[Area differentials of sex imbalance in births].

Along with a rapid decline in fertility to below replacement levels in South Korea, a new demographic phenomenon has emerged: a distorted sex ratio at birth. In 1993, the sex ratio was 115.6, with significant differences in this ratio by region. "An analysis of national survey data showed that in the areas with the most distorted imbalance, people not only have a stronger preference for sons and more favoring attitude toward sex-selective induced abortion, but also actually have more sex-selective induced abortions after checking the sex of the fetus. This analysis proved that the sex imbalance is being caused by means of sex-selective induced abortions after checking the sex of the fetus." (SUMMARY IN ENG)

[Spontaneous abortion and genetic natural selection].

Author reviews the recent researches related to the genetic causes of spontaneous abortions as well as the results of the cytogenetic examinations of gametes, early embryos and clinical miscarriages. In the background of more than half of first trimester spontaneous pregnancy losses severe chromosomal aberration of the fetus can be found. About 60% of abnormal karyotypes is formed by autosomal trisomies, the rest is divided among polyploidies, X-monosomy and different structural anomalies. The incidence of fetal trisomies correlates with maternal age, besides balanced translocations of parents and delayed fertilisation can also play a role in the etiology of fetal chromosomal abnormalities. Regarding that most of these abnormalities accidentally develop because of meiotic or mitotic error there is no great risk of recurrence. Women who have had one spontaneous abortion will soon get pregnant again and give birth to a healthy child. The abortion of fetuses with abnormal karyotype is the appearance of natural genetic selection.

Risk of spontaneous abortion following induced abortion is only increased with short interpregnancy interval.

We set out to study the risk of spontaneous abortion following a first trimester induced abortion as a function of the interpregnancy interval between two pregnancies. The cohort study is based on the following databases: Danish national registries: the Medical Birth Registry (MBR), the Hospital Discharge Registry (HDR), and the induced Abortion Registry (IAR). All primigravid women in the time period from 1980 to 1982 were identified in the MBR, the HDR and the IAR. A total of 15 727 women who terminated the pregnancy with a first trimester induced abortion were selected as the induced abortion cohort, and 46 026 women who did not terminate the pregnancy with an induced abortion constituted the control cohort. By register linkage all subsequent pregnancies which were not terminated by induced abortion were identified from 1980 to 1994. Only women who had a non-terminated pregnancy following the index pregnancy were selected. Women whose first pregnancy was terminated following a first trimester induced abortion had a risk of spontaneous abortion of 11.0% vs. 9.4% in the control cohort. This relative difference of 1.17 was not statistically significant in logistic regression analyses. An increased risk was only found for women who had an interpregnancy interval of less than 3 months (OR=4.06, 95% C.I.=1.98-8.31). The abortion method, vacuum aspiration with dilatation or evacuation with dilatation did not modify this elevated risk. Overall the study did not show an increased risk of spontaneous abortion following one or more induced abortions, except for women with a short interpregnancy interval between an induced abortion and a subsequent pregnancy. We recommend women who have a first trimester induced abortion be advised to wait at least 3-6 months before trying to become pregnant again.

The incidence of repeat induced abortion in a randomly selected group of women. A retrospective study.

Because of the suspected medical complications resulting from repeat abortions and the uncertainty of data from studies of repeat abortion that rely on patients' reports, we examined the records of 403 women who obtained abortions between 1975 and 1980 at an obstetrics-and-gynecology clinic within a prepaid-membership health maintenance organization. We found that: (1) 15.9% of the women had had an abortion in the year prior to the abortion for which they were selected for our study, (2) 45.4% of the women had had at least one previous abortion, and (3) 16.9% had had at least two previous abortions. These results are higher than those reported in other studies of repeat abortions and warrant speculation about the relationship of age, marital status and age combined with marital status to abortion history.

Deaths caused by pulmonary thromboembolism after legally induced abortion.

Pulmonary thromboembolism is an infrequent but serious complication of induced abortion. Of the 104 abortion-related deaths reported to the Center for Disease Control in 1972 through 1975, 10 (9.6 per cent) involved fatal pulmonary embolism; eight of these cases were proved at autopsy. All but one of the women had pre-existing risk factors for thromboembolic disease, including obesity, previous thrombophlebitis, use of oral contraceptives, or type A blood. All but one case occurred in women who had received general anesthesia. In addition, four of the eight women had undergone a concurrent sterilization procedure at the time of the abortion. Preventive efforts should focus on identifying women at high risk for thromboembolic events prior to the abortion procedure and then selecting the abortion procedure least likely to produce postoperative embolism.

Transfusions of leukocyte-rich erythrocyte concentrates: a successful treatment in selected cases of habitual abortion.

Forty-nine women who suffered recurrent spontaneous abortion of unknown etiology were studied for cellular reactivity and blocking antibody in one-way mixed lymphocyte culture before and after their receipt of three transfusions of leukocyte-rich erythrocyte concentrates from third-party donors. Those 38 of the 49 women who had no blocking antibody all developed significant blocking activity after the transfusion series. Twenty-five of them have become pregnant since, and only one aborted again. The blocking activity demonstrable in 11 of 49 women was increased after the transfusions. Subsequently five of them became pregnant, and all aborted again. We later found that these five women, who considered themselves to be in perfect health, all had serologic signs of autoimmune disease. We advise against transfusion treatment of women who habitually abort without preceding immunologic investigation, because a population of habitual aborters may contain women with yet undiagnosed autoimmune disease, who would be worse off after blood transfusion. We conclude from our results that a selected population of habitual aborters, that is, those without blocking antibody, benefits from transfusion treatment.

Relationship between abortions and seroprevalences to selected infectious agents in dairy cows.

The aim of this study was to determine whether the occurrence of abortion is related to the seroprevalence of abortion-causing infectious agents. In a cross-sectional study, cattle from dairy farms in Switzerland that were defined as having an abortion problem were divided into two groups: cows with a history of abortion within the previous 3 months (cases) and cows without a history of abortion (controls). A positive titre to Leptospira spp. was associated with an increased probability of being a case (OR = 1.74, 95% CI = 1.21-2.47). There were interactions between Coxiella burnetti titre and parity, and between Chlamydia psittaci and C. burnetti titre and breed. Multiparous cases after the second lactation with a positive titre to C. burnetti were less likely (OR = 0.42, 95% CI = 0.22-0.82) to be cases. Swiss Browns (Swiss Braunvieh and Brown Swiss) with a positive titre to C. psittaci and Swiss Browns with a positive titre to C. burnetti were more likely (OR = 1.63, 95% CI = 1.13-2.37 and OR = 1.79, 95% CI = 1.15-2.78, respectively) to be cases. Parity alone was not associated with the occurrence of abortion.

Spontaneous abortion in immunodeficient SCID mice.

PROBLEM: Immunodeficient SCID mice on the CB-17 have been used to test the role of "rejection" in a xenogeneic blastocyst transfer model of recurrent miscarriage, but interpretation of the data requires knowing syngeneic within-species matings have a high success rate and do not require immunotrophic factors expected only in immunocompetent non-T-cell deficient mice. METHOD: Resorption rates were studied in a SCID CB-17 barrier facility that provided the mice used to test the role of immunology in the resorption model. RESULTS: Spontaneous resorption in syngeneically mated immunodeficient SCID mice on the CB-17 background occurred at an unexpectedly high rate and could not be prevented by treatment with anti-asialo GM1 antibody or GM-CSF, both of which are effective in ameliorating abortion in DBA/2J-mated CBA/J mice. Immunocompetent CB-17 +/+ mice showed an even higher rate of loss. The latter was also not affected by treatment with antiasialo GM1 antibody or by GM-CSF and was not prevented by tetracycline (which is effective in the DBA/2-CBA/J system) or progesterone treatment. Mating experiments showed a scid/+ x scid@+ cross gave the highest rate of loss, and it appeared that the presence of +/(+)-type embryos in the uterus could be augmenting abortion with selective discrimination against scid/scid embryos. High abortion rates were associated both with appearance of a coagulase-negative Staphylococcus sp. in feces and with loss of one component of the SPF flora. Decidual tissue from mated CB-17 +/+ mice showed premature release of TNF-alpha in absence of TGF-beta 2-related suppressor activity, and vascular lesions (fibrinoid necrosis), varying in extent, were associated with both scid/scid x scid/scid and +/+ x +/+ pregnancies. TNF-alpha also appeared prematurely in pregnant scid/scid mice, but the levels were lower (and areas of necrosis smaller than in +/+ x +/+ pregnancies). Outcrossing onto a C57B1/6 background dramatically reduced the abortion rate, indicating an important genetic effect on susceptibility with heterogeneity protecting against abortion. CONCLUSIONS: SCID mice on the CB-17 background do not have a high rate of successful syngeneic pregnancies, and a TNF-alpha induced vasculopathy may be responsible. Abortion was not caused by immunodeficiency leading to loss of immunotrophism because immunocompetent non-SCID CB-17 mice had a higher rate of loss. Factors augmenting the abortion rate included the presence of embryos of the +/+ genotype in the uterus and treatment with anti-asialo GM1 antibody. Abortion rates were not reduced by treatments effective in the DBA/2-mated CBA/J mouse model but were reduced by re-establishing a new colony with defined flora (a temporary effect) and by outcrossing mice with a different (C57B1/6) background. Together, the data suggest an infectious trigger (identity uncertain) of the vasculopathy and an important genetic influence on susceptibility with heterozygosity and a SCID mouse mutation providing against abortion a degree of protection.

Contraception and induced abortion in Armenia: a critical need for family planning programs in eastern Europe.

OBJECTIVES: The purpose of this study was to determine the number of induced abortions per woman and the reasons for selecting induced abortion among parous Armenian women. METHODS: A consecutive series of 200 women attending an abortion clinic in Yerevan, Armenia, were queried by a physician about their reproductive histories. RESULTS: Women younger than 20 years of age reported a median of 1 and women older than 40 years reported a median of 8 induced abortions in their lifetimes (overall median = 3). Lack of contraceptive information was the major reason cited for not using contraception. CONCLUSIONS: Induced abortion is the major form of birth control among parous Armenian women. Concerted public health campaigns are needed to inform women and their physicians in Armenia and other Eastern European countries about alternative contraceptive methods.

Association of endometriosis and spontaneous abortion: effect of control group selection.

Endometriosis has been associated with an increased incidence of spontaneous abortion, compared with the abortion rate of the general population. To assess whether a separate control group would affect these conclusions, we studied 139 consecutive infertility patients with laparoscopically proven endometriosis to determine the incidence of spontaneous abortion. Ninety-five of these patients underwent conservative surgical resection of endometriosis, and 44 patients opted for expectant management. There was no significant difference between these two groups in average age, duration of infertility, or proportion of patients with primary infertility. The average spontaneous abortion rate before diagnosis for all patients was 63.1%. After surgical treatment, the abortion rate dropped to 0% (P less than 0.000001) for all stages of disease. However, even in those patients who received expectant management only, the abortion rate fell to 16.7% and 21.4% for mild and moderate endometriosis, respectively (P less than 0.001). These results suggest that the spontaneous abortion rate in untreated endometriosis may not be as high as previously reported and may not be significantly different from the rate in the general population. The data also emphasize the need for well-defined control groups when assessing the effects of a treatment regimen.
