Folie à deux: an old diagnosis with a new technology.

The implantable cardioverter defibrillator (ICD) is the treatment of choice for life-threatening ventricular tachyarrhythmias. Despite this, indications for explantation have been described. Psychiatric complications have been considered in the decision to not re-implant a previously explanted device but never as a primary indication for explantation. The case of a woman who requested ICD explantation amidst concerns that the titanium used in its construction was absorbing harmful negative electromagnetic field energy from the environment resulting in intermittent bouts of fatigue, diaphoresis and an overall sense of impending doom is presented. She and her husband had significant psychosocial impairment because of this belief system. The present case is the first known report of a psychiatric complication as the primary indication for ICD explantation. It is illustrative of diagnostic dilemmas, competency and the importance of quality of life.

Relationship patterns in "folie à deux".

"Folie à deux" is characterized by the communication of delusional ideas from one subject to other ones who have been living closely with him for a long time, usually his relatives. Before illness shows, there is a leader-follower relationship between partners, who are lacking of external sources of pleasure and are socially or culturally isolated. The dominating partner usually is a paranoic subject who strongly needs his ideas to be accepted by other people, while the dominated partner is a dependent subject who shares the other one's ideas in order to get pleasure from him and who is not able to criticize these ideas because of the lack for external influences. The two partners project hostility into the external world because, if the dominated partner does not share the other one's ideas, hostility will be projected into him and he will become very anxious. Interpersonal mechanisms in "folie à deux" are similar to those which take place in brainwashing, hypnosis and psychotherapy.

Cocaine abuse and delusions of parasitosis.

We report two persons in their thirties with the shared delusion of parasitosis. Both abused cocaine, and both recovered from their delusions after a period of abstinence. Although the literature does not stress that a specific cause is usually found for delusions of parasitosis, this may be changing for persons in groups among whom cocaine and possibly other illicit drug use is common. It is a service to these desperate patients to establish their confidence and then discretely ask about the use of illegal drugs, especially cocaine.

[2 psychoses and a symbiotic link].

On the basis of the pathology described by Lasègue & Falret, this papers intends to further a psychoanalytic distinction between both types of psychosis which exemplify 'folie à deux'. Hypothesis are proposed which refer to the difference in the psychic structure, in view of the 'body into pieces' experience in Type I delirium cases and a mainly neurotic organization in Type II delirium cases. A case in brought up where part of the interview reveals the rigid as well as sealed bond organization between significant and meaning. Reference is also made to the qualities of symbiotic attachment between both partners in the delirious intercourse, and to the importance the consideration of this diagnostic difference has with regards to the therapeutic strategy.
