Face imaging reconstructive morphography. A new method for physiognomic reconstruction.

A new method of facial identification--Face Imaging Reconstructive Morphography (FIRM)--is herein described. This method permits the construction of objective composites of facial features based on precise cephalometric measurements. The anthropometric measurements are derived from radiographic films taken by the Analytic Morphograph (KLS). FIRM is a rapid, objective, and reproducible method that is distinctly better than the current methods of forensic-anthropological identification.

Reconstruction of the upper lip with lip switch (Abbe flap reconstruction).

The lip switch operation (Abbe) is useful for full thickness defects of the upper lip. Careful design of the flap with pedicle take-down and inset of the flap after 2 weeks, leads to a predictably good reconstructive result. The flap is most useful for 30-50% full thickness defects of the upper lip.

The dendritic extent of motoneurons in frog brachial spinal cord: a computer reconstruction of HRP-filled cells. With comments on dendritic reconstruction methodologies.

A lateral and a medial motoneuron in the brachial spinal cord of the leopard frog, Rana pipiens, were labeled by horseradish peroxidase applied to the ventral root. Their dendritic trees were traced, analyzed, and plotted using a computer-microscope system. Some dendrites of the medial motoneuron crossed the midline of the spinal cord, but no dendrites of the lateral motoneuron crossed the midline. Nevertheless, the total dendritic length of the lateral motoneuron exceeded that of the medial motoneuron. The peak number of dendritic branch segments of the medial motoneuron was located at a greater distance from its soma than that of the lateral motoneuron. Three-dimensional reconstruction and rotation of the dendritic trees revealed that the dendrites of the medial motoneuron had a greater rostrocaudal extent than those of the lateral motoneuron. When compared to reports of Golgi-impregnated motoneurons, our results suggest that the HRP technique labels dendrites more completely. However, use of the HRP technique may introduce greater errors in the subsequent measurement of dendritic segments due to nonuniform tissue shrinkage.

[Value of autologous chondrocyte transplantation in the reconstruction of experimental cartilage defects. Part I. Extent of defect, macroscopic appearance of reconstructed articular surface and microscopic analysis of predominant tissue].

Articular cartilage defect is one of the main reasons of osteoarthritis. Currently, tissue engineering techniques are the methods concerning better cartilage reconstruction. The aim of this part of the study was macroscopic evaluation of degree of defect feeling, macroscopic appearance of repair tissue and microscopic analysis of predominant tissue after autologous chondrocytes transplantation. Repair of partial thickness cartilage defect on distal part of femur was evaluated (25 adolescent rabbits). Procedures were performed in II groups: I--autologous chondrocytes transplantation under periosteal flap, II--periosteal graft. Chondrocytes were isolated from the cartilage specimens by enzymatic digestion and cultured in vitro. The regenerates were inspected 4, 8 and 12 weeks after the operation. Macroscopic analysis in group I, in most cases revealed filling of the defect with tissue resembling surrounding cartilage. In group II the defect was partially filled, and there was many fissures and cracks in all regenerates. In microscopic analysis in group I, after 4 and 8 weeks following the transplantation the tissue similar to juvenile hyaline cartilage predominated. After 12 weeks it resembled mature hyaline cartilage. In group II, in all cases fibrous cartilage was observed after 4, 8, 12 weeks. Obtained results indicate, that macroscopic and microscopic characteristics of repair tissue after autologous chondrocytes transplantation more closely resembled hyaline cartilage, than in periosteal graft group. 12 weeks after autologous chondrocytes transplantation the repair tissue reached maturity, and demonstrated microscopic characteristics of hyaline-like cartilage. The method of autologous chondrocytes transplantation provides potential for clinical application.

[Forum: reconstruction of the traumatised thumb. Reconstruction of the amputated thumb by progressive lengthening of the 1st metacarpal using mini-fixator. Apropos of 2 clinical cases].

The authors report two cases of reconstruction of a mutilated thumb by progressive elongation of the first metacarpal without the need for bone grafts, using a minifixation elongation device, which is very easy to use. It allows distraction which can be performed by the patient himself and adjustment to the desired length, up to 40 mm.

Use of reconstruction plates in conjunction with soft-tissue free flaps for oromandibular reconstruction.

The plate-flap option has a place in oromandibular reconstruction: It allows an expedient method to obtain an excellent cosmetic and functional result with minimal donor site disability. It is best for lateral and posterior low-volume defects in the debilitated, the elderly, and in those patients with a poor prognosis. This technique may be used as a preliminary to vascularized bone grafts in patients receiving postoperative radiotherapy. This method is no panacea. These are very specific indications for its use. Failure usually requires rescue with a vascularized bone graft. With proper selection of patients, however, this will rarely be necessary. A major weakness in the plating systems available at present is their tendency to fracture if they remain in place too long. The gauntlet is, therefore, thrown down to those in a position to develop more reliable hardware.

[Joint reconstruction as a cause of limited knee joint extension after anterior cruciate ligament reconstruction using a bone-tendon-bone autograft].

The authors report on a group of 3 patients who have developed restricted knee extension following acl reconstruction using central one-third of the patellar tendon as an autogenous graft. A fibrous nodule, varying in size from 15 to 20 mm, occupying the intercondylar notch and presenting mechanical block to full extension was found at arthroscopy. Removal of the nodule resulted in improved extension to about minus 3 degrees after surgery and 0 degrees at 6 weeks follow-up. No complications occurred. Histologically, the nodule consisted of focally hyalinized fibrous tissue.

Ridge reconstruction with hydroxylapatite. Part 2. Ridge reconstruction based on residual anatomic structure.

The second part of this article shows how the problems of atrophy and resorption of the residual ridge can be corrected with hydroxylapatite to obtain ideal ridge form and maximal prosthetic rehabilitation. The high degree of efficiency of the system is based on a thorough knowledge of the anatomic structures involved, on the use of the cephalogram as a guide, on the correction of adverse intermaxillary relationships and on the recourse to a staging of procedures when required.
