Influence of reinforcement design on the loss of marginal seal of provisional fixed partial dentures.

Loss of marginal cement seal, initial fracture, and stiffness of three four-unit provisional restorations were tested. Loss of marginal cement seal was monitored using strain gauges located occlusogingivally at the distal of the second molar abutment and the mesial of the first premolar abutment of a four-unit posterior fixed provisional restoration with two pontics. The provisional restorations were then placed in an Instron testing machine and loaded to initial fracture. Stiffness was calculated from the Instron recordings. The provisional designs were permachrome band-reinforced, wire-reinforced, and a nonreinforced control. Analysis showed: (1) the band-reinforced design withstood a significantly greater load before loss of cement seal (P< or =.05); (2) the band-reinforced design had a significantly higher stiffness (P< or =.05); and (3) there was no difference between the three designs for load at initial fracture. Within the same sample it was found that for both band- and wire-reinforced designs the premolar abutment lost the marginal cement seal at a significantly lower load than the molar (P< or =.05). For the control there was no significant difference for load at marginal cement seal loss between abutments.

Retrospective audit of patients with advanced toothwear restored with removable partial dentures.

The dental records of 50 patients with advanced tooth wear restored with removable prostheses were examined. Retrospective data were collected with regard to source of referral, presenting complaint, aetiological factors, clinical features, dentures provided, details of failures and maintenance. The maximum follow up period was three years. The ratio of male to female patients was 4:1 and the age range 31-75 years. Failures were recorded in 38% of patients with provisional and 64% with definitive dentures. The most common failure was fracture or wear of the incisal or occlusal surfaces. The majority of failures were addressed by adjustment of the dentures and the audit confirmed the need for regular maintenance.

Salvaging of adjacent fixed partial dentures using an overcasting technique.

Occasionally, clinicians are faced with a situation in which a solitary tooth between two bridges requires extraction. When this type of situation occurs, we must ponder the dilemma of how to replace the missing tooth. Options include placing an implant where the tooth was extracted or remaking one large multiunit bridge. Implants are not always possible or practical, and remaking a multiunit bridge that involves the abutments of the two existing bridges can be challenging and becomes much more expensive to the patients. The overcasting technique is a treatment modality that offers a solution in these particular situations.

The effect of glass fiber reinforcement on the fracture resistance of a provisional fixed partial denture.

PURPOSE: This study determined the load required to fracture a three-unit provisional fixed partial denture restoration, which had been reinforced with an experimental glass fiber reinforcement. MATERIAL AND METHODS: Provisional fixed partial dentures (n = 5) were fabricated from a resin of polyethyl methacrylate powder and n-butylmethacrylate liquid. The fixed partial dentures in the control group were unreinforced. In the other groups, the fixed partial dentures were reinforced either with one, two, or three unidirectional glass fiber reinforcements and one glass fiber weave reinforcement. The load was applied to the fixed partial dentures by a steel ball placed in the cavity in the middle fossa of the pontic tooth. A longitudinal section of the fixed partial denture was made to determine the position of the reinforcements. Means were compared by analysis of variance. RESULTS: The load required to fracture the unreinforced fixed partial denture was 614 N, while incorporation of one unidirectional reinforcement increased the load to 660 N, two reinforcements to 818 N, three reinforcements to 827 N, and three reinforcements with one weave reinforcement to 973 N. It was found that the unidirectional reinforcements were positioned on the side of the occlusal surface of the fixed partial denture, namely, the side of compression during loading. CONCLUSION: The results suggest that, even though the glass fiber reinforcements were positioned on the least favorable side of the fixed partial denture in terms of the physical properties of the materials, these reinforcements considerably increased the fracture resistance of the provisional fixed partial denture.

Comparison of effects of bleach and cleansing tablet on reflectance and surface changes of a dental alloy used for removable partial dentures.

Proper hygienic care of removable partial dentures is important for maintaining a healthy oral mucosa. A daily home care routine is the responsibility of the patient to maintain oral hygiene. A dentist must inform the patient about denture cleansers used for plaque control. Common denture cleansers may negatively affect the surface of alloys used to make partial dentures. The purpose of this study was to evaluate whether bleach and cleansing tablet have deleterious effects on the surface of a dental alloy used to fabricate removable partial dentures. Nineteen test specimens were prepared from CoCr dental alloy. The specimens were cast into 10 mm long x 10 mm wide x 1 mm thick. Fresh solutions made from a commercial bleach and a commercial cleansing tablet were used for each treating. Three groups (commercial denture cleansing tablet, bleach and tap water) were tested. Six specimens were immersed in 200 mL of tap water containing a single denture cleansing tablet. Two spoons of the commercial bleach was added to 200 mL of tap water. Seven specimens were treated. The control specimens were immersed in 200 mL of tap water. Six specimens were the control group. Each specimen was immersed for 30 min per day in the solution for a period of 30 days. The specimens were analyzed for reflectance with a spectrophotometer, for surface changes with a light microscope and weight which was measured with sensitive balance before and after treatment. The SPSS program was used for the statistical analysis. The Kruskal-Wallis Test and Wilcoxon Signed Ranks Test was used. No significant changes of reflectance was shown in the Kruskal-Wallis Test (p > 0.05). The Wilcoxon Signed Ranks Test was performed for comparison into groups. The difference between before and after treatment was statistically significant for the commercial denture cleansing tablet and the commercial bleach (p = 0.028) (p < 0.05). Reflectance was decreased after treatment. Within the limitations of this study, the commercial cleaning solutions had influenced the decrease of reflectance on the surface of the dental alloy. This solutions should not be limitlessly used on dentures with metal components, because they caused clinically significant reflectance changes on the surface of the alloy after 30 days.

The partial denture over a fixed guide bar appliance: a technique for restoring partially edentulous quadrants. Case report.

Replacement of a number of lost teeth in partially edentulous quadrants has been accomplished using a technically simple Tissue-borne, Partial Denture over a Fixed Bar (PDOFB) Appliance. The appliance offers a similar degree of comfort and aesthetic appeal to that of a fixed bridge, but is specifically designed to reduce the high occlusal loads that normally are applied to the abutment teeth during mastication. It has been used where, because of the excessive distance between the abutment teeth, or because of the poor periodontal health of the abutment teeth, the construction of conventional crowns and bridges was contraindicated. The indications for, and the construction of such an appliance are described.

11-year follow-up study of resin-bonded fixed partial dentures.

The clinical performance of 325 resin-bonded fixed partial dentures (RBFPD) placed between 1984 and 1995 was reviewed. The primary indications of the 264 patients treated were trauma and aplasia. Two hundred eighty-three of the resin-bonded fixed partial dentures were primary restorations, and 42 of the restorations were secondary and tertiary after the loss of the primary resin-bonded fixed partial denture. Including the rebonded restorations, the survival rate calculated using the Kaplan-Meier method was 76% after 5 years and 60% after 10 years. Analysis of the data showed that casting alloy, pretreatment of the bonding surfaces, luting agent, and mobility of the abutment teeth were decisive prognostic factors for success. Base metal alloys, silicoating, mesh retention, and immobile abutments were positive factors. In contrast to other reports in the literature, retentive abutment preparation did not result in a higher survival rate than unprepared abutments. Small restorations in the bonding area did not affect the survival. No differences in survival rate were found for resin-bonded fixed partial dentures in the maxillae or mandible, or in the anterior and posterior regions. Multiunit resin-bonded fixed partial dentures had a smaller probability of survival than three-unit resin-bonded fixed partial dentures. Carious lesions were found on 4% of the abutment teeth.

Rotational path of insertion for removable partial dentures with an anterior saddle.

The provision of removable partial dentures remains a viable treatment modality for many partially dentate patients. Replacing missing anterior teeth with a removable partial denture using a rotational path of insertion provides improved retention and appearance. The use of a rotational path of insertion also provides additional retention for a removable partial denture restoring combined anterior and posterior bounded saddles. Two case reports demonstrate the use of the rotational path of insertion for partial denture design.

Repair with a surveyed cast clasp while patient retains the partial denture.

A technique is described that allows a removable partial denture with a broken clasp or a removable partial denture in which an abutment has been extracted to be restored by the reattachment of a new cast clasp component or a complete surveyed clasp assembly. The technique is applicable to most clasp designs and can include attachment to the acrylic denture base or the metal major connector. The technique is distinguished from other repair techniques by (1) providing a cast clasp replacement, (2) allowing the patient to retain the prosthesis during the repair, (3) generating a precisely formed surveyed clasp assembly by the laboratory, and (4) including an efficient transfer mechanism for the precision clasp created in the laboratory to be attached to the removable partial denture in the dental office. A two-step impression procedure is used to ensure proper relation of the partial denture to the abutments.

Ecologic changes in the oral cavity caused by removable partial dentures.

1. The introduction of a removable partial denture into the oral cavity adversely affects the prevailing ecologic situation in terms of plaque formation. 2. The rehabilitative effect of a removable partial denture may be safeguarded by controlling plaque formation by strict personal hygienic measures on the part of the patient. In particular, proximal surfaces adjacent to denture bases should be pointed out to patients as surfaces to which they must give special attention. 3. Simplification of the design of removable partial dentures can reduce their damaging potentialities. 4. Further prophylactic measures, such as topical application of fluorides and perhaps other chemicals, should also be taken.
