Effect of testicular macrophage conditioned media from rats with autoimmune orchitis on Leydig cell function.

PROBLEM: The aim of this study was to investigate the influence of immune-activated testicular macrophages obtained from rats with autoimmune orchitis (EAO) on Leydig cell steroidogenesis. METHOD OF STUDY: EAO was induced in rats by active immunization with testis homogenate and adjuvants. Testicular and peritoneal macrophages from rats with EAO were isolated and cultured for 24 hr. Testosterone (T) production by purified Leydig cells incubated in vitro with macrophage-conditioned media (CM) from rats with EAO or control rats was measured. RESULTS: An increase in T production by Leydig cells incubated with CM from testicular, but not peritoneal, macrophages of rats with EAO was observed. This increase was dose-dependent up to a concentration of 30% CM; proportions higher than 35% exhibited an inhibitory effect. CONCLUSIONS: Immune-activated testicular macrophages obtained from rats with EAO induced both stimulatory and inhibiting steroidogenic effects on Leydig cells in vitro and not the exclusively inhibitory action that has widely been attributed to activated macrophages. This dual effect probably depends on the ability of these cells to synthesize different molecules that may exert opposite effects.

Allergic orchitis lesions are adoptively transferred from vasoligated guinea pigs to syngeneic recipients.

Histopathology typical of allergic orchitis developed in testes of inbred guinea pigs 16 months after vasoligation. A similar histopathology was found in unoperated testes after unilateral vasoligation. Peritoneal exudate cells from vasoligated guinea pigs transferred identical lesions to syngeneic recipients. The testicular lesions in long-term vasoligated guinea pigs have an immunological basis.

Malakoplakia of the testis and its relationship to granulomatous orchitis.

The chance finding of two cases of undiagnosed malakoplakia in the files of the Department of Pathology, Queen's University Belfast prompted a systematic study of all cases of inflammatory testicular and epididymal disease diagnosed in that department during a 43-year period from 1934 to 1977. A total of 71 cases was studied, and no additional example of malakoplakia was found. In the Belfast collection, there is one other example of testicular malakoplakia and therefore a total of three cases in 74 examples of inflammatory testicular and epididymal disease (4.3% incidence). In the series there were 10 cases of granulomatous disease (13.5%). The world literature on testicular malakoplakia has been reviewed, and typical and atypical features have been determined. Also a comparison has been made between testicular malakoplakia and granulomatous orchitis, and the relationship between these conditions is discussed.

Role of beta 2-adrenoceptors in the biological effect of autoimmune orchitis spleen cells.

Autoimmune orchitis induced an increment in beta 2-adrenoceptor populations in intact mouse spleen lymphocytes. Normal and autoimmune lymphocytes incubated with soterenol increased the mechanical response of isolated atria. Autoimmune cells were more effective than normal cells in inducing this response. Soterenol or spleen cells alone did not modify the contractility at the concentration used. Inhibitors of beta 2-adrenoceptors of spleen cells completely blunted the reaction between soterenol and lymphocytes, while when atria were exposed to butoxamine, mechanical activity induced by soterenol plus lymphocytes was not affected. Cell-free supernatants of lymphocytes exposed to soterenol elicited the reaction in the same way as soterenol-treated lymphocytes. Direct contact of cells with the assay organ was not necessary. Inhibitors of cyclooxygenase on lymphocytes blocked the reaction of soterenol-treated lymphocytes on atria, while inhibitors of lipoxygenase(s) completely blocked the reaction of atria exposed to soterenol-stimulated lymphocytes or supernatants. These results suggest that soterenol reacts with beta 2-adrenoceptors of normal and autoimmune cells. From this reaction, soluble factors are released that in turn trigger stimulation of the atrial contractility as a consequence of the release of oxidative products of the lipoxygenase(s) pathway of arachidonic acid from atria. The high activity of atria in the presence of autoimmune spleen cells is probably related to the increment in number of beta 2-adrenoceptors of these cells.

Suppression of experimental allergic orchitis and cellular immune response in the guinea pig by pretreatment with testis antigen in incomplete Freund's adjuvant.

Pretreatment of guinea pigs with homologous testicular antigen (TA) in incomplete Freund's adjuvant (IFA) markedly reduced both the incidence and severity of experimental allergic orchitis (EAO) when animals were subsequently challenged with TA in complete Freund's adjuvant (CFA) and examined at 2 weeks after challenge. The pretreatment rendered lymph node lymphocytes, particularly T cells, specifically unresponsive to the subsequent orchitogenic challenge as judged by antigen-induced in vitro lymphocyte proliferative response and blastogenic factor production, whereas antisperm antibody formation was not affected. The suppression induced was of a transitory nature, lasting 3 weeks after orchitogenic challenge, but between 4 and 5 weeks after challenge the development of EAO and aspermatogenesis became evident in parallel with the restoration of cellular immune responsiveness. Cyclophosphamide treatment 3 days before orchitogenic challenge abolished the preventive effect of pretreatment with TA in IFA. Transfer of serum obtained from pretreated animals failed to prevent actively sensitized recipients from developing EAO, which does not favor a possible occurrence of serum blocking factors in protected animals.

In vivo effects of cyclosporin A: abrogation of the induction of experimental allergic orchitis and sparing of the generation of suppressor cells.

Cyclosporin A (CyA; 10 mg/kg/day) administration for 14 days, starting on the day of immunization with testicular antigen (TA) in complete Freund's adjuvant (CFA), almost completely abrogated the induction of experimental allergic orchitis (EAO), delayed skin reactivities to both TA and purified protein derivative of tuberculin (PPD), and antisperm antibody response in strain 13 guinea pigs when examined 2 weeks after immunization. The unresponsive state induced was of a transitory nature. After cessation of CyA treatment, EAO-eliciting capability and cellular immune responsiveness recovered in relatively short time, while restoration of humoral immune responsiveness was delayed. Transfer of lymph node cells taken from CyA-treated, EAO-suppressed animals at 2 weeks postimmunization into normal syngeneic recipients inhibited the induction of EAO and delayed skin response. The suppressor cell activity appeared to be at least in part antigen-specific. It seems likely, therefore, that the profound suppression of EAO by CyA could be explained as due to the additive effect of the inhibition of helper/inducer T cell generation and the sparing or activation of suppressor cell populations.

Vasectomy to prevent acute epididymo-orchitis after prostatic adenomectomy: still a controversy.

There has been a controversy for almost 80 years about the usefulness of vasectomy in the prevention of acute epididymo-orchitis (EO) after prostatic adenomectomy (PA). In the last few years, improved surgical equipment and new more effective antibiotics have drastically diminished the incidence of this complication. The purpose of this report is to show that because of the devastating effect of the disease, and in spite of its low incidence there is still a clear indication for vasectomy, and also that infected urine, a preoperative indwelling urethral catheter, duration of postoperative catheterization and postoperative complications do not affect the incidence of post-PA acute EO.

Epididymo-orchitis as a complication of brucellosis.

A study of 84 patients who were diagnosed with epididymo-orchitis between July 1987 and September 1993 is presented. Brucellosis was a complication in 14 cases (17%). All 14 cases had elevated agglutination titers. Brucella blood culture was found to be positive in 4 of 14 cases (28.5%). Standard therapy regime (streptomycin plus tetracycline) was effective in 13 of 14 (93%) cases.

Fine needle aspiration cytology of tubercular epididymitis and epididymo-orchitis.

OBJECTIVE: To study the role of fine needle aspiration cytology (FNAC) and ancillary studies in the diagnosis of tubercular epididymitis or epididymo-orchitis. STUDY DESIGN: Forty patients with tubercular epididymitis or epididymoorchitis diagnosed on FNAC underwent a detailed clinical workup, imaging and microbiologic studies before being started on antitubercular treatment (ATT). One patient underwent orchiectomy. RESULTS: Clinically, the disease presented in patients of all ages usually as a scrotal swelling or rarely as a scrotal sinus (3) or abscess (3) or as part of disseminated tuberculosis (2). Three patients gave a history of previous tuberculosis. Scrotal sonography confirmed the involvement of the epididymis, testis or spermatic cord in each case. FNAC was diagnostic in 27 aspirates (epithelioid cell granulomas with caseation) but nondiagnostic in the rest. Tubercular etiology was confirmed directly by detection of acid-fast bacilli (AFB) on FNA smears in 24 (60%) patients and urine samples in 11 and indirectly in 9 patients with negative AFB by using a combination of a positive Mantoux test (5 of 9), presence of caseating granulomas on FNA smears (7 of 9) and therapeutic response to ATT (9 of 9). CONCLUSION: FNA as a minimally invasive technique plays a prime role in the diagnosis of tubercular epididymitis and epididymoorchitis. It provides adequate material for cytologic and microbiologic examination and helps to avoid unnecesary orchiectomy.

Tuberculous orchitis diagnosed by fine needle aspiration cytology.

OBJECTIVE: To study the role of the fine needle aspiration cytology (FNAC) in the diagnosis of testicular lesions. STUDY DESIGN: Seven adult males (55-68 years) presented to the clinic with unilateral enlargement of the testis for the previous few months. Clinically, a malignant tumor was suspected. Fine needle aspiration was performed for rapid diagnosis. RESULTS: The smears revealed many groups of epithelioid histiocytes, giant cells, a few plasma cells and occasional spindle-shaped fibroblasts. Acid-fast stain revealed a few acid-fast bacilli. The cases were diagnosed as tuberculosis and treated with orchiectomy and antituberculous drugs. The patients did well. Histologic sections confirmed the cytologic diagnosis. CONCLUSION: FNAC is useful in the diagnosis of tuberculous orchitis.

Testicular function after unilateral bacterial epididymo-orchitis.

In order to determine whether bacterial infection of the gonads may account for some male-related infertility, the fertility status of 45 young men who developed gonococcal urethritis and then epididymo-orchitis were studied prospectively. The fertility potential of the subjects was evaluated through history of paternity, repeated semen examinations, serum follicle-stimulating hormone determinations, and testicular biopsy. Before infection, 14 married men (100%) had proved their fertility through their children. Two years after infection, only 21% of these fathers and 40% of all subjects produced semen considered adequate for conception. The semen values in the rest were comparable to those of infertile/subfertile men. Although the lesion was clinically confined to one testis, testicular biopsy and follicle-stimulating hormone studies showed that testicular damage involved both testes. The study showed that bacterial gonadal infection may result in permanent azoospermia or oligospermia and this without question may result in male-related barrenness.

Successful interferon-alpha 2 a therapy for a patient with acute mumps orchitis.

Involvement of one or both gonads in postpuberty mumps is common and can often lead to infertility. Our report discusses the course of disease of a patient with acute mumps orchitis. Only 2 days after administration of interferon, clinical symptoms had drastically improved, and the infection subsided without any after-effects. We therefore recommend immediate interferon therapy, as soon as postpuberty mumps manifests, to prevent testicular atrophy and resulting infertility.

Acute Disease↗

The painful scrotum: torsion vs epididymo-orchitis.

In summary, an acute disorder of the scrotum in a child without previous urologic history must be viewed as torsion until proved otherwise by surgical exploration. No deleterious effects have been reported from such an approach, which requires a minimal period of hospitalization and little discomfort to the child. The shorter the interval between the onset of symptoms and surgical exploration, the greater the likelihood that the testicle will be viable later. The diagnosis of epididymo-orchitis in a healthy child without a history of urologic abnormality or evidence of lower urinary tract infection has no place in the differential diagnosis of the child with the painful scrotum.

A 3-year-old boy with tuberculous epididymo-orchitis.

The authors report the case of a 3-year-old child with tuberculous epididymo-orchitis. The only presenting symptom was testicular, epididymis and scrotal swelling and pain. Diagnosis was reached after histopathological examination of epididymis and testis tissues. The response to antitubercular drugs given with prednisolone was rapid. This case emphasizes the importance of considering tuberculosis in differential diagnosis of testicular and epididymal enlargement in young children in an endemic area despite the absence of systemic, pulmonary and urinary manifestations.

Management of epididymo-orchitis in Genitourinary Medicine clinics in the United Kingdom's North Thames region 2000.

A questionnaire survey and case notes audit reviewing management of epididymo-orchitis (E-O) by 34 Genitourinary Medicine (GUM) clinics located in the North Thames was undertaken. Twenty-two clinics (65%) returned completed questionnaires and audited a total of 83 newly diagnosed cases. All participating clinics offer microscopy of urethral smears and screening for Neisseria gonorrhoeae and Chlamydia trachomatis to all patients, regardless of age. However, greater numbers of clinics would offer routine microbiology of mid-stream urine (MSU) samples (20/22, 91% versus 16/22, 73%) and scrotal ultrasound (5/22, 23% versus 1/22, 5%) to patients aged over 35, compared with men under 35. Half of the cases audited were due either to sexually transmitted infections (STIs) (41/83, 49%), or associated with ascending urinary tract infections (4/83, 5%). No obvious infectious cause was identified for 38/83 cases (46%). Reported management was appropriate for the causative conditions diagnosed and accorded with the UK National Guidelines for this and related conditions.

Lymphocyte proliferative response in vitro and its cellular dependency in guinea-pigs with experimental allergic orchitis.

The development of cellular hypersensitivity was measured by the proliferative response of lymph node cells in vitro and a skin test to partly purified testicular antigen. There was a slight stimulation of lymph node cells and of the delayed cutaneous response at 1-2 weeks after sensitization with testicular antigen in Freund's complete adjuvant and then, after a decline in response, there was a pronounced peak at 6 weeks after sensitizaton. The early response coincided with the onset and active inflammatory stage of the effect on the testis, while the major peak was consistent with a state of aspermatogenesis. The proliferative response of unseparated lymph node cells could not be attributed to the function of T or B cells acting alone, although at 6 weeks after sensitization B-cell populations did become slightly responsive to antigen. The response of T lymphocytes was enhanced by the presence of B lymphocytes or peritoneal macrophages at 2 and 6 weeks after sensitization, but B cells did not appear to interact with macrophages. We conclude that T cells are essential to the proliferative response of lymph node cells during the induction of experimental allergic orchitis in guinea-pigs.
