Riding the waves of change in health system material management: team building in times of uncertainty.

Within the health care industry, reorganization brings new challenges and opportunities for improving work-related processes. This article examines one Midwestern health system's response to the systemic change that may occur after restructuring. The consolidation of diverse and separate material management departments within one integrated material management division prompted a concerted team development and training effort. In this model, management and non-management staff worked collaboratively to achieve a unified work group that will continue to evolve and grow with the needs of the organization.

[A report on staff development in advanced knowledge and technology of gene diagnosis for medical technologists by the Japanese Association of Medical Technologists: the view of medical technologists].

With advancement of the molecular biology, gene diagnosis is widely utilized in clinical application of medicine. For medical technologists, it is necessary to receive continued education to practice such advanced scientific trends. The Japanese Association of Medical Technologists established a working group for a staff development program of gene diagnosis and chromosome analysis in 1996, and has continued its activities in making inquiries about the present conditions, publishing a textbook, and providing seminars. The internal laboratory utilization of gene diagnosis was 8.7%(213 of 2437 hospitals). Based on the fact that about half of the hospitals use external laboratory, staff development for the internal utilization of gene diagnosis is an urgent issue. We have been providing seminars to meet the educational needs of our members. In addition to those activities, we have continued our efforts in providing a manual with clinically useful information, standardizing methods, establishing an information network, and conducting a controlled survey. The role of the working group is now shared by the local prefecture to further increase the numbers of those with expertise in gene diagnosis. We, medical technologists, need to have a global view of professional growth, and also to cooperate with academic societies related to gene diagnosis to establish a certification system.

Dollars, debts and duties: lessons from funding Australian general practice.

This study investigated changes in resource allocation and activities of Australian Divisions of General Practice associated with new funding procedures which link monies to nominated outcomes. The study involved analysis of annual reports and strategic plans, and semistructured telephone interviews with key personnel from 27 divisions of general practice. The main outcome measures were: number of activities in various nominated health areas; total and median expenditure per activity in each area; and methods of resource allocation. Despite a modest increase in funding to the total general practice divisions program over the two year period, expenditure decreased in the National Health Priority Areas of mental health, diabetes, cardiovascular disease, injuries and cancer. There was increased expenditure in the priority area of immunisation, which received dedicated funding. There was greatly increased expenditure in the areas of information technology and services to GPs (including continuing medical education, professional development and workforce issues). The ease of defining and measuring outcomes influenced the choice of activities. In 1996, activities were linked to formal needs analyses in approximately 20 per cent of cases. The most frequent driving force for projects was enthusiastic GPs. In 1998, resource allocation decisions were more explicitly linked to formal needs analyses; however, the standard of the needs analyses varied widely between divisions. Changes in funding procedures which use nominated outcomes as the major accountability mechanism may produce unexpected, and unintended results, including significantly decreased expenditure in areas with outcomes which are hard to define and measure but which are important for health improvement.

A model for the long-term planning of physician workforce in Hungary.

A computer programme for the long term (25 years) projection of the physician workforce was developed. The input variables were the admittance rate of the medical schools and the unemployment rate of the active physicians, the output variables were the number of active physicians. The age and sex structure, the retirement age were taken into account. Different scenarios were made to compare the impact of the output of medical faculties and the unemployment rate of doctors on the number of active of the physicians in five years intervals for a twenty-five years period. The input of the young graduates varied between 800-600/year, the unemployment rate between 4.5% and nil. At end of the 25 years projection period the reduction of physicians was between 9.6 and 25.0%. The aim was to find a professionally acceptable, socially tolerable and politically eligible optimal relationship among the three variables.
