The dental health of pre-school children in a deprived urban community in Glasgow.

OBJECTIVE: This study investigated the dental health status of pre-school children in a deprived urban community in Greater Glasgow. The aim was to gather baseline data to support the need for a multi-agency dental health programme for this age group and against which trends in dental health could be measured over time. METHOD: A defined deprived community was identified and an area profile compiled. Children attending the five nursery schools in and around the area were examined using the standardised criteria adopted by BASCD/SHBDEP. RESULTS: Two hundred and forty-eight children were examined representing 75.8% of those on the nursery rolls. Caries prevalence and mean dmft rose from 64% and 3.14 for three to three and a half-year-old children to 86% and 6.14 for four and a half- to five-year-old children. This latter figure was higher than the Scottish and Health Board averages for five-year-old children (2.93 and 3.5 respectively). Those from the most deprived postcode sector had significantly worse dental health than those resident in other areas (mean dmft = 6.50 compared with 3.77). They also had significantly more unrestorable lesions. Overall, the Care Index (ft/dmft x 100) was 3.03 which is less than the Scottish average of 7.8. CONCLUSION: The dental health of nursery school children in and around the Possilpark area of Glasgow is worse than both the Scottish and health board averages for five-year-old children. Those resident in the most deprived sector of the community have significantly worse dental health. The main components of dmft were untreated decay and missing teeth. The Care Index was low.

Caries, diet, dental knowledge and socioeconomic variables in a population of 15-year-old Israeli schoolchildren.

A population of 163 10th grade, 15-yr-old students of a Jerusalem high school were surveyed for caries prevalence, cariogenic dietary habits, dental knowledge and socioeconomic variables. A mean level of 7.75 DMFT was found, confirming results of previous studies which indicate a general increase of caries prevalence in Israel over the past two decades. None of the independent variables measured constituted significant predictors of caries prevalence. However, father's geographic origin, mother's educational level and social class according to father's occupation demonstrated significant associations with caries treatment levels as expressed by the FT and DT components of the DMFT index.

Association between some educational indicators and dental caries experience of 12-year-old children in developing countries: an ecological approach.

OBJECTIVE: The aim of this study was to investigate the association between some educational indicators and dental caries experience of 12-year-old children in developing countries. BASIC RESEARCH DESIGN: The ecological association between DMFT levels amongst 12 year olds (DMFT-12 index) with six educational indicators (adult literacy rate, mean years of schooling, pupil-teacher ratio for primary level, educational expenditure as a percentage of gross national product, primary enrollment ratio and percent completing primary level) has been studied using developing countries as the unit of analysis. Pearson's correlation analysis and stepwise linear multiple regression technique were used to identify the significantly associated educational indicators with the DMFT-12 index. RESULTS: A negative association between DMFT-12 index and percent completing primary education level has been observed (r = -0.509; p < 0.01). Also, stepwise regression analysis results have shown that only percent completing primary level education is significantly associated with DMFT-12 index scores (partial regression coefficient = -0.042; 95% confidence intervals: -0.064, -0.021). CONCLUSION: Dental caries experience of 12-year-old children appears to be highest in countries with low percent of primary level completion. Percent completing primary level education may be considered a good predictor of DMFT-12 index in developing countries.

[Dental caries in preschoolers and students in Monte Patria, region 4].

An epidemiological study on caries prevalence was undertaken on school children and pre-school children aged from 2 to 14 years, of Monte Patria, a small city located in the North of Chile, following the criteria established by the World Health Organization. It was found that the 90.54% of the studied sample had caries, the 21% of the patients lost permanent teeth due to caries, and the presence of filled teeth was detected only in 1.75% of the children. DMFT index of the sample was 5.16 and dmft index was 4.60.

[Epidemiological survey of dental caries in primary schoolchildren in Condeixa-Nova during 1986/87].

An epidemiological survey on the prevalence of dental caries was carried out among the population of the primary schools in the municipality of Condeixa-a-Nova. A total of 716 children from both sexes, 6-13 years old, was involved, being comparatively higher the number of boys. The values of CPO which we found were not high, being of great interest on the component of carious teeth without treatment (lack of professionals? lack of motivation?). It was verified that the CPO index was rather higher among girls than among boys.

[The oro-dental status of children 6-15-years of age in Pays de la Loire, Vienne and Deux Sèvres].

This survey was conducted on a representative sample of schoolchildren aged 6 to 15 years from Pays de la Loire, Vienne and Deux-Sèvres. The results showed that the DMFT and DMFS indices increased with age. The girls had a higher caries experience. The children from rural zones had a higher caries prevalence than those of urban areas. However the differences were not statistically significant. Relatively low values were recorded between 6 and 15 years for the plaque index, the calculus index and the gingival index.

[Simplified caries index for ages 4 to 6 and 7 to 10 (deciduous and mixed dentition)].

The purpose of this research is to obtain a simplified caries index. Four hundred and twenty children, both sexes, 4 to 10 years of age were divided in two groups: 4-6 years and 7-10 years. For caries prevalence, on the deciduous dentition, we used a modified GR JEBBEL index and for permanente teeth, the criteria established by KLEIN et alii, 1938. Considering the cumulative effects of caries, in the mixed dentition, we considered the sum of dmfs and DMFS. A simplification of the total index was performed, for the ages 4 to 6 years considering the teeth 54, 61, 75, 82 and for mixed dentition we added the tooth 26 and 26. The results showed in both dentition, that simplified caries index showed significant correlation with the total index.

Lessons learned from a pilot study on the health status of children from itinerant populations.

The pilot study is used to guide development of a research design or to test an already developed plan; however, results of pilot work often are not reported. Pilot work can yield many valuable lessons and provide investigative training to the principal investigator and as well as evidence to funding agencies that the research is feasible and worthwhile. The pilot study described here is preliminary work on health disparities in two itinerant populations: carnival and migrant farmworker children. This pilot study had three aims: (a) identify the most productive methods to recruit subjects from these two "invisible" populations; (b) test feasibility of the research protocol; and (c) collect preliminary data on the children's health status for future research proposals.

Self-reported tooth loss and bone mineral density in older men and women.

It has been reported that postmenopausal women with osteoporosis have a higher than expected number of dentures and fewer teeth than women without osteoporosis. The relationship between self-reported tooth loss and bone mineral density (BMD) at the hip and spine in 608 men and 874 women, aged 65-76 years, was examined in a cross-sectional study. BMD was measured using dual X-ray absorptiometry. Twenty-four per cent of men and 27% of women had no natural teeth. There was a consistent decrease in BMD with increasing numbers of teeth lost in men. This relationship was independent of age, body mass index and smoking habit (trochanter r = -0.14, p < 0.0005; Ward's triangle r = -0.1, p < 0.005; and lumbar spine r = -0.07, p < 0.05) with between 5% and 9% difference in mean BMD at various sites between men who had all their teeth and men who had no teeth. There was no significant association between self-reported tooth loss and BMD in women. Tooth loss is associated with lower BMD in men; this relationship is less consistent in women.

Oral conditions in a population of young adults in Strasbourg.

The oral conditions of 1,993 young adults, 16-30 years old, constituting a statistically representative random sample of the population of Strasbourg, were studied. Plaque, calculus, and gingival indices were determined as well as DMFT and DMFS caries indices. All these indices showed a highly statistically significant linear correlation with age. Sex had a significant influence on caries indices but not on plaque, calculus, or gingival indices. Socioprofessional conditions had a very significant influence on caries and gingival indices but not much on plaque index and not at all on calculus index.

[Dental caries, urinary fluorides and enamel opacities].

The aim of this study was to analyse with different statistic models the relation between fluoride urinary concentration, fluorosis and dental caries in terms of water fluoridation. 3237 schoolchildren aged 5 to 21 years were examined by a single dentist in accordance with the guidelines of the World Health Organization and following the standardized system for diagnosing, recording and analyzing dental caries data of Möller & Poulsen (1973). The index of Thylstrup & Fejerskov (1978) was applied to measure the prevalence and severity of dental fluorosis. Urinary fluoride concentration in 3022 children and water fluoride concentration were measured. The results of this study show a close relation between a high percentage of urinary fluoride, the presence of opacities and a low caries index.
