Acute infarction during triple-vessel coronary angioplasty due to acute occlusion of a nonattempted vessel.

During angioplasty of the left coronary artery, an acute thrombotic occlusion of the right coronary artery occurred at the site of a borderline stenosis. The source of the unexplained chest pain at the end of the procedure was identified but the next day when inferior Q-waves appeared. It was documented by repeat angiography. The coronary angiogram of the right coronary artery preceding the angioplasty of the left coronary artery is likely to have triggered the occlusion. Chest pain during or after angioplasty without apparent cause in the dilated artery must prompt a contrast medium injection into the nonattempted artery, particularly if the latter had undergone a diagnostic study at the beginning of the procedure.

Vacuole formation in the endothelium of rat extremity vessels depends on fixation techniques and vessel type.

Applying immersion fixation for electron microscopy, huge clear endothelial membrane-bound vacuoles of 0.1-3 microm diameter were noted in the extremity veins of Sprague-Dawley rats. Histological and electron microscopic histochemical methods were applied to determine whether they were the product of programmed cell death or any other kind of cell damage. Image analyzer was used to measure the total area of the vacuoles in the endothelium cells. Neither lipid content nor acidic phosphatase activity could be identified in the vacuoles. In saphenous and brachial veins, the vacuoles occupied 20.6 +/- 2.21% and 18 +/- 2.45% of the endothelium, respectively. Venous endothelium of two different strains of rat also contained the vacuoles. No such structures appeared in extremity arteries. Long-term tilting did not influence vacuolization. Using in vivo whole body fixation, only pinocytotic and dense microvesicles, but no vacuoles were noted. In conclusion, the clear vacuolar structures represent neither lipid inclusions nor secondary lysosomes. The method of tissue fixation is critical when venous endothelial vesicles are investigated. It is presumed that the vacuoles originated from intra- or intercellular microstructures, and that in case of the collapsible vein segments, their size is increased under the pathological-hypoxic and low-pressure-conditions of in vitro fixation.

The prognostic role of vessel productive changes and vessel density in oligodendroglioma.

The recognition of the anaplastic variant of oligodendroglioma is difficult, since it is not easy to identify histological prognostic factors. Among the latter, vascular productive changes have been inconsistently put in relation with survival. In 95 cases of operated oligodendrogliomas, endothelial cell hyperplasia, microvascular proliferations and capillary density were studied by histological and immunohistochemical methods. Capillary density was evaluated on CD31-stained sections by a grid of 100 squares placed in the ocular of the microscope. Statistical analysis was performed in order to compare these parameters with survival. A nodular growth pattern was observed more frequently among tumor grades 3-4 than among tumor grades 1-2. Endothelial cell hyperplasia was more frequent in nodular growth pattern, but it did not correlate with survival. The highest capillary density was found in nodular growth pattern, but it did not correlate with survival as well. Microvascular proliferations correlated with survival only in univariate, but not in multivariate analysis. Age, extent of surgical removal, year of surgery, post-operative Karnofsky score and MIB-1 LI remained associated with survival, as observed in a previous study.

[Digital subtraction angiography in percutaneous transluminal angioplasty of vessels of the extremities and renal vessels].

The new imaging method digital subtraction angiography (DSA) permits intravenous arteriography. 1259 patients were thus investigated over one year. The lower extremities comprised 21,4% of assessment areas. 120 investigations in 95 patients were based on an indication of percutaneous transluminal angioplasty (PTA). DSA can be used before and after the PTA. The small extent of invasiveness and the shortening of in-patient admission are particular advantages preoperatively. These advantages, in addition to the possibility of out-patient evaluation, are also true for DSA in comparison to the conventional arteriography. Thus the combination of both methods has been found to be very suitable.

Vessels within vessels in the myometrium.

We have encountered a peculiar vascular architecture in the myometrium wherein arteries are found free-floating within cleft-like spaces. Using different colored dye injections in the uterine arteries and veins, we demonstrated that these spaces are venous channels. This was confirmed by immunoperoxidase staining for CD34, which enhanced the cells lining these spaces. A review of 81 hysterectomy specimens showed that this vascular architecture was present in 42 cases (52%), while it was identified in the parenchyma of only two mastectomy specimens among the 45 specimens from different organs studied. A strong association existed between the presence of this architecture and history of menorrhagia (p = 0.0116). This peculiar vascular architecture might be important in the pathogenesis of menorrhagia and the development of intravenous leiomyomatosis. Pathologists should also be able to recognize these spaces as vascular channels in the event that malignant cells are identified within them.
