BACKGROUND: During radiotherapy the pacemaker-patient is exceptionally endangered by ionising radiation, because a damaging impact on the pacemaker's circuit is possible. PATIENTS AND METHODS: Guided by experiences of long standing dealing with pacemaker-patients during radiotherapy in our medical centre, we demonstrate possibilities to accompany these patients with well coordinated interdisciplinary co-operation. So the possible risks by ionising radiation can be identified and restricted. Our investigations on explanted pacemakers will explain the influence on programmable pacemaker systems. Standard values for the dose of 9-MV-photons will be recommended. RESULTS: The radiation dose, the pacemaker system is exposed during radiotherapy, should be kept as small as possible by suitable methods (e. g. shielding, selection of radiation quality, shifting of the pacemaker system to a region of less doses), to ensure that the functions and the duration of life is not effected. Because of the different manufacturing technologies and the scattering of the product parameters within a homogeneous set of pacemakers we are not able to specify a guaranteed value for the radiation resistance. CONCLUSIONS: The doses of pacemaker should be as minimal as possible. The patient's pacemaker system has to be controlled in adequate periods, in particular during radiotherapy. If the accumulate dose on the pacemaker system exceeds 5 Gy despite of all efforts, the pacemaker should be exchanged after the radiotherapy.

INTRODUCTION AND OBJECTIVE: The hypothesis that ventricular pacing is the most important factor in the survival of patients with Sick Sinus Syndrome remains controversial. The aim of this report was to determine independent clinical variables to predict survival in paced SSS patients. METHODS: Retrospective, nonrandomized study of 153 patients with a mean age of 69 +/- 11 years, who received an initial pacemaker for Sick Sinus Syndrome between 1980 and 1994: 65 physiologic pacing (32 dual chamber, 33 atrial) and 88 ventricular pacing mode. After a maximum follow up of 177 months (median 57 months) the end point was total mortality. RESULTS: Total mortality was 21%. Using univariate analysis, single chamber ventricular pacing, age > or = 70 years and NYHA > or = 2, was associated with a higher risk for total mortality (4 times increased risk for ventricular pacing compared to other pacing modalities). Independent predictors using multivariate analysis were: 1) NYHA > or = 2 (p < 0.05). 2) Coronary artery disease (p < 0.01). 3) Chronic obstructive pulmonary disease (p < 0.05) and 4) Gender (p < 0.05). CONCLUSIONS: Mortality in patients with the Sick Sinus Syndrome is strongly predicted by preimplant baseline clinical variables. Our data indicate that the role of ventricular pacing mode remains inconclusive. A large, randomized study is necessary to confirm these results.

ECG investigations of cardiodynamics were performed in 47 patients subjected to isolated ventricular electrocardiostimulation on obsidan treatment. The drug was given in a single dose or in a course. Cardiodynamic effects were evaluated in 3 groups of patients: with hyperkinetic, eukinetic and hypokinetic circulation (17, 19 and 11 patients, respectively). Cardiohemodynamic effects of obsidan are shown to depend on type of circulation and features of intracardiac hemodynamics (fixed cardiac rhythm, atrioventricular dissociation, pathological ventricular asynchronism, valvular blood regurgitation). This potentiates cardiodepressive effects of obsidan in patients with hypokinetic circulation who experience deterioration of cardiac insufficiency.
