[Cytokeratin expression in the epithelial cells of nasal polyp].

OBJECTIVE: To study the expression of cytokeratin in epithelial cells of nasal polyp and its significance. METHOD: The monoclonal antibody of AE1, AE3, CK7, CK14, CK19, CK20 were used to determined the corresponding cytokeratin expression in epithelial cells of nasal polyp with immunohistochemical staining. RESULT: 1. AE1, CK7, CK19 staining were positive in various pathological epithelium of nasal polyp. AE1, CK19 showed positive in total layer of epithelium. CK7 was positive in goblet cells and basal cells of the pseudostratified epithelium. 2. AE3, CK20 staining were positive in part of simple epithelium and stratified epithelium. 3. AE1, AE3, CK7, CK19 staining showed focal positive in transitional epithelium. CONCLUSION: CK7, CK19 are the one of the essential assembly type of cytokeratin in epithelium of nasal polyp. During the process of inflammatory reaction, the expression of partial cytokeratin subtype would lose while a new cytokeratin pattern would appear.

Nasal polyps herniating through lacrimal-nasal bony ostium mimicking dacryocystocele: a rare cause of DCR failure.

PURPOSE: To describe a rare cause of dacryocystorhinostomy (DCR) failure resulting from polyps herniating through the lacrimal-nasal ostium in a patient with severe, recurrent nasal polyposis. DESIGN/METHODS: Observational case report. Clinical practice setting. RESULTS: Five years after DCR surgery, a 65-year-old male patient presented with epiphora and a soft cystic enlargement in the lacrimal sac area mimicking a dacryocystocele. The Jones 1 test was negative but the Jones 2 test positive. Computer tomography (CT) showed bilateral obstruction of the nasal space by soft tissue, herniating through the lacrimal-nasal ostium, suggestive of recurrent nasal polyposis. Transnasal polypectomy was performed, which re-established tear-drainage to the nose. CONCLUSIONS: Recurrent nasal polyposis may lead to DCR failure and may give rise to a soft tissue swelling at the site of previous DCR surgery.

Intercellular adhesion molecule-1 and accumulation of eosinophils in nasal polyp tissue.

OBJECTIVE: To investigate the relationship between intercellular adhesion molecule-1 (ICAM-1) and the accumulation of eosinophils in nasal polyp tissue to better understand the mechanism of airway eosinophilic inflammation. METHODS: The expression of ICAM-1 and its natural ligand, lymphocyte function-associated antigen-1 (LFA-1), in normal nasal mucosa from 6 controls and in nasal polyp tissue from 19 patients with nasal polyposis were determined with immunohistochemistry. With dual immunohistochemistry and May-Griünwald Giemsa stain (MGG), the expression of LFA-1 and infiltrating eosinophils in nasal polyp tissue was observed. RESULTS: The expression of ICAM-1 and LFA-1 was stronger in the nasal polyp tissue than in normal nasal mucosa. There was a positive relationship between the infiltration of eosinophils and the expression of LFA-1 on eosinophils. CONCLUSION: Accumulation of eosinophils in nasal polyp tissue is associated with the counter-effect between adhesion molecules and its ligand on eosinophils.

[Cell infiltration in nasal polyps and it's pathological significance].

OBJECTIVE: To study the relation between inflammatory cells infiltration in nasal polyp tissues and it's pathogenesis. METHOD: Nasal mucosa from 10 normal individual and 4 patients of nasal polyps were studied with immunohistochemical technique and histopathologic examination. RESULT: The number of eosinophils, CD68 positive cells and monocytes was significantly higher (P < 0.05) in nasal polyps tissues than in the normal controls. CONCLUSION: Infiltration of inflammatory effector cells such as eosinphils and chronic inflammatory changes of the nasal mucosa might play a role in nasal polyps sprouting.

Endothelial nitric oxide synthase/soluble guanylate cyclase system in human nasal polyps.

The aim of our study was to analyze the level of expression of the endothelial nitric oxide synthase (eNOS)/soluble guanylate cyclase (sGC) system in nasal polyps and control nasal mucosae. The study was performed in polyps from 15 patients and nasal mucosae from 11 subjects operated on the nasal septum (control group). The expression of endothelial nitric oxide synthase (eNOS) and soluble guanylate cyclase (sGC) was determined in nasal mucosae. Western blot analysis demonstrated that eNOS protein was overexpressed in the nasal polyps with respect to control nasal mucosae. Immunohistochemistry also demonstrated that the vascular endothelium of nasal polyps contained higher amounts of eNOS protein than control nasal mucosae. Moreover, the beta(1) subunit of sGC was also overexpressed in the nasal polyps, which was associated with an increased content of cyclic GMP in the nasal polyps with respect to nasal control mucosae. In human nasal polyposis, there is an overexpression of the eNOS/sGC system. Further studies are needed to assess whether this overexpression is involved in the genesis of nasal polyposis.

Association of stem cell factor expression in nasal polyp epithelial cells with aspirin sensitivity and asthma.

Mast cells constitute a significant proportion of cells infiltrating nasal polyp tissue, and epithelial cells may release stem cell factor (SCF), a cytokine with chemotactic and survival activity for mast cells. We aimed to assess the expression of SCF in human nasal polyp epithelial cells (NPECs) as related to patients' clinical phenotypes. Nasal polyp tissues were obtained from 29 patients [including nine with aspirin (ASA)-hypersensitivity and 12 with bronchial asthma] undergoing polypectomy for nasal obstruction. Epithelial cells were obtained following 6-week culture of nasal polyps explants. The SCF released into the culture supernatant was assessed by enzyme-linked immunosorbent assay (ELISA) and total SCF mRNA in the polyp tissue was determined by semiquantitative reverse transcriptase polymerase chain reaction (RT-PCR). For the whole group of patients, the number of polypectomies correlated with expression of SCF mRNA (r = 0.62; P < 0.005), SCF protein in the NPECs supernatants (r = 0.39; P < 0.05) and with density of mast cells in epithelial layer (r = 0.37; P < 0.05) and stromal layer (r = 0.5; P < 0.01) of nasal polyps. The SCF/beta-actin mRNA ratios were significantly higher in ASA-hypersensitive (AH) asthmatics (median 0.97, range: 0.8-1.5) when compared with ASA-tolerant (AT) patients (median 0.5, range: 0.1-0.7; P < 0.001). The SCF protein concentration in NPEC supernatants was also significantly higher in AH asthmatics (median 1.10 pg/microg DNA, range: 0.4-1.9) when compared with AT patients (median 0.1 pg/microg DNA, range: 0.02-1.2; P < 0.001). In the subpopulation of ASA-sensitive asthmatics the number of polypectomies correlated also with the density of mast cells and eosinophils in the polyp tissue.

Overexpression of Fas-ligand in human nasal polyps.

Apoptosis mediated through the Fas/Fas-L system is essential in regulating immune function, developing organs, and conferring immune privilege. To illustrate the role of the Fas/Fas-L system in the pathogenesis of human nasal polyps, we investigated the transcripts and protein level of the Fas-L gene in 8 human nasal polyp tissues and 7 nasal turbinate mucosa specimens using reverse transcription-polymerase chain reaction and Western blotting. Localization of Fas-L was performed with immunohistochemistry. The transcripts of the Fas-L gene were detected at similar levels in both polyps and nasal mucosa. There was a significant overexpression of Fas-L protein on nasal polyps compared to nasal mucosa. Fas-L-positive cells were localized on the epithelial layers of cystically dilated glands and the down-growing epithelium of nasal polyps. Fas-L may play an important role in the pathogenesis of human nasal polyps, including cystic degeneration of submucosal glands and conferring of immune privilege to nasal polyp formation.

The presence of IgE on the surface of lymphocytes in nasal polyps.

The presence of IgE-bearing lymphocytes in nasal polyps was correlated with the patients' atopic status. Following surgical removal, the polyp tissue was treated with hyaluronidase and a single-cell suspension was obtained. Lymphocytes were isolated by gradient centrifugation, and the fluorescent antibody technique was used to study the presence of various immunoglobulin markers on the surface of lymphocytes. The presence of IgE-bearing cells was correlated with serum IgE levels, history of allergy, and skin test reactions. Patients with a positive atopic history had intermediate to high serum IgE levels. There was no correlation between IgE level and skin reactivity in these patients. Good correlation was obtained between the number of IgE-bearing cells in nasal polyps and serum IgE levels in atopic patients. The IgE-bearing cells represented 10 to 40 per cent of total immunoglobulin-bearing lymphocytes. No IgE-bearing cells were detected in five of six patients with a negative atopic history and negative skin tests. Thus IgE may be synthesized within nasal polyps of atopic patients, and the polyps in atopic patients may have a different etiology from those in nonatopic patients.

A case of metastatic nasopharyngeal carcinoma in benign nasal polyps.

An unusual case of recurrent nasopharyngeal carcinoma occurring within the stroma of benign nasal polyps is described. The patient was primarily treated with radiotherapy at first diagnosis. Two years later, he was diagnosed to have benign nasal polyps and had endoscopic sinus surgery. The recurrence was only diagnosed from histological examination of the polyps. The necessity for preoperative histological examination of any nasal polyp in patients with a past history of nasopharyngeal carcinoma and the importance of histological examination of all nasal polyps removed at surgery are emphasized.

A histological study of formation and growth of nasal polyps.

Chronic infection and allergy are considered to be the two major etiologic factors of nasal polyps. Whatever its cause may be, the initial phase of nasal polyp formation is represented by increased exudation from vessels, edema of the lamina propria and the bulging of the nasal mucosa. Nasal polyps were histologically classified into three types: edematous type, glandular and cystic type (ductal type) and fibrous type. Those of the edematous type and the glandular and cystic type are in the active stage of tissue reaction because of permeation of vascular fluid and marked infiltration of round cells. Contrastingly, the fibrous type polyps are in the healing stage of tissue reaction because of marked proliferation of fibroblasts and collagen fibers. The morphological structure of the nasal glands involved in the bulging mucosa determines the type of nasal polyps, whether edematous type or glandular and cystic type.

Nasal polyps--a preliminary diagnosis.

It is often considered that where nasal obstruction can be attributed to the presence of nasal polyps that the diagnostic process is at an end. The fact that the majority of simple nasal polyps are benign reinforces this view. Linked with this, the fact that the majority of serving soldiers are healthy, may lead the unwary into not suspecting some of the other types of nasal polyp.

[Expressions of chloride channel ClC-2 and ClC-3 in human nasal polyps].

OBJECTIVE: To investigate the role of chloride channel ClC-2 and ClC-3 in human nasal polyps. METHOD: Two chloride channels ClC-2 and ClC-3 were studied in 27 cases of nasal polyps with immunochemistric staining. RESULT: Nasal polyps revealed strong positive staining of ClC-2 and ClC-3, the ClC-2 and ClC-3 proteins localized mainly in epithelial cells, sub-epithelial mucous glands without significant distinction. The number of positive cells of ClC-2 and ClC-3 are 17.6 +/- 6.33 and 23.49 +/- 6.14. CONCLUSION: The chloride channels ClC-2 and ClC-3 may play a role in the formation of nasal polyps.

Distributional characteristics of sulfated glycosaminoglycans in normal human nasal mucosa and nasal polyp.

The results of this study suggest that an altered expression pattern of sulfated glycosaminoglycans (GAGs) in nasal polyp may not function as a charge restriction barrier for vascular permeability, contributing to the leakage of protein and fluid. Sulfated GAGs are found on the vascular endothelial surface and in the extracellular matrix in various tissues and organs, suggesting that these materials constitute a negatively charged screen restricting the movement of circulating plasma molecules. This study was designed to elucidate the distributional characteristics of sulfated GAGs in normal human nasal mucosa and nasal polyp in order to understand their roles in the formation of nasal polyp. The expression and localization of sulfated GAGs in normal human nasal mucosa and nasal polyp were detected light microscopically with the histochemical method using poly-L-lysine-conjugated colloidal gold followed by silver enhancement. Sulfated GAGs in normal human nasal mucosa were distributed in the epithelial layer, vascular endothelial surface, submucosal gland and extracellular matrix. In nasal polyp, intense staining was also seen in the glandular structure and epithelial layer. However, the vascular endothelium and extracellular matrix exhibited either a weak reaction or no reaction.

[Quality of life survey on patients with chronic rhinosinusitis and nasal polyps].

OBJECTIVE: To explore the health-related quality of life (QOL) status of patients with chronic rhinosinusitis and nasal polyps. METHODS: One hundred and twenty patients with chronic rhinosinusitis and nasal polyps and two hundred individuals passing health examination were enrolled by random and their QOL scores were assessed by using QOL instruments including existing SF-36 questionnaire (Chinese version ) and SNOT-20 questionnaire translated into Chinese, of which clinimetric and psychometric properties were tested. RESULTS: The feasibility, reliability, validity, and responsibility of Chinese version of SNOT-20 questionnaire all passed the test. By the assessment of SF-36 questionnaire, it revealed that scores of six domains such as physical functioning, role physical, bodily pain, mental health, vitality, and general health from patients with chronic rhinosinusitis and nasal polyps were lower than that of control group except social functioning and role emotional (P < 0.05). It showed by Chinese version of SNOT-20 questionnaire that patients exceeded healthy individuals in the scores of twenty items of three domains including physical problems, functional limitations, and emotional consequences, of which the most five important items affecting health status were respectively need to blow nose, thick nasal discharge, lack of a good night's sleep, dizziness, and post-nasal discharge (P < 0.05). CONCLUSIONS: QOL instruments such as Chinese version of SF-36 and SNOT-20 questionnaires can effectively differentiate the QOL status between patients with chronic rhinosinusitis and nasal polyps and healthy individuals. The negative impact of chronic rhinosinusitis and nasal polyps on patients' QOL includes physical functioning, role physical, bodily pain, mental health, vitality, general health, and emotional consequences. The problems of nasal discharge, sleep, and dizziness should be sufficiently emphasized in clinical treatment.

Characterization of the eosinophil chemokine RANTES in nasal polyps.

Previous studies have demonstrated that the cytokine RANTES (Regulated And Normal T cell Expressed and Secreted) has been shown to be a potent mediator of eosinophil chemotaxis in vitro and of leukocyte recruitment. Because eosinophils are the hallmark cells in nasal polyposis, we hypothesize that RANTES is locally produced within the nasal polyp microenvironment and is responsible for the eosinophil recruitment seen in nasal polyposis. To begin to test this hypothesis, we evaluated nasal polyps from 17 patients and 3 control specimens for distribution and content of RANTES using immunohistochemical techniques and enzyme-linked immunosorbent assay technology. Our immunohistochemical studies demonstrated that in nasal polyposis, RANTES antigen staining occurred predominantly within eosinophils and epithelial cells. To quantify the relative levels of RANTES in normal and nasal polyp specimens, tissue homogenates were prepared, quantified, and normalized to protein levels. We detected RANTES in all 17 nasal polyp tissue homogenates (566 +/- 16 pg/mg total protein). The RANTES levels in nasal polyp homogenates were nearly 40-fold higher than the RANTES levels in normal tissue (15.7 +/- 28.2 pg/mg total protein). Thus, it appears that increased expression of RANTES by eosinophils and epithelial cells within the nasal polyp microenvironment promotes eosinophil recruitment and activation within nasal polyps. We hypothesize that RANTES induces increased recruitment and activation of eosinophils, presumably contributing to the increased tissue changes associated with nasal polyposis.

[Expression of hypoxia-inducible factor-1alpha in nasal polyps and its correlation with vascular endothelial growth factor and inducible nitric oxide synthase].

OBJECTIVE: To detect the expression and distribution of hypoxia-inducible factor-1alpha (HIF-1alpha) in nasal polyps, to explore the relationship of HIF-1alpha with vascular endothelial growth factor (VEGF) and inducible nitric oxide synthase (iNOS), and to investigate the role of HIF-1alpha in the pathogenetic mechanism of nasal polyps. METHODS: Using the techniques of immunohistochemistry and reverse transcription polymerase chain reaction, the expressions of HIF-1alpha, VEGF, iNOS protein and HIF-1alpha mRNA were examined. The specimens were obtained from patients underwent SMR surgery during the same period, including thirty-one nasal polyps and ten inferior turbinate tissues. RESULTS: According to immunohistochemical research, the expressions of HIF-1alpha, VEGF and iNOS were higher in nasal polyps than those in inferior turbinate tissue (P < 0.01); The result of RT-PCR showed that there was no significant difference between the expression of HIF-1alpha mRNA in nasal polyps and inferior turbinate tissues (P > 0.05); No correlation was found between the expression of HIF-1alpha and the clinical stages of nasal polyps (F = 0.42, P > 0.05); There was a positive correlation between the expression of HIF-1alpha and that of VEGF (r = 0.630, P < 0.05). No significant correlation was found between the expression of HIF-1alpha and that of Inos (r = 0.144, P > 0.05). CONCLUSIONS: Although the mRNA level of HIF-1alpha did not change much, the expression of its protein was significantly higher in nasal polyps than in inferior turbinate tissues. There was no correlation between the expression of HIF-1alpha and the clinical stages. A positive correlation was found between HIF-1alpha and VEGF, but no correlation was detected between the HIF-1alpha and iNOS. In conclusion, our study indicates that hypoxia may play an important role in the pathogenetic mechanism of nasal polyps.

[A study on the expression of IL-8 and IL-3 in human nasal polyp and polyposis].

OBJECTIVE: To study the expression of IL-8 and IL-3 in human nasal polyp and polyposis. METHOD: Using immunohistochemical methods detect the expressions of IL-8 and IL-3 in 52 cases of middle turbinate, nasal polyp and nasal polyposis, and calculating the positive area percentages in each group. All datas were analysed with SNK test, Kruskal-Wallis test and Mann-Whitney test. RESULT: There were very significant differences between either two groups about the expressions of IL-8. There were significant differences between either two groups about the expressions of IL-3. CONCLUSION: IL-8 is not only one of the causes in nasal polyp, but also plays an important role in the recurrence of nasal polyposis. The expression of IL-8 in polyp is different from that in polyposis, it indicate that polyp and polyposis have different mechanism. IL-3 is important for the accumulation of eosinophil and is a key factor in the mechanism of polyposis.

[Expression of intercellular adhesion molecules in nasal polyps tissue and its significance].

OBJECTIVE: To study the expression of intercellular adhesion molecule-1/lymphocyte function-associated antigen-1 in nasal polyps tissue and its significance. METHODS: Frozen sections of 9 normal nasal mucosa and 19 nasal polyp samples were studied with immunohistochemical technique and immunohistochemical and MGG dual staining method with ICAM-1 and LFA-1 monoclone antibodies. RESULTS: Expression of ICAM-1 and LFA-1 were stronger in nasal polyps tissue than in the normal nasal mucosa, and there is positive relationship between the expression of ICAM-1 and the expression of LFA-1 on eosinophils. CONCLUSION: High expression of ICAM-1 and LFA-1 in nasal polyps tissue showed that interaction of ICAM-1/LFA-1 may participate in the inflammatory reaction and prompt adhesion and migration of eosinophils. High expression of LFA-1+ on eosinophils may play an important role in the pathogens of nasal polyps.
