Bioblock therapy.

An orthopedic treatment philosophy has been presented. Attention is drawn to the fact that the facial bones of both experimental animals and human beings respond extensively to physiologic and pathologic forces, and it is suggested that these could be mimicked to advantage. A system of appliances designed to achieve this is described. Some case reports which appear to show induced skeletal change are presented, but it will be some time before the real contribution of bioblock therapy can be assessed scientifically. Meanwhile, we should perhaps look more closely at our patients' faces and consider treating them at an earlier age.

Ceramic shade prescriptions for work authorizations.

The basic application provides essential data for ceramic veneer fabrication. Three-dimensional information divides the space for body and enamel components. The second body shade selection improves shade matching in the middle one-third of the tooth and enhances the appearance of the enamel component. Dentists are encouraged to recognize the influence each component has upon the total shade composition. A format is established for recording clinical information. The use of available ceramic systems is seemingly unlimited. The method presented provides flexibility for recording the most detailed shade information in simple graphic and verbal terms. Technical simplification is a by-product of complete, precise work information. Clinical judgements based on color and ceramic knowledge, as well as experience, can be conveniently converted into concise component descriptions. Using the suggested guidelines with reasonable care, a comfortable experience base for prescribing difficult shades can quickly develop.

The microscopic appearance and effect of toothbrushing on extrinsically stained metal-ceramic restorations.

To clarify the nature and longevity of the extrinsic stain layer of metal-ceramic restorations, a series of experiments were undertaken. It was found, with microscopic examination, that there is little or no diffusion of the stain into the underlying porcelain and that at a thickness of 22 microns the extrinsic stain layer is grossly evident to an observer. Using a fluoride dentifrice caused no significant loss of either stain or surface integrity with 120,000 brush strokes (equivalent to 8.5 years). At 160,000 strokes (equivalent to 11.4 years) there was evidence of loss of surface integrity.

A retrospective analysis of one implant system in an oral surgery practice. Phase I: Patient satisfaction.

A questionnaire was used to rate patients' satisfaction, expectations, and level of discomfort associated with implant treatment. The questionnaire was sent to 262 patients who had been treated with Core-Vent implants between 1984 and 1988. Over 60% of the patients contacted completed the questionnaire. The distribution of the respondents was evaluated according to age, sex, occupation, and marital status. Statistical interpretation of the data revealed that (1) measures of the level of satisfaction and expectation were significantly less for housewives when compared with other occupations; (2) satisfaction significantly correlated with measures of expectations and lack of discomfort; and (3) expectations correlated with lack of discomfort. In general, most subjects were extremely satisfied with the treatment they received.

The practice of complete denture prosthodontics by selected dental graduates.

Graduates of a dental school in the southeastern United States were surveyed for the procedures they used in practicing complete denture prosthodontics. Of the 530 graduates representing 15 graduating classes, 80% returned the questionnaires. Most respondents (94.8%) made complete dentures, but only 3.5% devoted more than 20% of their practice time to this phase of prosthodontics. In practice, the average graduate performed approximately one half of the techniques that had been judged academically to be clinically essential. Compliance decreased proportionately with the number of years in practice. Factors affecting compliance include the influence of peer practitioners and continuing education courses. The content of the predoctoral curriculum needs to be reviewed for its relevancy to continuing education and to dental practice.

Effects of osseointegrated implants on psychological and social well-being: a literature review.

This article considers three research topics on the social and psychologic aspects of osseointegrated implants. First, the motivations of patients who enter therapy are described. Second, research studies on the psychologic effects of dental implants are critically examined. Although these studies provide consistent evidence in support of osseointegrated implants, there are many issues that make interpretation of results problematic. Finally, some suggestions for future research are made.

Social and economic factors in prosthodontic practice and education.

Prosthodontics represents a highly developed body of knowledge and skill that spans multiple disciplines. Numerous studies can be cited that address biologic, mechanical, or materials science factors that influence decisions about patient care. On a daily basis as clinicians, teachers, or patients, we experience an interplay of social, economic, and psychologic conditions that similarly influence treatment decisions. This article explored a rationale for including a clear, explicit emphasis on knowledge and skill development for prosthodontic practice and education related to social, psychologic, and economic factors.

The surgical-orthodontic correction of maxillary deficiency.

To date surgeons have primarily emphasized advancement of the retrusive or retodisplaced maxilla. There has been no emphasis on combined surgical-orthodontic treatment sequencing in this deformity. Moreover, the maxilla may be deficient not only anteroposteriorly but also vertically and/or transversely. Frequently, when the maxilla is deficient, two or three of these spatial components of the deformities coexist to various degrees. Thus, the term maxillary deficiency is used here to describe the general nature of the skeletal deformity.
