[Surgical treatment of induration in penis plastica].

In the past 5 years the authors have removed the scar tissue in the case of the induration of the penis plastica of 43 patients by their special surgical method. Operation had to be repeated in the case of 5 patients. Twenty-five patients were cured, 15 improved and 3 remained unchanged after operation. The method, if applied with the necessary skill, is a recommendable method for the treatment of the induration of penis plastica. If, after operation the restoration of the function is not perfect, the application of supplementary conservative therapy is justified.

Orgotein in the treatment of plastic induration of the penis (Peyronie's disease).

Eighteen patients with Peyronie's disease have been treated with the anti-inflammatory metalloprotein Orgotein, which exhibits a pronounced superoxide dismutase activity. Only patients with severe symptoms were selected. The drug was injected monthly into the indurated areas of the penis. Marked improvement was noted, especially regarding the loss of pain on erection. Only one adverse reaction has occurred.

Orgotein, a new drug for the treatment of Peyronie's disease.

Twenty-three patients with Peyronie's disease have been treated with the new anti-inflammatory metalloprotein drug orgotein, which exhibits pronounced superoxide dismutase activity. Administration was done into the indurated areas of the penis by a special syringe. Evaluation of signs and symptoms included measurement of the induration size and consistency as well as the degree of the deviation of the penis. The patients responded well to orgotein therapy, especially regarding the loss of pain on erection. On long term results also a diminishment of the induration size and penis deviation on erection was observed; the drug administered intraplaqueally was of outstanding safety.

[Is there a thrombophlebitis or subcutaneous lymphangitis of the penis?].

Twelve patients suffering from subcutaneous dorsal induration of the penis were investigated. The diagnosis of a lymphangitis or thrombophlebitis depends on clinical features. It is of nonvenereal origin, and the treatment, if necessary, consists in local or systemic antiinflammatory or antibiotic therapy.

Ultrasound diagnostics as a method of investigation of plastic induration of the penis.

Using the ultrasound technique of visualization (grey-scale technique) and a method of 'water bath', the authors obtained echograms of plastic indurations of the penis. The echograms can be immediately photographed and interpreted. By their quick, painless and simple performance, without contrast medium and hazardous irradiation, the authors gain an advantage over some other diagnostic methods.

[Cavernous vegetative innervation and hemodynamics in patients with fibroplastic induration of the penis and erectile dysfunction].

Different versions on pathogenesis of Peirony's disease (PD) and erectile dysfunction in this disease made us to investigate cavernous innervation and hemodynamics in patients with fibroplastic induration of the penis. We have detected characteristic changes of cavernous electric activity and histochemical picture in biopsies of all our patients with PD. Of them, 61% had cavernous hemodynamic disorders. Our findings suggest that early affection of cavernous innervation may trigger PD pathogenesis and erectile dysfunction in this disease.

[Treatment of Le Peyronie's disease combining pharmacology and physical therapy].

Of the multiple conservative treatment modalities available for plastic induration of the corpora cavernosa, we chose to combine physical (laser therapy) and drug therapy (potaba) in an attempt to achieve a synergistic effect. We report on 32 cases submitted to this combined treatment modality. Patients were evaluated for pain, curvature, impotence and plaque before and after treatment.

Induratio penis plastica Peyronie's disease. Clinical features and etiology.

The clinical and laboratory findings in 106 patients with Peyronie's disease as well as histopathological examinations of biopsies from the plaques were studied. The clinical symptoms and signs were in general similar to those found in previous studies, but bone marrow smears showed an increased number of plasma cells and lymphocytes in 18 of 24 examined. Biopsy of the plaque in cases of long-term symptomatology disclosed a fibrosis poor in cellular components. In patients with a short history of the disease and a tender induration, an inflammatory component of the specimens with perivascular accumulation of lymphocytes and balooning of endothelial cells in the small vessels was seen. Characteristic cells with "cross-banded" nuclei, described earlier only in Dupuytren's contracture and experimental fibrosis, was observed for the first time in Peyronie's disease. Based on these findings a combined traumatic-immunological etiology is suggested.

Results of radiotherapy for Peyronie's disease.

PURPOSE: To retrospectively review the results of radiotherapy for Peyronie's disease. PATIENTS AND METHODS: In the time interval 1983-2000, 154 patients in our clinic were irradiated for Peyronie's disease. Of those, 101 had at least one complete follow-up data set and are the subject of this study. In the majority of patients, penis deviation was between 30 and 50 degrees , there were one or two indurated foci with a diameter between 5 and 15 mm. Pain was recorded in 48/92 patients. Seventy-two of the 101 patients received radiotherapy with a total dose of 30 Gy, and 25 received 36 Gy in daily fractions of 2.0 Gy. The remaining patients received the following dosage: 34 Gy (1 patient), 38-40 Gy (3 patients). Mean duration of follow-up was 5 years. RESULTS: The best results ever at any time during follow-up were an improvement of deviation in 47%, reduction of number of foci in 32%, reduction of size of foci in 49%, and less induration in 52%. Approximately 50% reported pain relief after radiotherapy. There were 28 patients with mild acute dermatitis and only 4 patients with mild urethritis. There were no long-term side effects. CONCLUSION: Our results compare well with those of other studies in the literature. In our patient cohort, radiotherapy was an effective therapy option with only very rare and mild side effects.

Conservative management options for Peyronie's disease.

Peyronie's disease is an acquired condition that presents clinically with a palpable induration, a distinct plaque, or a curvature of the erect penis. At times, the erections are painful. Erectile dysfunction (ED) often is associated with Peyronie's disease. The current era of phosphodiesterase therapy for the treatment of ED seems to have increased the number of patients presenting for treatment of Peyronie's disease. During the past decade, significant advances have been made in understanding the pathophysiology of the disease, resulting in numerous nonsurgical therapies proposed for the treatment of Peyronie's disease. These medical treatments can be administered systemically, locally, or intralesionally. Unfortunately, there are few randomized, placebo-controlled trials to evaluate most of these proposed therapies. In evaluating therapies for Peyronie's disease, it must be remembered that the natural disease history includes spontaneous resolution of pain and even a small improvement in penile curvature in some men. This article reviews the natural history, scientific understanding, and nonsurgical treatment options for the mild to moderate Peyronie's deformities. Surgical therapy remains the mainstay of treatment for severe and refractory cases of Peyronie's disease that result in a physical or psychologic dysfunction.

[Radiotherapy in benign diseases: Morbus Peyronie].

PURPOSE: The induratio penis plastica (IPP) or the so-called Morbus Peyronie is a burdening disease for patients with three main symptoms, induration, deviation and pain. The etiology of this fibrosing and plaque forming disease is largely unknown up to now. PATIENTS AND METHODS: The presented data refer to a retrospective univariant examination of 67 patients, which have been treated for IPP with soft X-rays in the Dermatological Clinic of the Technical University of Munich between 1990 and 1995. The aim of the study was to examine, how far a progression of the disease can be stopped with soft X-rays and how the pain symptomatic is reduced. RESULTS: In 58 of 67 examined patients (86.6%) a progression of the disease could be stopped. 25 out of 67 patients (37.3%) complained of pain before therapy. This symptomatic mostly improved totally in 21 patients (84% of the patients with pain). A complete or partial dissolution of the indurations was to be noticed in 41 of 70 indurations (58.6%). In 23 of 60 patients (38.3%) an improvement or totally regression of the deviation was observed. It could be shown that therapeutic outcome significantly correlates to a shorter duration of anamnesis (p < 0.05), smaller plaque size (p < 0.025) and a tendency to lower age of the patients. CONCLUSION: Radiotherapy of Morbus Peyronie is extremely effective and has a low rate of side effects.

[Surgical treatment of plastic induration of the penis].

In a period of two years 16 patients with induration penis plastica were observed; 11 needed treatment. The therapy was performed according to the degree of severity with vitamin E or local injections of hyaluronidase-dexamethasone. When the conservative therapy failed the foci were removed by operation and the defect was covered by lyophilised dura. The results are rather encouraging. Disturbances of the erection could not be observed. In selected cases of successless conservative therapy the excision with covering of the dura seems to represent an accessible alternative.

[Radiation treatment of plastic induration of the penis].

Between 1969 and 1984, a group of 54 patients with plastic induration of the penis was treated at the Medical Radiation Institute of the University of Tübingen. During the first years conventional X-ray irradiation was applied, and since 1974 patients were submitted to 3 or 5 MeV electron irradiation of a linear accelerator, until a total focal dose of 20 Gy was accumulated. 78% of these patients were between 50 and 60 years old. Sixteen patients suffered from a simultaneous Dupuytren contracture, one patient had a fibrosis of mamma virilis. The treatment results reached from satisfactory until very good in 69% of all cases. Complete regression or a considerable improvement was obtained for indurations in 59%, for deviations in 69%, and for erection algesia in 79%. Radiogenic damages can be excluded in case of a gonad exposition of 0.2 Gy. There were no permanent cutaneous lesions after a total focal dose of 20 Gy. The prognostic importance of an early treatment is underlined; late stages presenting ossifications can be treated only by surgery. For routine diagnosis of site, extent, and possible mineralization of the regions involved, soft ray phallography performed with the technique of mammography can be used.

[Complex therapy of plastic induration of the penis].

The data of 56 patients with plastic induration of the penis treated at the Department of Urology of Semmelweis University Medical School are presented. After a survey of the literature and of the therapeutic possibilities the surgical solution worked out by the authors recommended. Conservative treatment alone cannot secure a cure and its pre-operative application will not improve the chances of a cure. A palpable knot is the indication for surgery. Postoperative cease on ultrasonic treatment. If for some reason (concomitant illness, oppositions of the patient to operation) only conservative treatment can be applied, ultrasonic therapy is believed the most suitable.

[Induratio penis plastica].

BACKGROUND: To evaluate the benefit of radiotherapy with fast electrons in induratio penis plastica (IPP). PATIENTS AND METHODS: Within 7 years we treated 58 patients with manifested clinically induratio penis plastica. We applied single dose of 3 Gy 2-3 times weekly and a maximum dose of 24-30 Gy. Follow-up was 2 years and included documentation of subjective discomforts (induration, deviation and painful erection). RESULTS: Induration (27.6%) and deviation (24.1%) of the erected penis are little improved, but in two-third of patients (65%) with painful erection defined relief could be obtained. It could also be shown that patients with short history of symptoms had best chances for a successful treatment. Side effects were not seen. CONCLUSION: In our experience radiotherapy is not suitable for all stages of IPP. Only patients in the early stages of IPP-especially the ones who suffer from painful erection-should be considered suitable for treatment with ionising radiation.

[Tunica vaginalis testis autografting for Peyronie's disease: report of two cases].

Two patients with refractory Peyronie's disease were treated by tunica vaginalis testis autografting following surgical excision of indurated tunica albuginea of the corpora cavernosa. One patient could achieve relief of pain and a straight erection of the penis 3 months after surgery. Another patient could not obtain a satisfactory erection during the follow-up of 6 months, but could have sexual intercourse after 7 months. A tunica vaginalis testis autograft might be an ideal substitute for tunica albuginea because it has several advantages: it causes no rejection at the recipient site; it is flexible enough to accommodate to expansion; there is no development of sebaceous glands and hair follicles that may occur in the use of a dermal graft; and it is easy to obtain.

Plaque excision and dermal graft in the surgical treatment of plastic induration of the penis (Peyronie's disease).

Excision of the fibrous plaque from the full thickness of the tunica albuginea of the corpora cavernosa of the penis was performed in 16 patients with Peyronie's disease in the course of seven years. The resulting gap was covered with a dermal flap. The dorsal vascular and neural bundles were separated from the tunica albuginea together with Buck's fascia. This method caused only very slight damages in the circulation and innervation of the penis. The patients were satisfied with the straightness of their penis. The method has the following advantages: it may be applied even in case of extensive, cartilaginous, calcified plaques; the hourglass deformation of the penis may be corrected; it does not result in the shortening of the penis; the complete excised plaque may be examined histologically.

[Historical considerations on La Peyronie's disease. Very little has changed!].

Peyronie's disease on plastic induration of the corpus cavernosum was described by La Peyronie in 1743. Initial clinical descriptions, based on 5 cases, were extremely clear. A surprising similarity is seen between comments on pathology and etiopathogenicity made in the past and current concepts. As early as 1886 it was suggested that inflammation was followed by fibrosis. Similarly, it has been known from 1850 that diabetes and Dupuytren's disease are frequently associated with Peyronie's disease. Although surgical removal of nodules and the use of a prosthesis were not mentioned, a thesis by Merle in 1899 suggested that treatment should consist of contralateral plication to reduce the effect of deviation of the fold resulting from the disease itself.
