Chondrosarcoma of bone: a statistical analysis of prognostic factors.

From 1980 to 1988, 30 cases of chondrosarcoma of the bone were treated surgically. Twenty-seven cases were followed from 6 months to 8.7 years (mean, 3.2 years). In all cases, a clinicopathological review was performed and the significance of prognostic factors were analyzed. The five-year survival of total patients was 88%. The survival was related to adequacy of treatment and histologic grade of the tumor. The five-year survival of adequately treated patients was 100%, but in inadequately treated patients it was 68%. At 8.7 years, 100% of the patients with grade-1 or grade-2 tumors survived, compared to 50% of the six-month survival rate in patients with the grade-3 tumors. None of the grade-1 or grade-2 chondrosarcomas metastasized, while 75% of grade-3 chondrosarcomas metastasized. The incidence of local recurrence was influenced by adequacy of treatment. At the final follow-up, the recurrence rate in the adequately treated group was 7%, while in the inadequately treated group it was 57%.

[Statistical analysis of risk factors for hypertension in obese children].

The aim of this work is the recognition, if any, of connection of arterial hypertension and some risk factors in obese children. In the schools of two little towns of our Region 114 subjects were selected, 57 males and 57 females, aged between 6, 5 and 15 years, with a body weight superior to that corresponding to the 95th percentils in the same age class. The blood pressure values were related to age and height and it was decided to consider hypertese those children whose pressure values corresponded to 95th and 97, 5th, respectively in the same class of age and height. The number of hypertese children in our group demonstrate that diastolic pressure in the height scale results especially increased among the age group 13-15 years in connection to size (females 75%, males 41, 7%). The connection between family diabetes and family hypertension have been shown significant. It can be pointed out that the risk factors for hypertension may be: age over 13 years, feminine, sex, family diabetes, family hypertension.

A statistical analysis of predictive factors of response to human lymphoblastoid interferon in patients with chronic hepatitis C.

OBJECTIVE: To determine markers predictive of effective interferon treatment for patients with chronic hepatitis C virus (HCV) infection, we studied 80 Japanese patients treated for 6 months with natural interferon-alpha. METHODS: Serum samples were tested for HCV RNA by two-stage polymerase chain reaction (PCR). HCV RNA were grouped into four genotypes by amplification of core-gene sequences by PCR with type-specific primers, and the level of HCV RNA was measured by competitive PCR. HCV RNA was detected in all patients before the interferon treatment. For the purposes of this study, a complete response was defined as the elimination of HCV RNA for at least 6 months after termination of the treatment. RESULTS: HCV RNA was eliminated from the sera of 27 patients (33.8%) at the termination of the interferon treatment, and the elimination was sustained throughout a 6-month follow-up (complete response). Four of eleven variables proved to be associated with a complete response when assessed by univariate analysis. With multiple logistic regression analysis assessment, however, only two variables (HCV RNA level (p < 0.001) and genotype (p = 0.048)) were significant. CONCLUSION: These results suggest that factors associated with the HCV infection are more important than patient characteristics for effective interferon treatment of patients with chronic HCV infection and that a low level of HCV RNA and HCV of genotype III are useful predictors of a complete response.

Modified Blalock-Taussig shunt: statistical analysis of potential factors influencing shunt outcome.

From January 1986 to December 1991, 100 modified Blalock-Taussig Shunts (MBTS) using PTFE (polytetrafluoroethylene) grafts were performed on 86 patients with complex cyanotic congenital cardiac malformations at this hospital. The ages of patients ranged from 15 days to 22 years. Thirty-seven were operated upon in infancy and one in the neonatal period. Varying sizes of grafts were used: 3 mm in 3 cases, 4 mm in 18, 5 mm in 44, 6 mm in 34 and 8 mm in one. The mean follow-up period was 13.5 months (ranged from one to 50 months). There were one hospital mortality and one morbidity. One year shunt patency rate was 90%. Univariate analysis showed the age of the patient operated upon and graft size were risk factors for shunt patency. With an encouraging 90% early shunt patency rate, we can now adopt MBTS as an alternative without hesitancy. Yet, the surgical technique and perioperative care of those cyanotic neonates receiving this palliation still remain a challenge.

[Applying matching matrix, factor analysis and clustering analysis to investigation of characteristics of syndromes of phlegm and blood stasis in patients with coronary heart disease].

OBJECTIVE: To investigate the characteristics of syndromes of phlegm and blood stasis in patients with coronary heart disease by multiple statistical methods of matching matrix, factor analysis and clustering analysis, and to provide some references for classification and normalization of diagnosis of syndromes of phlegm and blood stasis of coronary heart disease. METHODS: The correlations among 46 kinds of symptoms in syndrome of non-phlegm and non-blood stasis, syndrome of blood stasis, syndrome of phlegm and syndrome of phlegm-blood stasis blocking in 200 patients with coronary heart disease were analyzed by matching matrix, factor analysis and clustering analysis. RESULTS: The manifestations of tongue and pulse in syndromes of phlegm and blood stasis were significantly different from those in syndrome of non-phlegm and non-blood stasis. The pathogenesis of viscera in syndromes of phlegm and blood stasis lied in the heart and kidney, and the syndrome of deficiency of heart qi was the most common one while the syndrome of deficiency of kidney qi took the secondary place. The syndrome of phlegm was often accompanied by syndrome of deficiency of spleen qi. Only 15 ones of 46 clinical symptoms showed high frequency in concomitant appearance in syndrome of blood stasis, syndrome of phlegm and syndrome of phlegm-blood stasis blocking. Apart from having the common symptoms in syndrome of deficiency in origin, the syndrome of phlegm especially showed white and greasy fur and slippery pulse as well as distention and fullness of chest and abdominal distension; the syndrome of blood stasis showed purplish tongue and ecchymosis on tongue as well as fixed pain; and the syndrome of phlegm-blood stasis blocking showed the main symptoms of both syndrome of phlegm and syndrome of blood stasis. CONCLUSION: The statistical methods of matching matrix, factor analysis and clustering analysis are convenient, and can definitely indicate the clinical characteristics and syndrome differentiation of viscera of different syndromes of phlegm and blood stasis, which are beneficial to further research of diagnosis and differentiation of such syndromes of coronary heart disease.

Selection of biomarkers by a multivariate statistical processing of composite metabonomic data sets using multiple factor analysis.

We introduce a statistical approach for integrating data from several analytical platforms. We illustrate this approach using (1)H-(13)C Heteronuclear Multiple Bond Connectivity nuclear magnetic resonance spectroscopy ((1)H-(13)C HMBC NMR) and Pyrolysis Metastable Atom Bombardment Time-of-Flight mass spectrometry (Py-MAB-TOF-MS) to perform metabolic fingerprinting on cattle treated with anabolic steroids. Multiple factor analysis (MFA) integrates complementary aspects from NMR and MS data into a unique metabolic signature describing the biomarkers related to the dose-response. This work also indicates that, from a practical point of view, metabonomics and other "-omics" biotechnologies can benefit significantly from a generalized multi-platform integrative approach using multiple factor analysis.

Morphological abstraction of thyroid tumor cell nuclei using morphometry with factor analysis.

Various morphonuclear studies by digital image analysis have successfully been applied to quantify the nuclear morphology, including chromatin distribution pattern, in cytology of various organs; however, the majority of past reports have not shown correlation between the quantitative data by digital image analysis and cytological findings in practical diagnosis. In this report, we present the usefulness of morphological abstraction to combine the objective data and subjective observation in cytological diagnosis. Randomly selected, 100 cells in each Papanicolaou-stained ABC smear samples of 39 benign and malignant thyroid tumor cases were studied. Gray-level image data provided seven parameters for nuclear size, four parameters for nuclear shape, and 16 parameters showing the nuclear chromatin patterns from high-dimensional texture analysis of using co-occurrence and run-length matrices. To statistically abstract nuclear morphology, factor analysis was used. Factor analysis classified morphological nuclear characters as abstraction parameter into five abstract parameters composed of nuclear size, shape, heterogeneity, and contrast and homogeneity of chromatin pattern. The nuclei of papillary carcinoma showed larger size, more irregular shape, and higher contrast of chromatin pattern than those of the benign group. The follicular carcinomas have larger nucleus in each cell and more monotonous chromatin pattern among cells in each case than those of the benign group. Morphological abstraction by morphometry with factor analysis may provide a practical approach to the detection of the underlying characteristics of nuclear morphology in aspiration biopsy cytology.
