The place of radioisotopic lung function studies in paediatrics.

An analysis of the clinical value of radioisotopic lung function (RLF) tests has been carried out on the results of studies in 123 children with heart or lung disease. The overall incidence of useful RLF tests was 53%, but in children between 6 months and 6 years the incidence was higher (69%). The incidence of useful RLF test was similar whatever the question to be answered, although rather higher when information about differential pulmonary blood flow was required. Lists of conditions are presented for which the incidence of useful RLF tests is likely to be high (72%), moderate (46%), and low (14%).

Quantitative measurement of faecal blood loss: comparison of radioisotopic and chemical analyses.

Blood loss in faeces was assessed by three different methods in five patients with recurrent iron deficiency. In short term (12 day) studies chemical analysis of complete stool collections for haemderived porphyrins (HemoQuant) gave results closely correlated with those obtained by measuring stool loss of 51Cr-labelled red blood cells. Whole body counting for 59Fe was relatively insensitive to small blood losses but allowed losses to be followed up over longer periods. Chemical analysis of faecal porphyrins thus provides a satisfactory alternative to radioisotopic techniques in short term quantitation of faecal blood loss, while longer term whole body counting of 59Fe may still be appropriate in a few patients for the detection and quantification of intermittent blood losses.

Radioisotope dilution technique for determination of zinc absorption in vivo.

A radioisotope dilution technique was adapted for determination of the absorbability of dietary zinc. In 16 growing rats given a casein diet with 12.8 mg Zn/kg dry matter, a mean apparent Zn digestibility of 72.4% was observed in a 6-day balance period 9 days after an intramuscular injection of 65Zn-ZnCl2. There was no significant difference in the specific radioactivity of zinc in blood plasma on day 15 and in urine collected from day 9 to day 15 used to assess the fecal excretion of endogenous zinc. Consequently, there was good agreement between the corresponding mean zinc absorbability (94.8 vs. 95.3%) calculated from these data.

Increased nuclear damage by high linear energy transfer radioisotopes applicable for radiodirected therapy against radiologic malignancies.

High linear energy transfer radioisotopes carried by appropriate agents have been proposed for receptor-directed radiotherapy. Two such classes of isotopes are Auger electron and alpha-emitting nuclides. To determine the relative cytotoxicity and nuclear damage to cells produced by these two classes of nuclides, we compared bromine-80m (80mBr), an Auger-electron-emitting radionuclide with a 4.4-hour half-life, with bismuth-212 (212Bi), an alpha-emitter with a 1-hour half-life. Because of the short path length of the Auger electrons, 80mBr was radiotoxic only when incorporated into DNA, such as in the form of [80mBr]bromodeoxyuridine ([80mBr]BrUdR). Both agents induced linear increases in chromosome aberration frequency, however, [80mBr]BrUdR caused multiple aberrations including the shattering of parts of the chromosomes. While, in contrast, a 2-hour exposure of cells to 212Bi, chelated to DTPA, a form which does not enter the cell, induced much less extensive chromosome damage. Exposure to equivalent activities of Auger electrons or alpha-particles results in 5 times more damage in Auger-electron-exposed cells. However, estimates of dose suggest they are equally toxic. Unlike Auger electrons, alpha-particles did not need to be in as close proximity to the DNA to have clastogenic and radiotoxic effects.

DETECTION OF PERICARDIAL EFFUSION BY RADIOISOTOPE HEART SCANNING.

A marked difference between the cardiac silhouette on the six-foot chest roentgenogram and the cardiac blood pool, determined by radioisotope scanning, has been shown to be consistent with pericardial effusion and/or thickening. It has also been observed that the cardiac blood pool is separated from the liver margin by the interposition of pericardial fluid and/or thickening. This separation was not demonstrated in the presence of a normal pericardium. To appreciate these features, 400 muc. of radioiodinated human serum albumin and 50 muc. of colloidal radiogold were used for scanning. The former outlind the blood pool and the latter demonstrated the position of the liver.

[Studies on the remineralization of enamel using radioisotopes].

Enamel of human and bovine teeth was found to show sorption of free calcium ions after damage caused by short-term exposure to diluted hydrochloric acid or mechanical damage. The experiment was carried out using radioisotopes 32P and 45Ca and it was shown that majority of incorporated ions remained in the enamel after prolonged washing out with water as well as after brushing with toothpaste. A considerable stability of binding of the incorporated phosphate ions was noted in the damaged enamel after exposure to calcium salt solution, and marked stability of binding of the incorporated calcium ions after exposure to phosphate solution.

[A radioisotope method for assessing the anaphylactogenicity of polymer drugs].

A radioisotope technique has been developed for the quantitative assessment of microcirculation disorder in a local anaphylactic focus of guinea pigs sensitized with horse immunoglobulin G and polyethyleneoxide conjugate with 2,4-dinitrophenol. The technique is based on the use of a radioactive tracer, dextran derivative, labelled with radionuclide 125I. The method can be used for studying a safety of new polymer-containing drugs.

[Ectopic or migrating spleen and supernumerary spleens: detection and identification with 99mTc, 51Cr and 113In radioisotopes].

Congenital malformations of the spleen are rare. We report a patient with an ectopic spleen located in the left iliac fossa, which was excised in order to prevent severe complications such as torsion of the pedicle, rupture or infection. A second patient with idiopathic thrombocytopenic purpura presented with an accessory spleen located retroperitoneally. Removal of this spleen following removal of the normotopic one resulted in cure of purpura. The clinical, radiological and radioisotopic studies used to locate and identify these malformations are described.

Radioisotope investigations of haematological disorders (excluding sickle cell disease) in Sierra Leone.

Using 51Cr, 125I, and 59Fe, red cell survival, blood volumes and certain aspects of iron metabolism were investigated in Sierra Leoneans referred for further evaluation of various blood disorders. The data provided clarification of the nature of the anaemia in some patients and demonstrated the haemolytic and erythropoietic role of the spleen in others with splenomegaly. Blood volume values in healthy individuals were found to be similar to those of normal subjects in other populations. Increased plasma volumes commonly described in non-African patients with anaemia and splenomegaly were also recorded in this group of Sierra Leoneans. Information obtained from these studies indicates that facilities for radioisotope investigations would be an asset in the diagnostic evaluation of complex blood disorders and in haematological research in developing African countries.

In vitro radioisotopic labeling of the L-3 stage of Brugia malayi.

Ten radioisotopes (133Ba, 207Bi, 109Cd, 51Cr, 59Fe, 203Hg, 54Mn, 125Sb and 75Se) were used to label the L-3 stage of Brugia malayi. The in vitro uptake at 4 hours in a buffered salt solution was greatest for 207Bi and exceeded that of 203Hg, by a factor of 6 and the rest of the radionuclides by factors of 26 or more. Dimethylsulfoxide (DMSO) (1%) in the medium only slightly enhanced the uptake of 207Bi but increased 203Hg uptake by a factor of 2.5. After incubation in non-radioactive medium only 2% of the 207Bi was retained; for 203Hg the retention was 70% or better. Increasing the labeling time increased total uptake and retention of the radionuclides. The retention of the 203Hg injected intraperitoneally into jirds (Meriones unguiculatus) in the form of labeled L-3s was followed over a 16 day period by counting the jirds in a whole animal NaI crystal detector. One L-4 stage of B malayi was recovered at the end of this period.

[Leptospira distribution in the soil of a natural focus of the infection (an attempt at the radioisotopic labelling of infected voles)].

The mass radioisotope (32P) labelling of all tundra voles excreting Zeptospira, infected urine was carried out over the area of 1 ha, and the results of the experiment analyzed by the method of planar coherent graphs, showed the irregular (spotted) distribution of "infected spots", i. e. soil patches contaminated by Zeptospira. The borders of individual small foci of infection did not coincide with the outlines of soil and vegetational divisions of the area. The spatial structure of the "extraorganismic" part of the Zeptospira population was originally formed by the carriers and thus determined by their mobility, their manner of using the territory. Later this structure was changed by selective elimination under the influence of living conditions for Zeptospira in the soil. The "infected spots", subjected to such selection, seem to be capable of constantly sustaining Zeptospira; the presence of these organisms is also due to the fact that they are regularly added into the soil with urine excreted by the carriers visiting the same spots of their territory.

[Scanning the subarachnoid space of the spinal cord following endolumbar and cisternal administration of colloid solutions of radioisotopes].

The patency of the subarachnoid space of the spinal cord was studied by means of radioisotopes in 82 patients, among whom 18 had tumors of the spinal cord, 22 had herniation of the intervertebral disk, 18 had complicated fractures of the spine, and 11 had inflammatory diseases of the spinal cord and its meninges. Colloidal solutions of 198Au, 111In, and 131I were used for endolumbar and cisternal infusion. Scanning of the vertebral canal provides for the diagnosis of the level, degree, and extent of its compression caused by various vertebro-medullary pathologic conditions. The method is sparing, minimum doses of isotopes are used, and no complications are produced.

Radioisotopic method for the measurement of lipolysis in small samples of human adipose tissue.

To facilitate the study of adrenoreceptor response in small needle biopsy samples of human subcutaneous adipose tissue, we developed a dual radioisotopic technique for measuring lipolysis rate. Aliquots (20-75 mg) of adipose tissue fragments were incubated in a buffered albumin medium containing [3H]palmitate and [14C]glucose, each of high specific activity. In neutral glycerides synthesized in this system, [14C]glucose is incorporated exclusively into the glyceride-glycerol moiety and 3H appears solely in the esterified fatty acid. Alpha-2 and beta-1 adrenoreceptor activation of tissue incubated in this system does not alter rates of 14C-labeled glyceride accumulation, but does produce a respective increase or decrease in the specific activity of fatty acids esterified into newly synthesized glycerides. This alteration in esterified fatty acid specific activity is reflected in the ratio of 14C:3H in newly synthesized triglycerides extracted from the incubated adipose tissue. There is a high correlation (r = 0.90) between the 14C:3H ratio in triglycerides and the rate of lipolysis as reflected in glycerol release into the incubation medium. The degree of adrenoreceptor activation by various concentrations of lipolytic and anti-lipolytic substances can be assessed by comparing this ratio in stimulated tissue to that characterizing unstimulated tissue or the incubation medium. This technique permits the study of very small, unweighed tissue biopsy fragments, the only limitation on sensitivity being the specific activity of the medium glucose and palmitate. It is, therefore, useful for serial examinations of adipose tissue adrenoreceptor dose-response characteristics under a variety of clinical circumstances.

Effects of radiolabeling monoclonal antibodies with a residualizing iodine radiolabel on the accretion of radioisotope in tumors.

The effect of using a "residualizing" iodine radiolabel, dilactitol-iodotyramine, for radioimmunolocalization of antibodies to tumors was investigated. This tracer is designed to be lysosomally trapped after catabolism of the labeled antibody. mAbs RS7 and RS11 were used for in vivo and in vitro studies on the uptake and retention of radioisotope into tumor cells. Both are murine IgG1 mAbs with pancarcinoma reactivity, which react with integral membrane glycoproteins. mAb RS7 has been shown to be relatively rapidly catabolized by the antigen-bearing cell line Calu-3, whereas RS11 is catabolized more slowly in the same cells. An 111In- or 88Y-p-isothiocyanatobenzyl-diethylenetriamine pentaacetic acid conjugate was also tested because these radiometals are known to be lysosomally trapped, and iodination via chloramine T was used to provide a baseline. In vitro, a substantial increase in retention of the label by cells was observed when the dilactitol-tyramine DLT- or 111In-labeled mAbs were used, and the improvement gained by the use of these residualizing labels was greater with the use of the rapidly catabolized mAb (RS7) than it was with the more slowly catabolized mAb (RS11). In biodistribution studies in nude mice bearing Calu-3 tumor xenografts, a dramatic improvement in the tumor accretion of the radiolabel was seen with the use of the 131I-labeled DLT- or 88Y-labeled mAbs. For example, at day 7 the percentage of injected dose/g in the tumor was 5.54 +/- 1.47% (SD), 38.06 +/- 8.04%, and 43.18 +/- 19.50% for the conventionally iodinated, DLT- and 88Y-labeled RS7, respectively. Dosimetry calculations performed on the biodistribution data predict increases of approximately 8- and 4-fold in the absorbed dose to tumor with the use of 131I-labeled DLT- and 90Y-labeled mAbs, respectively, compared to the conventional 131I. In contrast to in vitro findings, these results were similar for both RS7 and RS11, suggesting that the use of DLT may be advantageous for most of the mAbs binding to the cell surface, including antibodies that are catabolized relatively slowly. The advantage of 131I-labeled DLT over 90Y is due to the longer physical half-life of the 131I.

Radioisotope investigations in some hemolytic disorders of children.

Ferrokinetics and life-span of erythrocytes were investigated with 59Fe and 51Cr respectively, in 15 children with thalassemia major, aged between 18 months and 15 years (five of which had been splenectomized), and in 8 children with hereditary spherocytosis aged between 12 and 14 years. In all cases studied with 59Fe there was low tracer incorporation in erythrocytes, but no correlation of this parameter to the amount of Hb F. The use of radioisotopes in studies of hemolytic disorders of children is stressed, primarily when splenectomy is considered. Investigation of ferrokinetics in thalassemic children is useful for monitoring advanced stages of the disease.

Radioisotope cisternography in acute viral encephalitis. A reappraisal.

Five cases of presumed acute viral encephalitis with convulsions were examined with radioisotope (RI) cisternography six and 24 hours after an intrathecal injection of 1 mCi of pentetic acid labeled with either ytterbium 169 or indium 111. All cases showed abnormalities with this study. The "cold" areas observed with RI cisternography were well correlated with abnormal foci on the EEG. Although the findings are nonspecific, the CSF dynamics and patency of the subarachnoid space are easily examined by RI cisternography without appreciable complications. It is a useful supplementary diagnostic method to depict the extent of lobar abnormalities of cerebral cortex, particularly at an early stage, that either narrow or obliterate subarachnoid space and CSF pathways.

Dextran's effects on stressed lenses: water, electrolyte, and radioisotope studies.

To evaluate the beneficial effects of dextran 40 as an additive to infusion solutions, we studied an experimental model of lens stress with use of buffered, low calcium (Ca++)-containing solutions. Incubation in low Ca++ solutions (pCa = 10.7) for ten hours (stress period) resulted in lens swelling and electrolyte imbalances that were irreversible even with reincubation in physiologic, normal Ca++-containing media (pCa = 2.7) (recovery period). The addition of 6% or more of dextran to the media inhibited lens water gain during the stress period. It also rendered the resultant electrolyte imbalances reversible during the recovery period, thus exerting a protective effect. Radioisotope-tracer studies showed that dextran improved the ability of the lens to accumulate rubidium chloride Rb 86 and reduced its efflux during both the stress and recovery periods. Although dextran did not markedly decrease sodium chloride Na 22 uptake by lenses under stress, it did allow the lens to remove the 22Na during the recovery period.
