[Evaluation of the socio-economic effect of using immune anti- meningococcal plasma in pediatric practice].

Analysis of the socioeconomic effectiveness of the use of immune antimeningococcal plasma in patients with the grave patterns of meningococcal infection has shown that introduction of the preparation into multimodality therapy results not only in a considerable medicosocial effect (reduction of lethality, lowering of the patients' stay at a hospital, a more rapid, as compared with the control group, removal of the symptoms of intoxication, elimination of hemodynamic disorders underlying the infectious toxic shock) but also in an appreciable economic effect.

[Effect of Archman health resort factors on the bile chemistry of chronic cholecystitis patients].

During remission, patients (n-83) with chronic cholecystitis showed the presence of lithogenic bile, a disorder which was eliminated under the action of Archman (the Turkmenian SSR) health resort factors. The efficacy of health resort treatment manifested itself in the improvement of the patients' clinical status, normalization of the kinetics of the gallbladder and bile chemistry. The choleretic and cholelytic properties of Archman mineral water permit recommending its use both for the purpose of correcting bile lithogenicity and prevention of the lithogenesis.

[Status of nitrogen balance and parenteral feeding in peritonitis].

Under study were parameters of protein metabolism and state of nitrogen balance in 83 patients with peritonitis, in 29 of them (with diffuse peritonitis) partial parenteral nutrition was performed. It reduced the degree of protein catabolism, facilitated elimination of metabolic disorders. Reinforcement of the ration of the caloric-nitrogen nutrition in order to completely fill up the energetic and plastic expenditures of the organism at the early postoperative period will entail danger of superflous transfusion load.

[Artefact--diagnosis and therapy from the surgical viewpoint].

Factitious wounds are often overlooked when patients are treated. The experiences we have had with ten patients (nine women and one man) are discussed. A typical case report of a 19 year old female, who had been hospitalized 19 times, demonstrates the magnitude of the diagnostic and therapeutic problems involved in such cases. If the basic psychological disorder cannot be eliminated, medical treatment will remain unsuccessful.

Classification of chronic venous disorders of lower extremities and pelvis.

Variability of hemodynamic situations in different forms and stages of chronic venous disorders of lower extremities and the pelvis as well as the mushroom growth of surgical techniques used in their treatment make the author consider the correct choice of individual treatment to be the main problem. In the first place such treatment should be directed towards adequate correction of the deep venous circulation disorders providing for elimination of the main pathogenetic factors (e.g. occlusion of the main veins, pathologic impact of the gravitational column, blood regurgitation at standing and its flotation in ambulation, arterio-venous run-off, etc.). The author has revealed a classification based on systematization of most frequently observed pathology, that allows a laconic functional and topical diagnosis and provides phlebological patients with individualized treatment.

[Thrombohemorrhagic syndrome in the clinical aspects of food poisoning].

The system of hemostasis was subjected to a comprehensive study in 116 adult patients with food toxoinfections of varying severity. The moderate and severe forms were primarily marked by the signs of hypercoagulation and high aggregation of formed elements of the blood. The symptoms of hypocoagulation dominated in extremely severe disease patterns. Intravenous infusion of crystalloid solutions (Kvartasol and Trisol) in an amount of 30-100 ml/kg with regard to the degree of dehydration promoted the elimination of the disorders cited.

[Basic theory of nervous system disorders: generative, determinant and systemic mechanisms].

The paper deals with the virtually unknown and little studied aspect of the pathogenesis of nervous disorders, which consists in emergence of new pathological integrations in the central nervous system (CNS). The formation and activity of these integrations are realized by endogenous mechanisms intrinsic to the damaged nervous system. The pathological integration of hyperactive neurons as their aggregate is a generator of pathologically enhanced excitation (GPEE). GPEE is capable of producing intense and uncontrolled impulse flow and developing in certain conditions self-sustained and even increased activity. The CNS region where GPEE is formed, becomes hyperactive, alters the other, linked with it, CNS structures and induces the formation of systemic pathologic integration in the form of a new pathodynamic organization, i.e. pathologic system (PS). This system-forming CNS region plays a role of a key, determinant part of PS. The main biological feature, which makes PS different from physiological systems, it is inadaptive or direct pathogenic significance for the organism. The neuropathologic syndrome is a clinical manifestation of PS activity. Each neuropathologic syndrome has an appropriate PS, and its specificity and content depend on which CNS structures constitute PS. Due to plasticity processes, the positive links between the PS part become consolidated with time and as a result the PS resistance to endogenous sanogenetic mechanisms and curative therapeutic treatment increases. In creating GPEE in a certain, pathogenetically important CNS region, one can reproduce the corresponding neuropathologic syndrome in modelling experiments. The basic principle in the treatment of nervous disorders consists in elimination of their pathologic systems.(ABSTRACT TRUNCATED AT 250 WORDS)

Depression and its treatment in cardiac patients.

In general medical-surgical hospital services, depression is the most common reason for seeking psychiatric consultation in behalf of patients with cardiovascular disease. The nontreatment of depression or the use of a psychotropic agent mismatched to a patient's particular cardiac condition or individual sensitivities has considerable negative impact. Therefore, a systematic approach should be used in the differential diagnosis of depression in cardiac patients, to eliminate other psychiatric disorders and to assure the correct treatment strategy. Physicians can develop an appropriate rationale for the use of a psychopharmacologic regimen in cardiovascular settings through recognition of the diagnostic criteria for depression and through comprehensive knowledge of the pharmacologic properties and possible cardiovascular effects of these vital treatments. Standard and alternative pharmacotherapies for depression in cardiac patients are reviewed.

Human requirements of iodine & safe use of iodised salt.

Iodine deficiency is the most common preventable cause of mental deficiency. Remarkable success has been achieved by the use of iodised salt to correct this deficiency in many industrialised countries since 1920. The Government of India has adopted a strategy to iodise all edible salt in the country to overcome iodine deficiency. Universal salt iodisation is the principal public health measure for eliminating iodine deficiency disorders. Daily iodine intakes of up to 1000 micrograms, appear to be entirely safe. In India, the likelihood of exceeding this level is quite small. Iodised salt does not cause any side effects. Iodine in iodised salt does not carry risks for persons who are already iodine sufficient. iodisation of salt at the current level of fortification (15-30 ppm iodine) keeps intakes well within a safe daily range for all populations, irrespective of their iodine status.

Prevalent diseases of ostrich chicks farmed in Canada.

In Canada, ostriches are now slaughtered for their meat and hides. The mortality rate in ostrich farming is highest in chick units. An increased chick survival rate impacts positively on production and profit. This paper will focus on common health disorders that affect chick production costs. These are discussed under the following categories: digestive, orthopedic, respiratory, and integumentary disorders. Methods for elimination or reduction of these mortality factors are also discussed.

Progress in salt iodization and improved iodine nutrition in China, 1995--99.

In 1993, the State Council of China announced the policy to virtually eliminate iodine-deficiency disorders (IDD) by 2000 and adopted universal salt iodization (USI) as the national strategy. Biennial province-based monitoring from 1995 onward aimed at capturing the use and iodine content of household salt, along with urinary iodine concentrations among schoolchildren from the same households. This paper reports on the progress made in China toward the goal of virtually eliminating iodine-deficiency disorders on the basis of 85 population-representative surveys in China's provinces during 1995--99. The percentage of households using adequately iodized salt (iodine > or = 20 mg/kg) increased from 43.1 % in 1995, to 82.2% in 1997, to 89.0% in 1999. In 1999, at least 90% of the households in 15 (48%) of the 31 provinces used adequately iodized salt, and a median urinary iodine concentration of less than 100 microg/L in children was reported in only one province. Across provinces, the median urinary iodine concentrations in children were positively correlated in each survey year with the median household salt iodine contents (combined r(s) = 0.74, p < .001) and with the proportions of households using adequately iodized salt (combined r(s) = 0.81, p <.001). Also in each survey year, the percentage of children with urinary iodine concentrations of at least 300 microg/L was correlated (combined r(s) = 0.69, p < .001) with the proportion of households using salt with iodine content of at least 40 mg/kg. The median urinary iodine concentration in children had reached 300 microg/L or more in 13 provinces (42%) by 1999. In a little more than five years, then, China has achieved outstanding progress toward the goal of virtual elimination of IDD through USI. Policy recommendations include improvement of quality assurance by salt manufacturers, along with a modest reduction in the mandated salt iodization levels.

[Arachidonic acid in the liver, ovary and uterus of cows with various reproductive capability].

Research was carried out at two localities ecologically different, i.e. in the "relatively" clean region of Klatovy, designed according to the standards of the Czech Ministry of Environment as a region with above-average quality environment and in the considerably affected region of Prague-East, designed as a region with below-average quality environment. The first group included 33 cows from herds with good fertility (average conception rate after 1st insemination 61.5 and service period 88 days) and the dairy cows were eliminated for reasons different as reproductive disorders (Klatovy), in the second group all 18 cows were eliminated because of reproductive disorders (Prague-East) and they were from the herds with much lower fertility (average conception rate after 1st insemination 46.9% and service period 107 days). Samples of liver, uterus and ovarian tissues of the cows were processed according to the method after Jarý et al. (1987), analysed on the gas chromatograph Varian--Aerogramm 2100. The data obtained were processed on a computer IBM PC--AT with a numeric co-processor, and the programme Quatro Pro, version 2.0 was used. Differences in average amounts measured in both regions are objectively characterized in Figs. 1, 2 (with marked scatterings) and correspond to the amount given in Tab. I. As regards the saturated acid, stearic acid and oleic acid, no remarkable differences were found according to the localities (Klatovy and Prague), with the exception of the average contents of those acids in uterine horns. In that case the region of Klatovy is dominating.(ABSTRACT TRUNCATED AT 250 WORDS)

[Acquired reactive perforating dermatosis. Successful treatment with allopurinol in 2 cases].

Perforating disorders represent a heterogenous group of dermatoses characterized by transepithelial elimination of dermal structures. Primary perforating disorders should be distinguished from secondary perforating disorders in which perforation with transepithelial elimination is a rare component of a variety of dermatoses. The primary perforating disorders are hyperkeratosis follicularis et parafollicularis in cutem penetrans (Kyrle's disease), elastosis perforans serpiginosa and perforating folliculitis. Acquired reactive perforating dermatosis (also known as acquired reactive perforating collagenosis) together with the hereditary variant of the reactive perforating collagenosis represent further examples of the primary perforating disorders. We report on 84 year old and 96 year old female patients with an acquired perforating dermatosis. Both of the patients additionally showed diabetes and hyperuricemia. Oral administration of allopurinol (100 mg daily) led to a healing of the disseminated skin lesions in 1-2 weeks. After a follow-up period of 6 months, both patients were in complete remission. On one hand, these results prove again the existence and the severity of this disease, and on the other hand suggest an immunomodulating or differentiation-promoting action in addition to the uricostatic effect of allopurinol.

The unwitting sacrifice problem.

The diagnosis of bipolar disorder has been linked to giftedness of various sorts and this raises a special problem in that it is likely that the condition has a genetic basis. Therefore it seems possible that in the near future we will be able to detect and eliminate the gene predisposing to the disorder. This may mean, however, that, as a society, we lose the associated gifts. We might then face a difficult decision either way in that it is unclear that we are preventing an unalloyed bad when we diagnose and eliminate bipolar disorder through prenatal genetic testing and yet if we allow the individual to be born we are condemning that person to being an unwitting sacrifice in that they might well suffer considerable net distress as a result of our need to keep our gene pool enriched in the relevant way.

[Screening of patients referred to our clinic for odontogenic focal diseases].

During 1997 and 1998 261 patients suffering from suspected focal diseases were investigated for the presence of persisting chronic inflammatory disorders in the oral cavity. In 83% of the patients pulp necrosis, chronic inflammation of the apical area and the periodontal tissues could be revelated. The most commonly diagnosed disorders were pulp necrosis, chronic apical periodontitis and periodontitis complicata respectively. Most frequently these diseases could be detected in chronic cardiac disorders, uveitis and dermatological diseases. In the future follow-up studies parallel with the elimination of the dental disorders monitoring of blood and serum parameters will be conducted in order to obtain data of th systemic effect of the persisting chronic dental diseases.

DSM-IV field trials for the disruptive behavior disorders: symptom utility estimates.

OBJECTIVE: We tested the predictive utility of symptoms for proposed DSM-IV definitions of the disruptive behavior disorders using indices corrected for symptom and diagnosis base rates. METHOD: The field trials sample consisted of 440 clinic-referred youths who were consecutive referrals to a heterogeneous group of mental health clinics. Multiple informants were interviewed to determine the presence of symptoms and diagnoses. RESULTS: Some symptoms which were either not in DSM-III or DSM-III-R, or were modifications of DSM-III-R symptoms, had greater diagnostic efficiency than did several existing symptoms. Symptom utility estimates were generally similar for different ages and genders, although some interesting age and sex trends emerged for a few symptoms. CONCLUSIONS: The results supported the inclusion of more restricted definitions of "lying" and "truancy" to increase their association with a conduct disorder diagnosis and they supported the elimination of "swearing" in the oppositional defiant disorder criteria. In addition to their relevance for developing optimal criteria for DSM-IV, these results can aid DSM-IV users by providing a useful guide to the relative efficiency of individual symptoms based on data from a large heterogeneous clinic population.
