Compliance and absenteeism in school children: implications for helminth control.

The validity of epidemiological surveys and the success of helminth control programmes based on the diagnosis of infection depend upon an accurate screening procedure. The success of all school-based control programmes, whether they involve prior diagnosis or not, depends on the level of school attendance and school enrollment. The degree to which compliance and school absenteeism may affect estimates of helminth infection and the coverage of treatment was investigated using empirical data from a survey conducted in 3 rural schools in Jamaica. Two sequential stool samples were requested from each child aged 9 to 12 years and screened for the presence and intensity of helminth infection using the Kato thick smear technique. Of the 696 children recruited into the study, 94% agreed to participate but only 90% of these children actually provided a stool sample for diagnosis and only 74% returned the second stool. Children infected with moderate to heavy loads of Trichuris trichiura were less likely to comply fully with the protocol than the uninfected children. They also took longer to comply and were absent from school more often than their uninfected counterparts. Increasing the sampling effort increased the compliance of all infected, and heavily infected, children. By giving them the option to comply, heavily infected children were under-represented by the sampling procedure. This has important implications for the design of control programmes and epidemiological surveys.

Orthopedic disorders, morbidity, and sick absenteeism in men with different levels of serum gamma-glutamyltransferase.

All orthopedic treatment administered between 1970 and 1982 was analyzed in random subsamples of men 28, 38, and 48 years old being screened for serum gamma-glutamyltransferase (GGT) levels. Of 1,151 screened participants, 631 men had been treated for fracture, distortion, contusion, low-back pain with or without sciatica, or radial epicondylitis. The extent of care and incidence of the different diagnoses over the 13 years, as well as the number of sick days incurred by these men during 1981, were investigated for different GGT brackets. Those men with the highest GGT values (greater than or equal to 1.34 mu kat/liter approximately 80 IU) had 4-6 times more fractures and consulted surgeons 6-13 times more often than those men with the lowest GGT values (less than 0.35 mu kat/liter approximately 20 IU). Epicondylitis and distortion showed the opposite frequency distribution, with the highest incidence occurring in the lowest GGT brackets. Increasing GGT values were also correlated with sick leave days. Men in the highest decile for sick leave had 25 times more fractures than those men with no sick absenteeism. We conclude that an accumulation of alcohol-related orthopedic disorders occurs among men with high GGT values.

Economic burden and absenteeism from influenza-like illness in healthy households with children (5-17 years) in the US.

BACKGROUND: School children (5-17 years) function as vectors in influenza epidemics. The objective of this study was to quantify the burden of influenza-like illness (ILI) on US healthy households with school-aged children. METHODS: The Medical Expenditure Panel Survey database of 1996-2002 was queried to compare annual total medical expenses and absenteeism incurred by otherwise healthy households with school-aged children that reported experiencing ILI to similar households that did not experience ILI. RESULTS: We identified 12,850 healthy households with at least one school-aged child. Households reporting one or more ILI episodes incurred $281.47 higher total annual medical expenses than those without an ILI. Employed members in households with ILI lost an average of 1.12 more workdays from their illness compared to those in the households without ILI. Furthermore, households with ILI in which all members were employed and medically insured lost an additional 0.89 workdays due to caring for other ill household members. School children in households with ILI missed 1.91 more school days annually and 4.87 more school days if the household had three or more students. The total annual additional cost to a household experiencing ILI, including medical expenses and productivity losses was $538.75 for fully employed and insured households and $424.83 if one or more members were unemployed or uninsured. CONCLUSIONS: ILI increase economic burden among households with school-aged children and lead to more school and workdays lost. Initiatives to vaccinate school-aged children should be explored.

[A morbidity information system based on school absenteeism].

We present a pilot experience about the introduction of a information system designed for the knowledge of infant morbidity (4-15 years) in a 13 school week period in a representative sample of 60 schools of the Valencian Community. We studied school absences that lasted three days or longer. Reports of teachers and parents were the source of information (100% response rate). Medical certification was also requested to the pediatricians and general practitioners of the area of the schools (12.74% response rate). To test the validity of this information system, absences caused by hepatitis were compared with the registry of Notifiable Diseases System (NDS) for the same period. We recorded 3,968 absences, with a cumulative incidence of 100.68 per 1,000 (C.I., alpha = .05, +/- 4.97) for females, and 119.4 (C.I., alpha = .05, +/- 18.36) for males. These differences were statistically significant. There was a concordance of 80.25% of the hepatitis cases between the registry of absenteeism and that of the NDS (chi 2 goodness of fit test = 3.16, p = .754). Although absences shorter than 3 days were not taken into account, the information system provided relevant and valid estimates of morbidity. Moreover, collaboration of teachers was considered very positive, which led us to use this health information system in five health areas of the Valencian Community.

Workplace absenteeism and alcohol use: a sequential analysis.

This investigation examined the day-to-day relationship between alcohol use and workplace absenteeism among a sample of participants (N = 280) employed in 1 of 3 large companies located in the northeastern U.S. With a semistructured interview, information was collected from employees about specific days of drinking during a 1-month period and marked on a calendar. Data about employees' absences during the same target time period were collected from the companies' human resource departments and were also marked on a calendar. A significant relationship was found between alcohol use and workplace absences; workers were roughly 2 times more likely to be absent from work the day after alcohol was consumed.

Objective stress factors, accidents, and absenteeism in transit operators: a theoretical framework and empirical evidence.

The authors used observational job analysis as a conceptually based technique to measure stress factors unbiased by worker appraisal with 81 transit driving tasks on 27 transit lines. Stressor dimensions included work barriers that interfere with task performance due to poor technical-organizational design, time pressure, time binding (autonomy over time management), and monotonous conditions. Line-specific average stressor values were assigned to 308 transit operators who mainly worked the particular line. Logistic regression analyses showed associations for high work barriers and sickness absences (odds ratio [OR] = 3.8, p = .05). There were elevated risks for work accidents for high time pressure operators (OR = 4.0, p = .04) and for the medium time-binding group (OR = 3.3, p = .04) and significant (alpha = .20) unadjusted interaction terms for barriers and time pressure in predicting accidents and absences, and barriers and time binding in predicting absences. Findings suggest guaranteed rest breaks and flexible timing for accident prevention and removal of work barriers for reducing absenteeism.

School absenteeism after upper gastrointestinal endoscopy in children.

BACKGROUND AND AIMS: To examine the functional impact of upper gastrointestinal endoscopy as a day procedure, particularly in relation to subsequent school attendance. METHODS: Symptoms and morbidities were prospectively recorded from school-aged children during observation in hospital and for 3 days at home after endoscopy by using a structured questionnaire. Reasons for school absence were identified. RESULTS: Sixty children (31 boys, 29 girls) were enrolled in the study (mean age 10.6 +/- 2.8 years, range 6.1-16.2 years). Following the procedure, symptoms were reported at home in 68.3% (same day), 56.7% (day 1) and 20% (day 2).The commonest symptoms were sore throat, tiredness and dizziness. Twenty-nine children (48.3%) did not attend school on the day following the procedure but most (26 of 29) had returned to school by the second day. The main reason for their absence was residual physical discomfort related to the procedure (55.2%). CONCLUSIONS: Persisting physical discomfort and school absenteeism are common following upper gastrointestinal endoscopy in children.

The relationship of HIV status, type of coagulation disorder, and school absenteeism to cognition, educational performance, mood, and behavior of boys with hemophilia.

Psychological and educational data were analyzed for all school-aged males with hemophilia at the Hemophilia Center of Central Pennsylvania (N = 66). Mean IQ (113.5) was higher than normal, and 2.4 times as many boys with hemophilia were enrolled in gifted programming than is the state average for boys. However, there was a disproportionately high prevalence of attention-deficit/hyperactivity disorder (ADHD; 28.3%), learning disability (LD; 15.8%), and graphomotor weakness. These were not significantly associated with HIV status or type and severity of coagulation disorder. School absenteeism was high but was not significantly related to academic achievement, IQ/achievement discrepancy, need for educational intervention, or diagnosis of ADHD or LD.

Work absenteeism in men who are labelled hypercholesterolaemic at screening.

Screening participants are frequently informed that they are hypercholesterolaemic. This study aimed at analysing whether such information induces a feeling of ill health, reflected in increased work absenteeism. A total of 1594 male construction workers aged from 20 to 59 years participated in an occupational health based screening programme. Men with plasma cholesterol greater than or equal to 6.9 mmol.l-1 (n = 184) were informed that they were hypercholesterolaemic and that they were at increased risk for heart attack. Comparison of health insurance data for the calendar year before and after screening, revealed that the relative risk for an hypercholesterolaemic subject to increase the annual number of sick-listing episodes was 1.01 (95% confidence interval: 0.93-1.10 compared to normocholesterolemic men. The annual number of days on sick leave carried a relative risk of increasing by 0.92 (95% confidence interval: 0.75-1.14). The results suggest that if patients know they are hypercholesterolaemia it does not necessarily mean that they perceive themselves as really ill.

Absenteeism and mortality of workers exposed to electromagnetic fields in the French Electricity Company.

The objective of this study was to describe the health status of electricity workers exposed to electromagnetic fields during their job. Two groups of exposed workers were studied from 1978 to 1993: the live line workers (n = 121) and the substation workers (n = 232.7) of the French Electricity Company (EDF). A control group was randomly selected from all the company non-management male employees; one control for each exposed subject was matched for the first year of employment. Absenteeism indices and mortality rates were computed and compared in the exposed and control groups. The absence rates were 1.98% in the substation workers and 2.5% in the control group (p < 0.001) and 2.7% in the live-line workers and 2.8% in the control group (NS). No effect of the length of exposure was found. However the medical causes of sickness absence were different: exposed employees had less psychiatric and respiratory diseases but more accidents at work than their control group. Relative risks of accidents at work were 1.2 95% confidence interval (CI) = 1.08-1.33[ for substation workers and 3.22 (CI = 1.78-5.88) for live line workers. EDF electromagnetic field exposed workers seemed not to be affected by any specific health problems except for an excess of accidents at work.

A preliminary investigation: effect of a corporate fitness program on absenteeism and health care cost.

A random sample of 517 employees was studied to determine differences in health care costs and absenteeism among exercisers and nonexercisers during the start-up of a corporate health and fitness program. Exercise was associated with decreased illness absence among female exercisers (47 v 69 hours, P less than .05) and there was a trend for illness absence to be inversely related to advancing age among exercisers, whereas illness absence increased among nonexercisers. Total health care costs among exercisers was lower (male $561, females $639) than among nonexercisers (male $1,003, females $1,535). Due to the large variation in the individual cost, the differences between exercisers and nonexercisers were not statistically significant. Ambulatory health care cost for nonexercisers (males $486, females $883) were significantly higher than the costs for exercisers (males $408, females $243). Because the differences were found upon program initiation, they were thought to be characteristics of exercisers and not due to exercise itself.

Predicting risk of back injuries, work absenteeism, and chronic disability. The shortcomings of preplacement screening.

Preplacement screening is put forth by some as an effective method for preventing low back disability at the workplace. Although there are still large gaps in knowledge regarding the natural history of and risk factors for back disability, it is clear that the majority of costs are derived from the small proportion of those that will suffer with back symptoms for 6 months or more. Thus, if preplacement screening is to have an impact on health care spending and compensation costs, the screening test will have to be predictive not so much of all back injuries, but rather of work absenteeism and especially chronic disability. This paper presents some criteria for evaluating a screening program and explores the effect of choosing different outcomes for evaluation. The literature on the effectiveness of a number of preplacement screening techniques is examined. Current legislation related to preplacement screening is also described. The natural history, etiologic factors, and prognostic markers for back injuries need to be elucidated further before effective screening strategies can be developed to reduce the incidence of back injuries and back disability in the population.

Absenteeism among employees who participated in a workplace influenza immunization program.

To assess the effectiveness of a free workplace immunization program at 3M's St. Paul, Minnesota locations, we examined the difference in sick leave hours taken from November 15, 1996, through March 15, 1997, for employees who had and did not have an influenza vaccination prior to the previous year's four-month influenza season (November 15, 1995-March 15, 1996). Among the 2,622 employees who self-reported that they were not immunized in the previous year, there were, on average, 1.2 fewer hours of sick leave taken during the 1996-1997 influenza season than the comparable time period one year earlier (P < 0.05), although the exact reason for the absenteeism was not determined. In particular, we observed that female employees younger than 50 years of age with two or more children took 3.1 hours less sick leave in the year they were immunized, compared with the preceding year (P < 0.0001). Among the 895 subjects who were immunized in both years, employees took 0.7 hours more sick leave during the 1996-1997 influenza season than the previous year (P = 0.46). Based on our findings, consideration should be given to workplace immunization programs. However, we urge caution in applying a "one-size fits all" approach to any cost-savings analysis from a company-sponsored immunization program because the workplace is not a homogeneous [corrected] environment, with regard to employees' age, gender, medical history, and home environment. All of these factors may directly or indirectly contribute to the risk of acquiring influenza and any of its complications.

Job stress and major coronary events: results from the Job Stress, Absenteeism and Coronary Heart Disease in Europe study.

AIMS: The intention of this study is to investigate the relationship of the demands/control/strain model with hard coronary events in an epidemiological, prospective, multicenter, European study. METHODS AND RESULTS: Six cohorts (Brussels, Ghent, Lille, Barcelona, Göteborg and Malmö) from four European countries (Belgium, France, Spain and Sweden) consisting of 21 111 middle-aged male subjects participated between 1993 and 1996 in the baseline survey of the Job Stress, Absenteeism and Coronary Heart Disease in Europe (JACE) study. The Karasek strain model of psychological demands (five items)/control (nine items) was used. During a mean follow-up of 40 months 185 acute coronary events or coronary deaths were observed. Age-adjusted hazard ratios (HRs) for developing an acute coronary event were 1.46 [CI 95% confidence interval (1.08-1.97)] for high against low psychological demands and 1.53 (95% CI 1.0-2.35) for strained (high demands plus low control) against relaxed (low demands plus high control) groups. After adjustment for standard cardiovascular risk factors the HR for developing a coronary event for those above or equal to the median against those below the median of psychological demands was 1.46 (95% CI 1.08-1.97) whereas the HR for strained against relaxed groups is 1.46 (95% CI 0.96-2.25). Sensitivity analyses confirmed the robustness of the results. CONCLUSION: In this European, multicenter, prospective, epidemiological study the Karasek job strain model was an independent predictor of acute coronary events, with the psychological demands scale emerging as the important component.

The impact of rheumatoid arthritis on medical expenditures, absenteeism, and short-term disability benefits.

OBJECTIVES: The objectives of this study were to estimate medical expenditures, absenteeism, and short-term disability costs for workers with rheumatoid arthritis (RA) and to estimate the relative costs of RA over a 12-month period. METHODS: Using data from nine U.S. employers, direct and indirect costs for 8502 workers with RA were compared with costs for a matched group without RA. Regression analyses controlled for factors that were different even after propensity score matching. RESULTS: Average total costs for workers with RA were $4244 (2003 dollars) greater than for workers without RA. RA was the fourth most costly chronic condition per employee compared with cancers, asthma, bipolar disorder, chronic obstructive pulmonary disease, depression, diabetes, heart disease, hypertension, low back disorders, and renal failure. CONCLUSIONS: RA is a costly disorder and merits consideration as interventions are considered to improve workers' health and productivity.

Is there a relationship between job satisfaction and absenteeism?

This study attempted to address the problem of stress and burnout in hospital nurses by exploring factors which affect job satisfaction and the relationship between these and absence rates. Fifty nurses working in the elderly care unit of a district general hospital were asked to complete a job satisfaction questionnaire, specifically developed to include factors appropriate to the study, together with Maslach's burnout inventory and Beck's hopelessness scale. The hypothesis that job satisfaction is related to absenteeism was not supported. However, significant correlations between some variables were isolated. These correlations were tested using Spearmans risk correlation co-efficient. It is hoped that the job satisfaction questionnaire will become a useful instrument in screening those at risk of burnout, and a diagnostic aid to help consider the factors which affect job satisfaction. The implications of the study point to the need for the prevention of stress from environmental factors by the adoption of organizational strategies to promote positive working conditions, and the development of coping strategies in the form of stress awareness seminars, regular staff support groups and counselling services.

What degree of work overload is likely to cause increased sickness absenteeism among nurses? Evidence from the RAFAELA patient classification system.

AIM: This paper reports a study examining whether nurses' work overload is associated with increased sick leave and quantifying the loss of working days from work overload. BACKGROUND: The RAFAELA patient classification system indicates nursing care intensity in relation to an optimum and is one of the few validated monitoring instruments of patient-associated workload among nurses. However, it is not clear whether work overload is a risk factor for increased sickness absenteeism, an important occupational problem in health care. METHOD: An observational cohort study was carried out with 877 nurses, 31 wards and five Finnish hospitals. Patient-associated workload scores from the RAFAELA system were based on a 6-month monitoring period in 2004. Records of 12-month self certified (1-3 days) and medically certified (>3 days) periods of sick leave in the same year were obtained from employers' registers. FINDINGS: The mean workload was 9% (sd = 8%) above the optimum. There was a linear trend between increasing workload and increasing sick leave (P < or = 0.006). Among nurses with workload > or =30% above the optimum the rate of self certified periods of sick leave was 1.44 (95% CI 1.13-1.83) times higher than among those with an optimum workload. The corresponding rate ratio for medically certified sick leave was 1.49 (1.10-2.03). These excess rates of sickness absence resulted in 12 extra sick leave days per person-year. CONCLUSION: Measuring nurses' workload may be an important part of strategic human resource management of nurses to reduce sick leave among nurses.

Research note: truants and other absentees.

Truants living in a depressed urban area are compared with Other Absentees from school living in the same area. The results show few differences in the social and financial circumstances of the two groups but considerable differences in the children's behaviour and in family relationships. The results are discussed in the light of other research which distinguishes truants from school refusers.
