Hypertrophic and keloidal scars: an approach to polytherapy.

BACKGROUND: Cryotherapy and intralesional corticosteroids have been used separately or in combination as a treatment for hypertrophic and keloidal scars. The addition of silicone gel to the treatment schedule might be of interest. METHODS: Twenty patients with hypertrophic and keloidal scars received two 15-s cycles (in total 30 s) of cryotherapy treatments at every session (once monthly for 12 months) with intralesional injections of trimcinolone acetonide 0.1% (10-40 mg/mL, once monthly for 3 months). Topical application of silicone gel was added three times daily (for 12 months). The control group included 10 patients who received treatment with silicone sheeting. RESULTS: After 1 year there was improvement in all the parameters studied, especially in terms of symptoms, cosmetic appearance and associated signs (P < 0.0001) compared to baseline and compared to the control group. CONCLUSION: Our study suggests that polytherapy may be an effective tool in the therapy of hypertrophic and keloidal scars.

Intralesional alpha interferon therapy in papillary superficial transitional cell carcinoma of the bladder. A pilot study.

The tolerability, toxicity and therapeutic efficacy of recombinant interferon alpha-2a (rIFN alpha-2a), administered by intralesional injection, were evaluated in 15 patients with papillary superficial transitional cell carcinoma of the bladder, rIFN alpha-2a was delivered endoscopically in a single weekly dose of 3 x 10(6) IU for 4 weeks (total 12 x 10(6) IU). Transurethral resection of residual tumours was then performed. The response to treatment was assessed according to ultrasonographic, endoscopic and pathological findings. One patient achieved complete remission, 6 partial remission, 6 minor remission and 2 stabilisation of disease. All patients completed the course of treatment. A mild, transient, flu-like syndrome was documented after every injection. Immunological findings suggest that the antitumour effects of rIFN alpha 2a are not mediated through the immune system.

Incorporation of 15-hydroxyeicosatrienoic acid in specific phospholipids of cultured human keratinocytes and psoriatic plaques.

15-hydroxyeicosatrienoic acid, 15-HETrE, the 15-lipoxygenase product of dihomogammalinolenic acid (DGLA), can inhibit the biosynthesis of the proinflammatory eicosanoids leukotriene B4 (LTB4) and 12-hydroxyeicosatetraenoic acid (12-HETE). The purpose of the present study was to investigate the incorporation of [14C]15-HETrE in specific membrane phospholipids of cultured human keratinocytes in vitro. [14C]15-HETrE was rapidly incorporated into keratinocytes. When a plateau was reached after 3 hours, 15% of the added radioactivity was incorporated into lipids; 96.5% into phospholipids (PL) and 3.5% into neutral lipids (NL). Within the phospholipid classes, [14C]15-HETrE showed selectivity for incorporation into phosphatidylinositol (PI). The mean proportion of [14C]15-HETrE in the PI, phosphatidylcholine (PC) and phosphatidylethanolamine (PE) was 83.2%, 8.5% and 8.3%, respectively. We then investigated the incorporation of 15-HETrE in epidermal phospholipids of psoriatic skin intralesionally injected with 15-HETrE. Four patients took part in the study. In each patient four identical plaques were injected with 0.65 ml of 2.0 microM, 6.2 microM, 18.6 microM of 15-HETrE (0.4 micrograms, 1, 2 micrograms and 3.6 micrograms respectively) or 0.65 ml of 0.88% NaCl twice a week. After 3 wk keratome biopsies were obtained from the treated plaques. Phospholipids extracted from the skin biopsies were separated into major classes by two-dimensional thin layer chromatography. 15-HETrE was then released from specific phospholipids after treatment with phospholipase A2 and identified by reverse phase and straight phase high performance liquid chromatography. There was a dose-dependent incorporation of 15-HETrE into the specific phospholipids PI and PC. When expressed as ng 15-HETrE/micrograms phospholipid phosphate, 15-HETrE accumulated preferentially in PI.(ABSTRACT TRUNCATED AT 250 WORDS)

Intralesional steroids in Langerhans cell histiocytosis of bone.

Langerhans cell histiocytosis may involve single or multiple organ systems. Bone involvement is the most common feature. We have examined retrospectively the effects of 20 intralesional injections of steroids into bone in seven patients seen at our department from 1988 to 1993. Most of these injections (75%) relieved the symptoms, and no side-effects were observed. However, injections into the jaw were seldom effective. Our results suggest that the dose of the steroids administered is of importance.

Observations on the early results of treatment of recurrent respiratory papillomatosis using cidofovir.

OBJECTIVE: To document the response of two patients with severe recurrent laryngeal papillomatosis following treatment with intralesional cidofovir in conjunction with carbon-dioxide laser evaporation. SETTING: Tertiary referral centre. METHODS: Retrospective review of treatment of two patients followed up over a 12-month period. INTERVENTION: Microlaryngoscopy and carbon-dioxide laser evaporation of lesions followed by intralesional injection of cidofovir. OUTCOME MEASURES: Photodocumentation and descriptive statistical representation of intervals between endoscopic treatment. The disease was staged according to severity on endoscopy. RESULTS: Initially, both patients showed a marked improvement of disease. However, the disease relapsed to a significant extent. Overall, there was no demonstrable change in the frequency of required endoscopies despite subjective improvement of the airway. CONCLUSION: Cidofovir may be of some benefit in the management of recurrent respiratory papillomatosis, but further studies are still required.

Endoscopic and electroporation therapy of paranasal sinus tumors.

The purpose of this article is to introduce the concept of endoscopic and electroporation therapy for the treatment of sinonasal carcinomas. Electroporation therapy is a technique that combines intralesional injection of bleomycin combined with application of high-voltage square wave electrical impulse into the tumor. A transient increase in cell membrane permeability allows intracellular entry of bleomycin, with subsequent cytotoxicity. Two cases of sinonasal cancer were treated with this technique. One patient had complete response and is free of disease at 20 months. The other had partial response with control of the disease up to a year. Endoscopic electroporation therapy is potentially a minimally invasive method for the treatment of sinonasal malignancies in an experimental setting where other treatment options do not exist. Phase II/III national clinical trials have been initiated to study the efficacy of electroporation therapy.

Subconjunctival total excision in the treatment of chronic chalazia.

A technique of subconjunctival total excision was used to treat 34 chronic chalazia. Another 34 were treated by incision and curettage and a further 32 by intralesional injection of long acting steroid. Subconjunctival total excision showed a higher success rate (94.12%) compared to the incision and curettage group (76.47%) and the injection group (75%). As the difference was statistically significant and due to other advantages we recommend subconjunctival total excision as a primary approach in the treatment of chronic chalazia.

Intralesional immunotherapy for warts using a combination of skin test antigens: a safe and effective therapy.

Warts are a common dermatologic problem. There are many treatments available. Intralesional injection of a skin test antigen has been shown to be efficacious at eradicating all warts when only a part of one wart is injected. The aim of this study was to determine whether injection of the combination of Candida albicans, mumps, and Trichophyton skin test antigens was more efficacious than and as safe as single antigen injection. Seventy-one percent of subjects had resolution of their warts with the injection of the combination of skin test antigens in this open label single arm study.

Comparison of topical paromomycin sulfate (twice/day) with intralesional meglumine antimoniate for the treatment of cutaneous leishmaniasis caused by L. major.

This is an open study to compare the cure rate of cutaneous leishmaniasis caused by L. major and treated with either paromomycin sulfate or intralesional injection of meglumine antimoniate. Sixty parasitologically proven cases with 1-3 lesions were included and divided randomly into two equal groups; one group received 1 ml of meglumine antimonate intradermally every other day for 20 days, the other group received the ointment containing 15% parmomycin sulfate in urea twice daily for 20 days. The patients were clinically evaluated at 1 and 6 weeks after treatment was completed. The results of clinical evaluation at 1 week after treatment completed showed a cure rate of 18 out of 27 (66%) in the meglumine antimonate injected group and 20 out of 29 (68%) in the paromomycin sulfate treated group. The chi square test was used to compare the cure rate between the two groups and showed no significant difference (p = 0.85).

[The cutaneous leishmaniasis of the face in Gafsa area, Tunisia].

The cutaneous leishmaniasis is a frequent affection in Tunisia. In the Gafsa region it is a endemoepidemic infection. Our prospective study was carried out on 225 cases of cutaneous leishmaniasis of the face collected over two years in the outpatients' department of dermatology at Gafsa hospital. The aim of our study was to know more about the evolutive epidemio-clinical particularities of the cutaneous leishmaniasis of the face. The relative frequency of the cutaneous leishmaniasis of the face for the new patients reached 5%. An equal distribution between the two sexes was noted. The average age of our patients was 24 years old and 6 months old with extremes ranging from 3 months to 77 years. The nodular and ulcerated forms were the most frequent (35.3%), followed by impetiginoid forms (23%), then the lupoid forms (15%). The papular form was quite frequent (13%). Impetiginoid lesions occurred frequently in the child. Lupoid lesions appeared particularly on the nose. The complicated forms: the cutaneuos necrosis or erysipelas of the face appeared after intralesional injections of meglumine antimoniate (Glucantime). The intramuscular meglumine antimoniate was used in 172 patients, among them 47 needed two treatments or more because of a particular resistance of the lesions of the face. The pimple of Gafsa, term initially used for ulcerated skin nodules, was actually characterised by a lesional polymorphism. The prognosis was good however the risk of in aesthetic scar of the face was important.

The use of the full thickness skin graft in Dupuytren's contracture.

For those hand surgeons who have experienced early complications associated with limited fasciectomies (those who have not, have not done enough limited fasciectomies) and are frustrated by a high rate of recurrence or extension of the disease, incision of the cord and interposition of a full thickness graft is a technique to seriously consider. The procedure is not difficult to perform, but patience and attention to detail are prerequisites for success. That success is measured by complete or near complete release of the contracture with a minimum of morbidity, a nil recurrence rate, and extension rate of less than 10%. This technique is indicated for patients who have one or more elements of the Dupuytren's diathesis. Usually people older than 65 who develop Dupuytren's contracture do not have the diathesis, and their disease can be managed by limited fasciectomy and Z plasty skin lengthening. Finally, Dupuytren's disease presenting with no contracture can be managed effectively and conservatively by a series of intralesional injections of triamcinolone into the nodules and cords, the treatment of choice for all plantar nodules and knuckle pads.

Induction of interleukin 3 and tumor resistance by SSM, a cancer immunotherapeutic agent extracted from Mycobacterium tuberculosis.

Interleukin 3 (IL-3) activity was demonstrated when inguinal lymph node cells obtained from Bacillus Calmette-Guérin-sensitized mice (BCG-ILNC) were stimulated in vitro with SSM, an immunomodulator extracted from Mycobacterium tuberculosis. The IL-3 activity was first detected on Day 1 in culture fluids of BCG-ILNC stimulated with SSM, reached a peak on Day 3, and then gradually decreased. The activity was completely neutralized by treatment with anti-murine IL-3 monoclonal antibody (mAb). When BCG-ILNC were treated with anti-Thy 1.2 or anti-Lyt 1.2 mAb followed by complement, IL-3 was not produced in the culture fluids. However, IL-3 in the culture fluids was detected when BCG-ILNC were treated with anti-Lyt 2.2 mAb, anti-asialo-GM1, or anti-mouse immunoglobulin antiserum followed by complement. These results suggested that Lyt 1+ T-cells appeared to be required for the production of IL-3 from BCG-ILNC stimulated with SSM. In addition, low but significant IL-3 activity was also observed in sera of mice treated with SSM. However, serum IL-3 activity was not detected in mice treated with both SSM and Thy 1.2 or Lyt 1.2 mAb, whereas the activity was induced by SSM in mice treated with anti-Lyt 2.2 mAb or anti-asialo-GM1 antiserum. On the other hand, the in vivo growth of IMC tumors inoculated in BALB/c x DBA/2 F1 mice was significantly decreased by intralesional injection of culture fluids containing IL-3, as well as by SSM itself. This antitumor activity of the culture fluids was not altered when it was treated with mAbs for interleukin 1, interleukin 2, or anti-mouse gamma-interferon antiserum. The antitumor activity of the fluid was only eliminated when it was treated with anti-mouse IL-3 mAb. Since nonspecific resistance to tumors in mice stimulated with SSM appears to require Lyt 1+ T-cells, these results suggest that, in part, nonspecific resistance to tumors of mice stimulated with SSM may be developed through IL-3, which was produced by Lyt 1+ T-cells after SSM stimulation.

Intralesional triamcinolone acetonide in alopecia areata amongst 62 Saudi Arabs.

Evaluation of 62 Saudi Arabs with alopecia areata on monthly intralesional injection of triamcinolone acetonide, showed complete regrowth in 40 (63%) patients at 4 months. Regrowth was likely in young adults with few lesions (less than 5 patches), lesions of short durations (less than 1 month) patches less than 3 cm in diameter. Regrowth was poor when associated with atopy (2 patients) and mongolism (1 patient). Side effects of treatment were minimal and the drug was well tolerated.

Sclerosing of recurrent lymphangioma using OK-432.

The authors present two cases of lymphangioma of the cervicofacial region, treated with a new investigational drug in North America, OK-432 (picibanil), a sclerosing agent intralesionally injected. Both patients had been treated surgically and had recurrence of the tumor. Intralesional injection of OK-432 without aspiration was employed for the first patient, and after aspiration in the second patient. A change in consistency of the tumor, manifested by softening, was followed by marked shrinkage. No complication either locally or systemically has occurred during the follow-up period (10 to 16 months). In both cases, satisfactory results were obtained, resulting in definite reduction in size and improvement in cosmetic appearance. The authors recommend OK-432 intralesional injection for surgically challenging lymphangioma. Their results support those of a recent Japanese study using OK-432 as sclerosing therapy for unresectable lymphangioma.

Perilesional linear atrophic streaks associated with intralesional corticosteroid injections in a psoriatic plaque.

Perilymphatic atrophy can be a complication of intralesional corticosteroid injections given for the treatment of conditions such as psoriasis, alopecia areata, and keloids, and intraarticular corticosteroid injections given in diseases such as rheumatoid arthritis. It may become manifest as perilesional linear, depigmented, atrophic streaks, which are usually most prominent in patients with dark-colored skin.

Subglottic hemangiomas in infants: treatment with intralesional corticosteroid injection and intubation.

Six children with a subglottic hemangioma were treated in the Sophia Children's Hospital in the period 1982-1987 by means of intralesional corticosteroid injection, followed by intubation. After treatment all children were symptomfree. In 3 patients this result was attained after 1 injection, in 2 after 3 injections. One patient needed 5 injections. The average duration of intubation was 19 days (7-36). Three months (1/2-7 1/2) after the onset of therapy all patients were free of symptoms. No serious complications were observed. No patient needed a tracheotomy. The average follow-up period was 3.3 years (1 1/2-5 1/2). The authors feel that not only the effect of intralesional corticosteroids, but also local gentle pressure by the tube is of therapeutic importance. The above-mentioned treatment of subglottic hemangioma in children is now the treatment of choice in our clinic.
