A controlled comparison of the Pomeroy resection technique and laparoscopic electrocoagulation of the tubes.

The Pomeroy resection technique and laparoscopic electrocoagulation of the tubes are compared in a controlled study. Information was obtained during the patients' stay in hospital and at three and 12 months after operation. There were no significant differences between the study groups concerning age, social group, parity, contraceptive practices or age of the children. The surgery time, stay in hospital, periods of postoperative discomfort and sick leave were shorter in the laparoscopy group, and there were fewer postoperative complaints than in the laparotomy group. No differences in the impact on postoperative bleeding patterns were demonstrated.

[Causes and prevention of burnings caused by electrocoagulation during surgery (author's transl)].

During surgery accidential burnings caused by electrocoagulation are quite often. They are usually due to contact of patients with grounded parts of the table or the monitoring devices. Besides that burnings occur because of insufficient fixation of the neutral electrode. To prevent injuries the patient must be isolated carefully from every grounded metal. The neutral electrode should be fixed as close as possible to the area which is operated on and should be of a broad contact. Other more rarely occurring causes of burnings and their prevention are described.

A method of inducing chronic gastric and duodenal ulcer by electrocoagulation in rats.

The described method includes electrocoagulation of the serous membrane of the stomach or duodenum by means of surgical diathermy. Under Nembutal general anaesthesia laparotomy was done and a ball-tip electrode was applied to the exposed anterior wall of the stomach or duodenal bulb. At the site of its contact with the serosa coagulation developed visible as a white spot approximately the size of the electrode tip. The ulcerogenic process triggered in this way developed within three days after the procedure. A deep ulcer in the mucosa developing during that time assumed in the second week the shape of a crater. From that time on ulcer healing began and progressed as evidenced by gradual diminution of its size. Complete healing of an untreated ulcer occurred in most animals after about five weeks. The extent of the ulcer niche was determined under a stereoscopic microscope calculating its area in mm2. A comparison of ulcer area in the animals treated with drugs inducing ulcer healing with this area in control rats made possible quantitative evaluation of the therapeutic effect.

[Changes in gastric mucosa, liver and mesenteric vessels following bipolar electrocoagulation].

The implications of bipolar electrocoagulation with a modified probe (BICAP) were tested in rat gastric mucosa, liver tissue and mesenteric vessels. This was done during the coagulation and at different intervals, up to 14 days later, by intravital microscopy of the exposed organs. A histological treatment of the respective tissues ensued at the closing of each experiment. Applying a current of 25 watt and contact times lasting for 5 secs and longer, perfect tissue necrosis develops which has clean and straight edges. The extensions on the surface and into the depth of gastric mucosa match precisely the contact area with the instrument. In the deeper mucosal layers there is a fungiform spread of necrosis while in the liver it remains uniform and shows the same diameter as on the surface. As a late result and notwithstanding the confinement of the coagulation to the mucous membrane, a local peritonitis may develop. Within contractile vessels the action of bipolar coagulation stops the blood flow which is only irreversible, when the current and contact times are extended to bring about necrosis in neighbouring tissue. Within the microcirculation of the liver the blood flow is preserved up to the very boundary of the necrosis.

[Electroresection and electrocoagulation as therapy for vulvar neoplasms].

A survey of the results attained by the most prominent authors of the individual therapy methods is given. After electro-resection and -coagulation of the vulvar tumor followed by roentgen or telecobalt irradiation, patients are almost without pain. The primary mortality in 332 patients was 1,5% (= 5 cases) because lymphonodectomy was performed in only 66 cases (20%). The period of hospitalization lasts about four weeks and cosmetic healing is excellent. This therapy method can also be recommended for largely extended tumors as lack of local symptoms can be achieved up to 97%. Of the 332 patients treated at our hospital, almost equal numbers of patients were seen with carcinoma stage I and II and stage III and IV. In stage I and II (negative lymph nodes) 74,5% and in stage III and IV (positive lymph nodes) 38,4% were cured. 80% were aged between 61 and 90 years. In spite of this negative selection of patients which was also combined with the intercurrent mortality rate of 13% (= 43 cases), 193 of the 332 patients or 58,1% survived five years or longer. In the last five years the survival rate was even 62,2%. A survey of the results obtained by electrocoagulation in the last 30 years is given. Especially the enormous increase of high risks as diabetes (42,2%) and the great number of over-aged patients (83,3%) are emphasized.

[Electrocoagulation].

Telangiectasia, the varicosities of the lower limbs, are generally broken down by microsclerosis; but micro-sclerosis is often insufficient, even impossible. This is where electrocoagulation is useful. It would be more correct to call it diathermo-coagulation; histological examination does actually show a real bursting of the tissues. To put it into practice an apparatus is needed, producing a high-frequency current; a punctiform electrode is connected at the positive terminus. Using magnifying lenses, the phlebologist penetrates the minute vessel, in depth (1 mm - 1.5 mm) and at points close together. The sessions, which are painless, take place each month, that is, as soon as the scabs have disappeared. Three or four sessions are needed for blue telangiectasia, and at least five or six for red. Exposure to sun or even to open air is forbidden during the year of treatment. The break-down is definitive and need not leave a scar. Moreover, women (it is almost exclusively women who are treated for this) accept these conditions, as they are glad to be rid of unaesthetic hair on thighs, legs, and ankles. We draw attention to the fact that all treatment should be suspended during pregnancy, as most telangiectasia disappear on their own after the birth.

[Hormonal changes during electrocoagulation of the Gasserian ganglion under neuroleptanalgesia and the effects of administration of a beta blocking agent (author's transl)].

Hormonal changes, occurring during electrocoagulation of the Gasserian ganglion under neuroleptanalgesia, and the effects of intravenous administration of pindolol 0.4 mg on these changes were investigated in two groups of 10 and 12 patients of advanced age. At the end of the operation there were marked increases of the concentrations of catecholamines, a rise of the plasma cortisol and a fall of the serum insulin. The administration of the beta-adrenergic blocking agent did not induce any statistically significant changes of the determined hormones compared to the control group.

Transnasal electrocoagulation of the vidian nerve.

Vidian neurectomy has previously been reported as having a high success rate in alleviating secreto-motor rhinopathies. We report a prospective study of 24 consecutive patients who underwent transnasal vidian electrocoagulation. An overall improvement in symptoms was found in 59.1%. A worsening of overall nasal symptoms was complained of by 27.3%. The possible reasons for this are discussed.

[Palliative treatment of esophageal neoplastic stenosis using bipolar electrocoagulation probe].

OBJECTIVE, PATIENTS AND METHODS: The objective of this study was to evaluate the results of palliative bipolar electrocoagulation probe (BICAP) treatment in 26 patients with non surgical, obstructive esophageal cancer. The mean tumor length was 7.2 cm. The strictures were located as follows: 3 in the cervical esophagus, 8 in the thoracic esophagus, 13 in the distal third of the esophagus and 2 involved both the cervical and thoracic esophagus. Most lesions were circumferential (73% versus 27% non circumferential) and exophytic (78% versus 22% sub mucosal). Coagulation was carried out under direct endoscopic control in the forward direction. The success of treatment was evaluated on the basis of the degree of reopening achieved (easy passage of an endoscope 12 mm in diameter) and good functional results (improvement of dysphagia, scored from 0 to 4 using a standard grading scale, for more than 15 days). RESULTS: Twenty-six patients underwent a total of 45 BICAP treatments (31 initial sessions, 14 repeated sessions). Reopening was achieved in 92% of cases and good functional results were obtained in 85% (mean dysphagia score: 3.2 before treatment versus 1.1 after treatment). The improvement of dysphagia resulted in a significant improvement of general performance status and stabilization or weight improvement in 21 patients. The mean number of sessions necessary for good initial results was 1.2 +/- 0.4. After the initial treatment by BICAP, radiotherapy or radiochemotherapy were respectively associated in 4 and 10 patients. The median duration of improvement was significantly longer in patients who underwent radiochemotherapy as compared with patients treated by BICAP alone (22 weeks versus 4 weeks). During the follow-up, 9 patients required several BICAP treatments and at the end of their disease, 12 patients underwent other palliative procedures. Major complications occurred in 4 cases (2 esotracheal fistulas, 1 hemorrhage, and 1 aspiration pneumonia) and mortality related to the procedure was 8%. CONCLUSIONS: Palliative BICAP treatment of obstructing esophageal and cardial cancer provide quick relief of dysphagia but repeated treatment sessions are necessary to maintain initial improvement. The procedure requires a short hospitalization stay and can be easily accomplished in all cases regardless of the tumor features.

Effects of the molar ratio of hydroxide and fluoride to Al(III) on fluoride removal by coagulation and electrocoagulation.

The effect of the molar ratio of hydroxide and fluoride ions to Al(III) ions (gamma(OH) and gamma(F)) on coagulation and electrocoagulation (EC) was studied to solve the problem of the over addition of acid or base. The efficiency of defluoridation was approximately 100% when the sum of gamma(OH) and gamma(F) (gamma(OH+F)) was close to 3. This finding reveals that the fluoride ions and the hydroxide ions can co-precipitate with Al(III) ions and the formula of the precipitate is Al(n)F(m)(OH)(3n-m). However, when gamma(OH) was less than 2.4, the defluoridation efficiency, given that gamma(OH+F)=3, dropped as gamma(OH) fell, because the amount of aluminum polymer formed dropped. The efficiency of defluoridation of EC exceeded that of coagulation for equal gamma(OH) and gamma(OH+F), when gamma(OH+F)>3, proving the existence of an electrocondensation effect.

Optimization of the electrocoagulation process for the removal of copper, lead and cadmium in natural waters and simulated wastewater.

Chemical, electrochemical and flow variables were optimized to examine the effectiveness of the electrocoagulation process for the removal of copper, lead and cadmium. The electrochemical process, which uses electrodes of commercial laminate steel, was applied to simulated wastewater containing 12 mg dm(-3) of copper, 4 mg dm(-3) of lead and 4 mg dm(-3) of cadmium. The optimum conditions for the process were identified as pH=7, flow rate=6.3 cm(3) min(-1) and a current density between 31 and 54 A m(-2). When the electrode geometric area and time of electrolysis reached critical values, the copper removal reached a maximum value of 80%. A linear relationship was identified between the current density and the mass of generated sludge. In addition, a linear relationship was found between specific energy consumption and current density. The results of this investigation provide important data for the development of an industrial-scale electrolytic reactor.

Fundamentals, present and future perspectives of electrocoagulation.

Electrocoagulation is an electrochemical wastewater treatment technology that is currently experiencing both increased popularity and considerable technical improvements. There has been relatively little effort to better understand the fundamental mechanisms of the processes, particularly those that could provide design parameters to optimize the performances of this relatively simple and inexpensive technique. In a research programme to delineate the mechanisms of the fundamental processes involved in, the authors have realized that the technology has been insufficiently reviewed with emphasis on the fundamentals and their relationship to the performance of this technology. This paper presents an in-depth discussion and consideration of the factors that need to be addressed for optimum performance of this technology. Recent improvements of this technique and the theoretical model studies are also reviewed.

Arsenic removal via electrocoagulation from heavy metal contaminated groundwater in La Comarca Lagunera México.

Arsenic contamination is an enormous worldwide problem. A large number of people dwelling in Comarca Lagunera, situated in the central part of northern México, use well water with arsenic in excess of the water standard regulated by the Secretary of Environment and Natural Resources of México (SEMARNAT), to be suitable for human health. Individuals with lifetime exposure to arsenic develop the classic symptoms of arsenic poisoning. Among several options available for removal of arsenic from well water, electrocoagulation (EC) is a very promising electrochemical treatment technique that does not require the addition of chemicals or regeneration. First, this study will provide an introduction to the fundamental concepts of the EC method. In this study, powder X-ray diffraction, scanning electron microscopy, transmission Mössbauer spectroscopy and Fourier transform infrared spectroscopy were used to characterize the solid products formed at iron electrodes during the EC process. The results suggest that magnetite particles and amorphous iron oxyhydroxides present in the EC products remove arsenic(III) and arsenic(V) with an efficiency of more than 99% from groundwater in a field pilot scale study.

Treatment of levafix orange textile dye solution by electrocoagulation.

The decolorization of the levafix orange textile dye in aqueous solution by electrocoagulation using aluminum sacrificial anode has been investigated. The process performance is analyzed in terms of decolorization efficiency and the important cost-related parameters such as electrode and energy consumptions, as a function of initial pH, conductivity, current density, initial dye concentration and electrolysis time. The present study proves the effectiveness of electrochemical treatment for the textile dye solution. 95% decolorization efficiency may be obtained at suitable operating conditions such as; current density 100 A/m(2), operating time 12 min and initial pH 6.4. The corresponding electrode and energy consumptions during the electrolysis were found to be 1.8 kg Al/kg dye and 35 k Wh/kg dye.
