Bilateral parotid enlargement due to malnutrition under the influence of the media in an adolescent in Lithuania.

The elimination of censorship for the media in post-communist countries in transition has contributed to increases in the prevalence of several medical problems. Children and adolescents are particularly vulnerable to the messages conveyed through the media, which influence their perceptions and behaviour. We describe a case of bilateral parotid enlargement due to malnutrition under the influence of self-prescribed diet in an adolescent. A 15-year-old girl reported to our institution under suspicion of Sjögren's syndrome for medical advice. Two months ago she developed persistent bilateral parotid enlargement and a dry mouth. Her medical history revealed a weight loss due to "self-prescribed" reduce diet. Social questioning clarified high use of the media and influence on the body concept and self image. On extra oral examination, a diffuse parotid enlargement was seen bilaterally. The examination of the mouth showed a low moisture level of the intraoral mucosa. The unstimulated whole salivary flow rate was 2 ml in 15 min. Laboratory findings evidenced anemia (107 g/l). The serum albumin concentration indicated a reduced level (28 g/l). Search for antinuclear antibodies, anti-SSA antibodies, anti-SSB, -Sm, -RNP and anti-double-stranded DNA antibodies was negative. Evaluation for antibodies against hepatitis C, cytomegalovirus and Epstein-Barr virus infection and HIV rendered negative results. A histopathologic examination of labial salivary gland biopsy revealed a picture of sialoadenosis. From the above investigations, a diagnosis of sialoadenosis due to malnutrition was made.

Socio-economic transition, inequality, and mortality in Lithuania.

The study assessed inequalities in mortality of Lithuanian urban and rural populations throughout the period of socio-economic transition (1990-2000). Mortality from major causes of death, except cancers in females, was higher among the rural population. Inequality in mortality increased during the period of transition, especially among males, mainly due to more rapidly improving health of the urban population. Cardiovascular diseases and external causes made the largest contribution to the inequality. Differences in mortality of urban and rural populations point to greater social and psychological stress affecting the rural population, unhealthy life styles, inequities in accessibility of health care and lack of preventive programs in rural areas.

2002 summer fires in Lithuania: impact on the Vilnius city air quality and the inhabitants health.

The episodes with highly elevated concentrations of particulate matter (PM10), NOx, and CO were detected in the city of Vilnius between August and September 2002 and possible reasons were analysed. This increase was attributed to emissions from fires in the vicinity of the city of Vilnius when data on fire location and start, wind direction and concentrations of pollutants were analysed. The correlation coefficient between PM10 and CO, NO, NO2, NOx increased during the fire period in comparison with the same correlation after the fire period. During the fire episodes, there was an increase in concentration of ozone precursors, and meteorological conditions were favourable for photochemical reactions. Therefore, the hourly values for ozone exceeded the values common for Vilnius. Generally, during the episodes, the maximum of NO2 concentrations was 2 times higher than the limit value for NO2 laid down in the Council Directives 1999/30/EC [Council Directive 1999/30/EC of 22 April 1999 relating to limit values for sulphur dioxide, nitrogen dioxide and oxides of nitrogen, particulate matter and lead in ambient air. Official Journal L 163, 29/06/1999: 0041-0060], and that of PM10 was even 5.5 times higher than the limit value for PM10. Human health effects study in Vilnius indicated that the incidents of documented respiratory diseases and exacerbation of the bronchial asthma during the fire period were up to 20 times higher in comparison to periods with no fires.

Prevalence and epidemiological features of bovine viral diarrhoea virus infection in Lithuania.

The objectives of the present work were to estimate the level of bovine viral diarrhoea virus (BVDV) infection in cattle herds at the different Lithuanian districts and to determine factors influencing the course of BVDV infection. The studies were explored in 147 intensive dairy cattle breeding herds from 27 different Lithuanian regions in 1997-2001. BVDV infection was diagnosed in all investigated regions. The existing variations in the structure of cattle population determined different distribution patterns of BVDV infection. The number of seropositive animals ranged from 11.9 to 100%. It must be pointed out that 29.9% of the herds were not infected with BVDV and in 32.7% of the herds from 70 to 100% of cattle were seropositive to BVDV. A positive correlation between the number of seropositive cattle, and the size of herds and age of animals was determined. Sex of animal had no influence on the prevalence of BVDV. It was estimated that the annual incidence risk of infection with BVDV decreases with the animal age.

Confidentiality for adolescents seeking reproductive health care in Lithuania: the perceptions of general practitioners.

Confidentiality is a major determinant of the accessibility and acceptability of sexual and reproductive health care for adolescents. Previous research has revealed that Lithuanian adolescents lack confidence in guarantees of confidentiality in primary health care settings. This study aimed to assess the factors that affect general practitioners' decisions whether to respect confidentiality for Lithuanian adolescents under the age of 18. Twenty in-depth interviews were carried out with a purposive sample of general practitioners. The decision whether to respect confidentiality was found to be influenced by external forces, including the legislative framework and societal attitudes towards adolescent sexuality; institutional features in clinical facilities, such as the presence of a nurse during consultations and the openness of the medical record filing system; and individual factors, including GPs' relationships with adolescents' families and their personal attitudes towards sexual and reproductive health issues. The findings reflect the urgent need for a comprehensive policy to ensure adolescents' right to confidentiality in Lithuanian primary care settings, including legislative reforms, institutional changes in health care settings, professional guidelines and (self-)regulation, and changes in medical training and continuing medical education. Other ways to safeguard confidentiality in adolescent health services, such as establishing youth clinics, should also be explored.

The main citral-geraniol and carvacrol chemotypes of the essential oil of thymus pulegioides L. growing wild in vilnius district (Lithuania).

Thymus pulegioides L. with lemon and carvacrol odor form the major part of plants growing wild in all 10 investigated localities during 1995-1997. The main components of the citral-geraniol chemotype of lemon-scented essential oil are the following (%): geraniol (14.9-30. 8), geranial (trans-citral, 9.7-19.7), beta-caryophylene (6.0-11.4), nerol (4.1-11.8), and neral (cis-citral, 0.1-9.5). The essential oil of carvacrol chemotypes contain more compounds that are characteristic of the thyme genus (%): carvacrol (16.0-22.2), beta-bisabolene (11.1-20.2), beta-caryophyllene (11.1-19.1), gamma-terpinene (5.8-16.2), p-cimene (5.5-10.4), thymol (3.3-9.8), and carvacrol methyl ether (5.6-8.6). The correlation between the odor and composition of the essential oil will help the users of wild thyme to choose the necessary chemotype for their purposes.

Effects of mixed grazing of first- and second-year calves on trichostrongylid infections in Lithuania.

The objective of this study was to examine whether susceptible calves grazing together with second-year resistant heifers are less exposed to trichostrongylid infection than are calves grazing on their own. Two groups of animals representing each age category were turned out onto pasture on 24 May 1997 and grazed at comparable stocking rates. The grazing of calves and heifers together was compared to groups of each age category grazing separately. The results indicated that herbage larval counts were significantly reduced in the second part of the grazing season on the plot grazed by the mixed group compared to the plot grazed by the first-season calves only. The mixed grazing strategy protected the young calves and no clinical signs were observed in this group, while most of the calves that grazed alone exhibited clinical signs. The availability of herbage was reduced towards the end of the season, with subsequent competition for the grass forcing all the animals to graze the tufts around the faecal pats, where the quality of the grass is poor and the numbers of infective larvae are high. The effect of this was visible in the form of increased parasite burdens in the calves that were grazed together with the heifers, confirmed by increased blood serum pepsinogen concentrations and reduced daily weight gains in the second part of the grazing season. The lower numbers of infective larvae on the pasture were probably achieved through the heifers ingesting many of the larvae but subsequently depositing relatively few eggs, since they had acquired some degree of resistance against trichostrongylid infections during their first grazing season. Thus they did not suffer any parasitological ill-effects during mixed grazing with first-season calves.

Time trends in incidence of cervical cancer in Lithuania from 1983 to 1997.

This report examines time trends in both age-specific and age-standardized cervical cancer (ICD9-180) incidence rates among Lithuanian women. The study covers the period from 1983 to 1997. In case of the age-specific rates Poisson regression revealed the controversial results for the different age groups. The incidence rates increased (beta > 0) for the women below 50 years and decreased (beta < 0) for the women above 50 years. The average annual percentage changes were estimated using the age-drift model. In the age groups from 30 to 49 years old the incidence rate increased by 3.3% per year with the 95% confidence interval (CI) being 2.2-4.4. In the age groups from 50 to 69 years old the incidence rate decreased by 2.9% (95% CI: 1.9-3.8). In case of the age-standardized rates during the last few years some increase was observed, although no strict conclusions could be drawn. This observation together with the results of the age-specific rate studies predicts that in the near future the age-standardized rate will start growing.

Phosphate run-off in the Nevezis River (Lithuania).

The long-term (40 yr) observation results of phosphates-phosphorus concentration and its runoff in the Lithuanian fourth largest river Nevezis are analysed. Amounts and peculiarities of background runoff, agricultural runoff, and point source pollution from towns were studied for various periods. It has been determined that phosphates enter into the river Nevezis mainly from towns (76.5%), from agriculture only up to 16%, and the background runoff for 7.5%. Considerable agricultural influence was observed in 1979-1991, when the phosphate runoff increased to 22 kg P km(-2) a(-1) and formed in 1984 33% of the entire runoff. A relation of the phosphates runoff from agriculture, for the entire study period, was obtained with the number of animals in the river's basin. Since 1990, when the number of animals started to decrease, the runoff of phosphates decreased too. Now the phosphates runoff from agriculture formed only 2% of the entire runoff. Water in the river Nevezis is heavily polluted with phosphate, according to the EU general classification of the water quality of rivers, although the Lithuanian pollution norms for effluents discharging to surface water are not exceeded. Norms for effluents discharging in the rivers from point-sources pollution should be made stricter and the highest permitted ratio between the phosphorus load in the river and its water discharge should be established.
