A randomized controlled clinical trial of conventional and immediately loaded tapered implants with screw-retained crowns.

PURPOSE: Surgical, prosthodontic, and esthetic outcomes of conventional and immediately loaded, single, tapered, roughened-surface Southern implants in the anterior maxilla that were restored with screw-retained crowns were compared over 1 year. MATERIALS AND METHODS: Standardized surgical and prosthodontic procedures were followed and accepted criteria were used for assessment. RESULTS: There were no significant differences within or between the control and test groups for age, gender, bone quality or quantity, implant stability measurements at surgery, or implant length. CONCLUSION: After 1 year, the implants that had been immediately loaded with single provisional crowns at surgery and definitive crowns 8 weeks later were as successful as conventionally loaded 2-stage implants.

The periodontal-restorative interface in fixed prosthodontics: tooth preparation, provisionalization, and biologic final impressions--Part II.

The delicate dento-gingival attachment apparatus must be treated with utmost care during all phases of preparation in the crevicular region. There is little room for error within these minute dimensions. The learning objective of this article is to encourage the dentist to be ever vigilant when approaching the subcrevicular tissues during biomechanical tooth preparation, provisionalization, biologic final impressions, and final cementation. Part I, published in the January/February, 1994, issue of PP&A, discussed the emergence profile, retraction, preparation modalities, and provisionalization. Part II discusses final impressions and casts derived from information provided by morphologically correct provisionals, surgically directed provisionalization, and final functional and aesthetic rehabilitative results.

Life on the arc: principle-centered comprehensive care.

Today's dental practice is experiencing an evolution in the manner through which new materials and techniques are marketed and introduced. An increasing concern among the patient population regarding aesthetics contributes to the acceptance of a commodity dental philosophy, without questioning the reliability of the technique or new material. A principle-centered practice differentiates the product marketing from the viability of a restorative material in vivo. This article discusses the concept of a principle-centered practice and describes how to place quality products in a balanced system in which harmony exits between all components of the masticatory system: the teeth, the muscles, and the temporomandibular joints.
