Graded response and restitution hypotheses of ventricular vulnerability to fibrillation: insights into the mechanism of initiation of fibrillation.

According to the upper limit of vulnerability (ULV), failed defibrillation (DF) shocks reinitiate ventricular fibrillation (VF) by falling on the vulnerable period of one or more of the fibrillation wavefronts. The failed shock first induces reentry (stage I VF), which within few cycles degenerate to stage II VF. We developed 2 hypotheses of vulnerability that explain DF failure using isolated and intact in situ ventricles. Activation maps were constructed with high-resolution electrodes and action potential (AP) recorded with microelectrodes. According to the graded response (GR) hypothesis, reentry is formed when a critical shock strength induces a GR that transiently increases local refractoriness. The GR propagates and initiates distal regenerative activity that propagates around the site of block to reenter through it as it recovers. Ultrastrong shocks prevent reentry by converting unidirectional block to bidirectional block by excessive increase in refractoriness, a finding that supports the ULV hypothesis. In situ ventricle stimulus-induced termination of reentry and stage I VF (protective zone) could be explained by the GR hypothesis. The induced functional reentry with periods of 100 to 160 ms engages the steep (unstable) portion of the AP duration restitution curves (slope >1) that promotes meandering and breakup. This leads to transition from stage I to stage II VF (the restitution hypothesis). We conclude that the GR and restitution hypotheses provide an insight into the mechanism of ventricular vulnerability to fibrillation induced by a stimulus. These hypotheses provide a new paradigm for effective antifibrillatory strategies.

Quantifying the similarities within fold space.

We have used GRATH, a graph-based structure comparison algorithm, to map the similarities between the different folds observed in the CATH domain structure database. Statistical analysis of the distributions of the fold similarities has allowed us to assess the significance for any similarity. Therefore we have examined whether it is best to represent folds as discrete entities or whether, in fact, a more accurate model would be a continuum wherein folds overlap via common motifs. To do this we have introduced a new statistical measure of fold similarity, termed gregariousness. For a particular fold, gregariousness measures how many other folds have a significant structural overlap with that fold, typically comprising 40% or more of the larger structure. Gregarious folds often contain commonly occurring super-secondary structural motifs, such as beta-meanders, greek keys, alpha-beta plait motifs or alpha-hairpins, which are matching similar motifs in other folds. Apart from one example, all the most gregarious folds matching 20% or more of the other folds in the database, are alpha-beta proteins. They also occur in highly populated architectural regions of fold space, adopting sandwich-like arrangements containing two or more layers of alpha-helices and beta-strands.Domains that exhibit a low gregariousness, are those that have very distinctive folds, with few common motifs or motifs that are packed in unusual arrangements. Most of the superhelices exhibit low gregariousness despite containing some commonly occurring super-secondary structural motifs. In these folds, these common motifs are combined in an unusual way and represent a small proportion of the fold (<10%). Our results suggest that fold space may be considered as continuous for some architectural arrangements (e.g. alpha-beta sandwiches), in that super-secondary motifs can be used to link neighbouring fold groups. However, in other regions of fold space much more discrete topologies are observed with little similarity between folds.

Management of chronic visceral ischemia.

The triad of postprandial pain, weight loss, and diarrhea are the hallmarks of abdominal angina. An anastomotic meandering mesenteric artery noted on frontal arteriogram is valuable in signaling significant disease, but lateral views are the sine qua non for diagnosis. Early surgical correction either by local endarterectomy or bypass of a stenotic or occluded segment and accurate selection of cases results in long-term correction of the intestinal ischemia. When advanced occlusive disease of the mesenteric arteries is noted on the preoperative arteriogram of patients selected for aortoiliofemoral, renal artery, or aortic aneurysm reconstructive surgery, concomitant reconstitution of the mesenteric vascular circulation is advisable. It is our opinion that such an approach can be a significant deterrent to subsequent catastrophic bowel infarction from mesenteric arterial occlusive disease.

Conventional electromagnetic acoustic transducer development for optimum Lamb wave modes.

Lamb waves are normally utilized for inspecting thin metal sheets. Wheel type probes using piezoelectric oscillators have generally been used as the sensors for Lamb waves. Recently, the electromagnetic acoustic transducer (EMAT) has been developed and is beginning to be used as a Lamb wave detector. We have developed a useful type of transducer for Lamb waves. The new EMAT consists of a meander coil with a narrow distance of 2.5 mm and has a symmetrical structure in the vertical direction for both surface sides. The new EMAT can generate Lamb waves with variable wavelengths corresponding to the frequency range from approximately 300 kHz to 2.5 MHz and multiple modes, and can also generate selected symmetrical and anti-symmetrical mode Lamb waves. It is demonstrated that the optimum Lamb wave mode could be produced by the appropriate positioning of the EMATs and controlling the phase (same or inversed) of the electrical signal driving the device. The described EMAT can be used to examine steel (or other material) sheets of different thickness. It is also shown that the S0 (0.3 MHz) mode Lamb wave is the most effective for the deepest (up to 6 mm) penetration.

Sedimentation rates measurements in former channels of the upper Rhône river using Chernobyl 137Cs and 134Cs as tracers.

Former river channels are aquatic ecosystems with a different geomorphology generated by fluvial dynamics more or less linked to the main channel. They present different ecological successions to become terrestrial ecosystems and are thus supposed to have different sedimentation rates. The aim of this paper is to assess this sedimentation rate using radioactive tracer methodology commonly used in lake studies. Chernobyl impacts, expressed in 137Cs concentration and 137Cs/134Cs ratio, were determined in sediment cores. Sites (21) were sampled in the alluvial plain of the Upper Rhône River from 1989 to 1994. The contamination presented a high spatial heterogeneity. The maximum values encountered by site ranged between 34 and 541 Bq/kg of dry matter. The method generally gave good core profiles. Sedimentation rate ranged between 0.14 and 0.70 cm/year for the former meanders and between 0.14 and 2.86 cm/year for the braided channels. The sediment accumulation rates ranged from 0.03 to 0.25 g/cm2 per year and 0.03 to 2.26 g/cm2 per year, respectively. These values are similar to those found for Lake Geneva. The importance of the former channels in relation to the main channel is enhanced by the higher contamination and radionuclides retention. The sediment accumulation rate is related to the organic carbon content in the sediment. A comparison between two former channels with different productivity showed that the the allogenous driven system presents a high organic sediment accumulation rate with a low organic content in the sediment and inversely, a low organic sediment accumulation rate with a high organic carbon content was found for the autogenous drive system.

Nail fold capillaroscopy: normal findings in children and adolescents.

OBJECTIVES: Capillaroscopy is a simple diagnostic method that permits noninvasive in vivo study of the capillary network. Studies designed to standardize capillary normality in children are limited. This article presents the capillaroscopic findings in healthy children and adolescents, thus making the application of this methodology viable for patients in this age range. MATERIAL AND METHODS: Healthy children were recruited from a private elementary school and junior high school. Nail fold capillaroscopy was performed using a stereomicroscope at 16 times magnification, addressing the following parameters: capillary morphology, capillary enlargement, devascularization, microhemorrhage, and subpapillary venous plexus visibility (PVS). These parameters were related to age, sex, ethnicity, and local periungal conditions. RESULTS: The sample comprised 329 individuals with mean age of 8.2 years. We observed atypical capillary morphology in 118 of the studied cases (36%), mainly bizarre capillaries in 90 (27%), meandering capillaries in 32 (10%), and bushy capillaries in 20 (6%). The enlarged capillary phenomenon was uncommon, being observed in 30 cases (9%). The number of capillaries per millimeter varied from five to nine. Deletion areas were detected in only seven individuals (2%). The subpapillary venous plexus was not visualized in 13 (4%) cases. Younger children presented higher PVS scores and fewer capillaries/mm as compared with older children. PVS scores were lower in males and in nonwhite children. Other variables were not associated with sex or ethnicity. CONCLUSIONS: The normal nail fold capillary network in children resembles that observed in adults with some differences, such as a lower number of loops per millimeter, a higher PVS score, and a higher frequency of atypical loops. This information is important for the diagnostic evaluation of children in the context of autoimmune rheumatic diseases.

Atrial fibrillation: strategies to control, combat, and cure.

Atrial fibrillation is the commonest clinical arrhythmia, is increasing in incidence and prevalence, and is associated with substantial morbidity and mortality. The arrhythmia may be paroxysmal (self-limiting), persistent (amenable to cardioversion), or permanent. Especially in its paroxysmal form, atrial fibrillation may be initiated by rapidly firing foci, generally located in the proximal pulmonary veins. Sustained atrial fibrillation is maintained by an atrial tissue substrate capable of accommodating many meandering wavelets. With continuing arrhythmia, the electrophysiological properties of the atria change and further facilitate continuing fibrillation. Treatment is aimed at prevention of thromboembolic complications, restoration and maintenance of sinus rhythm, and control of ventricular rate during atrial fibrillation. With greater understanding of the arrhythmia mechanisms, it is becoming possible to offer targeted curative treatments to more and more patients.

Neural repair of the injured spinal cord by grafting: comparison between peripheral nerve segments and embryonic homologous structures as a conduit of CNS axons.

To study the differences between peripheral nerve (PN) and embryonic homologous CNS structures as a conduit for CNS axons, spinal cord segments in neonatal rats were removed at the mid-thoracic level and PN or embryonic spinal cord (ESC) segments were grafted into the vacancy. Neural connections across the graft were examined by the anterograde and retrograde tracing methods. Anterogradely labeled pyramidal tract fibers entered the PN segment meanderingly and were dispersed; most stopped at the caudal end of the graft. Although a small fraction of fibers re-entered the host spinal cord, they terminated near the graft-host interface without further extension. By contrast, similarly labeled fibers entering the ESC segments crossed the graft and extended further to reach the lumbar segments. The fibers were defasciculated in the graft but became fasciculated and oriented dorsally near the caudal end of the graft, and descended in the normal path. Consistent with these findings, the retrograde tracing study revealed that in animals with ESC segment grafts but not in those with PN segment grafts, many neurons in the upper brain structures were labeled with Fast Blue that was injected into the lumbar enlargement. The difference between the two kinds of graft as a conduit for CNS axons is likely to be due to differences in matching growing axons and their guidance cues through the graft-host interface.

Pattern recognition of the movement tracks of medaka (Oryzias latipes) in response to sub-lethal treatments of an insecticide by using artificial neural networks.

Specimens of medaka (Oryzias latipes) were observed continuously through an automatic image recognition system before and after treatments of an anti-cholinesterase insecticide, diazinon (0.1 mg/l), for 4 days in semi-natural conditions (2 days before treatment and 2 days after treatment). The "smooth" pattern was typically shown as a normal movement behavior, while the "shaking" pattern was frequently observed after treatments of diazinon. These smooth and shaking patterns were selected for training with an artificial neural network. Parameters characterizing the movement tracks, such as speed, degree of backward movements, stop duration, turning rate, meander, and maximum distance movements in the y-axis of 1-min duration, were given as input (six nodes) to a multi-layer perceptron with the back propagation algorithm. Binary information for the smooth and shaking patterns was separately given as the matching output (one node), while eight nodes were assigned to a single hidden layer. As new input data were given to the trained network, it was possible to recognize the smooth and shaking patterns of the new input data. Average recognition rates of the smooth pattern decreased significantly while those for the shaking pattern increased to a higher degree after treatments of diazinon. The trained network was able to reveal the difference in the shaking pattern in different light phases before treatments of diazinon. This study demonstrated that artificial neural networks could be useful for detecting the presence of toxic chemicals in the environment by serving as in-situ behavioral monitoring tools.

Short term dynamics of diatoms in an upland stream and implications for monitoring eutrophication.

A study designed to examine the effect of intermittent diffuse nutrient inputs on diatom assemblages in a small upland stream in northern England was confounded by other changes that occurred in the stream just before fertilisation started. A flora dominated by attached diatoms changed, over a period of about 3 weeks to one dominated by motile Nitzschia spp. In the absence of any likely human causes, the most likely reason for this change was undercutting of a meander upstream, releasing fine silts which favoured motile diatoms. Changes due to fertilisation by phosphorus were also recorded, with an increase in the proportion of Epithemia in samples, possibly favoured by the lower N:P ratio caused by the increased P concentration. The implication of these changes for monitoring eutrophication and ecological status in upland streams is discussed.

Enhanced delivery of naked DNA to the skin by non-invasive in vivo electroporation.

DNA delivery to skin may be useful for the treatment of skin diseases, DNA vaccinations, and other gene therapy applications requiring local or systemic distribution of a transgene product. However, the effective, consistent and patient-friendly transfection of skin cells remains a challenge. In a mouse model, we evaluated the effectiveness of intradermal injection of plasmid DNA followed by noninvasive in vivo electroporation (EP) as a method to improve transfection in skin. We achieved a several hundred-fold stimulation of gene expression by EP, sufficient to produce clinically relevant amounts of transgene product. We studied the effect of DNA dose and time after treatment as well as various EP pulse parameters on the efficiency of gene expression. EP under conditions of constant charge transfer revealed that the applied voltage was the main determinant for transgene expression efficiency while other pulse parameters had lesser effects. Patient-friendly, noninvasive meander electrodes which we designed for clinical applications proved equally effective and safe as plate electrodes. We also showed for the first time that noninvasive EP is effective in stimulating transfection and gene expression in human skin, particularly in the epidermis. Our findings demonstrate the applicability of EP-enhanced DNA delivery to skin for gene therapy, DNA immunization and other areas.

Inferior mesenteric artery aneurysm in Behçet syndrome.

We present a case report of an aneurysm of the inferior mesenteric artery, associated with occlusion of the celiac, superior mesenteric, and left renal arteries and severe stenosis in the right renal artery, in a 48-year-old patient with Behçet syndrome. The meandering inferior mesenteric artery, with an aneurysm 28 mm in greatest dimension, was the blood supply source for the intraperitoneal viscera. Aneurysm resection and reimplantation of the inferior mesenteric artery, and right renal artery bypass grafting with saphenous vein was performed. To our knowledge, this is the first reported case of inferior mesenteric artery aneurysm caused by Behçet syndrome.

Median arcuate ligament compression syndrome in monozygotic twins.

Twin 27-year-old women had symptomatic mesenteric ischemia caused by median arcuate ligament compression. Arteriography demonstrated severe celiac artery stenosis in one twin, celiac artery occlusion in the other, and proximal superior mesenteric artery narrowing with retrograde filling from a meandering mesenteric artery in both. Division of the ligament and direct celiac artery revascularization completely relieved symptoms in both patients. Median arcuate ligament compression of the celiac and superior mesenteric arteries can result in mesenteric ischemia. Documentation of this unusual syndrome in monozygotic twins suggests that the responsible anatomic relationships are congenital.

The gliding apparatus of peripheral nerve and its clinical significance.

Every nerve must have the capacity to adapt to different positions by passive movement relative to the surrounding tissue. This capacity is provided by a gliding apparatus around the nerve trunk. There is another level of gliding provided by the interfascicular epineurium which allows the fascicles to glide against one another. The clinical significance of the gliding apparatus in the context of external and internal neurolysis and nerve repair is discussed. An explanation is offered for the occurrence of the so-called meander-like deformity of fascicles, seen in nerve entrapment syndromes.

Experimental neodymium:YAG laser damage to acrylic, poly(methyl methacrylate), and silicone intraocular lens materials.

PURPOSE: To compare neodymium:YAG (Nd:YAG) laser effects on acrylic, silicone, and poly(methyl methacrylate) (PMMA) intraocular lens (IOL) polymers. METHODS: Ten Nd:YAG laser exposures were produced in each of 6 implantation-quality acrylic (Alcon MA60BM), silicone (Staar AQ1016), and PMMA (Alcon MC60BM) IOLs under identical conditions. Each polymer type was irradiated at 6 power settings (0.3, 0.5, 1.0, 1.5, 2.0, and 3.0 mJ) and at 2 focal points (midpoint of lens optic and on the posterior surface to which a cellophane membrane was affixed). The linear extent of the damage was measured using light microscopy. Specimens exposed to 1.0 mJ were processed for scanning electron microscopy. RESULTS: The damage threshold (> or = 5 microns depth) was 0.3 mJ for silicone and 1.0 mJ for acrylic and PMMA IOLs. At the clinically relevant power levels, 1.0 to 2.0 mJ, the depth of damage in the acrylic polymer was 11.9 to 30.5 times less than the depth in the silicone polymer. Similarly, the depth of damage in the PMMA polymer was 5.4 to 52.6 times less than the depth in the silicone polymer. The morphologic pattern of damage in the silicone IOL showed a deep, irregularly configured trough with meandering tendrils. Acrylic IOL damage morphology consisted of an ameboid-shaped entry site without radiating fractures and mild posterior penetration. Poly(methyl methacrylate) IOL damage consisted of a shallow focal trough with radiating fractures. CONCLUSIONS: The silicone IOL polymer had the lowest threshold for laser-induced damage and greater linear extension of damage than the PMMA and acrylic IOL polymers. Poly(methyl methacrylate) and silicone polymers exhibited collateral damage or ejected particulates adjacent to the entry site, whereas the acrylic polymer showed a discrete locus of damage.

Principles of rat subcortical forebrain organization: a study using histological techniques and multiple fluorescence labeling.

In the present study, we introduce new views on neuro- and chemoarchitectonics of the rat forebrain subcortex deduced from traditional and current concepts of anatomical organization and from our own results. It is based on double and triple immunofluorescence of markers for transmitter-related enzymes, calcium-binding proteins, receptor proteins, myelin basic protein (MBP) and neuropeptides, and on histological cell/myelin stains. The main findings can be summarized as follows: (i) the dorsal striatum of rat and other myomorph rodents reveals a small caudate equivalent homotopic to the caudate nucleus (C) of other mammals, and a large putamen (Pu). (ii) Shell and core can be distinguished also in the 'rostral pole' of nucleus accumbens (ACC) with the calretinin/calbindin and neuropeptide Y (NPY) immunostaining. The shell reveals characteristics of a genuine striatal but not of an extended amygdala (EA) subunit. (iii) EA and lateral septum show striking similarities in structure and fiber connections and may therefore represent a separate parastriatal complex. (iv) The meandering dense layer (DL) of olfactory tubercle (OT) forms longitudinal gyrus- and sulcus-like structures converging in its rostral pole. (v) The core regions of the islands of Calleja that border the ventral pallidum (VP) sharing some of its features are invaded by myelinated fibers of the medial forebrain bundle (MFB). The island of Calleja magna is also apposed to an inconspicuous, slender dorsal appendage of VP. (vi) The VP is composed of a large dorsal reticulated part traversed by the myelinated GABAergic parvalbumin-immunoreactive axons of the MFB and a slender ventral non-reticulate part close to the islands of Calleja. (vii) Considering their close association to the limbic system, ventral striatum (VS) and VP may represent the oldest part of basal ganglia, whereas dorsal striatopallidal subunits were progressively developed in parallel to the growing neocortical influence on motor behavior.

Picodroplet-deposition of enzymes on functionalized self-assembled monolayers as a basis for miniaturized multi-sensor structures.

We are reporting on a novel approach for structured immobilisation of enzymes on gold surfaces modified with monolayers of functionalised alkylthiols. The formation of enzyme spots is achieved by shooting very small volumes of an appropriate enzyme solution (down to 100 pl) onto a thiol-monolayer modified gold surface using a micro-dispenser. Formation of enzyme patterns is obtained by moving the micro-dispenser relative to the modified gold surface using a micro-positioning device. Enzyme spots with typical lateral dimensions of 100 microm are obtained, but also, more complex structures, e.g. lines or meander structures, can be achieved by multiple droplets dispensed during the concomitant movement of the micro-dispenser. The first enzyme layer on top of the functionalised thiol-monolayer is subsequently covalently immobilised using either carbodiimide activation of carboxilic headgroups at the enzyme or via already introduced activated ester functions at the monolayer. Immobilised enzyme activities of glucose oxidase and lactate oxidase patterns have been characterised by means of scanning electrochemical microscopy. The product of the enzyme-catalysed reaction, H(2)O(2), is detected with an micro-electrode in the presence of either or both substrates, glucose and lactate, leading to a visualisation of the corresponding enzyme pattern and the lateral enzymatic activity.

Visualizing muscle cell migration in situ.

BACKGROUND: Cell migration has been studied extensively by manipulating and observing cells bathed in putative chemotactic or chemokinetic agents on planar substrates. This environment differs from that in vivo and, consequently, the cells can behave abnormally. Embryo slices provide an optically accessible system for studying cellular navigation pathways during development. We extended this system to observe the migration of muscle precursors from the somite into the forelimb, their cellular morphology, and the localization of green fluorescent protein (GFP)-tagged adhesion-related molecules under normal and perturbed conditions. RESULTS: Muscle precursors initiated migration synchronously and migrated in broad, rather than highly defined, regions. Bursts of directed migration were followed by periods of meandering or extension and retraction of cell protrusions. Although paxillin did not localize to discernible intracellular structures, we found that alpha-actinin localized to linear, punctate structures, and the alpha5 integrin to some focal complexes and/or vesicle-like concentrations. Alterations in the expression of adhesion molecules inhibited migration. The muscle precursors migrating in situ formed unusually large, long-lived protrusions that were polarized in the direction of migration. Unlike wild-type Rac, a constitutively active Rac localized continuously around the cell surface and promoted random protrusive activity and migration. CONCLUSIONS: The observation of cellular migration and the dynamics of molecular organization at high temporal and spatial resolution in situ is feasible. Migration from the somite to the wing bud is discontinuous and not highly stereotyped. In situ, local activation of Rac appears to produce large protrusions, which in turn, leads to directed migration. Adhesion can also regulate migration.
