[Selected immunologic tests in industrial workers chronically exposed to harmful occupational factors].

160 workers, aged 19-60 (average 41.5 years), engaged in the production of prefabricated concrete elements and chronically exposed to ambient humidity and temperature fluctuations underwent preventive clinical immunological examinations. Eighty-seven subjects passed, within recent six months, upper respiratory tract infection. Forty-seven per cent of those who passed the respiratory tract infection exhibited enhanced immunological reaction of cellular type, ability of lymphocytes blastic transformation, the degree of lymphocytes immunoadherence inhibition in the presence of antigens, and lymphocytes B value. The changes in the circulatory system may be conditioned by chronic myocarditis.

The basics of immunological tests.

This review article describes the cells and the humoral factors of the immune system. It also explains what tests are available to assess the immune functions of patients. Each test listed states the amount of blood needed and the time required to perform the assays.

Experience with immunological tests in drug-induced hepatitis.

Nine patients are reported who developed hepatic injury following the administration of therapeutic agents. The drugs incriminated were prajmalium bitartrate (4 patients), quinidine (3 patients), procainamide and nifedipine (one patient each). In all patients the clinical features indicated an allergic process, which in three cases was substantiated by positive response to reexposure. The detection of immunologic processes in our patients by various in vitro methods supports the assumption that immune mechanisms may be involved in the production of drug-induced hepatic disease and confirm allergy.

[Strategy of the use of immunological tests in the topographical diagnosis of urinary infections].

Assays for bacteria-specific antibodies were performed in 423 hospital patients with urinary tract infection (UTI), 184 of whom also had serodiagnosis by passive haemoagglutination with the strain responsible for the infection. The results of serodiagnosis were expressed as the difference in dilution between the antibody titres found in the patients and those found in pooled sera from healthy subjects. On the basis of this study, a rationale for immunological investigations, particularly in adults, is suggested. When bacteria-specific antibodies are lacking the probability of a lower UTI is 98%; when they are present, the probability of an upper UTI is only 74.5%, but the figure rises to 95% with a positive passive haemoagglutination serodiagnosis. In patients with negative serodiagnosis and positive bacteria-specific antibodies, a topographical diagnosis of UTI by immunological methods becomes impossible. In children, these methods are of little use owing to their poor sensitivity in detecting bacteria-specific antibodies, but the information provided by serodiagnosis is useful for determining the site of infection.

[Use of an immunologic test for differential diagnosis of uterine chorionepithelioma and hydatid mole].

Polysaccharide complexes obtained by Westphal's method (modified by L. A. Zilbert et al) from the tissue of a primary tumour node and remote metastases (the lungs) of chorionoepithelioma of the uterus were used for differential diagnosis of chorionepithelioma of the uterus and hydatid mole. Investigations carried out showed that the reaction to the intradermal injection of polysaccharide complexes obtained from the remote metastases (the lungs) of chorionepithelioma of the uterus was highly specific and sensitive: positive reaction occurred only in the patients with chorionepithelioma of the uterus, and it was negative in the patients with hydatid mole. With a marked diminution (in the course of chemotherapy) of metastases in the lungs, when the immunological reaction to chorionic gonadotropin fell to 300--100 IU per litre of the urine, reaction to the intradermal injection of polysaccharide complexes from the metastases of chorionepithelioma of the uterus still remained positive.

Immunologic tests of value in diagnosis. 2. Complement.

Laboratory tests are available to assess the function of the complement pathway and to measure levels of individual complement components. The pattern of complement abnormalities is often helpful in suggesting diagnostic possibilities. For example, when when total hemolytic complement, C3, and C4 are all decreased, one of the rheumatoid diseases is likely. In addition, complement levels in spinal and synovial fluid may provide helpful diagnostic clues.

[Results of clinico-immunological testing of myelopid in chronic forms of multiple sclerosis].

Forty patients with a verified diagnosis of multiple sclerosis (MS), the majority of whom had the secondary progressive form of the disease, received a short-term treatment with myelopid (MP). After this 10 patients were given a long-term treatment with MP. The clinical trial was carried out by the double blind method using placebo. The patients' status was estimated by special evaluation scales (clinical), immunoassays (the content of T lymphocyte subpopulations, reaction of blast transformation of leukocytes to mitogens under different conditions of cultivation including that in the presence of 0.3 micrograms/ml MP, by assessment of the production and activity of interleukin-2), by the method of evoked potentials and NMR tomography. 60% of the patients manifested an improvement of the clinical status after the short-term MP treatment, which was accompanied by positive dynamics of evoked potentials and a decrease of the functional activity of the cells in the reaction of blast transformation of leukocytes. In such patients, MS ran a more favourable course during 0.5-1.5 years of observation. In 8 patients with the immunologic signs of the pathological process activation, the clinical status worsened after MP. The correlation and regression analyses allowed one to specify the clinico-immunologic indications and contraindications for MP administration. The clinico-immunologic changes noted were not recorded after placebo. In the patients receiving the long-term treatment, one could see a further decline of the functional activity of the cells in the reaction of blast transformation of leukocytes, a rise of the content of T suppressors/killers in the blood, a favourable clinical course of MS.(ABSTRACT TRUNCATED AT 250 WORDS)

Immunologic tests of value in diagnosis. 1. Acute phase reactants and autoantibodies.

Acute phase reactants are nonspecific indicators of tissue necrosis and/or inflammation but may be helpful in determining activity of disease. Rheumatoid factor is likewise rather nonspecific, but its presence is helpful in predicting the course, severity, and complications of rheumatoid arthritis. Numerous antinuclear antibodies have been identified in collagen vascular diseases; perhaps the most specific association is between anti-Sm antibody and systemic lupus erythematosus. Anti-smooth-muscle and antimitochondrial antibodies can aid in differential diagnosis of liver disease, while antithyroid antibodies can perform a similar function in diffuse goiter. Anti-parietal-cell and anti-intrinsic-factor antibodies are quite specific for pernicious anemia.
