Change in circulating blood volume following craniotomy.

OBJECT: The importance of monitoring circulating blood volume (CBV) during perioperative management is widely recognized in critically ill patients. The purpose of this study was to investigate the change in CBV following craniotomy by using indocyanine-green pulse spectrophotometry. METHODS: Circulating blood volume and plasma hormones related to stress and fluid regulation were measured five times: preoperatively, immediately postoperatively, and 1, 2, and 7 days after craniotomy was performed in 17 patients with a brain tumor or an unruptured aneurysm. The mean value of CBV preoperatively was 82 ml/kg, which decreased to 64 ml/kg (78%) immediately postoperatively and gradually recovered to 82 ml/kg on Day 7 postsurgery (p = 0.0069). The mean values of adrenaline, noradrenaline, arginine vasopressin, renin, and aldosterone were highest immediately postoperatively. The mean intraoperative balances of water and sodium were 1,090 ml and 113 mEq, respectively. Partial correlation coefficients of CBV to noradrenaline and serum sodium during the entire study were -0.430 (p = 0.0036) and 0.418 (p = 0.0048), respectively. CONCLUSIONS: Attention should be paid to decreased CBV following craniotomy, which is caused by the shift of fluid to interstitial spaces due to surgical stress. Hypovolemia can be suspected from a postoperative decrease in serum sodium.

Does central venous pressure reflect the circulating blood volume for the decrement of compliance just after esophagectomy?

This study investigates whether the pressure parameters obtained from the Swan-Ganz catheter (SGC) accurately reflect the circulating blood volume just after en bloc resection of the thoracic esophagus with regional lymph node dissection. It is well known that this operation induces severe hemodynamic changes and although the pressure parameters obtained from the SGC are an accepted means of monitoring circulating blood volume, we have often experienced a discrepancy between the SGC data and the clinical state. We examined the pressure parameters and diameter of the inferior vena cava (IVC) and left ventricle (LV), and the central venous compliance using SGC and echocardiography in ten patients who underwent esophagectomy for esophageal cancer. The central venous pressure, pulmonary arterial mean pressure, and pulmonary artery wedged pressure were significantly increased just after the operation compared with the preoperative levels, while the diameters of the IVC and LV decreased just after the operation. The compliances of the IVC decreased significantly just after the operation. The hemodynamic shift to the third space after esophagectomy induces decrement of the compliances of IVC. As the CVP does not always reflect the circulating blood volume, measuring the diameter of the IVC using echocardiography is extremely useful for monitoring circulating blood volume just after esophagectomy.

Urethral blood circulation during cardiopulmonary bypass.

The haemodynamic changes induced by extracorporeal circulation (ECC) are thought to be important in the induction of urethral strictures in open heart surgery when indwelling latex catheters are used. In the present study, 6 piglets were cannulated and connected to extracorporeal perfusion apparatus. Pump flows correlated with human ECC values with non-haemic prime were used. The mucosal and submucosal blood circulation in the urethra reduced by 66% during ECC (P less than 0.05). The brain and hepatic arterial flows increased. A significant reduction was seen in renal blood circulation. The changes in the urethral blood circulation during ECC correlated with previous findings. The reduced wash out levels of chemicals leaching from the indwelling latex catheters as a result of reduced local blood circulation are the main trigger for the induction of urethral strictures during ECC and in other shock-like circulatory disturbances in the human body.

[Metabolic syndrome and the pancreas. Status of blood circulation in the pancreas in metabolic syndrome in patients with different types of hyperlipoproteinemia ].

A study of blood circulation in the pancreas of 42 patients with the metabolic syndrome by the method of intracavitary rheography revealed a drop in the intensity of terminal blood circulation and presence of venous congestion with the activation of venous-arterial shunting. Hemodynamic disorders directly depended on the adiposity degree and patients' age, being evident as signs of a more obvious disorder of terminal blood circulation in patients with HLP of the II and III type.

[Clinical study of the treatment of endometriosis with promoting blood circulation and stasis removing method].

The treatment of endometriosis by blood circulation promoting and stasis removing method is based on the experience carried in the "Ji Yin Gang Mu" and the fact that this disease belongs to the category of pelvic stagnant blood. The study group consisted of 53 women with endometriosis. They were manifested as dysmenorrhea, menoxenia, ovarian chocolate cysts and enlarged uterus. The control group consisted of ten women with normal regular menstrual cycle. This article deals with the method of using the hemodynamic index of uterus arterial blood flow. After treatment the blood flow amount of uterus arteries of 53 cases (study group) obviously decreased and their uterus arterial blood flow speed reduced markedly as compared with pretreatment status, (P < 0.001 and P < 0.01) respectively, while pre-treatment group was significantly higher than that of control group (P < 0.001). After medical treatment for 3.5 months, symptoms such as dysmenorrhea and menstrual disorder basically disappeared. 22 ovarian chocloate cysts became smaller and 16 disappeared. The pregnancy rate was 45%. The data of this study suggest that the mechanism of treatment of promoting blood circulation to remove stasis might be closely related to the regulation of physico-chemical characteristics of blood and the adjustment of the function of hemodynamics.

[Development of water space volumes in calves during the first month of life. III. Changes in the plasma and circulating blood volumes].

The experiment was performed on the flock mentioned in part I. The plasma and circulating blood volumes have been determined using Evans Blue (T-1824). The plasma volume expressed in litres increased slowly but systematically during the examination period. A clear and statistically significant correlation between fluctuations in the absolute plasma amount and a calf age, its weight and body area has been observed. Calculated per kilogram and square metre of a calf body the plasma volume decreased significantly during the experimental period which fact found its statistical confirmation. The results correlated veritably and negatively with ages of the animals. The absolute volume of circulating blood in calves constitutions between 5th and 31st day of animals lives has increased but the differences found could not be confirmed statistically. A positive and veritable correlation has been observed between the obtained results and ages, weights and body areas of the examined calves. The circulating blood volume calculated both in terms of animals' weights and body areas during the first month of their lives decreased which fact has been statistically confirmed. The values as obtained in the experiment appeared negatively but veritably correlated with ages of the animals.

Blood cells and their role in regeneration. I. Changes in circulating blood cell counts during forelimb regeneration.

The possible role of leukocytes in regeneration was examined through the evaluation of quantitative changes in circulating blood cell counts during forelimb regeneration in adult newts. Leukocyte counts declined during the first 2 weeks, then returned to preamputational levels. Thrombocytes were also depressed (40-67%) throughout the progressive stages (9-30 days). In addition, lymphocytes were reduced, especially during the preblastemic phase (less than 15 days). In contrast, neutrophils were increased immediately following wound healing and during the differentiation stages, but were otherwise unchanged. These variations appear to reflect physiological changes occurring during regeneration and are consistent with a potential role for leukocytes in regeneration.

[Dependence of cardiovascular effects of adenosine on the blood circulation type in rats].

Administration of adenosine increased the RR interval in greater degree at hyper- than in hypodynamic blood circulation type. The AV-blockade and atrial flutter were maximal in rats with hyperdynamic type of blood circulation. Propranolol weakened the adenosine cardiotropic effect whereas cholinoblockators enhanced it. Neurohumoral regulation exerted no effect on the adenosine hypotensive action.
