[Intracavitary heart metastasis of testicular embryonic tumor].

Cardiac intracavitary metastasis are very uncommon. A 19 years old patient with an embryonic tumour of the testes extending into the right ventricle, which manifested as heart failure, is presented. To our knowledge, this is the first case report of a teratoma with carcinoma of the testis involving the right ventricle.

Multiple primary left ventricular myxomas with multiple intraventricular recurrences.

A 23-year-old male was operated on for two primary left ventricular myxomas of the "complex" type. Ten months later, abnormal echo reappeared and reoperation was carried out to excise 2 recurrent myxomas in the left ventricle. Multiple foci were most likely responsible for the recurrence. No recurrence has been detected for 20 months postoperatively. This may be the first reported case of multiple primary left ventricular myxoma with multiple recurrences in the left ventricular cavity.

[Cardiac rhabdomyoma in tuberous sclerosis. Presentation of a case].

We report of a case of cardiac tumor in a three months old infant, examined with echocardiography because a murmur was noted on routine examination. Further investigations brought out the diagnosis of tuberous sclerosis. Spontaneous regression of the tumor mass occurred in the following months. No signs of cardiac dysfunction of CNS involvement appeared. The peculiarity of rhabdomyoma as an early manifestation of tuberous sclerosis is emphasized.

Transesophageal echocardiographic diagnosis of multicentric left ventricular myxomas mimicking a left atrial tumor.

We report a case of multicentric left ventricular myxomas with prolapse of one myxoma into the left atrium during ventricular systole that mimicked a left atrial tumor. The transthoracic echocardiogram showed large masses in the region of the mitral valve leaflets consistent with vegetations or tumors. A computed tomographic scan of the chest demonstrated two distinct left atrial masses, one of which appeared to prolapse from the left atrium into the left ventricle. Intraoperative transesophageal echocardiography showed a large pedunculated mass moving between the two left-sided cardiac chambers with intermittent trapping of the mass in the left atrium. The mass was attached to the left ventricular posteromedial papillary muscle by a long stalk. Another adjacent large ventricular mass was also noted in the left ventricle. These findings were confirmed at operation, which also demonstrated a third small tumor arising from the atrial aspect of the posterior mitral leaflet.

[Solitary cardiac metastasis from carcinoma of the cervix].

Authors report the case of 37 years old women after radiotherapy for spinocelular cervical carcinoma which was presented by a solitary metastasis in the right ventricle causing right ventricle output tract obstruction. Cardiosurgical intervention as well as chemotherapy were not indicated for the great extension of cardiac involvement. The patient died from a massive pulmonary embolism. Echocardiography, although limited by the discrimination ability, seems to be the best method for the cardiac metastasis detection.

[Leiomyosarcoma of the right ventricle. Report of a case and review of the literature].

Primary cardiac leiomyosarcoma is a very rare condition. Only 15 cases have been reported in the literature. We report the case of a 27 year old man admitted to hospital for chest pain in March 1985. Echocardiography showed a right ventricular tumour which was completely resected at surgery. The diagnosis of leiomyosarcoma was confirmed by histological examination. After 22 months follow-up, the patient was still alive despite pulmonary metastases. The diagnosis of cardiac tumour used to be made post-mortem but since the introduction of new methods of investigation, especially 2D echocardiography, the diagnosis can be made at an early stage and allows rapid surgical resection, the only means of obtaining the histological diagnosis and of completely curing benign tumours.

[The recurrence of a right atrial myxoma. Apropos a case].

Cardiac myxomas are the most frequent cardiac tumor and they have the capability for growing again years after resection. We have recently reviewed such cases at the Instituto Nacional de Cardiología "Ignacio Chávez" and no cases of recurrence of these tumors was detected. In this paper we inform a case that had recurrence of the tumoral mass 4 years after successful resection of the primary one. There were two masses in the right atrium of different sizes. To our knowledge this is the first case with this characteristic.

[Left ventricular myxoma with mucocutaneous myxoid tumors--a case report].

A surgical treatment of left ventricular myxoma arising from the anterior wall of the left ventricle is reported. A 32 year old man with a history of transient syncopal attack had an echocardiographic diagnosis of left ventricular myxoma. He previously had had myxoid tumors excised from the nose and from the posterior part of the left thigh. Cardiac surgery was performed under extracorporeal circulation on December 20, 1989. A friable, gelatinous 5 cm long myxoma with a diameter of 3 cm was excised from the left ventricule. The postoperative course was uneventful. The clinical characteristics in all the reported cases of cardiac myxomas with associated myxoid tumors of the skin are reviewed. Cardiac myxomas in patients with mucocutaneous myxoid tumors have an atypical biological behavior with early development of myxomas, atypical location, multiple tumors and high rate of recurrent myxomas.

Heart transplantation in patients with malignant disease.

We have traditionally pushed the limits of conservative candidate criteria for heart transplantation. We have been gratified by our results in the aged, the diabetic, and the mortally ill. Our inclusion of patients with malignant disease underscores our philosophy to include patients as candidates for transplantation for whom the procedure has reasonable expectation of success. We report here our early results of heart transplantation in 11 patients with malignant disease. Our survival rate in this group is 100%, and all patients are leading active lives with no evidence of recurrent or metastatic tumor. Immunosuppression protocols were adjusted on an individual basis determined by the chemotherapy dosage, duration, and relation to transplantation. Whenever possible a 1-year disease-free interval after completion of adequate cancer therapy is desired before transplantation.

[Leiomyosarcoma of the pulmonary artery. Apropos of a case].

The authors report a case of sarcoma of the pulmonary artery in a 41 year old woman hospitalised for syncope on effort associated with effort dyspnoea. Chest Xray and echocardiography were "normal", leading to referral for pulmonary scintigraphy and angiography which showed multiple intravascular filling defects in the pulmonary arterial tree. A tumour implanted on a pulmonary valve was discovered at exploratory thoractomy. It was resected and the pulmonary valve replaced with a Carpentier-Edwards bioprosthesis. The histological findings were those of a leiomyosarcoma. The patient was reoperated 19 months later for a local recurrence but the tumour was too extensive for a complete ablation. The patient developed right heart failure and died the day after surgery. Sarcoma of the pulmonary artery is a rare tumour in adults and carries a very poor prognosis. The clinical course is initially insiduous and polymorphic but early diagnosis and complete excision are the only means of improving survival.

[Spontaneous regression of a heart rhabdomyoma in tuberous sclerosis. Apropos of a case].

A case of cardiac rhabdomyoma diagnosed during the neonatal period is presented. The condition remitted spontaneously, as was observed by monitoring its course both clinically and by echocardiography. Surgical treatment of this rare condition should be assessed with care, both because of the risk involved in surgery and because of the possibility of a spontaneous cure in rhabdomyomas, which are usually grouped together, as in our case, in a general picture of tuberous sclerosis.

[Right ventricular metastasis of renal cell carcinoma 10 years after kidney transplantation].

In a 61-year-old man, renal cell carcinoma of the right kidney was diagnosed 10 years after renal transplantation. Echocardiography revealed a right ventricular mass 5.5 x 3.5 m in size. The patient died suddenly 10 weeks later, post-mortem examination confirmed diagnosis of right ventricular metastasis of renal cell carcinoma. This case underlines the importance of echocardiographic examination in tumor patients if signs of heart failure, angina pectoris, embolism, or rhythm disturbances arise; or if cardiac murmur becomes audible, or heart size increases.

An unusual case of metastatic intracardiac plasmacytoma.

The authors report the case of a 36-year-old male who, following investigations for dyspnea, othopnea and peripheral cyanosis, was found to have metastatic intracardiac plasmacytoma. Diagnosis was made initially with transesophageal echocardiography and emergent cardiac surgery was performed. Postoperative transthoracic and transesophageal echocardiograms revealed residual masses in the right and left atrium. The patient was subsequently treated with systemic chemotherapy and was symptom-free within three months of treatment, with rapid decline of paraproteins in the serum and urine. This case of intracardiac metastatic plasmacytoma is deemed unique because cardiac involvement with multiple myeloma has not been studied in detail, largely due to the rarity of such a presentation.

[Right ventricular myxoma with near obstruction of the pulmonary artery].

Right ventricular myxoma is very rare. To our knowledge, only 23 cases in Japan have been reported in the literature. A 76-year-old male was admitted with complaints of cough and dyspnea. We found an abnormal floating mass in the right ventricular outflow tract by echocardiography and cineangiocardiography. The digital subtraction angiography showed that the following mass continuously lodged in the pulmonary artery with total occlusion of the left main pulmonary artery and near obstruction of the right pulmonary artery. Furthermore he had the constitutional signs of myxoma and the serum IL-6 level reached 41.7 pg/ml. Under extra-corporeal circulation, we salvaged the right ventricular outflow tract to the right lung, resecting the tumor, originated from the right ventricular septum and extended to bilateral main pulmonary arteries.
