OSCILLO-oscillating air mattress in neonatal care of very preterm babies.

In this pilot study a new device for respiratory stimulation with undulating oscillations and high frequency vibrations via an air mattress was tested as an additional therapy in 12 preterm infants with recurrent apnoeic episodes. The mean birthweight of the study population was 1760 g and gestational age 31.1 weeks. it was shown that an oscillating air mattress is an effective additional mode of treatment for apnoeic episodes in preterm infants. There was a clear reduction in rate of apnoeas in all 12 infants being transitory in 2 and more persistent in 10 infants. Besides restlessness in a few cases no side effects were observed. Whether this mode of treatment could lessen the need for drug medication with xanthines will require a larger randomized investigation.

Pressure reduction with a hospitalized population using a mattress overlay.

Billions of dollars are spent each year on treating pressure ulcers. With healthcare costs climbing and reform the order of the day, it is essential for researchers to identify a device which reduces pressure, is easy to use and is cost effective. This study used a Mini-Tipe pressure sensor to measure pressure readings over the sacral and trochanter areas of 17 subjects identified as being at risk for skin breakdown. Pressures were compared on a standard hospital mattress and an anatomically contoured mattress overlay. There was a 48 percent reduction in mean pressures over the sacral area and a 23 percent reduction over the trochanter. No correlations between pressures and demographic data were identified. Further research is warranted to determine the effects of variables found in an "at risk" population on pressure reduction with various products.

Pressure-reducing capability of Conforma II mattress overlay.

Pressure readings over the sacral and trochanter areas were obtained on 30 healthy adult volunteers. An anatomically contoured mattress overlay (Conforma II) was compared to a standard hospital mattress. The difference in the mean pressures over the sacral areas represented a 52% reduction in pressure, whereas, a 46% pressure reduction was obtained over the trochanter area. Body weight was significantly associated with higher mean pressures.

Distribution and density of house dust mites Dermatophagoides spp. (Acarina:Pyroglypidae) in the mattresses of two areas of Rome, Italy.

The distribution of Dermatophagoides farinae and D. pteronyssinus in two groups of houses in Rome was investigated by means of weekly sampling of dust on mattresses during the period of high mite density (May-July, 1994). All mattresses were infested; overall 4,179 live and dead mites were collected. In the two examined areas, a difference in species distribution was observed. D farinae was always the prevalent species, it was 94.7% in area A and 64.1% in area B; only in houses 9 and 10 D. pteronyssinus was the most abundant species (71.4% and 45.8%). Housekeeping practices were investigated by interviewing the tenants and relative humidily and indoor/outdoor temperature were measured during each weekly sampling. A correlation analysis revealed a significant relationship between housekeeping practices and mite density (r = 0.83). The poorest housekeeping practices produced the heaviest mite infestation, while good ones were associated with low densities.

Nimbus 3 alternating-pressure replacement mattress.

Alternating-pressure surfaces have been shown to reduce the incidence of pressure sores compared with standard hospital mattresses and pressure-reducing (constant low-pressure) surfaces. Huntleigh Healthcare has recently introduced the Nimbus 3 to its range of alternating-pressure mattress replacement systems. This product has been developed according to new medical device regulations and is indicated for the treatment of patients with all grades of pressure sores and for prevention in patients who are at very high risk of developing pressure sores.

Evaluation of the Porta-Warm mattress as a source of heat for neonatal transport.

One risk of transport of newborns to referral centers is hypothermia. Modern transport incubators have limitations in their ability to keep infants euthermic. As tested in the laboratory and during actual transport the Porta-Warm mattress extends the capability of the transport incubator to keep neonates euthermic by (1) reducing the time required to warm the incubator and (2) warming the incubator in cooler environements. The mattress is an effective adjunct to the transport incubator for keeping newborns warm during transport.

Vertical mattress suture for strabismus resection operation.

The suturing technique for strabismus resection has been simplified with the use of a single double-armed suture in the form of a vertical mattress. Synthetic ophthalmic suture material has made this technique still more desirable because of the tensile strength and tissue-gripping characteristics of these materials.

Guanine as a hygienic index for allergologically relevant mite infestations in mattress dust.

Since guanine is not only an essential constituent of vital nucleic acids, but also the main end product of nitrogenous waste excretion in arachnids, it is a potential candidate for a hygienic index for mite activity in house dust. The public health significance of these mites is based on their production of allergen in the home environment. In 101 mattress-dust samples, total number of mites and guanine content were positively correlated. The hygienic standard of 10 mites per 0.1 g dust (as derived from Danish investigations) was met in all samples with less than 0.06% guanine in the dry dust. Where the guanine content was more than or equal to 0.25% in the dry dust, mite numbers were higher than 10 mites per 0.1 g dust in 43 of the 44 samples. The three different groups (negligible mite content, possibly allergologically relevant mite concentration and allergologically unhygienic mite situation) could be easily discriminated on the color scale. The guanine-AZO-dye reaction in vacuum cleaner dust is a strong and promising candidate for an allergologically relevant hygienic index in the home.

Effect of heat treatments on the house-dust mites Dermatophagoides pteronyssinus and D. farinae (Acari: Pyroglyphidae) in a mattress-like polyurethane foam block.

The behaviour and survival of the house-dust mites Dermatophagoides pteronyssinus and D. farinae were studied under laboratory conditions in a mattress-like polyurethane foam block that was heated by an electric blanket. Mites obtained from a laboratory culture were introduced into the foam block which was sliced into seven horizontal layers and covered with an electric blanket. Measurements of temperature and relative humidity were taken between the layers and the number of living mites was established using the heat-escape method.

Air-mattress motion therapy for respiratory failure after cardiothoracic surgery.

PURPOSE: The standard management of respiratory failure after cardiothoracic surgery involves prolonged mechanical ventilation, drug support, and chest physiotherapy. We report the effectiveness of a novel type of respiratory physiotherapy, the air mattress V-CUE unit, which utilizes a computerized automatic combination of three movements: rotation, percussion, and vibration. METHODS: Among 189 patients who underwent various cardiothoracic operations, 6 were eligible for V-CUE application, after the development of respiratory failure caused by early adult respiratory distress syndrome, massive atelectasis, or pulmonary infection. RESULTS: The V-CUE unit was used to treat six postsurgical patients, aged 58-73 years (mean, 65.8 years); four who had undergone coronary artery bypass grafting, one who had undergone mitral valve replacement, and one who had undergone a bilobectomy. The mean duration of supportive V-CUE was 3.9 days (range 2-6 days), and the mean duration of mechanical ventilation was 2.9 days. There was no need for reintubation or tracheotomy, and all six patients recovered uneventfully. CONCLUSION: Our preliminary results indicate that V-CUE dynamic air therapy is effective for managing postoperative respiratory failure, adjuvant to or complementary to standard therapy.

Figure-of-eight vertical mattress suture technique for anterior flap suspension to overlying tissues in external dacryocystorhinostomy.

PURPOSE: To describe figure-of-eight vertical mattress suture technique in external dacryocystorhinostomy for a combined one-step closure of anterior mucosal flaps and overlying wound tissues and to evaluate its effectiveness on surgical outcome. DESIGN: A prospective, single surgeon, uncontrolled, interventional case series. METHODS: A total of 112 consecutive lacrimal drainage systems of 106 patients (84 women, 22 men; 100 unilateral, six bilateral) from June 2002 to January 2006 with acquired nasolacrimal duct obstruction without canalicular disease underwent external dacryocystorhinostomy with this modified technique. Relief of epiphora and anatomic patency were defined as success. Operative time and success rate were evaluated and advantages were stressed and compared with our previous reports. RESULTS: Mean age was 40.1 years (range, 6 to 75). The etiology was idiopathic in 109 patients and traumatic in three cases. Of 112 lacrimal drainage systems, epiphora was the presenting symptom in 88, combined epiphora and recurrent dacryocystitis in 15, and combined epiphora and mucocele in nine cases. Seven lacrimal drainage systems were revision cases. Mean follow-up was 26.3 months (range, 6 to 48). Success rate was 99.1% (111/112) and mean operative time was 38.2 minutes (range, 28 to 69). CONCLUSIONS: Such a modified suture technique reveals a rapid and simultaneous one-step closure of two different layers that speeds up surgical procedure; simplifies closure process as the knots are tied completely outside the wound, avoiding the disadvantages of suture tying in a small and deep area; eliminates the dead space between anterior flaps and overlying wound tissues; keeps anterior mucosal flap complex away from posterior flaps that prevents collapse back onto the anastomosis and decreases the possibility of mucosal adhesions and, therefore, fibrotic band formation with underlying tissues that may also be useful in cases with small sacs or osseous openings and in revision cases where scarring is a large concern.

Mattress double anchor footprint repair: a novel, arthroscopic rotator cuff repair technique.

In an effort to increase the immediate strength of a rotator cuff repair and to simulate the standard open reconstruction with its effective suture fixation, we have developed a novel technique for suture anchor reconstruction of the rotator cuff. The technique, termed mattress double anchor (MDA), is simple and adaptable. It makes use of 2 suture anchors that are placed independently and then connected by a suture loop. The technique produces a repair construct that distributes the stress across 2 anchors. The method also restores a large surface area for healing between the rotator cuff and the tuberosity.

Support surfaces: beds, mattresses, overlays-oh my!

The prevention and treatment of pressure ulcers are major concerns for health care providers across the care continuum. The selection of a support surface is an important component of a comprehensive pressure ulcer prevention program. The accepted standard in clinical practice for pre-venting pressure ulcers and other complications of immobility is to either turn patients manually at frequent intervals or to use a pressure-reducing device. The longer a patient is immobilized, the more profound will be the systemic complications. The costs associated with the complications of immobility are staggering in terms of human suffering, physiologic damage, and real dollars. A variety of specialty beds, mattresses, and overlays have been designed to address pressure, shear, friction, and moisture. Limited data exist regarding the efficacy of these products. Clinicians want to choose a support surface for their patients on the basis of product performance. With the push toward establishing standards for testing methods and reporting information, clinicians can look forward to making support surface decisions based on the evidence and outcome data resulting from controlled clinical studies and expert opinion and consensus.

Continuous mattress suture for all hand-sewn anastomoses of the gastrointestinal tract.

BACKGROUND: The continuous vertical mattress technique for anastomoses in the gastrointestinal or colorectal surgery has not been well reported in literature. METHODS: We used the technique for all hand-sewn anastomoses with double-armed monofilament absorbable suture (Glycomer 631). RESULTS: In the 266 consecutive anastomoses in 242 cases, there were 4 anastomotic leakages (1.5%) and 1 anastomotic stenosis (0.4%). CONCLUSIONS: The technique was feasible, time-saving, economical and with satisfactory results.

Everting mattress running suture: an improved technique of atrial anastomosis in human lung transplantation.

Significant complications related to the venous anastomosis have been reported in clinical lung transplantation, and it has been suggested that the incidence of pulmonary vein complications may have been previously underestimated. In our institution, we have adopted the horizontal everting mattress running suture as standard practice for the venous anastomosis in human lung transplantation. This technique is easier to perform than the regular over-and-over sutures, and allows suturing of the donor and recipient atrial cuffs in a straight line, opposing endothelium to endothelium, thus limiting the presence of redundant and potentially thrombogenic tissue in the lumen. This technique should help reduce the risk of venous anastomotic complications.

A clinical evaluation of an alternating-pressure mattress replacement system in hospital and residential care settings.

An observational study was carried out to compare wound healing on alternating-pressure mattress replacement systems (APMRS) and other surfaces in an elderly population in acute and residential care settings. Subjects were assessed for the reduction in their pressure ulcers at approximately two weeks and per day, and a visual analogue scale was used to record the patients' comfort. Seven and ten subjects were allocated to the Nimbus III APMRS (Huntleigh Healthcare Ltd) in the hospital and residential care settings respectively. There was no significant difference in the healing of the subjects' sores in the two areas either at two weeks or per day. Five people were allocated to control surfaces in the hospital setting (mainly APMRS; Pegasus Airwave, Pegasus Egerton) and ten in the residential care setting (mainly alternating-pressure overlays; AlphaXcell, Huntleigh Healthcare Ltd). There was no significant difference in the healing of subjects' sores in the two areas, either at two weeks or per day. The trial APMRS was found to be equally comfortable in either setting, and in both settings the control surfaces were not regarded as significantly different in terms of comfort. These findings, from a small sample, promote discussion about the use of pressure-relieving equipment in settings where there are older people who may be at particular risk from pressure damage and where nursing interventions are less intensive and routine.
