The ophthalmologic manifestations of sinus histiocytosis with massive lymphadenopathy.

Of 113 cases of sinus histiocytosis with massive lymphadenopathy, 13 patients had ophthalmic infiltrates. Eleven of the 13 had infiltrates in the orbital soft tissues, and five of these patients also had eyelid disease. One patient had infiltrates only within the eyelid, and one without disease in the orbit or eyelid had extensive infiltrates in the uveal tract. The microscopic differential diagnosis included a variety of lymphoreticular malignancies, storage diseases, histiocytosis X, rhinoscleroma, tuberculosis, and inflammatory pseudotumor of the orbit. These 13 patients with ophthalmic disease were similar clinically and pathologically to patients with sinus histiocytosis with massive lymphadenopathy who did not have ophthalmic disease.

Clofazimine: a review of its medical uses and mechanisms of action.

Clofazimine has been in clinical use for almost 40 years, but little is known of its mechanism of action. The primary indication for clofazimine is multibacillary leprosy, but it is useful in several infectious and noninfectious diseases, such as typical myocobacterial infections, rhinoscleroma, pyoderma gangrenosum, necrobiosis lipoidica, severe acne, pustular psoriasis, and discoid lupus erythematosus. Postulated mechanisms of action include intercalation of clofazimine with bacterial DNA and increasing levels of cellular phospholipase A2. Clinical experience, possible mechanisms of action, and side effects of clofazimine are summarized.

[Nasal angiocentric T cell-natural killer cell lymphoma with pancreatic localisation in a child].

The nasal angiocentric T-cell-natural killer cell lymphoma is an aggressive tumor, exceptional in Tunisia, which is observed especially in Asia, Mexico and South America. We report the case of an 11-year-old boy, presenting with an exuberant and ulcerative tumor of the right nasal pit. Radiological investigation evidenced a right ethmoïdo-frontal tumor. Evolution was characterized by the installation of abdominal pain and fever, the evidenciation of a mass within the head of the pancreas that compressed the extra hepatic biliary duct and Wirsung's canal and was associated to hyperamylasemia. Biopsy under ultrasound revealed a histiocytic nature. Nasal biopsy found histiocytic and lymphocytic cells and led to a diagnosis of rhinoscleroma. The absence of improvement under antibiotic treatment and the development of fever with leuconeutropenia oriented towards the diagnosis of a non-Hodgkinian malignant lymphoma. Medullary biopsy and immunohistochimic study confirmed a T/NK cells lymphoma, with medullary invasion. The child died from acute respiratory distress syndrome. The authors insist on the rarity of this bipolar location, in particular in children, emphasize the difficulty of the diagnosis and review the literature.

A recent immunological concept of some nasal diseases.

In this study, we have attempted to summarize and clarify the concept of nasal permeability to antigens such as inhaled particles, bacterial breakdown products and medicaments. The mature epithelium retains the capacity to allow macromolecules to penetrate by a pinocytotic mechanism, which is more pronounced during the neonatal period. The vast majority of individuals have no ill effects from the nasal transport of large molecules. However, when increased quantities of toxic or antigenic macromolecules gain access to the body because of a derangement in the protective mucosal barriers (namely: the Secretory IgA (SIgA), the mucous plug with the cilia, the intracellular digestion or interstitial immunoglobulins and macrophages), antigen absorption may be altered and result in either local or systemic disorders. The speculative concepts suggesting that clinical disease-states (e.g. allergy, atrophic rhinitis, rhinoscleroma, infection and malignancy), may be associated with altered mucosal permeability have also been discussed.

Scleroma of the lower respiratory tract: a bronchoscopic study.

Twenty cases suffering from rhinoscleroma were studied. Bronchoscopy was performed for detection of secondary involvement of the subglottic region, trachea and bronchus. The study revealed that the larynx was involved in 40 per cent, the trachea in 20 per cent and the bronchus in 10 per cent of cases. The intranasal lesion, endotracheal and endobroncheal lesions all occur in the form of an atrophic stage, granulomatous (proliferative) stage and finally a fibrotic or scarring stage. The role of bronchoscopy in early diagnosis, initial symptomatic treatment and in follow-up is discussed in the light of the available literature.

[Training of Polish physicians in rhinolaryngology in XIX century Vienna].

The eminent Viennese rhinolaryngologists in the XIX century were: Leopold Schroetter, Karl Stoerk, Johann Schnitzler, Ottokar Chiari, Markus Hajek and others. Stroerk, the pupil of Ludwig Türck was a head of rhinolaryngological clinic since 1891, he was also the author of valuable papers, among other things on rhinoscleroma. Schnitzler, the assistant of Johann R. von Oppolzer was a head of Viennese polyclinic. The therapeutical nihilism in Viennese clinics is mentioned. Ottokar Chiari, the assistant of Schroetter was an eminent rhinolaryngologist. Markus Hajek was a head of otorhinolarynoglogical clinic in Vienna performed in 1919. Theodor Billroth, professor of surgery in Vienna was the first to remove the larynx for cancer in 1873.

[Immunobiological properties and therapeutic effectiveness of preparations from Klebsiella bacteriophages].

The purified preparations of Klebsiella bacteriophages, viz. the monovalent preparation of K. pneumoniae bacteriophage and the polyvalent bacteriophage preparation for the treatment of infections caused by K. ozaenae, K. rhinoscleromatis scleromatis and K. pneumoniae sensu lato, have been obtained. The bacteriophage preparations have proved to be nontoxic and safe for laboratory animals after the intraperitoneal injection of these preparations followed by the pathomorphological study of the internal organs of the animals. The clinical study of the newly developed bacteriophage preparations in the course of the treatment of purulent inflammatory diseases in 109 patients has revealed that the preparations are not reactogenic and exhibit sufficient effectiveness in the therapy of ozena, rhinoscleroma and Klebsiella infections with different localization of the infectious process.

[Cervico-facial and ENT symptoms due to HIV infection in tropical area. About 253 Congolese cases].

In human immunodeficiency virus infection as well as in related syndromes, cervico-facial and otorhino-laryngologic manifestations are current. A retrospective study in Oto-Rhino-Laryngology service of Brazzaville University Hospital from December 1995 to November 2001 has been reported. 253 patients were selected from a total of 1352 consultations. The study population is young (average age: 34 +/- 4.8 years), and most of the patients are 30 to 49 years old (75.9%). Men represent 51% and women 49%. Although sexual multipartnership (59.7%) is the main risk factor, traditional practices (22.8%) are not neglictible in Africa. Among all cases, human immunodeficiency virus type 1 is found in 72.3% of cases. The affections are located in the neck (40.5%), ear (24.9%), pharynx (17.3%), rhinosinus (13.3%), oral cavity and vestibule (2.7%) and larynx (1.3%). These main affections are represented by: parotidosis (20.1%), peripheric facial paralysis (15.4%), pharyngeal candidiasis (14.6%), sinusitis (14.2%) and ganglial tuberculosis (11.5%). Lymphoma (7 cases), kaposi's sarcoma (7 cases) and epidermoid carcinoma (1 case) are the malignant affections identified in 15 cases (6%). The diagnosis of some affections like cystic parotiditis (11%), noma (1.6%), African histoplasmosis (0.4%) and rhinoscleroma (0.4%) constitutes this study particularity The Oto-rhino-laryngologist's role is important in early diagnosis of HIV infection as well as in the followed-up of patients.

[Analysis of 60 masses in the nasal vestibule].

OBJECTIVE: To summarize the treatment strategy of nasal vestibule masses. METHODS: Between May, 1982 and April,2003,60 patients with nasal vestibule mass were treated in our department. The clinical features of these masses were retrospectively analysed. RESULTS: Among 60 masses, 51 cases were benign tumors, 5 cases were inflammatory masses, and 4 cases were malignant tumors. All patients were surgically treated and discharged with complete recovery. Among patients with benign tumor and non-specific inflammatory mass, 37 patients with a median follow-up of 97 months without recurrence and malignant change. One patient with hidradenoma and one patient with rhinoscleroma recurred and another surgery was carried out. There was no recurrence after second surgery. 6 patients died of other diseases, and 11 patients lost follow-up. Two patients with malignant melanoma recurred after first surgery. Then the tumor was removed via lateral rhinotomy and died of local recurrence and liver metastasis. Two patients (one with papilloma carcinomatous change and another with non-Hodgkin lymphoma) were free of diseases 17 years or 5 years and 2 months respectively. CONCLUSIONS: Masses of different histological types can be found in the nasal vestibule. Surgical removal is the treatment of choice. The approaches are determined on the basis of site, size and kind of the masses.

[Forty-three tracheal stricture with Ni-Ti shape memory alloy stent].

OBJECTIVE: To evaluate the application of Ni-Ti shape memory alloy stent for treating tracheal stricture, including its indications and complications. METHODS: Forty-three patients clinical data were analyzed, among them, 15 suffered from tumor; 7 trauma; 4 granulation; 1 Wegener's granuloma, 2 rhinoscleroma, 1 without clear diagnosis; 7 were relapsing polychondritis; 1 were respiratory amyloidosis; 4 were tracheobronchial tuberculosis, 5 were tracheobronchomalacia. The stricture degree: Level I: 15 patients, Level II:24 patients, Level III:4 patients. The location of stricture in 5 patients extended to the subglottis area. All patients had tracheal stricture and accepted the treatment with Ni-Ti shape memory alloy stent. RESULTS: Forty-two patients breathed freely after the operation, 1 patient died after surgery. The tracheostoma in 18 patients was closed 34 days after operation. Among the remaining 8 patients with tracheostoma:3 patients left hospital with tracheal cannula; 1 patient died,2 patients were found stent shifted and no evident breath improving; 2 patients were found granulation and had secondary operation with laser. CONCLUSION: Ni-Ti shape memory alloy stent can expand the tracheal stricture quickly and thus improve the breath effectively, which is minimally invasive and simple. It can be used as a method in the treatment of tracheal stricture.

[First case of rhinoentomophthoromycosis observed in the People's Republic of the Congo (Congo Brazzaville)].

The first case of rhinoentomophthoromycosis to be recognised in the People's Republic of the Congo (Congo Brazzaville) is reported. The patients was a 55 years old congolese male. The early symptoms consist of a progressive nasal obstruction with a massa inside of the nostril, initially diagnosed as a rhinoscleroma. One year later the patient had a prominent distorsion of the face and the histological lesions were characteristic of an entomophthoromycose. Cultures were positive for Conidiobolus coronatus.

A peculiar lymph node anti-carcinoma reaction.

A case of a laryngeal extensive carcinoma determining a severe obstruction is reported, where the development of a monstrous adenopathy is followed by the regression of the initial tumour until complete laryngoscopical disappearance, as well as of obstructive and general symptoms. Cytological and histological examinations of the above-mentioned lymph node demonstrated (besides metastatic cells) a peculiar cellular reaction, similar to rhinoscleroma, which represented an original anti-carcinoma defensive mode. The medical importance of this very rare case is discussed and the probable immune origin of this reactive cellular complex is explained by analogy with "Hodgkinom", where granuloma are the immune response of the body against Sternberg-Reed cell.
