Long-acting contraceptive agents: norethisterone esters of arylcarboxylic acids.

The synthesis of esters of norethisterone (17 alpha-ethynyl-17 beta-hydroxy-estr-4-en-3-one) with acids containing a benzene ring is described, two methods of esterification being compared in terms of yield and convenience. The activities of these esters as long-acting contraceptive agents have been evaluated.

Expression of the oestrogen regulated pNR-2 mRNA in human breast cancer: relation to oestrogen receptor mRNA levels and response to tamoxifen therapy.

The pNR-2 mRNA is regulated by oestrogens in cell lines established from metastatic human breast cancer cells. The levels of the pNR-2 and oestrogen receptor RNAs have been measured in 96 tumour samples from patients undergoing surgery for breast cancer. Oestrogen receptor mRNA was detected in 90% of the 60 primary breast tumour samples from patients not receiving endocrine therapy at the time of surgery, whereas the pNR-2 RNA was detected in 57%. In primary tumours the expression of pNR-2 was entirely dependent upon oestrogen receptor RNA expression. When the 60 primary tumours were considered, pNR-2 and oestrogen receptor mRNA levels were significantly correlated. There was no significant correlation for pNR-2 positive tumours. pNR-2 mRNA levels were similar in tumours of pre- and post-menopausal patients and were independent of tumour differentiation and nodal status. Oestrogen receptor and pNR-2 mRNA levels were also measured in 21 tumour samples from patients receiving primary tamoxifen therapy. Eleven of these had shown an objective response and a significantly larger number of tumours from these patients contained pNR-2 mRNA than from patients who did not respond (chi 2 = 6.08, P less than 0.025).

The value of the basal temperature chart in the management of infertility.

The basal temperature chart (BTC) has been used as a means of screening the ovarian function of infertile patients and for monitoring their response to ovarian stimulation. During three years in a busy infertility clinic, specifically designated for cases of functional infertility, 491 patients were seen and 96 per cent of them kept valid basal temperature charts. The use of the BTC enabled therapy to be instituted early on in the course of investigations in those cases of infertility in which there was an endocrine factor. There were 420 patients in this category; of those who received treatment a high percentage became pregnant in contrast to those who were being observed. It is recommended that the BTC should be used early and more frequently in the management of cases of infertility, since it has been shown to serve as an inexpensive and convenient parameter of ovarian function.

Postcoital contraception.

Changes in sexual mores have led to the need for an effective emergency postcoital contraceptive agent. To meet this need various individual steroids, either alone or in combination, have been evaluated and shown to be effective in preventing pregnancy as a result of a single, unprotected coital act. No drug has received specific marketing approval in North America for this purpose. However, in western Europe, the combination of ethinyloestradiol and levonorgestrel is marketed specifically for use as a postcoital contraceptive agent. Intrauterine copper contraceptive devices have also been shown to be effective postcoital contraceptive agents, but their applicability is confined to a specific segment of the population. Other agents are also being investigated for their postcoital contraceptive effectiveness, including prostaglandins, anti-progestins, GnRH agonists, super agonists and antagonists, and HCG antagonists. Sufficient interest exists in postcoital contraception that the World Health Organization has undertaken, through their task force dealing with postcoital, once-a-month and menses-inducing agents, to develop other postcoital drugs.

Cytologic evidence of extensive keratotic reaction in women exposed to diethylstilbestrol in utero.

Squamous mucous membranes and squamous metaplastic epithelium sometimes undergo hypermaturation with the production of a keratin layer. Anucleated keratotic squamous plaques in smears are generally recognized as cytologic evidence of this altered maturation. This keratotic reaction was quantified in cytologic smears from 191 women exposed in utero to diethylstilbestrol (DES). Keratotic reaction was observed in the vaginal smear in 40% of the cases, in the cervical smear in 26% and in the endocervical smear in 19%; overall, a keratotic reaction was observed in at least one specimen from 48% of the women. These frequencies are higher than those reported in other studies. The observed frequency was age related. The significance of the hyperkeratosis, including its possible relationship to a lower dysplasia rate among DES-exposed women, is unclear. No conclusions can be drawn until more is known about behavioral factors in DES-exposed women.
