Isolation and characterization of cell-specific cDNA clones from a subtractive library of the ocular ciliary body of a single normal human donor: transcription and synthesis of plasma proteins.

A subtractive cDNA library was developed for the purpose of identifying cell-specific genes expressed within the human ocular ciliary body, a tissue responsible for regulating aqueous humor secretion and intraocular pressure. Partial DNA sequence of a large number of cDNA clones and homology searches of nucleic acid and protein databases revealed significant homologies to at least 90 independently known genes. A group of biologically significant genes that were previously not known to have transcriptional expression in the ciliary body, complement component C4; alpha 2 macroglobulin; selenoprotein-P; and apolipoprotein D, were further demonstrated by Northern hybridization. Antibodies to these and other proteins (i.e., tyrosinase-related protein and pigment epithelium-derived factor) confirmed their cell-restricted expression in ciliary epithelial cells (pigmented, nonpigmented), or vascular endothelial cells. We provide evidence that two human plasma proteins, complement component C4 and alpha 2-macroglobulin are metabolically labeled with [35S]methionine in ciliary processes explants, suggesting that the ciliary body is an organ of synthesis and secretion of plasma proteins present in aqueous humor. These results challenge the notion that plasma proteins in aqueous humor are imported from outside of the eye. The subtractive cDNA library reported in this work should be very useful for identifying potential candidate genes in ocular abnormalities affecting the ciliary body, or involved in the regulation of intraocular pressure.

[Ciliary body melanoma in a black patient: a case report].

Uveal melanomas are rare in the black population. We report the case of an 81-year-old black man who presented a ciliary body melanoma of the left eye. Ophthalmological examination revealed a mass arising from the ciliary body and choroid. Diagnosis of ciliary body melanoma, made from clinical examination and ocular ultrasound, may be difficult in some cases due to its similarity to other eye tumors. Uveal melanomas mainly affect Caucasians but they are not unknown in blacks.

Photodynamic therapy (PDT) of the ciliary body with silicon naphthalocyanine (SINc) in rabbits.

BACKGROUND AND OBJECTIVE: To investigate silicone naphthalocyanine (SINc; 0.5 mg/kg) for photodynamic therapy (PDT) of the ciliary body in pigmented rabbits. STUDY DESIGN/MATERIALS AND METHODS: SINc was dissolved in canola oil by heating, emulsified with Tween-80, and given by ear vein. Pharmacokinetics were studied in frozen sections by fluorescence microscopy using a CCD camera-based, low light detection system with digital image processing at 1 hr and 24 hr (12 rabbits, 24 eyes total). A Ti:Sapphire laser delivered light at 770 nm by contact fiberoptic (1,000 microns; 80 mW/cm2;20,40 and 80 J/cm2). Controls (5 rabbits), received laser light at 770 nm without SINc. For comparison, eyes received continuous wave Nd:YAG laser by fiberoptic contact (0.8-1.2 J). RESULTS: Localization studies showed intravascular distribution shifting to a ciliary body distribution at 24 hr. PDT at 1 hr and 24 hr postinjection showed a more selective destruction of the ciliary body at 24 hr. Ciliary processes treated at 24 hr showed infarction and marked edema with sparing of iris. Tissue thermal damage was minimal in PDT controls. Eyes treated with the Nd:YAG laser exhibited full-thickness thermal necrosis of iris, ciliary processes, and a fibrinous iridocyclitis. In contrast, eyes treated by PDT were quiet with thrombosis of superficial blood vessels. CONCLUSION: Tissue photon penetration is good at 770 nm and thermal effects from the exciting laser alone were minimal. The ciliary processes of pigmented rabbits exhibit a selective retention of SINc and on that basis can be selectively destroyed with a minimum on thermal damage to nontarget tissues.

Endoscopic photocoagulation of the ciliary body after repeated failure of trans-scleral diode-laser cyclophotocoagulation.

PURPOSE: To report the successful control of intraocular pressure with endoscopic cyclophotocoagulation after repeated failure of trans-scleral diode-laser cyclophotocoagulation. DESIGN: Interventional case report. METHODS: A 3 1/2-year-old child had intraocular pressure around 30 mm Hg in one eye despite full medical treatment and three procedures of trans-scleral diode laser photocoagulation. RESULTS: Endoscopic view of the ciliary body revealed many misplaced laser burns in the pars plana region. Using direct endoscopic visualization of the ciliary body, precise, confluent burns were applied to the ciliary body. Six months after the procedure, intraocular pressure has remained under 20 mm Hg. CONCLUSION: Endoscopic cyclophotocoagulation can effectively lower intraocular pressure after recurrent external diode-laser photocoagulation has failed to do so. The direct visualization during the endoscopic procedure is particularly advantageous in cases with atypical ciliary body morphology, such as in pediatric glaucoma.

Comparison of the muscarinic cholinoceptors in the rabbit ciliary body and the guinea-pig ileum.

Interactions of several muscarinic drugs with their receptors were studied in the ciliary body smooth muscle of rabbits and the ileal longitudinal muscle of guinea-pigs, using pharmacological and biochemical procedures. The dissociation constants of carbachol, pilocarpine, atropine and pirenzepine estimated by these procedures indicate no heterogeneity of muscarinic receptors in either tissue; thus both are probably of the M2 type. However, the density of the receptors in the ciliary body is lower than in the ileum. Pilocarpine, with lower intrinsic efficacy, demonstrated a pronounced organ selectivity when compared to carbachol as it was a potent full agonist in the ileum and a competitive antagonist in the ciliary body. These results suggest the importance of both receptor density and threshold as determinants of agonist potency.

Phenol oxidase activity of the iris and the ciliary body in rats.

Phenol oxidase activity of the iris and the ciliary body in normal young rats of three differently pigmented strains, Lewis (albino), E3 (light brown, hooded), and BDE (black, hooded), was studied using DL-Dopa. Applicability of the histochemical method was controlled with skin samples taken from the necks of the animals. In the pigmented strains, the appearance of newly formed pigment at the pigmented epithelium and stroma of the iris and ciliary body was observed. In the E3 animals, the reaction was slightly positive, while in the BDE rats it was more pronounced. No phenol oxidase activity was detected in the ocular structures of the albinotic rats.

Proton irradiation of malignant melanoma of the ciliary body.

This is our first case of malignant melanoma of the ciliary body treated with proton beam irradiation, a technique that we developed for irradiating choroidal melanomas. After 21 months of follow-up no growth of the tumour has been observed, and shrinkage of the tumour was noted on the follow-up photographs and by ultrasonography. The 32P uptake test, which was positive before treatment, turned negative 14 months after irradiation. The described technique of proton beam irradiation might offer an alternative for the treatment of ciliary body melanomas when the present techniques of iridocyclectomy cannot be applied because of the size of the lesion.

Choroidal and ciliary body blood flow analysis: application of laser Doppler flowmetry in experimental animals.

Laser Doppler flowmetry (LDF) measures the flux of red blood cells in a vascular network embedded in tissue. In the present report, choroidal and ciliary body blood flow was measured continuously using an LDF technique in pentobarbital anesthetized cats. Blood flow measurements were made from the choroid beneath the lateral and superior rectus muscles and from the surface above the ciliary body 2-3 mm posterior to the limbus. In some experiments, the sympathetic nerve to the eye was stimulated; in others, intraocular pressure (IOP) was monitored manometrically with a controlled saline infusion delivered into the anterior chamber to gradually elevate IOP. Norepinephrine (NE) and methacholine (MC) also were tested for their effects on choroidal blood flow when given intravenously and intra-arterially. Transient bilateral carotid artery occlusions produced a significant drop in blood flow, but flow did not decrease to zero probably due to collateral contribution of the vertebral circulation. Sympathetic nerve stimulation produced a frequency-dependent decrease in both choroidal and ciliary body blood flow with the latter exhibiting a lower threshold and a greater magnitude of effect. Gradual elevation of IOP produced a proportional decrease in choroidal blood flow. The level of intraocular pressure required to decrease blood flow was dependent on the existing ocular perfusion pressure. Both intravenous and intra-arterially administered NE produced dose-dependent decreases of choroidal blood flow despite graded increases of systemic arterial blood pressure. In contrast, MC produced dose-dependent increases of choroidal blood flow in conjunction with graded decreases of systemic blood pressure. The results of the present report demonstrate the validity of the LDF technique for performing continuous reading of blood flow changes in the choroid and ciliary body of experimental animals.(ABSTRACT TRUNCATED AT 250 WORDS)

Digital infrared photography assists in the detection of iris and ciliary body cysts.

PURPOSE: To report a case that illustrates the utility of digital infrared photography in the detection of iris and ciliary body cysts in an asymptomatic patient. MATERIALS AND METHODS: Digital infrared photography of the iris was performed in both eyes of a 50-year-old white woman who presented with bilateral endothelial pigment dusting and subtle iris transillumination defects. RESULTS: Infrared photography revealed a pronounced, rounded patch of transillumination in the periphery of the left iris. Also observed were focal, rounded regions of reduced transmittance in the peripheral iris of both eyes. These findings suggested the presence of bilateral iris and ciliary body cysts. High-resolution ultrasound biomicroscopic images were consistent with this suspicion. CONCLUSIONS: Digital infrared photography of the iris may be useful in the detection and evaluation of iris and ciliary body cysts.

Purification and properties of anionic glutathione S-transferase from bovine ciliary body.

An anionic glutathione S-transferase was purified from bovine ciliary body by DEAE-agarose chromatography and affinity chromatography on GSH-agarose and Orange A. The enzyme accounts for about 25% of total soluble glutathione S-transferase activity of the tissue. The purified enzyme has a molecular mass of about 50,000 Da and is composed of two identical subunits of about 25,000 Da. The enzyme has a pI of 5.8. The enzyme conjugates GSH with 1-chloro-2,4-dinitrobenzene, p-nitrobenzyl chloride, 1,2-epoxy-3-(p-nitrophenyl)propane, 1,2-dinitrobenzene and 3,4-dinitrobenzoic acid. The Km values for 1-chloro-2,4-dinitrobenzene and GSH are 0.40 mM and 0.57 mM respectively. Haematin is a non-competitive inhibitor (Ki = 4.5 microM) when tested with various concentrations of 1-chloro-2,4-dinitrobenzene. The enzyme shows no glutathione peroxidase activity with either H2O2 or cumene peroxide as substrate. On the basis of substrate specificities, pI values, amino acid composition and peptide maps, it is concluded that the ciliary-body enzyme is probably identical with the anionic form of glutathione S-transferase from bovine lens and liver.

Ouabain-sensitive Na-K ATPase response in the rabbit iris-ciliary body after lensectomy-vitrectomy.

We studied ouabain-sensitive Na-K adenosine triphosphatase (ATPase) activities in the iris-ciliary body of rabbit eyes after lensectomy-vitrectomy. Changes in enzyme activities were quantitatively investigated in the plasma membrane of iris-ciliary body at 0 or 7 hours and at days 1, 3, 7, and 14. The specific activity of Na-K ATPase rose to significantly higher levels than the control value at 7 hours following surgery, but returned to the baseline value after 7 days. In addition, we evaluated enzyme activities after lensectomy-vitrectomy during which SF6 or silicone oil was injected. The specific activity of Na-K ATPase following the injection of SF6 or silicone oil was significantly higher than the control value at 7 hours and did not return to the normal value even after 14 days. Consensual reaction, demonstrated by increased Na-K ATPase activity, also was found in the contralateral unoperated eyes of SF6- and silicone oil-injected rabbits. The increased Na-K ATPase activity in the iris-ciliary body after experimental surgery may play an important role in restoring swollen tissues.

Adrenergic receptor subtypes in rabbit iris-ciliary body membranes: classification by radioligand studies.

Receptor subclasses in iris-ciliary body cell membranes were determined by direct binding of radioactive dihydroalprenolol (DHA), yohimbine (YOH), WB-4101 WB) and prazosin (PRZ), classified respectively as beta 1 + beta 2, alpha 2, alpha 1, and alpha 1 subtype selective ligands (based on binding to brain adrenergic receptors). Specific binding was defined with appropriate unlabelled agonist and antagonist drugs in each case. Binding data were analysed by library programs of the PROPHET computer system. Subclass specificity was also determined indirectly by binding competition of the labelled ligand (at a three- to 10-fold Kd concentration) with increasing concentrations of 'cold' agonist or antagonist. Ligand binding parameters of unlabelled drugs were obtained from Dixon and Scatchard plots of the data. The adrenergic receptor density of iris-ciliary body is approx. 600 fmol mg-1 protein, of which 20-25% are primarily beta 2 receptors. Three distinct subpopulations of alpha-receptors, representing 10, 25 and 40-45% of the total, bind PRZ, WB and YOH, respectively, each with high specificity for its corresponding ligand but with 10- to 1000-fold lower specificity for the other two ligands. The majority of alpha-adrenergic receptors are of the alpha 2-subtype. A small population of receptors are similar to vascular postsynaptic alpha 1-receptors, while a larger subpopulation may have characteristics intermediate between alpha 1- and alpha 2-subtypes.

Toll-like receptor 4 and CD14 expression in human ciliary body and TLR-4 in human iris endothelial cells.

We investigated the expression of the functional endotoxin receptor proteins Toll-like receptor-4 and CD14 in human eyes. Toll-like receptor-4 and CD14 proteins were detected by immunohistochemical analysis of sections of whole human eyes embedded in paraffin with monoclonal antibodies against human toll-like receptor-4 (HTA-125), human CD14 (RPA-M1), or as a control, an irrelevant mouse IgG1k (MOPC-21). Incubation of explants with a neutralizing anti-toll-like receptor-4 monoclonal antibody was used to determine if lipopolysaccharide stimulation of tumor necrosis factor or interleukin-6 secretion was dependent on Toll-like receptor-4 activity. Reverse transcription-polymerase chain reaction was used to detect mRNAs for toll-like receptor-4, tumor necrosis factor-alpha, interleukin-1beta, interleukin-6 and interleukin-8, 3 hr after stimulation of cultured iris microvascular endothelial cells. By immunohistochemistry, human ciliary body non-pigmented epithelial cells showed strong expression of the endotoxin receptor proteins, toll-like receptor-4 and CD14. Toll-like receptor-4 antibodies significantly inhibited lipopolysaccharide-stimulated tumor necrosis factor secretion by the ciliary body. Toll-like receptor-4 mRNA was constitutively expressed in iris endothelial cells and slightly down-regulated by endotoxin. mRNA levels for tumor necrosis factor-alpha, interleukin-1beta, interleukin-6 and interleukin-8 were all increased by endotoxin treatment. This is the first report that shows intraocular (ciliary body and iris) expression of toll-like receptor-4, other than in cornea. Our results show that the ciliary body also expresses CD14, which is anatomically colocalized with toll-like receptor-4. This suggests a potential interaction between both molecules during endotoxin activation of ciliary body cells. The juxtaposition of toll-like receptor-4 and CD14 in the anterior uveal tract helps to explain the sensitivity of the iris/ciliary body to bacterial endotoxin as seen in the standard animal model of endotoxin-induced uveitis.

Malignant nonteratoid medulloepithelioma of the ciliary body in an adult.

BACKGROUND: The authors describe a 62-year-old man with a malignant nonteratoid malignant medulloepithelioma of the ciliary body with extension into the anterior choroid and posterior chamber. He developed gradual loss of vision in his right eye, and a clinical diagnosis of malignant melanoma of the ciliary body and choroid was made. RESULTS: Histopathologic examination of the enucleated eye showed a malignant nonteratoid medulloepithelioma of the ciliary body with extension into the anterior choroid and posterior chamber. CONCLUSION: This report provides clinicopathologic evidence that intraocular medulloepithelioma can occur rarely in an adult. The lesion clinically may mimic a malignant melanoma. The authors also summarize five previously reported cases of this unusual lesion.

Ciliary body endophotocoagulation during pars plana vitrectomy for pediatric patients with vitreoretinal disorders and glaucoma.

PURPOSE: To investigate the efficacy of ciliary body endophotocoagulation during pars plana vitrectomy in eyes of pediatric patients with medically uncontrolled glaucoma. METHOD: We compared preoperative and postoperative intraocular pressure and visual acuity in five eyes of five patients aged between 10 months and 14.5 years who had 180 degrees of ciliary body endophotocoagulation with pars plana vitrectomy. RESULTS: Preoperative intraocular pressure on medical therapy for glaucoma ranged from 30 to 55 mm Hg (median, 35 mm Hg). Postoperative follow-up ranged from 12 to 24 months, with a median of 16 months. Postoperative intraocular pressure on medical therapy ranged from 12 to 35 mm Hg (median, 25 mm Hg) at 6 weeks, from 12 to 33 mm Hg (median, 29 mm Hg) at 6 months, and from 12 to 29 mm Hg (median, 27 mm Hg) at 12 months. Twelve months after surgery, mean intraocular pressure reduction of 20 mm Hg was statistically significant (P = .020). CONCLUSION: Ciliary body endophotocoagulation during pars plana vitrectomy may be an effective treatment for pediatric patients with simultaneous uncontrolled glaucoma and vitreoretinal disorders.

Ciliary body thickness increases with increasing axial myopia.

PURPOSE: To assess the relationship between ciliary body thickness and refractive error. DESIGN: Prospective observational case series. METHODS: We enrolled 75 eyes of 75 patients with no history of intraocular surgery or use of anti-glaucoma medication. Axial length was measured by A-scan ultrasonography. Ciliary body thickness (CBT) was measured by two masked observers on radial-section images of the temporal corneoscleral limbus, acquired with ultrasound biomicroscopy (UBM) at 2 and 3 mm posterior to the scleral spur (CBT2 and CBT3, respectively). RESULTS: Refractive error was inversely correlated with CBT2 and CBT3 (r = -.64 and r = -.52, P < .0001, respectively). Axial length was correlated with CBT (r = .66 and r = .57, P < .0001, for CBT2 and CBT3, respectively). CONCLUSION: CBT increases with increasing myopia and axial length.
