Epidermal growth factor in the healing of corneal wounds, epidermal wounds and partial-thickness scalds. A controlled animal study.

The effects of the topical application and the intraperitoneal administration of epidermal growth factor (EGF) on the healing of epidermal wounds, and of partial-thickness scalds in rats and of corneal wounds in rabbits were evaluated. The parameters measured were the daily percentage body weight change, time to complete healing of the wounds, the evaporative water loss from the wound surface and the rate of new formation of epithelial cell layers on the epidermal and corneal wounds. Wounded animals treated with 0.9% NaCl were used as controls. After removal of the corneal epithelium the local application of EGF stimulated the regeneration process appreciably. No significant healing advantage from the treatment with EGF was observed, however, in epidermal wounds and partial-thickness scalds. The result does not warrant any clinical evaluation.

Adenosine triphosphate bioluminescence as a method to determine microbial levels in scald and chill tanks at a poultry abattoir.

According to Hazard Analysis of Critical Control Points (HACCP) programs developed for the poultry industry, poultry processing waters should be actively monitored to minimize cross-contamination between chicken carcasses. In order to monitor HACCP programs, a test is required that provides results on a real time basis. A modified adenosine triphosphate (ATP) bioluminescence test has been developed that can assess microbial levels in poultry processing waters within 15 min. A study was conducted to determine the effectiveness of this test for examining scald, prechill, and chill tank waters. The results showed that the modified ATP bioluminescence method gave results comparable to plate counts. The microbial levels were dependent on the tank and the time of sampling. The highest microbial levels were detected in the scald tank. In all three tanks, the microbial levels increased over time during the day.

[A sudden death case in the bath with peculiar scald].

A 60-year-old man was found dead in the bathtub of his house on the 2nd of February. Erythema with a clear border was observed on the right side of the face, trunk and around the knees, though the left side of the face, the hips and the feet were normal. The water heater was set outside of the bath, and the heated water flowed from the heater to the bath through the upper pipe. In order to determine the mechanism of the peculiar scalding, changes in the water temperature in the bathtub were measured under the same conditions. An hour after turning on the water heater, the surface water temperature of the bath was 73 degrees C. Zonal gradation of the temperature was observed. At the level of the opening for the lower intake pipe, it was 50 degrees C. The border of the scalding was consistent with the water layer above 50 degrees C.

[The effects of zinc on liver metallothionein in scalded rats].

OBJECTIVE: To investigate the changes in liver metallothionein (MT) in scalded rats. METHODS: The scalded rats in deficient zinc condition were fed with diets containing zinc 4 micrograms/g, 10 micrograms/g, and 80 micrograms/g respectively. The changes in serum zinc, liver zinc and liver MT were observed. RESULTS: The serum zinc decreased, liver zinc and MT increased. The higher diet zinc levels were, The higher serum zinc, liver zinc and MT became. The correlation analysis showed that the changes between liver zinc level and liver MT was positive relationship(r = 0.85, P < 0.01). CONCLUSION: Serum zinc decrease, liver zinc increase after burn. This is connect with increment of MT synthesis in liver, zinc is inducer to MT. It can improve synthesis of liver MT.

[The effects of resuscitation with hypertonic saline solution on the postburn pulmonary injury in scalded dogs].

OBJECTIVE: To explore the preventive and curative effects of resuscitation with hypertonic saline solution on postburn pulmonary injury in scalded dogs. METHODS: Mongrel dogs inflicted by 40% TBSA III degree scald were employed as the model. Sixteen dogs were randomly divided into two groups with 8 in each group, i.e. hypertonic saline solution group (H group) and isotonic saline solution (N group). The changes in cardiac output (CO), cardiac index (CI), pulmonary arterial pressure (PAP) and pulmonary vascular resistance (PVR) were determined within 48 postburn hours (PBHs). Pulmonary tissue samples were obtained for the measurement of pulmonary water and hydrocarbon (C2 H4 and C2 H6) contents. Bronchoalveolar lavage fluid (BALF) was harvested for white blood cell (WBC) counting and classification and for the detection of the changes in PLA2 activity and LTC4 content. RESULTS: Compared to those in N group, the CO and CI in H group increased significantly while the pulmonary water content, the hydrocarbon content, the WBC count, and PLA2 activity and LTC4 content in BALF all decreased obviously. CONCLUSION: Hypertonic saline solution might be beneficial in the management of early postburn pulmonary injury.

[Effects of nitric oxide (NO) on the cardiac function in severely scalded rats].

OBJECTIVE: To investigate the effects of NO on the cardiac function in severely scalded rats and to elucidate its mechanism. METHODS: Ninety Sprague-Dawley (SD) rats inflicted with 30% TBSA III degree scalding were employed as the model and divided randomly into five groups, i.e. burn (B), shamburn (SB), L-arginine (L-Arg), N-nitroso-L-argininemethylester (L-NAME) and L-Arg + L-NAME groups. Cardiac functional indices, myocardial content of malondialdehyde (MDA) and myocardial Ca (calcium) transport were examined in the rats of all the groups, respectively. RESULTS: Compared to those in B group, the mean arterial pressure (MAP), left ventricular summit pressure (LVSP), the +/- dp/dt max in left ventricle pressure and the cardiac Lo in L-NAME group increased obviously. While those in L-Arg group decreased significantly but those in L-Arg + L-NAME group exhibited no remarkable change. The MDA content in L-NAME group was evidently lower than that in B group, while the calcium transport index of sarcoplasmic reticulum in L-NAME group increased obviously. CONCLUSION: Nitric oxide, massively produced postburn, exerted inhibitory effects on cardiac function by means of the cytotoxicity of its owns and other free radicals, which might be closely related to the decrease of its calcium transport function of sarcoplasmic reticulum.

[The effects of rapid fluid replacement on the hemodynamics of scalded shock dogs with delayed resuscitation].

OBJECTIVE: To investigate the effects of rapid fluid replacement on the hemodynamics of scalded shock dogs with delayed resuscitation and to explore the protocol for the correction of postburn shock in case of delayed resuscitation. METHODS: Mongrel dogs inflicted with 40% TBSA III degree scalding were employed in the study. Twenty -- four dogs were randomly divided into 3 groups, i.e. thermal injury control group (C), delayed even fluid infusion group (E) and delayed rapid fluid infusion group (R). The femoral arterial pressure (FAP), pulmonary arterial pressure (PAP), pulmonary vascular resistance (PVP) and systemic vascular resistance (SVR) were monitored before and 2, 6, 8, 12, 24, 36 and 48 hours after injury. RESULTS: (1)All The dogs in C group died within 36 postburn hours (PBHs), while those in E and R groups survived more than 48 PBHs. (2) FAP and CO in all the dogs decreased obviously postburn while SVR increased evidently. After fluid infusion, FAP and CO bounced back while SVR decreased, especially in R group. (3) PAP, PAWP, CVP and PVR recovered rapidly but within normal range in R group. CONCLUSION: Rapid fluid resuscitation was beneficial and feasible for the correction of burn shock due to delayed resuscitation.

[The significance of the expression of the HSP70 and HSP90 in the intestinal mucosa in scalded rats during early postburn stage].

OBJECTIVE: To investigate the changes in the expression, the tissue contents and the distribution of heat shock protein (HSP) 70 and HSP90 in the intestinal mucosa in scalded rats during early postburn stage and to elucidate the effects of heat shock response of intestinal mucosal cell on burn victim's systemic pathophysiology. METHODS: The scalded rats were taken as the model. ELISA, immunoblotting analysis and immunohistochemical methods were employed to analyze and study the expression, the tissue contents and the distribution and their functional status of HSP70 and HSP90 in the intestinal mucosa. RESULTS: (1) The free HSP70 content in the intestinal mucosa exhibited significantly transient decrease in the early postburn stage. (2) The total contents of HSP70 and HSP90 in the intestinal mucosa increased obviously postburn. (3) There exhibited remarkable heterogeneity of the molecular structure of HSP70 in the intestinal mucosa in the early postburn stage. CONCLUSION: The expression and the changes in the contents and the tissue distribution of the two HSPs in the intestinal mucosal cell might play important roles in the cellular stress reaction of the intestinal mucosa and even in the intestinal barrier functional mechanism.

[The effects of escharectomy en masse during shock stage on the secretion of cytokines from macrophage in scalded rats].

OBJECTIVE: To explore the effects of the plasma from rats undergoing escharectomy en masse during shock stage on the secretion of TNFalpha, IL-6 and IL-8 by macrophage. METHODS: Rats inflicted by 30% III degree scald were employed as the model. Sixty-three rats were randomly divided into escharectomy (E) and scald (C) groups. The plasma contents of TNFalpha, IL-6 and IL-8 in rats in two groups were determined. The plasma from rats in two groups was respectively co-cultured with rat peritoneal macrophages for 6 hours and the TNFalpha, IL-6 and IL-8 in the supernatants of the culture were assayed. RESULTS: Plasma contents of TNFalpha, IL-6 and IL-8 were evidently increased in both E and C groups. But the contents of these cytokines in E group decrease and were much lower than those in C group at 12 postburn hour. The sera from two groups of rats could all stimulate the secretion of TNFalpha, IL-6 and IL -- 8 from peritoneal macrophages, but the serum from C group possessed stronger stimulating ability than that from E group. CONCLUSION: Escharectomy en masse during shock stage could weaken the ability of burn sera to induce the secretion of TNFalpha, IL-6 and IL-8 by macrophages. It was therefore beneficial to the prevention of postburn development of systemic inflammatory response syndrome.

[The dynamic changes of serum TNFalpha level in scalded rats after the administration of rhGH during the early postburn stage].

OBJECTIVE: To examine the dynamic change of serum TNFalpha level in scalded rats after the administration of rhGH during the early postburn stage. METHODS: Forty-two male sanitary Sprague-Dawley rats were randomly divided into three groups, i.e. normal control (N), burn with early administration of rhGH (BA) and burn without rhGH (BN) groups. The rats were inflicted with 25% similar 30% TBSA deep partial thickness scalding on the back. rhGH in dose of 6 U/kg was administered subcutaneously via abdominal skin starting from the 1st postburn day (PBD). Rat serum TNFalpha levels were determined by ELISA in BA and BN groups at 3, 6 and 10 PBDs. RESULTS: The rat serum TNFalpha level in BA was lower than that in BN groups on 3 PBD. Whilst the TNFalpha levels in BA were much higher than those in BN groups on 6 and 10 PBDs. CONCLUSION: Administration of rhGH at early postburn stage might abate the early systemic postburn reaction.

[The effect of heparin on the deep second-degree burn in scalded rats].

OBJECTIVE: To investigate the effects of heparin on deep second-degree burn in scalded rats. METHODS: Thirty Sprague-Dawley rats inflicted with 20% TBSA deep second-degree scalding on the back were employed as the model and were randomly divided into two groups. i,e heparin (H) and normal saline groups (N). Heparin (100 U . kg(-1) . day(-1)) was injected subcutaneously to the rats in H group while normal saline to those in N group once a day. Wound healing rate and immunological indices were determined simultaneously. RESULTS: (1) The wound healing time was much shorter in H group than that in N group (P < 0.005). The granulation tissue growth condition examined by LM and EM was much better in H group that than in N group. (2) The lymphocytic transforming ratio in H group was evidently increased (P < 0.005). And the phagocytic function of the macrophages increased significantly (P < 0.05). But there was no difference between the two groups in terms of hemoltytic plaque test (P > 0.05). CONCLUSION: Injection of heparin subcutaneously could accelerate burn wound healing and strengthen the systemic cellular immune potential and the macrophage phagocytic capacity of the host, but exerted no effect on the humoral immunity.

[The Influence of stress inhibition on the plasma levels of LPS, pro-inflammatory and Th1/Th2 cytokines in severely scalded rats].

OBJECTIVE: To explore the influence of stress inhibition on the plasma levels of LPS pro-inflammatory and Th1/Th2 cytokines in severely scalded rats. METHODS: Sprague-Dawley (SD) rats inflicted by 30% TBSA of deep partial thickness burn were employed as the model and randomly divided into burn with immediate resuscitation (A) and burn with immediate resuscitation and soluble cocktail (B) groups. Plasma was harvested from peripheral blood at different postburn time points for the determination of the levels of LPS, IL-1alpha, IL-6, TNFalpha, IL-8, IL-2, IFN-gamma, IL-4 and IL-10 in the rats of the two groups. And the rats inflicted by sham scalding were taken as control group (C). RESULTS: The postburn plasma levels of LPS, IL-1alpha, IL-4 and IL-10 increased gradually, while the plasma levels of IL-6, TNF, IL-8, IL-2 and IFN increased initially and decreased thereafter. The increasing ranges of LPS and these inflammatory cytokines were higher in A group, in which the increases of IL-1alpha, IL-6 and IL-4 appeared earlier in A (6 PBH) than those in B (12 PBH) groups. CONCLUSION: Prompt fluid resuscitation and stress inhibition could delay and ameliorate the postburn inflammatory reaction, decrease the production of Th2 cytokine and partially restore the production of Th1 cytokine after 48 PBH.

[The effects of gut -- origin lipopolysaccharide translocation on the apoptosis of lymphocytes in scalded rats].

OBJECTIVE: To investigate the relationship between lymphocyte apoptosis in spleen and mesenteric lymph nodes (MLNs) and gut -- origin lipopolysaccharide (LPS) translocation after severe scalding in rats. METHODS: Wistar rats inflicted 30% TBSA III degree scalding were employed as the model. Serum LPS levels in portal vein and vena cava were determined by limulus amebocyte lysate (LAL) test. Apoptotic lymphocytes in paraffin -- embedded spleen and MLNs sections were detected with TdT -- mediated and dUTP nick end labeling (TUNEL) and peroxidase (POD) staining. The images were taken by cooldccd camera system, and the count and optical density (OD) value of apoptotic lymphocytes were analyzed with software Spot and Imagine proplus 4.10a (IPP4.10a). RESULTS: Serum LPS levels in portal vein and vena cava increased sharply in rats postburn (P < 0.01) and reached a peak value at 6 postburn hour (PBH). LPS level in portal vein was higher than that in vena cava (P < 0.01) in the period of 3 to 48 PBHs. The results of TUNEL -- POD staining and the count of apoptotic lymphocytes showed that the apoptotic cells were few in normal MLNs, but increased obviously postburn (P < 0.01), especially at 3 PBH. On the contrary, apoptotic lymphocytes were concentrated in spleen cortex before injury and decreased obviously postburn (P < 0.01), though a slight recovery was found after 24 PBH. Optical density of apoptotic lymphocytes was significantly reduced in spleen at 24 PBH, But the OD value of apoptotic lymphocytes in MLNs increased significantly within 48 PBHs when compared with that before injury (P < 0.01). CONCLUSION: Gut -- origin endotoxemia in rat could occur in early postburn period. The LPS translocation might induce lymphocyte apoptosis in MLNs. The decreased apoptotic lymphocytes in spleen implied abnormal immune function in spleen.

[The influence of escharectomy during shock stage on skeletal muscle proteolytic rate in severely scalded rats].

OBJECTIVES: To observe the changes in proteolytic rate and the influence of escharectomy and skin-grafting during shock stage on the proteolytic rate of skeletal muscle in severely scalded rats, and to investigate necessity of escharectomy during shock stage. METHODS: Wistar rats afflicted with 30%TBSA (total body surface area) full-thickness burn on the back were randomly divided into 3 groups: burn injury group (group B), escharectomy during shock stage group (group S), and escharectomy after shock stage group (group N). Escharectomy and skin-grafting was performed on the postburn day 1 (PBD1) and PBD4 in group S and N. The extensor digitorium longus (EDL) muscle was collected using a technique without injury to the muscle, and the proteolytic rate of the EDL was determined by the in-vitro muscle incubation system with sufficient oxygen supply and amino acid automatic analyzer. RESULTS: The total proteolytic rate (TPR) in rat EDL muscles increased significantly (t > 2.988, P < 0.01) on PBD1, but no prominent changes were observed at other time points. There were no statistically significant changes in TPR between group S and group N, but the myofibrillar proteolytic rate (MPR) in rat EDL muscles increased markedly at each time point after scald (t > 2.988, P < 0.01). The MPR in group S decreased significantly as compared with that in group N on PBD2, 4, 5 (t = 2.311, P < 0.05; t = 3.054, 3.319, P < 0.01), and the mean values were 3.17, 2.33, 1.75 nmol/wet weight in g/3 h, respectively. No significant changes were observed between group N and S. CONCLUSION: The high protein catabolism of skeletal muscle can be alleviated by escharectomy and skin-grafting during shock stage, and it is beneficial to organics.

[An experimental study of the intracellular distribution and the activation of mitogen-activated protein kinases in myocardial cells in scalded rats].

OBJECTIVE: To investigate the intracellular distribution and the activation of mitogen-activated protein kinases (MAPKs) in myocardial cells in scalded rats. METHODS: Wistar rats were used in this study and forty of them were inflicted with 40% III degree scald on the back and eight normal ones as control. The samples of plasma and myocardial tissue were harvested at 1, 3, 6, 12 and 24 postburn hours (PBHs), and samples were also obtained from normal rats as control. The plasma CK-MB activity was determined by routine method. The activation states of all the members of MAPKs [p38 kinase, ERKs (extracellular signal-regulated protein kinases) and JNK (c-Jun N-terminal kinase)] in the myocardial tissue samples were detected by Western blotting. The tissue slides were stained by immunohistochemistry methods. RESULTS: Activation of p38 kinase and ERK with nuclear translocation was found postburn, especially during 1, 3 and 6 PBHs (P < 0.01). But there was no activation of JNK during 1 - 24 PBHs. The plasma CK-MB content increased at 3 PBH and reached summit at 12 PBHs (P < 0.05 - 0.01). CONCLUSION: p38 kinase and ERK signal pathways might play important roles in the early postburn injury of myocardial cells, inducing myocardial injury.

Effects of countercurrent scalding and postscald spray on the bacteriologic profile of raw chicken carcasses.

In June and September 1988, the USDA Food Safety and Inspection Service sampled raw chicken carcasses at a federally inspected slaughter establishment in Puerto Rico to determine the effects of changing the scalding equipment on bacterial contents of raw poultry products. The scalding equipment was changed to a countercurrent configuration, with a postscald hot-water rinse cabinet that sprayed carcasses as they exited the scalder. Analysis of 250 carcass-rinse samples collected at preevisceration, prechill, and postchill sites over 7 days indicated that carcasses had mean aerobe plate counts of log(10)3.73 before evisceration, 3.18 before chilling, and 2.87 after chilling; Enterobacteriaceae counts of log(10)2.70 before evisceration, 2.25 before chilling, and 1.56 after chilling; and Escherichia coli counts of log(10)2.09 before evisceration, 1.61 before chilling, and 0.89 after chilling. Salmonellae were found on 24% of the carcasses before evisceration, on 28% before chilling, and on 49% after chilling. Although bacterial count reductions were significant at all 3 sites, the proportion of carcasses contaminated with salmonellae in this study was higher at the postchill than prechill site (49 vs 28%). This no doubt was caused by cross-contamination in the chiller. These percentages indicated that although simple scalder changes contributed substantially to the improvement of the bacterial quality of chicken carcasses, additional interventions in the chilling process (such as chlorination of chill water) are important to control cross-contamination and to preserve the positive effects obtained by the scalder changes.

[An experimental study on the change in neuropeptide substance P in the burn wound skin in scalded rats].

OBJECTIVE: To investigate the change in neuropeptide substance P (SP) in the burn wound skin in scalded rats. METHODS: The SP distribution density in the rat skin samples from the burn wound, peri-wound area and remote area of scalded rats were determined by immunohistochemistry technique. The mRNA expression of SP was observed by in situ hybridization. RESULTS: The distribution density of SP in the skin samples from the above three areas decreased obviously since 15 mins postburn, dropped to the lowest level during 6 - 12 postburn hou (PBH), and then recovered thereafter. Comparatively, the recovery in peri-wound area occurred earlier. In addition, there was emigration of macrophage-like large cells with positive SP immunoreactivity from local blood vessels in the dermal layer of the burn wound and peri-wound areas. The large cells were closely related to the local SP-containing nerve fibres at 12 PBHs. Intensified staining of the cells was identified at 24 PBHs with the disintegration of the cells into pieces with release of large amounts of positive SP immunoreactive granules, followed by disappearance of the cells at 48 PBHs. It was indicated by in situ hybridization that the preprotachykinin A (PPTA) mRNA encoding and synthesizing SP was expressed in various cells at 6 PBHs. CONCLUSION: SP might be synthesized and released from cutaneous nerve ending or from local inflammatory cells after skin burn injury.

Rectal temperature changes of Wistar rats with second-degree scald burn in hot and humid environment following immediate cooling therapy.

OBJECTIVE: To observe the changes in rectal temperature (Tr) after immediate cooling therapy for Wistar rats with superficial second-degree scald burn in hot and humid environment, and evaluate the effect of the dressing material for cooling. METHODS: Twenty-four Wistar rats were randomly divided into 4 groups (n=6), namely normal temperature control (NTC), normal temperature cooling therapy (NTCT), hot and humid control (HHC), and hot and humid cooling therapy (HHCT) groups. Superficial second-degree scald burns were induced in the rats, followed by interventions with cooling therapy in the two therapy groups at dry bulb temperature (Tdb) of 35.33+/-0.35 degrees Celsius with relative humidity of 70.81%+/-1.38%, whereas the two control groups were treated at Tdb of 26.33+/-1.29 degrees Celsius with relative humidity of 71.05%+/-4.57% without dressing for cooling therapy. The exposure time of each group was 120 min, and the Tr was recorded every 20 min. RESULTS: On the basis of comparisons between the measurements taken at 7 different time points, we found that the Tr of the rats was elevated in hot and humid environment (P<0.001) and decreased when cooling therapy was applied (P<0.001). Interactions between the environmental temperature and cooling therapy were noted in their influence on Tr (P=0.003). As the exposure time was prolonged, Tr slowly decreased in NTC group, mildly fluctuated in NTCT group, but elevated in HHC and HHCT groups with gradual increase of the differences between the measurements taken at the same time point. CONCLUSION: Application of the dressing material on the abdomen for cooling therapy can efficiently lower the Tr, which may prevent the progression of the heat injury.
