[Physiology of the horny layer: aspects for prevention and rehabilitation of occupational dermatitis (author's transl)].

The manifestation of professional eczematous dermatitis is conditioned by prolonged damage and subtoxic alteration of the epidermal horny layer (so-called "2 phases eczema"). The stratum corneum has three different functions of a barrier: protection against water-loss, alkali, and physical forces. The following rules apply: 1. Substances permeate the epidermis the more the higher their concentration and the less the thicker the stratum corneum (Fick's law). 2. Continuously applied alkaline solution is sustained by the stratum corneum without remarkable structural damage up to pH 10. 3. The stratum corneum becomes dry and brittle if its water content diminishes below 10%.

Occupational dermatitis in the manufacture of color television tubes.

An outbreak of dermatitis occurred in the Flow Coat sector of a large and modern color television factory. After investigating the working procedures in this area, a risk of contamination of the skin and clothing with ammonium dichromate was found when ammonium dichromate was weighed and mixed with polyvinyl alcohol (PVA). All of the workers involved in this process were clinically examined; 9 of 18 showed dermatitis to the back of the hands and forearms. Three of these workers were transferred from the sector and were cured of their dermatitis. Patch tests were carried out using 40 allergens, including those used in the Flow Coat process. The tests for PVA and for the phosphorescent blue, green, and red pigments gave negative results at the three concentrations tested. A high incidence of sensitization was found; 9 of 18 (50%) of the workers were found to be sensitized to ammonium dichromate, which is used as a fixer. The preventive measures adopted consisted of improving the quality of the personal protective equipment (PPE) and transferring the weighing and mixing of dichromate to the company laboratory. During the next 18 months, there were no new cases of dermatitis in the Flow Coat sector.

Occupational dermatitis to ultraviolet-cured acrylic-based inks in computer hard disc manufacturing.

OBJECTIVE: Ultraviolet-cured acrylates and their various components and applications are reviewed in this report. METHODS: A 26-year-old woman involved in silk screening computer discs with ultraviolet-cured inks developed an acute allergic contact dermatitis on her hands and forearms. Patch testing revealed a number of strong reactions to epoxy resin and many multifunctional acrylates. RESULTS: The only one listed on the material safety data sheet to which she reacted was tripropyleneglycol diacrylate (TRPGDA). The other positive reactions likely represent cross-reactions. The positive reaction to epoxy is possibly relevant and attributable to nonhardened epoxy resin contaminant in the epoxy prepolymer. CONCLUSION: This case report shows a new application for ultraviolet-cured acrylate based inks in computer hard disc manufacturing.

Occupational dermatitis in animal feed mill workers.

In an epidemiological study of occupational dermatitis in 15 different northern Italian animal feed mills, 204 workers were interviewed, examined and patch tested to 34 allergens, selected from the additives most commonly used in the animal feed mills under consideration. The prevalence of occupational contact dermatitis was 13.7% (28/204): 7.8% (16/204) irritant contact dermatitis and 5.8% (12/204) allergic contact dermatitis from animal feed additives. Among the latter, there were sensitizations, to our knowledge hitherto unreported in the literature: to indigo carmine (2 cases), monensin sodium (1 case), thiabendazole (1 case), methylchlorpindol (1 case) and amprolium hydrochloride (1 case). 3.9% (8/204) of the workers complained only of pruritus sine materia on exposed areas of the body. Contact dermatitis increased with respect to duration of employment: the difference in the rate of contact dermatitis between workers employed in animal feed mill for greater than 10 years and those employed for less than 10 years was statistically significant (p less than 0.05). 6.8% (14/204) had latent sensitivity, which was more frequent in workers with less than 1 year of employment with respect to more experienced colleagues.

Diagnosis of occupational dermatitis by general practitioners and employment medical advisors: a pilot study.

This pilot study examined how closely general practitioners and employment medical advisors agreed when jointly investigating occupational dermatitis. The criterion for admitting a patient to the study was dermatitis on one or both hands. This presented most commonly among men aged 41-60 years and women aged 21-40 years. There was some reluctance among general practitioners to take part in the study, probably owing to the medico-legal problems associated with this condition. General practitioners and employment medical advisors were equally likely to attribute a case of dermatitis to occupational factors if the patient was employed in industry; but if the patient was in nonindustrial employment, general practitioners were more likely to consider the possibility of an occupational origin than employment medical advisors. These differences suggest that before initiating a wider study more information is required concerning the trigger mechanisms for dermatitis on the hands in order to reduce the diagnostic variability.

Occupational dermatitis in the aircraft industry: 35 years of progress.

A comparison of the occupational dermatitis occurring in the same aircraft factory during 2 separate decades, 1955-1965 and 1981-1990, is presented. Subungual pulpitis is highly specific to this industry, because of the handling of resins and sealing agents. The number of cases dropped from 122 to 40, in accordance with progress in preventive medicine and technological changes in the factory. Irritant contact dermatitis nevertheless remained appreciable, while allergic contact dermatitis greatly decreased.
