Immunohistochemical localization of keratin, vimentin and myosin in salivary gland tumors.

An immunohistochemical study on keratin, vimentin, and myosin was performed in 117 specimens of human salivary gland under normal and several neoplastic conditions. In normal glands, positive immunostaining for keratin was observed in the inner cells of all ductal systems, whereas myosin and vimentin were the cytoskeletal components of myoepithelial cells. In pleomorphic adenoma, the inner cells showing a tubular pattern demonstrated positive immunostaining for keratin, and the outer cells as well as the neoplastic cells with a solid and myxoid pattern exhibited positive immunostaining for all antibodies. Monomorphic tubular and trabecular adenoma, and adenolymphoma showed positive immunostaining for keratin, although one case of tubular adenoma exhibited positive immunostaining for all antibodies. Squamous cell carcinoma, adenosquamous carcinoma, and mucoepidermoid tumor showed positive immunostaining only for keratin, but one case of adenocarcinoma, two cases of adenoid cystic carcinoma, and clear cell tumor disclosed positive immunostaining for keratin and vimentin.

Myoepithelial cells in salivary gland neoplasms.

Archival paraffin sections from normal salivary gland tissue and salivary gland neoplasms were stained by immunoperoxidase technique with a well characterized cytokeratin antibody (PKK1). In normal parotid tissue, myoepithelial cells and peripheral cells of larger ducts were selectively stained. In pleomorphic adenomas, most cells were stained, the staining being somewhat stronger towards the duct lumina. In basal cell adenomas, only cells adjacent to the duct lumina were stained where a differentiation of cells into peripheral and ductal was seen. In adenolymphomas basal cells were stained, and in oncocytomas small elongated cells reacted with the PKK1 antibody. Only a few duct cells in an acinic cell carcinoma were reactive and in mucoepidermoid carcinoma, peripheral epidermoid cells were strongly stained. In adenoid cystic carcinoma, mostly duct cells were stained whereas the peripheral ones remained unstained. Although the intermediate filament protein expression is very stable during tumorigenesis, the staining with the presently used monoclonal antibody in salivary gland neoplasms differed markedly from what could be expected according to current views on the participation of this cell type. This supports our view that cells in tumors should be characterized on the basis of their staining, i.e. state of differentiation and not on their presumed histogenesis.

Sialadenoma papilliferum.

Sialadenoma papilliferum is a rare benign papillary intraoral tumour most frequently affecting men over the age of 50. The tumour bears many similarities to adenolymphoma of the parotid and may be considered a variant of the latter. A case is presented and the literature is reviewed.

[Ultrasonographic diagnosis of the parotid gland tumors--experience with 310 patients].

The diagnostic value of ultrasonography of parotid gland tumors is to determine if the tumor is benign or malignant. In the period from 1984 to 1995, ultrasound examinations were performed on 310 patients with a space-occupying lesion of the parotid gland, using real time 3.75 MHz, 5 MHz and 7.5 MHz transducers. From a histological standpoint, there were 246 benign tumors, namely 144 pleomorphic adenomas, 35 adenolymphomas, 23 other adenomas, 14 cysts, 9 neurinomas, 3 lymphomas, 9 hemangiomas, 3 lymphangiomas and 6 lymph nodes. Another 64 were malignant tumors, namely, 7 mucoepidermoid carcinomas, 7 acinic cell carcinomas, 7 adenoid cystic carcinomas, 15 adenocarcinomas, 7 squamous cell carcinomas, 4 undifferentiated carcinomas, 10 carcinomas in pleomorphic adenoma, 3 malignant lymphomas, 2 metastatic carcinomas and 2 other carcinomas. According to our criteria for ultrasonographic diagnosis of the parotid gland tumors, the benign or malignant pattern was determined by the following findings; shape, boundary echo, internal echo and posterior echo. But in some cases we could not differentiate the two echogram patterns, and we call them the intermediate pattern. The total accuracy rate of the diagnosis of the 310 patients was 79.0%. (The intermediated pattern was found in 39 patients and these cases were considered to be misinterpreted.) In 268 primary parotid gland tumors (210 benign and 58 malignant tumors) excluding recurrent cases, lymph nodes, hemangiomas and so on, the sensitivity was 62.1%, the specificity was 91.4% and the total accuracy rate was 85.1%. The accuracy of sonography in determining whether a tumor was benign or malignant was affected by the tumor size; tumors smaller than 2 cm in diameter showed a tendency to be diagnosed as benign and tumors larger than 6 cm in diameter as malignant. In the 268 cases, the importance of the pattern of the boundary echo and the internal echo in differentiating benign and malignant tumors was examined retrospectively. In the 210 primary benign tumors, the benignity of the tumors was assessed correctly in 84.3% by the boundary echo and 85.7% by the internal echo. In the 58 primary malignant tumors, the malignancy was assessed correctly in 58.6% by the boundary echo and 43.1% by the internal echo. Strong echos, which are the signs of a malignant pattern, were seen in 14.8% of the benign tumors, and in 51.7% of the malignant tumors. It can be concluded that ultrasonography is very reliable for the examination of parotid gland tumors.

Salivary gland tumours in Congo (Zaire).

From the files of the Department of Pathology of Kinshasa University Hospital, a retrospective study on salivary gland tumours in Congo (Zaire) had been carried out in order to determine their histological and epidemiological profile. Salivary gland tumours are rare in Congo, with a relative frequency of 1.8%. Pleomorphic adenoma (55.1%) and adenoid cystic carcinoma (15.9%) are the most common tumours. Parotid gland (36.6%) is the most frequent location of salivary gland tumours in this country. People aged between 30 and 39 years (19.9%) (Mean: 37.4 +/- 16.2 years) and females (M/F = 0.8/1) are most affected. No case of adenolymphoma is recorded in this series.

[Oncocytic carcinoma of salivary gland].

OBJECTIVE: To study the histological and immunohistochemical characteristics of oncocytic carcinoma of salivary gland. METHODS: 7 cases of oncocytic carcinoma of salivary gland were investigated by HE, PAS, PTAH staining and immunohistochemistry. RESULTS: Among the cases, 4 were male and 3 were female. The average age was 49 years old. 3 cases occurred in the parotid, 1 cases in submandibular, 1 cases in sublingual and 2 cases in minor salivary gland of palate. Investigated histologically, the tumour cells are characterized by oncocyte. The evidence of cellular atypia, obviously increased mitoses and infiltrative growth pattern are indicative of malignancy. Immunohistochemically, alpha 1-ACT, alpha 1-AT, LF, TF and CEA were all strongly positive in the normal salivary cells and adenolymphoma, while in oncocytic carcinoma, alpha 1-ACT and alpha 1-AT were moderate positive and LF, TF and CEA were light positive. CONCLUSION: 7 cases were characterized by oncocyte and showing infiltrative growth, alpha 1-ACT, alpha 1-AT, LF, TF, CEA were positive in oncocyte.

[The influence of heat-induced epitope retrieval on endogenous avidin-binding activity (EABA) and blocking of EABA in immunohistochemistry].

OBJECTIVE: To study the influence of heat-induced epitope retrieval (HIER) on endogenous avidin-binding activity (EABA) and to establish an effective way to block EABA in immunohistochemistry. METHODS: Systematically screening EABA in 164 (679 samples) formalin-fixed and paraffin-embedded human tissues including 76 (102 samples) normal tissues and 88 (577 samples) tumor tissues as well as 4 (80 samples) formalin-fixed and paraffin-embedded rat normal tissues using tissue array (tissue chip), HIER, immunohistochemistry and egg white solution blocking. In addition, EABA was also examined in 9 (15 samples) human frozen tissues. RESULTS: (1) EABA was detected in frozen tissues. (2) No staining for EABA was seen in formalin-fixed and paraffin-embedded tissues. (3) EABA was revealed after the tissues treated with microwave HIER. (4) The density of signal for EABA was variable from tissue to tissue and cell to cell. (5) The signals of EABA expressed in scatter or diffuse in tissues and in granular form in cytoplasm. (6) EABA was found in a wide range of epithelial tissues, especially in gland epithelia of normal and tumor tissues. These included kidney, adrenal cortex, liver, C cells of thyroid gland, oxyphil cells of parathyroid, fundal gland of stomach, sebaceous gland of skin, duct of salivary; oncocytoma and papillary adenocarcinoma of kidney and thyroid gland, adenolymphoma of parotid, carcinoma of liver cell, adenocarcinoma of stomach, colon, prostate, gall bladder and endometrium, and so on. (7) EABA was easier revealed by higher pH value buffer (EGTA pH 9.0) than that with lower pH value (EDTA pH 8.0 and citrate pH 6.0). (8) The revealed EABA could be effectively blocked using 20% egg white solution. CONCLUSIONS: HIER could unmask EABA in formalin-fixed and paraffin-embedded tissues. The unmasked EABA present in a wide range of human normal and tumor tissues as well as in rat normal tissues. The EABA could influence routine immunohistochemistry staining when using (strept)avidin-horseradish peroxidase detective system. The egg white solution could effectively block EABA and eliminate the influence of EABA on immunohistochemistry.

[Analysis of the proportion of salivary gland tumors in 528 elder patients].

OBJECTIVE: To analyse the clinical characteristic, evolution and prognosis of undifferentiated carcinoma with lymphoid stroma(malignant lymphoepithelial The purpose of this study was to investigate the present proportional ratio of salivary gland tumors in elder patients, to provide references for early diagnosis and treatment clinically. METHODS: The medical data of 528 elder patients with salivary gland tumors treated in our department in 1990s were reviewed and analyzed. RESULTS: The number of salivary gland tumors in elder patients had increased in recent years. These tumors were frequently found in patients of 60-74 years. The ratio of benign to malignant was 1.7:1. In benign tumors, the proportion of adenolymphoma approached pleomorphic adenoma. While in malignant tumors, adenoid cystic carcinoma accounted the most. These tumors were most commonly found in the parotid gland, followed by the palate. The prevalence of female had an increase trend. CONCLUSION: The proportion of salivary gland tumor in elder patients in 1990s was different from that in 1980s. This change should attract enough attention form the surgeons.

English Abstract↗

cGMP phosphodiesterase activity evaluation in human carcinoma of salivary glands.

The aim of this study was to evaluate differences of cGMP-PDE activity in salivary glands, between a control group and different benign tumour groups and, where present, with malign tumour groups. Endogen cGMP was evaluated too. The enzymatic reaction used the method of Spoto et al., with minor variations. The samples were organized in six groups: A (Adenolymphoma and Warthins tumour); B (Pleomorphic Adenoma); C (Basaloid Adenoma); D (Myoepitelioma). The control group was represented by healthy patients. In A and B groups, we have analyzed malign pathologies (Adenocarcinoma and Parotid Lymphoma) The benign tumours have more activity than controls, especially in Myoepitelioma (D) but with a decrement in the C group, which presents lower values of cGMP than the other three groups, where the concentration is similar. Between A and B groups, the activity values of malign tumours are similar, higher than controls and than the other benign pathologies, but not higher than in myoepitelioma. The cyclic concentration is similar for malign pathologies, with concentrations lower than controls, similar to Basaloid Adenoma (C).

Salivary gland tumours in Ibadan, Nigeria: a study of 295 cases.

Over a period of 21 years 295 primary salivary gland epithelial tumours were collected and studied. These tumours constituted 2.8% of all head and neck tumours seen at the University College Hospital. Ibadan, over the same period. Two hundred and one (68.1%) were in the major glands and 94 (31.9%) were in the minor intra-oral glands, with the parotid and palatal glands being most frequently involved. There was no statistically significant difference in the sex ratio, and the incidence of tumour gradually increased with age to a peak during the 4th decade. Pleomorphic adenoma was the commonest benign lesion while mucoepidermoid and adenoid cystic carcinomas were the common malignant lesions seen, and were more prevalent in older males. Unlike in other African studies, adenolymphoma, acinic cell carcinoma, adenocarcinoma, and undifferentiated carcinoma types were all identified.

[The value of sialography in the differential diagnosis of parotid masses].

Sialographic manifestations of 82 patients with parotid masses were studied to determine the value of sialography in the differential diagnosis. Multiple filling defects indicate metastatic tumor, tuberculosis and sarcoidosis. The occurrence of calcification suggests tuberculosis or sarcoidosis. The parotid cyst may show the sialographic appearance similar to that of adenolymphoma. The appearance of adenitis of parotid lymph nodes may be similar to that of malignant tumor when the inflammation extends into the parenchyma. Sometimes the appearance of eosinophilic lymphatic granuloma may be similar to that of malignant tumor, but the former is more extensive. The displacement of entire gland suggests extrinsic lesion. Although the displacement of branch ducts may occur in the hypertrophy of parotid, there is no defect in acinar filling. At times it is not easy to differentiate the parotid masses by sialography alone Case histories, results of laboratory and other examines, such as gray scale ultrasound, should be combined to improve the diagnosis.

[Papillary cystadenocarcinoma of the submaxillary gland. A rare diagnosis].

The authors describe a case of a "separate" type of salivary adenocarcinoma-like tumor. The growth is described as a papillary cystadenocarcinoma of the submaxillary gland, which should be distinguished from degenerate adenolymphoma. This malignant tumor, which remains "enclosed" until late, grows slowly and should be checked regularly over a protracted period of time following surgical excision performed in accordance with cancer therapy specifications.

[Unusual autoimmune and neoplastic associated diseases in Sjogren's syndrome].

In an unselected series of 12 patients with Sjögren's syndrome the following autoimmune diseases were observed: severe myxedema due to Hashimotos thyroiditis in four, subclinical thyroid hypofunction in one, diffuse hyperthyroidism in one, glomerulonephritis in two, chronic active hepatitis in one, lupus erythematodes disseminatus in three (one with idiopathic thrombocytopenic purpura), classical rheumatoid arthritis in two. A rare familial occurrence was seen in two sisters. The only male in this group exhibited coexistence of a benign adenolymphoma of the parotid gland (Warthin's tumor) with a malignant non-Hodgkin lymphoma.

[The value of fine needle aspiration cytology in suspected neoplastic salivary gland enlargement].

Imaging offers little support in the management of salivary gland masses suggestive of a neoplastic lesion. There are also contraindications for a surgical biopsy in many cases. Fine-needle aspiration cytology (FNAC) is not yet widely recognized as a diagnostic tool. To date, 206 FNAC were carried out from 1986 through 1993 on 181 consecutive patients and were reviewed in the present study. Histological confirmation was possible in 174 tests, while 32 were confirmed on clinical follow-up. In sum, 192 samples were sufficient for interpretation, 10 were questionable by our standards and 4 were non-diagnostic. One hundred-seventy-one samples were true-negative, 27 true-positive, 4 false-negative and 4 false-positive. Sensitivity was 87.1% and specificity 97.7%. Out of 141 primary diagnostic procedures in which a final histologic diagnosis was available, FNAC was able to determine histogenesis in 113/124 benign lesions and 9/17 malignant masses. These included 65/67 pleomorphic adenomas and 21/22 adenolymphomas. In 8 cases a diagnosis of "adenoma" was made. Difficulties in interpretation were found in lesions that were mucoepidermoid carcinomas and, in part, adenoid cystic carcinomas. No complications occurred. Provided that there was sufficient experience in performing the aspiration technique and in cytologic interpretation, FNAC was found to be a quick, reliable, low-cost, easy-to-perform method with low risk in the management of nearly all benign and most malignant salivary gland lesions.
