Interim overdentures.

The construction of an interim overdenture using existing removable partial dentures with natural tooth crowns and artificial teeth can be a simple and economical method of providing patients with dentures while tissues heal and teeth are prepared and restored. A more definite prognosis for both the patient and his remaining dentition can be established before the final overdenture is completed. The procedures necessary to provide three types of interim overdentures have been outlined. Patients tolerate this method of changing their dentitions extremely well.

Custom characterization of the provisional restoration.

The fabrication of esthetic custom provisional restorations that provide optimum esthetics and function accomplished with minimal chairside time is described. The effects of mamelons, translucency, and incisal haloes are reproduced to mimic natural dentition. The technique is based on a provisional shell that is made in the laboratory and relined intraorally for a more precise fit. In addition, it can be characterized chairside to better satisfy the esthetic demands of the patient. The laboratory-processed shell allows us to attain optimal physical characteristics with auto-polymerizing methylmethacrylate resin.

The processed provisional splint in periodontal prostheses.

A technique has been described whereby a heat-processed provisional splint was fabricated with the factors of esthetics and long-term serviceability being of prime importance. Additional advantages of the use of a well-adapted provisional restoration in a periodontal prosthesis are as follows: (1) They facilitate periodontal treatment by allowing total visibility and access to surgical sites when the splint is removed. (2) The splinting effect may enhance healing and periodontal-ligament reattachment by stabilizing mobile abutments. (3) The patient's ability to render adequate home care for a fixed prosthetic restoration may be evaluated and reinforced before making case restorations. (4) The patient's cooperation is assured during an extended period of restorative care when an esthetic, comfortable, provisional splint is provided.

Immediate/early loading of dental implants: a report from the Sociedad Española de Implantes World Congress consensus meeting in Barcelona, Spain, 2002.

BACKGROUND: Immediate/early loading protocols are becoming frequently used in implant dentistry, but the prerequisites for achieving good results and the limitations of such protocols are not fully known. Moreover, the terminology used in immediate/early loading is still confusing. PURPOSE: The purpose of this article is to present the outcome of a consensus meeting on immediate/early loading. MATERIALS AND METHODS: A consensus meeting was organized during the Sociedad Española de Implantes World Congress in Barcelona on May 23, 2002, with the objective to present and discuss the experiences from immediate/early loading protocols in dental implant treatment. The purpose was also to discuss definitions of the terminology used in immediate/early loading. The consensus meeting agenda included presentations from invited experts, followed by a consensus discussion. RESULTS: A consensus statement was agreed on. CONCLUSIONS: Multiple independent investigators have demonstrated that immediate/early loading of implants is possible in many clinical situations; however, additional documentation is required.

Implant prosthodontics contribute to restorative dentistry.

This paper is directed toward dental practitioners who accomplish the prosthodontic procedures for dental implants. It includes a discussion of most of the major steps in current implant dentistry. Several implant systems are compared with a look at the future of implant prosthodontics.

Complete artificial dentition supported by endosseous implants: a case report of total in-office treatment.

This case report demonstrates the construction of a complete restoration of the dentition by the surgical placement of endosseous titanium implants that support a fixed prosthesis in each jaw. The positioning of the implants and teeth in the prostheses are important factors for a successful long-term result. Distribution of the occlusal biting forces over as many implants as possible is important. Off-axial occlusal biting forces should be diverted to the anterior prostheses, where the forces are not as great and the posterior teeth are designed with flat occlusal surfaces that separate during excursionary chewing movements. Medial mandibular flexure caused by the contraction of the medial pterygoid muscle can be addressed by constructing the prosthesis in segments. This is so as not to have a rigid entity encased in flexing bone that may induce stress to the bone, leading to loss of implant integration and failure. Segmenting also insures an appropriate fit of the prosthesis with respect to casting and porcelain firing distortion. Lip support by means of a flange in the prosthesis may be necessary when there has been a large amount of bone loss from edentulous resorption. Cleaning and maintenance of the prostheses every 3 to 6 months is essential.
