An assistive-technology intervention for verbose speech after traumatic brain injury: a single case study.

OBJECTIVE: To assess an assistive-technology intervention for neurobehavioral change, coordinated with a Web-based data recording system. PARTICIPANT: Male, mid-thirties, with a complex history, having verbose speech subsequent to traumatic brain injury. DESIGN: BAB' single-case. INTERVENTION AND MEASURES: The intervention (B and B' trials) was a digital recording of the participant's voice, stating "be brief," delivered at fixed intervals by a personal digital assistant. Utterance start and stop times were monitored with a wireless, Web-enabled device and were recorded in a server-side database. RESULTS: Utterance frequency did not change. However, the total utterance time was reduced during B trials, attributable to the reduced frequency of lengthier utterances. CONCLUSIONS: The target behavior was modified successfully. The participant adopted a specific cue-appropriate strategy. Since a variety of alternative response strategies may be required of a person, given a range of context-specific social practices and community settings, the importance of context-sensitive assistive technology cue repertoires is discussed.

Interobserver reliability of a CT-based fracture classification system.

OBJECTIVES: This study was designed to determine whether the interobserver reliability of a fracture classification scheme applied based on a single, carefully defined, computed tomography (CT) cut is greater than those previously reported for systems designed for use with plain radiographs. DESIGN: Observer review of selected cases. SETTING: Four, level one, trauma centers. PATIENTS: Pretreatment CT scans of patients with calcaneus fractures were screened by the authors. Thirty cases were selected that had an appropriate semicoronal CT image. Ten orthopaedic traumatologists who were members of the Orthopaedic Trauma Association and had a minimum of 5 years postresidency experience were selected as reviewers. INTERVENTION: The reviewers were provided with a digital CT image for each case as well as written and diagrammatic representations of the Sanders classification system. The observers then classified each fracture according to the Sanders classification. RESULTS: : The mean kappa value for interobserver reliability for fracture types I-IV was 0.41 +/- 0.02 (mean +/- standard error of the mean; range, 0.07-0.64). Observers disagreed by more than 1 fracture type (ie, I vs. III or II vs. IV) in 10% of the cases. Observers agreed on the location of the fracture lines (A, B, C) in 90% of type II fractures and 52% of type III fractures. CONCLUSIONS: The results indicate that in a carefully controlled paradigm, the interobserver reliability with a classification system based on interpretation of a single, carefully defined CT image was no better than the results reported for the same classification system used with full CT data or for other classification systems used for various fractures in the skeleton. Agreement in identifying the location of the fracture lines was very good for simple fractures but much worse for complex injuries. Additional study may determine whether the use of a full complement of CT images can improve reliability in classification of complex injuries.

Particulate silicone for vocal fold augmentation: morphometric evaluation in a rabbit model.

To study the augmentation of the paralyzed vocal fold in response to the injection of particulate plastics in a rabbit model, we performed a prospective, experimental, and controlled study in 45 New Zealand rabbits with surgical vocal fold paralysis. We compared a control group (acute unilateral recurrent nerve section) with a group that underwent polytetrafluoroethylene (PTFE) injection and with another group that underwent polydimethylsiloxane (PDMS) injection. Equal volumes of implant material were injected. The animals were killed either 6 weeks or 6 months after intervention and compared by histomorphology using a digital measuring system. The injection sites showed a substantial foreign body giant cell reaction against the PDMS and PTFE, surrounded by a light, fibrous stroma. No differences in the frequency of displacement or disaggregation of implants were observed. No extrusion to the laryngeal lumen, signs of migration outside the larynx, or malignant changes were seen. No differences were found in the length of the vocal fold between the PDMS group and the PTFE group. The vocal fold was thicker in the PDMS group than in the PTFE group, but a statistically significant difference was found only for the anterior and middle thirds at 6 weeks. In the PDMS sample, the thicknesses were similar in the 6-week and 6-month subgroups. The group with injection of PDMS yielded histologic images compatible with greater and more stable medialization than did the group with injection of PTFE.

Can quantitative capnometry differentiate between cardiac and obstructive causes of respiratory distress?

STUDY OBJECTIVE: To determine whether quantitative measurement of end-tidal carbon dioxide (ETCO2) can differentiate between cardiac and obstructive causes of respiratory distress. DESIGN: Prospective observational study. SETTING: Emergency department (ED) of a tertiary care hospital. PATIENTS: Adult patients who presented to the ED with moderate-to-severe dyspnea. Patients were excluded if they were unable to cooperate with the performance of peak expiratory flow rate (PEFR) or ETCO2 tests, were younger than 18 years of age, or had received prehospital intervention for their respiratory distress. INTERVENTIONS: Physicians obtained an ETCO2 level and PEFR prior to ED pharmacologic intervention. A hand-held capnometer with digital read-out was used to obtain the ETCO2 level. The patient's age, sex, initial vital signs, breath sounds and medication history, the presence or absence of diaphoresis and/or orthopnea, the duration of symptoms, the chest radiograph interpretation, and final diagnosis were also recorded. MEASUREMENTS AND RESULTS: Forty-two patients were eligible for inclusion in the analysis. The mean ETCO2 level was 31.1+/-9.4 mm Hg; the mean PEFR was 161.3+/-53.1 L/min. The ETCO2 levels for pulmonary edema/congestive heart failure (CHF) patients differed significantly from those of asthma/COPD patients (27.1+/-7.8 mm Hg vs 33.4+/-9.6 mm Hg; p=0.0375). However, no single ETCO2 level was found to be a reliable predictor of diagnosis. CONCLUSION: ETCO2 levels for pulmonary edema/CHF patients differ significantly from those of asthma/COPD patients. However, no single ETCO2 level reliably differentiates between the two disease processes.

[The simultaneous occurrence of inguinal hernia and colorectal cancer--case reports].

INTRODUCTION: The simultaneous occurrence of inguinal hernia and colorectal cancer is rare, but it is well known by surgeons. We call attention to the synchronous diseases with the help of cases accumulating in our department and with the support of literature. CASE REPORTS: A 76-year-old male hernioplasty by Lichtenstein was performed on, and during the exploration because of ileus having developed in the postoperative period a stenosing rectum carcinoma was discovered. 87-year-old inguinal hernial male patient's sigmoid tumor was justified as a result of complex examination completed before surgery. The resection of sigmoid and simultaneously left-sided hernioplasty were carried out transabdominally with implantation of polypropylene mesh. With a 81-year-old male presenting bilateral inguinal hernia a carcinoma of rectum was found with digital examination. A rectum resection by Dixon was made, and in second stage inguinal herniorrhaphies are planned after cytostatic treatment. With a 94-year-old male patient arriving with incarcerated hernia a stenosing carcinoma of rectum was palpated with rectal digital examination, but the patient refused all intervention. After reposition of abdominal wall hernia of an 81-year-old female patient a tumor was found--through colonoscopic examination--30 cm from anal opening, but the patient rejected any kind of operation. CONCLUSION: With the contemporaneous appearance of two diseases should be taken into consideration, when long existing hernia rapidly becomes larger, and complaints emerge with the hernial patients. At this point the complex examination of colon is inevitable during the operative preparation.

A prospective comparison of 3 new-generation pulse oximetry devices during ambulation after open heart surgery.

OBJECTIVE: To assess the clinical performance of 3 new-generation pulse-oximetry signal-processing software systems (Philips FAST, Masimo SET, and Nellcor N-3000) during ambulation after open-heart surgery. DESIGN: Prospective, convenience sample. SETTING: Cardiac surgical progressive care unit in a 629-bed, not-for-profit, tertiary-care teaching hospital. PATIENTS: Status post-cardiac-surgery patients (n = 36) during their first postoperative ambulation. INTERVENTIONS: None. PROTOCOL: Randomization was used for digit and hand selection, and all 3 devices were used continuously during ambulation. Data on dropouts, false alarms, and correlation with heart rate were recorded. We continuously measured arterial oxygen saturation via pulse oximetry during ambulation with all 3 devices. RESULTS: Pairwise comparisons indicated significant differences among the 3 devices for data dropout and false alarm. In repeated-measures analysis, the Nellcor N-3000 had the greatest likelihood of data dropout (odds ratio of 31.9 to Masimo and 5.6 to Philips, at the 95% confidence interval). However, the converse was true for false alarms; the Masimo had the most false alarms, with an odds ratio of 17.9 to Nellcor and 2.3 to Philips, at the 95% confidence interval. There were also significantly more dropouts with all 3 devices when readings were taken on a hand on an arm from which a radial graft had been taken (p = 0.004). For heart-rate correlation, the mean absolute difference among the 3 devices was similar: Philips = 4.3 beats/min, Masimo = 5.1 beats/min, and Nellcor = 3.0 beats/min. CONCLUSIONS: There are significant differences among the 3 devices with regard to dropout and false alarms. High numbers of dropouts are problematic because no pulse-oximetry patient information is available during dropout. However, false alarms are even more problematic, because they desensitize clinicians to alarms and call into question the accuracy of displayed data. While our data highlight the statistical differences between the studied oximeters, the clinical implications of these differences warrant further study.

Digital imaging fiber-optic trans-illumination, F-speed radiographic film and depth of approximal lesions.

BACKGROUND: Detection of early carious lesions is a prerequisite to an optimal preventive and minimal surgical intervention strategy. The authors conducted this study to determine whether Digital Imaging Fiber-Optic Trans-Illumination (DIFOTI) (Electro-Optical Sciences, Irvington, N.Y.) could be useful in evaluating early approximal lesions and to compare radiographs produced with F-speed film with both histologic lesion depth and cavitation. METHODS: The authors created artificial approximal lesions in vitro in extracted teeth over 14 weeks and imaged them using a "bitewing-like" view every two weeks with DIFOTI and F-speed radiographic film. At the end of the 14 weeks, the authors examined the lesions for surface cavitation using visual and tactile methods. They then thin-sectioned the lesions and subjected them to histologic analysis using polarized light microscopy (PLM). RESULTS: DIFOTI was not able to measure the depth of a lesion in any of the samples. It was, however, able to show surface changes associated with early demineralization as early as two weeks. The depth of a lesion measured using F-speed radiographic film was not statistically different from the depth of a lesion measured with PLM histologic analysis (P > .05). None of the lesions showed any signs of surface cavitation after 14 weeks of demineralization. CONCLUSIONS AND CLINICAL IMPLICATIONS: DIFOTI technology should not be used to decide between surgical or chemical treatment strategies based on lesion depth. We found that F-speed radiographic film was accurate in the approximating the depth of the lesion histologically. Ideally, the clinical decision whether to cut the tooth should be made based on cavitation rather than histologic lesion depth.

Quantification of plagiocephaly and brachycephaly in infants using a digital photographic technique.

OBJECTIVES: The aims of the study were: (1) to develop a technique to quantify plagiocephaly that is safe, accurate, objective, easy to use, well tolerated, and inexpensive; and (2) to compare this method with tracings from a flexicurve ruler. DESIGN: A case-control study of 31 case infants recruited from outpatient plagiocephaly clinics and 29 control infants recruited from other pediatric outpatient clinics. PARTICIPANTS: Infants in the study had been diagnosed with nonsynostotic plagiocephaly or brachycephaly and were between 2 and 12 months old. INTERVENTIONS: Infants' head shapes were measured using (a) digital photographs of a head circumference band and (b) a flexicurve ruler. Flexicurve tracings were scanned, and both the digital photos and the scanned flexicurve tracings were analyzed using a custom-written computer program. MAIN OUTCOME MEASURES: The oblique cranial length ratio was used to quantify cranial asymmetry, and the cephalic index was used to quantify the degree of brachycephaly. RESULTS: The infants tolerated the photo technique better than the flexicurve. Also, mothers preferred the photo technique. There was less within-subject variance for the photos than for the flexicurve measurements. The results suggested that an oblique cranial length ratio of >or= 106% can define plagiocephaly and that a cephalic index of >or= 93% can define brachycephaly. CONCLUSIONS: The photographic technique was better accepted and more repeatable than the flexicurve measuring system. We propose that "normal" head shape is indicated in infants with both an oblique cranial length ratio of less than 106% and a cephalic index of less than 93%.

Acquired rotational digital deformity in children as a result of finger sucking.

Although most attention to the harmful effects of finger sucking by infants has been on the potential dental deformities, finger deformities also occur. The three cases presented in this article describe radial rotation of the index finger as a result of persistent finger sucking. In the majority of cases rotational deformities spontaneously resolve once finger sucking ceases. In a few cases, particularly when the habit is unduly prolonged, deformities may persist and cause functional impairment. In these patients surgical intervention is indicated. In our cases the rotational digital deformities were treated successfully by rotational osteotomy of either the metacarpal or the proximal phalanx.

Role of digital subtraction fluoroscopic imaging in detecting intravascular injections.

Complications of unintended intravascular penetration and subsequent injection of local anesthetics and/or other drugs occasionally are devastating. Multiple reports have related to unrecognized intravascular placement of the needle. The generally accepted technique during interventional techniques is intermittent fluoroscopy. In fact, this may miss vascular uptake due to rapid washout. Thus, digital subtraction, the commonly accepted standard for documenting angiography and venography, may be a useful tool for documentation of and/or avoidance of intravascular placement of the needle and subsequent injections with interventional techniques. This case report involves description of digital subtraction to enhance visualization of contrast distribution during injection. Three series of images are included from atlanto-occipital joint, caudal epidural catheterization, and a cervical transforaminal injection. The case reports demonstrate that digital subtraction fluoroscopic imaging is superior to intermittent or live fluoroscopy in detecting intravascular injections with interventional techniques.

Family practice residents' compliance with preventive medicine recommendations.

We conducted a medical records survey of 270 randomly selected patients 20 years of age or older seen by family practice residents in a university-based training program. This study concerned residents' compliance with certain preventive recommendations made by four expert groups: the American Cancer Society, the Canadian Task Force on the Periodic Health Examination, the Immunization Practices Advisory Committee, and the National Institutes of Health Consensus Development Conference on treating elevated cholesterol. The interventions studied were breast examination, mammography, pelvic examination, Pap testing, digital rectal examination, stool guaiac slide testing, serum cholesterol screening, and pneumococcal and influenza vaccinations. We calculated the percentage of eligible patients receiving each of these interventions. The highest rates of resident compliance with health screening recommendations were for Pap testing in women 20-34 and 35-59 years of age (31.7% and 22.6%, respectively), breast examination in women 20-39 years of age (27.3%), and influenza vaccination (23.0%). There were no other rates of compliance that exceeded 20%. Our data thus indicate that residents in this program did not comply well with health maintenance recommendations.

Parenchymal perianeurysmal cyst in the brain: case report.

OBJECTIVE AND IMPORTANCE: Parenchymal perianeurysmal cysts are rare, and only seven cases have been reported. We present a case report with a 30 month follow-up on this topic. The possible etiopathogenetic mechanisms of cyst formation are discussed. CLINICAL PRESENTATION: A 54-year-old man with a 5-month history of headache and a computed tomography scan showing a giant parenchymal cyst located in the right temporal lobe with a mural enhanced nodule was admitted to our neurosurgical department with the diagnosis of cystic brain tumor. Magnetic resonance imaging followed by digital subtraction angiography identified the enhancing nodule as a large right middle cerebral artery aneurysm. INTERVENTION: Surgical treatment was performed; the aneurysm was clipped and the cyst evacuated. Postoperative digital subtraction angiography confirmed the clipping of the aneurysm at the neck. Serial magnetic resonance imaging controls showed the permanent collapse of the cyst. CONCLUSION: Parenchymal perianeurysmal cysts are rare. In the presence of parenchymal cysts neighboring main vessels, the possibility of a perianeurysmal cyst should be considered. In regard to the etiopathogenetic mechanisms responsible for the cyst development, the action of multiple coexisting factors seems to be the most applicable.

Impact of a social marketing media campaign on public awareness of hypertension.

BACKGROUND: Barriers to high blood pressure (BP) awareness and control are exacerbated by poor knowledge of the consequences and uncertainty regarding how to and who should direct care. We developed a social marketing hypertension awareness program to determine baseline awareness, knowledge, and treatment behavior, and then studied the impact of a targeted, media intervention among randomly surveyed adults at risk in a representative urban community compared to a control community immediately and 6 months after the intervention. METHODS: The program consisted of three random-digit telephone surveys conducted in two mid-sized Ontario cities to determine high BP awareness, knowledge, and treatment behavior. Using baseline knowledge and attitudes toward high BP in both communities, a social marketing awareness strategy and mass media intervention campaign incorporating television, radio, print, direct to patient, and interactive techniques was developed and implemented in the test city only. Both test and control cities were resurveyed immediately after and at 6 months post-media intervention to detect change and decay. RESULTS: A sample of 6873 men and women more than 35 years old who were aware of their high BP demonstrated a high prevalence of high BP in the general population ( approximately 34% in both communities). At baseline this population had poor knowledge of their own BP numbers and poor understanding of the diseases related to high BP. Although few considered high BP a health concern, they had good understanding of lifestyle interventions for high BP prevention and control. The number of the respondents who claimed to have high BP increased immediately after intervention in the test city (38%; P < .02), whereas the number of respondents who were treated and uncontrolled decreased (P < .05) compared to control. There was a significant increase in patients' knowledge of consequences and in their perception that they were most responsible for high BP control in the test city (P < .005) compared to control. At 6 months, no further changes were observed in those claiming to have high BP in either city, whereas decay to baseline in those treated but not controlled and those claiming responsibility for their BP control was observed in the test city. No changes were observed in the control city accept for an increase from baseline to 6 months in the percentage claiming to be treated but uncontrolled. We were unable to determine whether the increase in number treated but uncontrolled was due to a higher treatment rate, similar treatment rate but more patients being uncontrolled, or a combination of these scenarios. CONCLUSIONS: High BP is very prevalent in adults and knowledge of lifestyle options for management is encouraging. In the short-term, although our media awareness program increased the number of respondents claiming to have high BP and patient self-efficacy for BP control, this was not maintained. We did not change knowledge of consequences or importantly, the health importance of BP control among those at risk. Hence, in addition to a mass media campaign, attention should be focused on dissemination of awareness knowledge information through medical professionals at the point of care.

Development and Validation of Machine Learning Models for Predicting Early Cognitive Decline Using Home Sensor-Derived Behavioral Data: Sensors in-Home for Elder Wellbeing (SINEW) Cohort Study.

BACKGROUND: As the global population continues to age, the prevalence of geriatric conditions, including dementia and frailty, is also increasing. Early identification of individuals at an elevated risk of these conditions, such as those presenting with mild cognitive impairment (MCI) or prefrailty, can provide a critical window for prompt intervention aimed at preventing or reversing disease progression. To promote such early identification, there is a burgeoning interest in the use of digital sensor technology and predictive modeling. OBJECTIVE: This study aimed to use a continuous, home-based monitoring sensor system for older adults to distinguish those exhibiting normal aging from those with MCI, early dementia, prefrailty, or frailty, and to predict their transition from normal aging to one of these conditions. METHODS: This longitudinal cohort study will recruit 200 community-dwelling adults aged ≥65 years with normal cognition or MCI at baseline. A multi-sensor system will be installed in participants' homes, including passive infrared motion sensors, door contact sensors, bed sensors, medication box sensors, wearable activity bands, and Bluetooth proximity beacons. These devices will continuously capture spatiotemporal activity patterns, mobility indicators, sleep behaviors, and medication-taking routines. Annual assessments will include standardized cognitive tests (eg, Montreal Cognitive Assessment, Mini-Mental State Examination, Rey Auditory-Verbal Learning Test, digit span, Color Trails Test, semantic fluency, Stroop), frailty measures (modified Fried phenotype, gait speed, grip strength), mental health scales, sleep quality, and psychosocial indicators. Sensor-derived features-such as gait variability, activity regularity, sleep fragmentation, and medication adherence patterns-will be integrated with clinical data to develop supervised machine learning models. Planned approaches include logistic regression, random forests, gradient boosting, and deep learning. Model performance will be evaluated using cross-validation and independent test sets. Primary metrics will include area under the receiver operating characteristic curve, sensitivity, specificity, precision, recall, and F1-score. Models will be benchmarked against gold-standard clinical diagnoses and validated using temporal subsets of the dataset. RESULTS: Enrollment for this study started in November 2019 and will continue until March 2030. As of June 2025, we have enrolled 138 participants. Full data analysis has yet to begin. CONCLUSIONS: We aim to develop a reliable and effective sensor system for in-home use that will facilitate the early detection of cognitive and physical decline. In so doing, it will add to our current understanding of digital biomarkers. It is common for older adults to seek clinical intervention only when their cognitive impairment has already reached an advanced stage. The implementation of readily deployable sensor systems within community settings presents us with opportunities for prompt intervention, which holds the potential for delaying or reversing disease progression and allowing for a greater number of functional and meaningful years.

Implementing a novel digital health platform for self-management of postmenopausal osteoporosis: A qualitative study of user experiences, perspectives and implementation outcomes.

BACKGROUND: Osteoporosis self-management requires scalable support, and digital health platforms may meet this need. This study aimed to characterise the experiences and perspectives of postmenopausal women who participated in a 12-month randomised controlled trial (RCT) of a digital voice assistant (DVA) delivered osteoporosis self-management intervention, and to assess key implementation outcomes. METHODS: This was a qualitative analysis of interviews with postmenopausal women from the intervention arm (DVA group) of the RCT. The DVA program broadcast education videos, medication reminders, home-based exercise, nutrition advice and monthly quizzes through a DVA device. Semi-structured interviews were recorded, transcribed and managed in NVivo through reflexive thematic analysis, guided by the Practical Planning for Implementation and Scale-Up and Proctor's implementation outcome taxonomy frameworks. Evidence weighting summarised participant coverage and code density. RESULTS: Twenty-two of 25 (88%) DVA group participants completed semi-structured interviews. Thematic analysis identified seven themes mapped to Proctor's implementation outcomes. Evidence weighting indicated strong support for the intervention's appropriateness and acceptability, moderate support for its adoption, fidelity, feasibility and sustainability, and limited support for costs. Participants valued clear audiovisual guidance, conversation-based interactions with natural language, and flexible home-based access to self-management. CONCLUSION: Digital health platforms for osteoporosis self-management appear feasible, acceptable and sustainable among postmenopausal women. Findings indicate that these platforms are approaching readiness for evaluation in implementation-focused settings, contingent on streamlined content, reliable delivery modalities, accessible user support, clear privacy regulations and pragmatic pricing models.

Acute pulmonary thromboembolism: comparison of the diagnostic capabilities of conventional film-screen and digital angiography.

STUDY OBJECTIVE: To compare digital to conventional film-screen pulmonary angiography for the diagnosis of acute pulmonary embolism (PE) in a clinical population. DESIGN: Retrospective review of patient data, ventilation/perfusion (V/Q) lung scintigraphy reports, and pulmonary angiographic reports. SETTING: University hospital, division of interventional radiology. PATIENTS AND METHODS: Patient data from 307 film-screen and 266 digital angiograms were analyzed for demographics, V/Q lung scintigraphy findings, and pulmonary artery pressures to define patient populations. The interpretations of film-screen pulmonary angiography were then compared with digital angiography interpretations for the entire group of interventional radiologists as well as the two interventionists who practiced throughout the study interval to determine any difference in rates of diagnosis of acute PE between the two techniques. RESULTS: There was no significant difference between the patient populations studied by film-screen or digital techniques for the data reviewed. Digital angiography utilized significantly more contrast material (digital, 173 mL; film-screen, 145 mL; p < 0.01) and a greater number of angiographic views (digital, 3.6 views per patient; film-screen, 3.4 views per patient; p = 0.04) when compared with film-screen angiography. There was no difference between the two techniques in the rates of diagnosis of acute PE, for individual radiologists or overall. CONCLUSIONS: Digital and film-screen pulmonary angiography possess equivalent diagnostic capabilities for acute PE as used in a clinical setting.

Psychosocial mediators of a nurse intervention to increase skin self-examination in patients at high risk for melanoma.

This prospective study examines psychosocial mediators of an efficacious skin self-examination (SSE) intervention that includes provision of a whole-body digital photography book depicting the entire skin surface. Individuals (n = 100) with established risk factors for melanoma were recruited from the Memorial Sloan-Kettering Cancer Center Pigmented Lesion Clinic during their initial dermatologist visit and were randomized to receive a photobook immediately (n = 49) or 4 months after intervention delivery (n = 51). Potential mediators included self-efficacy and response efficacy drawn from Social Cognitive Theory, melanoma worry, and SSE anxiety drawn from Self-Regulation Theory, and skin cancer knowledge, and skin awareness. Only self-efficacy was a significant mediator, accounting for 8% of the total effect of photobook enhancement on SSE adherence at 4 months.
