Epidermoid carcinoma arising in parotid adenolymphomatous lesion with microdeposit of amyloid substance.

A case of an epidermoid carcinoma which arose in a cystic adenolymphoma-like lesion of the parotid gland is presented. Histologically, in the major part, the tumor tissue showed a papillary cystadenoma lymphomatosum-like pattern with the stroma consisting of mainly plasma cells and lymphocytes, but the epithelial element was mostly multilayered, showing a keratinous differentiation. In some areas, sebaceous-like cells and oxyphilic cells were found in the epithelial component. In the surrounding fibrous tissue, the invasive growth of the epithelial cells could be observed. Microdeposit of amyloid substance was found in the lymphoid stromal tissue and also in the intercellular space of the epithelial element. A mass of epithelioid cells and cholesterol crystal clefts were observed in the granulomatous tissue which proliferated around the tumor tissues. About 4 months after surgical excision, a recurrent tumor, which was well differentiated squamous cell carcinoma, appeared on the same site as the primary tumor.

Immunohistochemical investigation of lysozyme, lactoferrin, alpha 1-antitrypsin, alpha 1-antichymotrypsin and ferritin in parotid gland tumors.

Presence of lysozyme, lactoferrin, alpha 1-antitrypsin, alpha 1-antichymotrypsin and ferritin was examined by the immunoperoxidase method in 15 consecutive parotid gland tumors as well as in normal parotid gland tissue. Lysozyme and lactoferrin were detected in intercalated duct cells of normal tissue and in the epithelial component of pleomorphic adenomas. alpha 1-antitrypsin, alpha 1-antichymotrypsin and ferritin were found in both epithelial and mesenchymal components of pleomorphic adenomas but not in normal parotid tissue. In the epithelial component of adenolymphoma only alpha 1-antichymotrypsin and lactoferrin were observed. The results would support a tentative histogenetic link between the intercalated duct cell and the epithelial component of the pleomorphic adenoma.

Coexpression of intermediate-sized filaments in sialadenoma papilliferum and other salivary gland neoplasms.

The neoplastic cells present in a sialadenoma pappiliferum were found by immunoperoxidase method and immunofluorescent staining technique to co-express 3 different types of intermediate-sized filaments (IFs) defined by monoclonal antibodies to cytokeratin, vimentin and desmin. When other salivary gland tumors such as 18 pleomorphic adenomas, 15 adenolymphomas, 2 oxyphilic adenomas, 7 mucoepidermoid tumors, 5 acinic cell tumors, 8 adenoid cystic carcinomas and 6 adenocarcinomas were examined immunohistochemically for the expression of IFs, no tumors with all 3 types of IFs observed in sialadenoma papilliferum were found.

Malignant Warthin's tumour: an ultrastructural study.

Undifferentiated malignant cells in a Warthin's tumour (adenolymphoma) were studied by light and electron microscopy. The ultrastructure of these cells indicated that they were poorly differentiated adenocarcinoma arising from the glandular component of the Warthin's tumour. Some differences between this case and previous ultrastructural studies are described.

Significance of preserving the posterior branch of the great auricular nerve in parotidectomy.

OBJECTIVE: Sensory disturbance due to excision of the great auricular nerve in patients who have undergone parotidectomy sometimes causes discomfort to the patients. In order to reduce the postoperative discomfort of the pinna, we tried to preserve the posterior branch of the great auricular nerve. METHODS: Forty patients with parotid tumor were included in this study. Twenty-one of these patients had pleomorphic adenoma, 16 had adenolymphoma and 3 had a low grade malignant tumor. Sensations of the pinna and the quality of life (QOL) after parotidectomy were evaluated using a 0-100 Visual Analogue Scale (VAS) assessed at 2 weeks, 1 month, 2 months, 3 months and 6 months after parotidectomy. RESULTS: The posterior branch of the great auricular nerve was preserved in 26 out of 40 patients (65%). No difference was observed in the incidence of complications except sensory disturbance of the pinna with this surgical procedure as compared to the surgical technique where the great auricular nerve was excised. The VAS score for the sensation was significantly higher in the group of patients whose great auricular nerve was preserved at 2 months (35.0+/-20.8 vs. 18.5+/-9.2), 3 months (64.4+/-18.3 vs. 26.4+/-13.8) and 6 months (66.9+/-16.2 vs. 26.6+/-11.4) after parotidectomy. The VAS score for the QOL was also significantly higher in the group of patients whose great auricular nerve was preserved at 2 months (50.3+/-21.8 vs. 35.1+/-14.5), 3 months (69.5+/-27.5 vs. 45.9+/-22.6) as well as 6 months (71.9+/-24.1 vs. 45.7+/-19.1) after parotidectomy. CONCLUSION: Preservation of the posterior branch of the great auricular nerve during parotidectomy is valuable in order to reduce the postoperative sensory disturbance of the pinna that follows conventional surgery. It further helps to improve the QOL of these patients after parotidectomy.

Facial nerve function in 100 consecutive parotidectomies.

The risk of facial nerve paresis after parotidectomy is thought to be within acceptable limits, although it is difficult to find published data regarding the specific magnitude of this risk. This study reviews the subject and reports postoperative facial function in 100 consecutive patients who had parotidectomies at the Henry Ford Hospital during a 9-year period. Permanent weakness of a major branch was identified in 2 of 77 patients having lateral lobectomy for parotid disease. Both patients demonstrated marginal mandibular paresis after surgery for adenolymphoma. No weakness was noted in 16 patients undergoing total parotidectomy. No unanticipated nerve disability was noted in 4 patients having partial nerve sacrifice in extended procedures; 3 patients had complete sacrifice of the nerve.

Immunohistochemical studies on the epithelial and lymphoid components of Warthin's tumour.

The lymphoid and epithelial elements of three Warthin's tumours (adenolymphomas) were studied by immunohistochemical technique. All classes of immunoglobulin (Ig)-producing cells were found, except IgE immunocytes. The numbers of cells/mm2 section area (median and range) were: 20.4 (15.2--27.2), 4.8 (2.8--12.4). 4.8 (0.4--9.6) and O (0--1.2) for IgG-, IgA-, IgM- and IgD-producing cells respectively. This gives percentage class ratios (68:16:16:0) which are quite different from those found in normal parotid tissue (3.6:91:3.1:2.6). Thus, the immunocytes found in Warthin's tumour are clearly polyclonal and show a class distribution comparable to that of lymphoid organs such as the tonsils. Also as in such organs, there were lymphoid follicles with mantle zone of B lymphocytes bearing membrane-bound IgM and IgD, and germinal centres showing reticular IgM staining. Apical staining for IgA and secretory component (SC) was shown in the luminal cells of the tumour epithelium, giving a fluorescence pattern similar to that of normal striated ducts. Selective transport of IgA apparently taken place into the cystic spaces of the tumour. Carcino-embryonic antigen (CEA), normally absent from striated ducts, was not generally present in the tumour epithelium, but was found in small groups of cells, especially in papilliferous parts of the epithelium. Warthin's tumour therefore seems to be composed of a fairly inactive normal lymphoreticular tissue and a neoplastic duct epithelium, most of which is highly differentiated with retention of immunoglobulin transport.

[Expression of C-erbB-2 oncogene mRNA in salivary gland tumors].

OBJECTIVE: To study relationship of C-erb-2 oncogene mRNA expression and histotype, tumorigenesis and biological behavior of salivary gland neoplasms. METHODS: Using 32P labeled oligonucleotide as probe, the dot blot technique was used to study the expression of C-erbB-2 oncogene mRNA in salivary gland neoplasm with normal salivary gland as control. RESULTS: With the expression in normal salivary gland as a standard, low C-erbB-2 mRNA expression was seen in adenolymphoma, basal cell adenoma. However, various degrees of over-expression of C-erbB-2 oncogene mRNA were detected in pleomorphic adenoma, mucoepidermoid carcinoma, acinic cell carcinoma, adenoid cystic carcinoma, papillary cystic carcinoma, and myoethelial cell carcinoma. CONCLUSION: C-erbB-2 oncogene mRNA over-expression in salivary gland neoplasm is related to biological behavior of salivary gland carcinoma.

Salivary gland tumours: is colour Doppler imaging of added value in their preoperative assessment?

Using a Toshiba SSA-270A Colour Duplex Scanner, 15 patients with suspected parotid and submandibular gland tumours had preoperative duplex scanning done. Peak systolic doppler shifts (fmax) were recorded and their vascularity was also subjectively scored. There were no arteries identified within the substance of contralateral normal glands. All recordings were analysed retrospectively and an independent histological assessment of tumours was made. Pleomorphic adenomas (n = 9) had a median peak systolic frequency (fmax) of 0.8 kHz, while adenolymphomas (n = 4) had a fmax of 1.65 and the two malignant tumours had fmax of 4.5 (P = 0.007 Kruskal-Wallis test). There appears to be a strong correlation between the fmax and the subjective vascular score (Corr 0.92). Colour duplex scanning is a non-invasive procedure which may be of help in the preoperative assessment of salivary gland tumours.

[Bilateral Warthin's tumor of the parotid gland].

Three cases of bilateral Warthin's tumor (adenolymphoma) of the parotid gland in patients treated in the ENT Department, Medical University of Gdańsk were presented. They were stated 3.3% of all Whartin's tumors in our institution. In two cases the contralateral tumor developed after 2.5 and 15 years and in one case arised synchronically. Our three patients were males, 50-65 years old, cigarette smokers more than 25 years. All of the patients were treated surgically--conservative parotidectomy was performed. We did not observe the complications and reccurence of the disease. Opinions on the diagnostic and therapeutic methods in that disease reported in Polish and foreign references are discussed.

[Combined diagnosis of parotid masses].

Computed tomography (CT), ultrasonography, sialography, and 99mTc scintigraphy were applied before operation to 108 patients with parotid masses. The results of each technique and the combined study of them were compared with the pathological diagnosis. Ultrasonography was found to be a very effective diagnostic aid in determining the presence of space-occupying lesion in the parotid. CT was the best technique to provide adequately reliable informations regarding the location of the tumor and the relationship between tumor and surrounding tissues. For diagnosing the nature of tumors, ultrasonography combined with sialography was reliable. 99mTc scintigraphy was better than other techniques in diagnosis of adenolymphoma. The diagnostic accuracy of combined diagnosis (90.7%) was higher than those of ultrasonography (83%), CT (80.5%), sialography (79%), and 99mTc scintigraphy (13.9%) alone. The advantage of combined diagnosis was particularly obvious for the diagnosis of low-grade malignant tumors.

Myoepithelial cells in salivary gland tumors. An immunohistochemical study.

Normal salivary glands and 55 salivary gland tumors were examined by immunostaining (immunoperoxidase [IMP] and immunofluorescence [IMF]) to identify myoepithelial cells (MCs) and speculate on their role in the histogenesis of the tumors. The classic (C) MCs of normal salivary glands stained by IMP with antibodies to cytokeratin and S100 protein and stained by IMF with the same antibodies and with antibodies to vimentin and actin. Modified (M) MCs of pleomorphic adenomas stained positively by IMP and IMF with all of the preceding antibodies. In many mucoepidermoid carcinomas, adenoid cystic carcinomas, and basal cell adenomas, variable numbers of CMCs and MMCs stained positively by IMP with anti-cytokeratin and anti-S100 protein antibodies. No MCs were detected in adenolymphomas or acinic cell carcinomas. We believe that MCs play a major role in the histogenesis of pleomorphic adenomas and may also be important in many mucoepidermoid carcinomas, adenoid cystic carcinomas, and basal cell adenomas.

[Primary and secondary neoplasms of the salivary glands in the province of Trieste (1977-1986). Histopathological review and epidemiological picture of 151 cases].

The authors carried out a histopathological and epidemiological study on 151 cases of primary and secondary neoplasms of the salivary glands. The parotid gland was involved in 117 cases (78.5%). The tumours were malignant in 20% (30 cases) of the lesions. The most common neoplasms were pleomorphic adenoma (74 cases) and adenolymphoma (35 cases). The occurrence of the latter were, for both sexes, directly correlated to the age of the subjects. The malignant neoplasms were also in direct relation to age. In 7 patients pleomorphic adenoma recurred (11.3%) and in 3 of these the tumour showed a carcinomatous change. The world standardized incidence of malignant forms in the province of Trieste was slightly lower than that for other countries with a similar socio-economic and demographic situation.

[Cystadenolymphoma of the parotid gland. A study of 40 cases].

A statistical review of 40 cases of adenolymphoma (Whartin's tumor) of the parotid gland is presented. In 10% of the cases, other tumors are found in the same concerned gland. Therefore, the authors perform a subtotal parotidectomy rather than a simple removal of the tumor of a superficial parotidectomy. This policy reduces the risk of facial nerve injury by repeated dissection.

Papillary cystadenoma lymphomatosum syndrome.

"Papillary cystadenoma lymphomatosum syndrome" is an unusual clinical constellation that was first described in 1970 as infected adenolymphoma syndrome. A case of papillary cystadenoma lymphomatosum syndrome is presented demonstrating the specific histopathology of the lesion as well as the typical clinical history of all reported papillary cystadenoma lymphomatosum complicated by acute onset of pain and sudden increase in size. Five other cases noted in the literature are compared to complete the clinical and histopathologic review of the entity.

[Autoimmune diseases of the salivary glands].

The structural and functional features of the salivary glands and their function associated with the immunocompetent system are described. The disturbance of such association, in the author's opinion, is conducive to the development of a peculiar tumour pathology (adenolymphomas) and autoimmune processes (Sjögren syndrome autoimmune parotitis). Based on the data from the literature two forms of this disease are distinguished: (1) a distinct nosological form developing as a result of various exo- and endogenous injuries, and (2) manifestations of other diseases, as a rule, of the autoimmune genesis accompanied by the secondary involvement of the salivary glands. The morphological picture in both forms is identical. It is suggested that in systemic involvement of the salivary, lacrimal and other glands and some autoimmune disease the term "Sjögren syndrome" be used, and the distinct disease be termed "autoimmune parotitis".

[Ultrasonic findings of parotid tumours in 40 patients].

Ultrasonic findings of 40 parotid tumours are presented in this paper. After a technical description proceeding they show the ultrasonic pattern of the three main types of parotid tumours: mixed tumours, adenolymphomas and carcinomas. The differential diagnosis between malignancy and non malignancy is carried out in 87,5% of cases. This rate of success is, according to literature data, similar to that of sialography. Since it is non invasive, the authors recommend sonography in the assessment of parotid tumours, rather than sialography.
