaginal delivery of premature twins. RESULTS: In vitro nitroglycerin induced a dose-dependent inhibition of spontaneous as well as oxytocin-induced myometrial contractile activity. Complete muscular relaxation was obtained at a concentration of 25-50 micrograms/ml. In vivo all patients had rapid effective uterine relaxation after intravenous injection of 100-200 micrograms nitroglycerin. CONCLUSION: Nitroglycerin administered intravenously seems to be a rapid and effective uterine muscle relaxant agent without overt adverse effects on mother or fetus.

Subarachnoid haemorrhage following spinal anaesthesia in an obstetric patient.

We describe an obstetric patient who presented for removal of a retained placenta. After insertion of the spinal anaesthetic, she developed a severe headache, and a subarachnoid haemorrhage was diagnosed. We discuss the differential diagnosis of the headache, the occurrence of intracranial haemorrhages after dural puncture and the future management of this patient.

Production of twin calves with in vitro fertilised embryos: effects on the reproductive performance of dairy cows.

Attempts were made to produce twin calves by implanting in vitro fertilised and matured (IVF) embryos into dairy cows. Twinning rates were consistently at the lower end of the range reported to have been obtained with in vivo derived embryos. The low pregnancy rates, higher than expected early embryonic losses, and increased fetal deaths and abortions recorded may indicate that IVF embryos are less viable. Fetal oversize in single calves was a significant problem in the first two years of the trial. The subsequent conception rates of cows that calved twins and those that calved singles were similar in spite of a much higher incidence of retained placenta and an increased interval to first oestrus in the cows bearing twins.

An early anaesthetic in Papua New Guinea.

A search for information about early anaesthetics administered in Papua New Guinea has revealed that an ether or chloroform anaesthetic was given, probably for a retained placenta, at Port Hunter on December 9, 1880. The anaesthetist or anaesthetic assistant was the Reverend George Brown, a Wesleyan Methodist missionary.

Outcome and subsequent fertility of sheep and goats undergoing cesarean section because of dystocia: 110 cases (1981-2001).

OBJECTIVE: To determine the outcome and subsequent fertility of sheep and goats undergoing a cesarean section because of dystocia. DESIGN: Retrospective study. ANIMALS: 85 sheep and 25 goats. PROCEDURE: Medical records were reviewed, and information was obtained on signalment, history, physical examination findings, anesthesia protocol, surgical technique, number of lambs or kids delivered, pre- and postoperative treatments, duration of hospitalization, and postoperative complications. Follow-up information was obtained through telephone conversations with owners. RESULTS: The proportion of sheep admitted to the veterinary teaching hospital during the study period that underwent a cesarean section (4.4%) was significantly higher than the proportion of goats that did (2.2%). Pygmy goats were overrepresented, compared with the hospital population. The most common reason for cesarean section was inadequate dilatation of the cervix. The most common surgical approach was via the left paralumbar fossa. Two hundred one lambs and kids were delivered, of which 116 were dead at delivery or died shortly afterward. Forty-two of the 65 dams with 1 or more dead fetuses had been in stage-2 labor for > 6 hours, and fetal death was significantly associated with a prolonged duration of dystocia. The most common complication following surgery was retained placenta (n = 49). Use of antimicrobials was associated with a lower rate of complications. All 16 dams that were rebred became pregnant and had no problems with dystocia. CONCLUSIONS AND CLINICAL RELEVANCE: Results suggest that cesarean section is an effective method of resolving dystocia in sheep and goats and does not adversely affect subsequent fertility.

Relationships of efficiency to reproductive disorders in Danish milk production: a stochastic frontier analysis.

Relationships of various reproductive disorders and milk production performance of Danish dairy farms were investigated. A stochastic frontier production function was estimated using data collected in 1998 from 514 Danish dairy farms. Measures of farm-level milk production efficiency relative to this production frontier were obtained, and relationships between milk production efficiency and the incidence risk of reproductive disorders were examined. There were moderate positive relationships between milk production efficiency and retained placenta, induction of estrus, uterine infections, ovarian cysts, and induction of birth. Inclusion of reproductive management variables showed that these moderate relationships disappeared, but directions of coefficients for almost all those variables remained the same. Dystocia showed a weak negative correlation with milk production efficiency. Farms that were mainly managed by young farmers had the highest average efficiency scores. The estimated milk losses due to inefficiency averaged 1142, 488, and 256 kg of energy-corrected milk per cow, respectively, for low-, medium-, and high-efficiency herds. It is concluded that the availability of younger cows, which enabled farmers to replace cows with reproductive disorders, contributed to high cow productivity in efficient farms. Thus, a high replacement rate more than compensates for the possible negative effect of reproductive disorders. The use of frontier production and efficiency/inefficiency functions to analyze herd data may enable dairy advisors to identify inefficient herds and to simulate the effect of alternative management procedures on the individual herd's efficiency.

Using activity and milk yield as predictors of fresh cow disorders.

The objective was to determine whether daily walking activity and milk yields could be used as predictors of metabolic and digestive disorders early in lactation. Data were collected from 1996 through 1999 from 1445 dairy cows in 3 Florida herds. Walking activity, milk yield, and other measures were collected from a computerized dairy management system. Mixed models analysis was used for data on cows before their first detected estrus, as identified by difference in activity. Healthy cows were defined as those without any metabolic or digestive disorder during the prebreeding stage, whereas a sick cow had an occurrence of those disorders at any time during the prebreeding stage. Metabolic disorders were ketosis, retained placenta, and milk fever. Digestive disorders included displaced abomasum, indigestion, reduced feed intake, traumatic gastritis, acidosis, and bloat. Data from cows with known cases of ketosis, left displaced abomasum, and digestive disorders were analyzed to determine changes in activity and milk yield before those specific disorders were clinically diagnosed. Although walking activity was generally lower among sick cows, cows with ketosis, left displaced abomasum, and digestive disorders had higher than average activity 8, 9, and 8 d, respectively, before each diagnosed disorder. Daily milk yields of sick cows were approximately 15 kg/d less than milk yields of healthy cows. Milk yields were lower by 6, 7, and 5 d, respectively, before diagnoses of ketosis, left displaced abomasum, and digestive disorders. Cows with ketosis, left displaced abomasum, and general digestive disorders could possibly be detected about 5 to 6 d earlier than clinical diagnoses based on changes in daily walking activity and milk yield.

Genetic selection for health traits using producer-recorded data. I. Incidence rates, heritability estimates, and sire breeding values.

The objective of this study was to determine the feasibility of genetic selection for health traits in dairy cattle using data recorded in on-farm herd management software programs. Data regarding displaced abomasum (DA), ketosis (KET), mastitis (MAST), lameness (LAME), cystic ovaries (CYST), and metritis (MET) were collected between January 1, 2001 and December 31, 2003 in herds using Dairy Comp 305, DHI-Plus, or PCDART herd management software programs. All herds in this study were either participants in the Alta Genetics (Watertown, WI) Advantage progeny testing program or customers of the Dairy Records Management Systems (Raleigh, NC) processing center. Minimum lactation incidence rates were applied to ensure adequate reporting of these disorders within individual herds. After editing, DA, KET, MAST, LAME, CYST, and MET data from 75,252 (313), 52,898 (250), 105,029 (429), 50,611 (212), 65,080 (340), and 97,318 (418) cows (herds) remained for analysis. Average lactation incidence rates were 0.03, 0.10, 0.20, 0.10, 0.08, and 0.21 for DA, KET, MAST, LAME, CYST, and MET (including retained placenta), respectively. Data for each disorder were analyzed separately using a threshold sire model that included a fixed parity effect and random sire and herd-year-season of calving effects; both first lactation and all lactation analyses were carried out. Heritability estimates from first lactation (all lactation) analyses were 0.18 (0.15) for DA, 0.11 (0.06) for KET, 0.10 (0.09) for MAST, 0.07 (0.06) for LAME, 0.08 (0.05) for CYST, and 0.08 (0.07) for MET. Corresponding heritability estimates for the pooled incidence rate of all diseases between calving and 50 d postpartum were 0.12 and 0.10 for the first and all lactation analyses, respectively. Mean differences in PTA for probability of disease between the 10 best and 10 worst sires were 0.034 for DA, 0.069 for KET, 0.130 for MAST, 0.054 for LAME, 0.039 for CYST, and 0.120 for MET. Based on the results of this study, it appears that genetic selection against common health disorders using data from on-farm recording systems is possible.

The association between previous 305-day milk yield and disease in New York State dairy cows.

Logistic regression was used to examine the association between milk yield and disease. We studied 8070 cows of second or higher parity from 25 New York State Holstein herds, calving between June 1990 and November 1993. Previous 305-d mature equivalent milk yield was used because most of the disorders studied occurred too early in lactation for current milk yield to be considered. Seven disorders were studied: retained placenta, metritis, ovarian cyst, milk fever, ketosis, abomasal displacement, and mastitis. A separate model was used for each disorder. Each model included, in additional to milk yield, parity, calving season, and heard. Results showed that higher milk yield was not a risk factor for any disease except mastitis. However, the association between higher previous milk yield and mastitis does not necessarily imply causation.

Physiological changes at parturition and their relationship to metabolic disorders.

Most of the metabolic diseases of dairy cows-milk fever, ketosis, retained placenta, and displacement of the abomasum-occur within the first 2 wk of lactation. The etiology of many of those metabolic diseases that are not clinically apparent during the first 2 wk of lactation, such as laminitis, can be traced back to insults that occurred during early lactation. In addition to metabolic disease, the overwhelming majority of infectious disease, in particular mastitis, becomes clinically apparent during the first 2 wk of lactation. Three basic physiological functions must be maintained during the periparturient period if disease is to be avoided: adaptation of the rumen to lactation diets that are high in energy density, maintenance of normocalcemia, and maintenance of a strong immune system. The incidence of both metabolic and infectious diseases is greatly increased whenever one or more of these physiological functions are impaired. This paper discusses the etiological role of each of these factors in the development of common diseases encountered during the periparturient period.

Effects of diseases on test day milk yield and body weight of dairy cows from Danish research herds.

The pre- and postdisease interrelationships of energy corrected test day milk yield and body weight of dairy cows caused by mastitis, three reproductive disorders (retained placenta, metritis, cystic ovaries), and seven metabolic disorders (milk fever, ketosis, decreased rumen motility, enteritis, left displaced abomasum, right displaced abomasum, and off feed) were quantified by using mixed models analysis with repeated measures of continuous data. The data were weekly recordings from 4414 lactations collected in three Danish research herds. High milk yield was a risk factor for ketosis and enteritis. Heavier primiparous cows were more likely to contract mastitis. Milk yield was decreased for a disease-specific period for all study diseases except cystic ovaries and right displaced abomasum. Metabolic disorders had a detrimental effect on body weight. The highest weight loss (69 kg) was associated with left displaced abomasum. The persistence of the weight loss differed considerably among study diseases. Almost all weight loss occurred up to and including the initial week after diagnosis, which emphasized the detrimental effect of the subclinical stage. However, weekly measured body weight seemed superior to weekly energy corrected test day milk yield for disease detection only for decreased rumen motility and left displaced abomasum. This study demonstrates the importance of the predisease level for accurate estimation of the loss of milk yield and body weight from disease.

[Curettage immediately after delivery].

The purpose of the study was to compare indications for curettage directly after delivery with histologic findings. Only in 51% there was a histologic confirmation of placental or membrane tissue. Residences indications of the suspected retained placenta were wrong in 70% cases. The curetting of 75% of the patients experiencing post partum bleeding revealed no fragments of placental or membrane tissue.

[Pregnancy termination in the second trimester with vaginally administered dinoprostone followed by intravenous sulprostone, for the indication 'foetal congenital defects'; results of a retrospective study].

OBJECTIVE: To obtain insight into treatment duration and complications of the currently accepted method for pregnancy termination in the second trimester using a combination of prostaglandine-E2 medications. DESIGN: Retrospective study of medical records. METHOD: Data were collected for all second trimester pregnancy terminations performed on foetal indication in the Erasmus Medical Center in Rotterdam, The Netherlands, in the years 1998-2001. The treatment consisted of vaginal administration of dinoprostone, followed by intravenous administration of sulprostone. RESULTS: A total of 134 pregnant women were involved in the analysis. The median age was 33 years and the median pregnancy duration 19 weeks. The median duration of hospital stay was 3 days (range: 2-11 days). The median duration of treatment for the total study population was 19 hours (4-172). For nulliparae this was 23 hours, and for multiparae 17 hours (p < 0.05). Of the total study population, 91 women (68%) delivered within 24 hours. The percentage of multiparae that delivered within 24 hours was higher than the percentage of nulliparae that delivered in this time (76% compared to 53%: p < 0.05). Operative removal of a--partially--retained placenta was conducted in 51 women (38%). This percentage was higher among women with treatment duration longer than 24 hours than among women who delivered within 24 hours (51% versus 32%; p < 0.05). The treatment duration and the complication percentage were less favourable than those described in a number of publications relating to the combination mifepristone and misoprostol.

[Haemorrhage delivery. About 65 cases].

The delivery haemorrhage is actually a problem of public health. It is responsible of 31.5 % of the maternal death in Tunisia. The goal of this work is to study the frequency of this complication, its gravity, its risk factors, its etiologists and its methods of treatment. It is a retrospective study. of 65 cases of delivery haemorrhage recorded to the obstetric gynaecology service "C" of the centre of motherhood and neonatology of Tunis during 4 years. The frequency of the delivery haemorrhage in our study is 1.19%. The middle age of the patient is of 31 years. Their middle parity is 2.4. Factors of risk taking out again our set are: gestational toxemia (35.4%), primiparity (33.8%), advanced maternal age (30.7%), pre-existent anaemia (24.6%). the uterine surdistension (21.3%), an abnormal middle length of labour (69.6%). use of oxytocin during labour (34%), induction (21.5%). Etiologists in our set are: atone in 63% of cases, retained placenta in 31.2% des cases, coagulopathie (9.2%), placenta previa (1.5%), uterine inversion (1.5%). The hold must be in charge multidisciplinary, systematized, precocious and dynamic.

[Selenium deficiency as a cause of secondary retention of the placenta in a herd of dairy cattle].

A farm is described, on which the placenta was retained in approximately forty per cent of the cows and on which selenium deficiency (0.03 +/- 0.010 micrograms/ml) and unduly reduced activity of glutathione peroxidase (GSH-Px) in the erythrocytes (65.2 +/- 26.4 u/g Hb) were observed. When 0.5 mg of Se per kg of dry matter of a mixture of concentrated feeds were added, the concentrations of Se and GSH-Px were restored to normal and the symptom consisting in retained placentae disappeared. This incident shows that selenium deficiency occurs in the Netherlands.

[Fertility in mares after an disturbed as well as an undisturbed puerperium. Significance of histological and cytological examinations of the uterus].

Because of the mare's ability to conceive very soon post partum a thoroughly performed control of the puerperal period is of particular importance. Cytological examinations of uterine content and histological examinations of uterine biopsy specimens provide more detailed information than microbiological examinations of uterine swabs. Uterine content and uterus biopsy specimens of 55 mares were examined histologically on days 3, 6 and 9 post partum. We observed a heavy polymorphonuclear reaction of the endometrium following disturbed puerperal processes especially in cases of retained placenta. This reaction of the endometrium as well as a delayed regeneration of the uterine epithelium and the uterine gland epithelium resulted in significantly lower fertility rates at the first post partum oestrus. In such cases the foal heat should therefore not be used for breeding.
