BACKGROUND AND METHODS: We evaluated nucleic acid sequence-based amplification (NASBA) and a galactomannan enzyme immunosorbent assay (GM-EIA) for the diagnosis of invasive aspergillosis (IA) in neutropenic febrile patients and for monitoring of its clinical course and outcome. Blood samples were collected twice per week from 128 patients with hematologic diseases during periods of neutropenic fever after undergoing chemotherapy or hematopoietic stem cell transplantation. A total of 448 blood samples were tested. RESULTS: There were 14 patients with IA (2 patients with proven IA and 12 with probable IA). The median index of the initial NASBA in the IA group was more than 10-fold higher than that in the non-IA group. Galactomannan antigenemia (index, >0.5) was detected with a sensitivity of 86%. In receiver-operator characteristic analysis, the cutoff index of NASBA for the presumptive diagnosis of IA was determined to be 5.0. Combination of these 2 parameters (either a GM-EIA index of >or=0.5 or a NASBA index of >or=5.0) improved the sensitivity of diagnosis to 100%. There was a close relationship between patient outcome and the kinetics of NASBA values: failure of negative conversion during treatment resulted in death in almost all cases. CONCLUSION: If either GM-EIA or NASBA results suggest IA, the diagnostic yield for IA could be improved, and NASBA could be a useful marker for predicting the clinical course and outcome of treatment.
PURPOSE: Nd:YAG laser membranotomy is considered a safe treatment of premacular hemorrhage (PMH) in adult patients, enabling rapid enhancement of visual functions. For children, however, pars plana vitrectomy (PPV) performed under general anesthesia has been the accepted treatment. In this report, the authors describe Nd:YAG laser membranotomy (LM) in two children with PMH complicated by hematologic disease. METHODS: Size of lesions was measured in optic disc diameters (DD). The authors performed three openings in the anterior surface of PMH with immediate intravitreal drainage of blood in both patients. RESULTS: Rapid enhancement of visual functions was followed. The authors observed no complications in 1 year of follow-up. CONCLUSIONS: LM may be a safe method of treatment of PMH in children in selected cases.
A comparative analysis was performed of hemogrammes in 134 liquidators of the Chernobyl accident with hemocirculatory disorders, in 69 non-irradiated patients presenting with like processes, in 18 patients with chronic lymphoid leukosis, and in 32 healthy subjects to determine the degree of risk for development of hematologic diseases. The risk was at its highest for thrombocytopenia and neutropenia which are laboratory signs of chronic lymphoid leukosis. The incidence of thrombocytopenia in the irradiated subjects is 4.48 times as much as in the non-irradiated individuals with the same diagnosis and 8.33 times as much as in normal subjects. The frequency of neutropenia in the irradiated persons is 3 to 7 times as much as in the non-irradiated presenting with like pathology, which fact suggests to us a pathogenetic role that the radiation factor plays role in the development of the above disorders.
Thirty-eight sex-mismatched bone marrow transplantation patients with various hematological diseases were followed-up using fluorescence in situ hybridization. Probes specific for various translocations, the X chromosome (DXZ1) and the whole Y chromosome (WCP Y), were used to assess successful engraftment and residual host cells. The combination of translocation and WCP Y probes enabled the identification of host and donor cells in addition to the identification of malignant vs. normal cells in the transplant recipient. Fifteen patients were sequentially followed up. The results obtained using the combination of translocation plus WCP Y probes were more reliable than those with DXZ1 plus WCP Y probes, or the translocation probe alone, especially when the percentage of residual leukemic cells detected by the translocation probe alone was around the cut-off level.
A new antibiotic cefminox (CMNX, MT-141) was administered to 54 patients with severe infections accompanying various hematological diseases. The rate of effectiveness was 55.3% on the whole. Mild liver dysfunction was observed in 3 cases, which recovered quickly following the stop of the administration.
To investigate the clinical features and outcome of severe pneumonia after allogeneic hematopoietic stem cell transplantation for malignant hematological disease, the clinical features of severe pneumonia, including symptoms, signs, blood-gas analysis and thoracic X-ray change were observed, the clinical therapeutical outcome is also evaluated. The results showed that after treatment with antibiotics or antibiotics + antiviral drugs or antibiotics + antiviral drugs + glucocorticosteroids, as well as mechanical ventilation, pulmonary changes in 17 patients with sever pneumonia completely disappeared, 1 case died. In conclusion, early diagnosis and proper treatment for complicated pneumonia after allogeneic hematopoietic stem cell transplantation can decrease the mortality.
BACKGROUND: Few studies exist on the neuropsychological sequelae of bone marrow transplantation (BMT) in pediatric patients. Published results show considerable discrepancies although studies of children with acute lymphoblastic leukemia undergoing high doses of cranial radiotherapy report short- and long-term loss of cognitive ability. OBJECTIVES: To determine the effects of BMT and the effect of anxiety on the pre-BMT assessment in a group of children with severe hematological disease treated in our center. METHODS: We performed a descriptive, prospective, longitudinal study of 54 children, aged 4-15 years, who were treated between 1987 and 1995. Twenty-two children were evaluated before and after BMT by means of the Weschler Intelligence Scale. To control for the effect of anxiety on the pre-BMT scores, the patients were divided into two groups according to the scores obtained in this test (group 1: IQ score 100; group 1: IQ100). RESULTS: Comparison of pre- and post-BMT scores for both groups revealed no significant differences. However, comparison of the results between groups revealed that group I scored lower in the post-BMT test than in the pre-BMT test while group I scored higher in the post-BMT test than in the pre-BMT test. CONCLUSIONS: Although comparison between the pre- and post-BMT results obtained from the whole sample showed no differences that indicated post-treatment sequelae, treatment-induced anxiety may have influenced the pre-BMT score.

Mutations of the P53 tumor suppressor gene represent the most common genetic lesion in human solid tumors. In hematological malignancies, P53 alterations are detected in the evolution of the chronic myelogenous leukemia to myeloid blast crisis. P53 mutations were also found to be associated with Burkitt's lymphoma and HTLV-1 related adult T-cell leukemia. These results support the notion that alterations of the P53 gene may be involved in the progression of these hematological diseases.
OBJECTIVE: To evaluate the use of a new reduced intensity of BuCy conditioning regimen for the treatment of malignant hematologic diseases in aged or intolerable patients receiving allogeneic hematopoietic stem cell transplantation (allo-HSCT) from the siblings. METHODS: Twelve patients with acute lymphoblastic leukemia (ALL, n = 4), acute myelogenous leukemia (AML-M(2), n = 2), chronic myelogenous leukemia (CML, n = 4), and myelodysplastic syndromes-refractory anemia with excess blasts (MDS-RAEB, n = 2) were intolerant of conventional myeloablative therapy because of age (older than 50 years) or having severe concurrent diseases. The median age was 49 years (range 42-64 years). Seven were males and five females. Two of the 12 patients were HLA one antigen-mismatched and the rest HLA identical with their donors. The low dosage conditioning regimen consisted of busulfan (2 mg.kg(-1).d(-1) for 3 days), Ara-C (2 g.m(-2).d(-1) for 1 or 2 times), cyclophosphamide (1.0 g.m(-2).d(-1) for 2 days) and anti-T-lymphocyte globulin (ATG 2.5 mg.kg(-1).d(-1) for 4 days, -5 - -2 day). Granulocyte colony-stimulating factor mobilized bone marrow and peripheral blood stem cells (PBSC) were harvested (1 patient using PBSC alone). All patients received cyclosporin A, short-term MTX and mycophenolate mofetil (MMF) for prophylaxis of acute graft-versus-host disease (aGVHD). DNA short tandem repeat (STR) sequence analysis, cytogenetics and molecular-biologic technique were used to analyze chimerism. RESULTS: All the patients were well tolerated the regimen, with no severe regimen related toxicity. In all the 12 patients, absolute neutrophil count > or = 0.5 x10(9)/L was achieved in 11 to 17 (median 15) days and platelet count > 20 x 10(9)/L in 10 to 23 (median 15) days after transplantation. Complete chimerism was achieved in 11 patients and 1 patient was in mixed chimerism at one month after HSCT. With a median follow-up of 14.5 (4.0-24.0) months, 7 of the 12 patients (58.0%) were alive and 5 (42.0%) of the 7 were disease-free. The probabilities of OS and DFS at 12 months were 75.0% and 48.1%. Five patients (41.6%) had aGVHD and four had local chronic GVHD with a cumulative probability of chronic GVHD of 41.5%. CONCLUSION: This reduced intensity conditioning regimen is well tolerated and safe for HSCT in the older patients or patients with severe concurrent medical conditions and can achieve full chimerism and long-term disease-free survival.
