[Systematic indications of various free transfer flaps in the area of the head and neck].

Many microsurgical free flaps have been described to tackle various problems in the head and neck. The authors present a system of indications which has proved to be suitable as judged by the functional and cosmetic results obtained. The harvesting of these flaps requires versatile training in surgical techniques for the different anatomical areas. This system of indications comprises the free fasciocutaneous forearm flap, jejunal loops, the fasciocutaneous parascapular flap, iliac crest flaps with vascularized bone and the parascapular cutaneous, latissimus dorsi osteomyocutaneous double flap.

Immediate reconstruction of mandibular defects with revascularized iliac bone grafts after radical surgery for osteosarcoma.

Excluding multiple myeloma, osteosarcoma is the most frequent primary malignant bone tumor. However, it is rare in the jaw bones, where it mainly occurs in young adult men. This report describes two mandibular osteosarcomas found in two males aged 32 and 30. In the former patient, osteosarcoma developed after bilateral mandibular sclerotic changes had been diagnosed radiologically as cementifying fibroma. This patient had received radiation therapy (56 Gy) because of a high grade astrocytoma of the brain three and a half years earlier. In the latter patient the tumor arose de novo in the mandibular molar area. The initial main symptom both patients was pain, which had been considered to be of dental origin because radiographs showed no evidence of malignancy. Radical surgery using immediate mandibular reconstruction with revascularized iliac bone grafts gave good results in both cases. The former patient died eight months after surgery because of recurrent astrocytoma and the latter is now free of disease five years after surgery. Immediate reconstruction of mandibular defects is possible with osteocutaneous groin flaps.

[Resection and reconstruction of mandible for treatment of two rare benign tumors].

Odontogenic myxoma and particularly juvenile fibromatosis are rare benign tumors in the maxillo-facial region. The aggressive local and sometimes voluminous development of these tumors may necessitate partial mandibulectomy. Autologous bone graft from the iliac crest is the best material for mandibular reconstruction because of its functional and esthetic results. If osteosynthesis of the graft is achieved by means of internal wiring, several weeks of intermaxillary fixation are required, whereas internal rigid fixation with plate and screws allows the patient to open the mouth more rapidly.

Ameloblastoma of the mandible: report of extensive destruction within 4 years.

A multicystic ameloblastoma of the mandible is reported in a 54-year-old white man who presented with a complaint of an ill-fitting partial denture due to a swelling in the posterior mandible. This case demonstrates the aggressive character of an ameloblastoma, causing extensive destruction of the mandible in a documented period of less than 4 years.

[Procedures and results with preoperative individual AO reconstruction plates].

As a rule continuous bony defects of the mandible after tumour resection are temporarily bridged with the AO reconstruction plate. To precontour the plate, CT-scans of the mandible at a scale of 1:1 6 mm above and parallel to the inferior margin are needed. Corresponding layers further cranial show the anatomy of the chin. Using a lateral cephalograph, the mandibular angle and the length of the plate in the ascending ramus are measured. Using the CT-scans, the lateral cephalograph, a prebent template and a corresponding model of the mandible, the plate is bent into shape. Of the nineteen preshaped reconstruction plates implanted, eleven did not need any intraoperative adjustment. The other eight plates needed only slight modifications of contour. Preoperative bending of the reconstruction plate reduces the time taken for the operation, protects the material and the plate is precisely shaped.

[Mandibular reconstruction with the titanium-hollow screw-reconstruction plate system (THORP)].

The THORP system is designed for bridging mandibular defects and for internal osteosynthesis of mandibular fractures. The major aspects and advantages of the THORP system compared with the conventional reconstruction plates and screws are: 1. The rigid fixation of the head of the screw to the plate, thus avoiding unphysiological loads to the bone underneath the plate. 2. The titanium-plasma coated and perforated hollow screws enable the development of direct bone-titanium contact at the area of the thread as well as the ingrowth of bone into the lumen and perforations. Further advantages are the preservation of small fragments such as the condylar process with only two screws and the stable fixation of bone transplants. The specially designed freely adjustable condylar prosthesis guarantees a correct guidance and function of the lower jaw. The fixation of this system to the lingual aspect of the jaw is particularly significant avoiding decubitus and perforation of the skin in the chin area. The results and the complications of 141 reconstructions of the lower jaw are discussed.

[Mandibular reconstruction. An historical overview].

In a historical review the most important technologies and auxiliary materials for lower jaw reconstruction are demonstrated and discussed. Personally developed implants for anatomical and functional reconstruction of the continuity and rigidity based on the latest experiences of bone surgery, are presented. The results of this development are demonstrated by several steps of mandibular reconstruction in a special clinical case. The discussion deals with the conclusions drawn from the historical review in comparison to our own clinical results.

[A case of metastatic lung tumor mimicking left atrial myxoma by echocardiography].

We report a case of lung metastatic tumor originated from submandibular osteosarcoma, which shows rare advancement and findings. This lung metastatic tumor advanced from left lower lung to left atrium through the left lower pulmonary vein, which penetrated to the left ventricle in the diastolic phase mimicking left atrial myxoma. 67Ga-citrate images showed that hot accumulation were recognized behind the heart in planar image and clear advancement from the lung to the heart in the SPECT image. 67Ga-citrate SPECT image should be available for cardiac tumors.

Neurofibrosarcoma of the mandible.

A case has been reported to illustrate the apparent transition of a neurofibroma into a neurofibrosarcoma. Biopsy of the inferior alveolar nerve, proximal to the lesion, was used to determine the extent of tumor extension. En block excision of the tumor was performed. The patient has been without evidence of recurrence for approximatley 3 1/2 years. Bone graft reconstruction has been completed and full function has been restored to the mandible.

[System of alloplastic bridging of mandibular defects].

Primary reconstruction of the mandible after extensive tumor resection is necessary when the median part is lost; in lateral defects of the mandible, however, it is not always indicated. Our own functionally stable plate system with the instruments used for mandibular reconstruction is described. Clinical cases are presented in which this system was applied in combination with silastic interposed as a temporary replacement of the mandible.

[Bone regeneration over plastic implants after mandibular resection breaking the continuity].

In young patients, bone regeneration may start from the region of the temporo-mandibular joint and from the peripheral mandibular stump in cases of continuity defects of the mandible. A resin graft implanted for soft tissue reconstruction is collar-like surrounded by bone which is developing into a functionally and anatomically full regenerate over many years, in correspondence to periosteal and desmal growth stimuli. In adults, regeneration is minimal.
