Reticulohistiocytoma involving the skin, subcutaneous tissue and a regional lymph node.

A 30-year-old female complained of a surface-eroded solitary nodule on the right thigh. Histologically, the dermal lesion consisted of uniform-sized polygonal cells with eosinophilic, 'ground glass' cytoplasm. Mitoses were infrequent. Under the histopathologic diagnosis of amelanotic melanoma, wide resection of the skin and dissection of the inguinal lymph nodes were performed. The subcutaneous tissue and a lymph node showed nodular proliferation of histiocytoid cells, in association with hemosiderin-laden multinucleated giant cells. The mononuclear cells were immunoreactive for factor XIIIa, while the multinucleated cells were positive for CD68, lysozyme and HLA-DR. In the lymph node tissue, a considerable number of mononuclear cells positive for CD68 were noted. CD34, alpha-smooth muscle actin, desmin and HMB45 were negative. Ultrastructurally, the mononuclear cells were rich in 100 nm vesicles and 180-350 nm lysosome-like granules. Interdigitation of the plasma membranes was seen in the multinucleated cells. The patient did not complain of joint symptoms, and has been disease-free for 5 years. The histologic and immunohistochemical features are consistent with so-called 'reticulohistiocytoma', though the site of histiocytic growth was unusual.

Oedema-preventing mechanisms in subcutaneous tissue of normal pregnant women.

Fluid transport between the plasma and interstitial fluid compartment is governed by the Starling forces, i.e. the capillary pressure (Pc), interstitial fluid hydrostatic pressure (Pi) and colloid osmotic pressure in plasma (COPp) and interstitial fluid (COPi). The COPp, COPi and Pi were measured in 10 normal pregnant women in the first and 10 women in the third trimester of pregnancy. Interstitial fluid was collected from subcutaneous tissue by implanted wicks and Pi was measured by the 'wick-in-needle' technique. The COPp was reduced from 23.2 mmHg in the first trimester to 21.1 mmHg in the third trimester. Concomitantly, COPi decreased from 13.1 to 8.4 mmHg on the thorax and from 9.6 to 5.5 mmHg at the ankle. Only small changes in Pi were observed. The more marked fall in COPi than in COPp indicates that a rise in Pc, in addition to hypoproteinaemia, contributes to increased capillary fluid filtration in pregnancy. The reduction in COPi opposes the increased filtration and thereby prevents a rise in interstitial fluid volume and oedema formation. These physiological changes imply a reduced safety margin against oedema formation in late pregnancy.

Excision of subcutaneous tissue for the treatment of axillary osmidrosis.

Axillary osmidrosis is a distressing problem. Medical treatment is often inadequate. Local excision of the apocrine and eccrine glands is the most effective method available but is often accompanied by significant morbidity. We report a modified surgical technique for the treatment of this condition. From January 1994 to December 1995, 46 patients (38 females, 8 males) with axillary osmidrosis have been treated by excision of the subcutaneous tissue via two transverse incisions, without removing skin. Sutures are used to anchor the skin to the axillary fascia. There is no need for a complicated tie-over dressing and postoperative arm restriction. All patients were followed up for a minimum of 6 months (average 11.6 months). The patients were asked to complete a questionnaire. The results of malodour elimination were classified as good, fair and poor. Forty-one (89.1%) of the patients had good results, four (8.7%) had fair results, and one (2.2%) had a poor result. The average convalescent time was 9.2 days. Twenty-six patients (57%) were very satisfied and recommended this procedure. Only one patient (2%) regretted having the operation. This operation has the advantages of a high success rate, low complication rate, and rapid recovery for the treatment of axillary osmidrosis.

Paraspiralatus sakeri n. g., n. sp. (Nematoda: Spiruroidea, Spirocercidae) from saker falcons, Falco cherrug in Saudi Arabia and the first report of larvae from the subcutaneous tissues of houbara bustards, Chlamydotis undulata macqueeni in Pakistan.

A new nematode genus and species, Paraspiralatus sakeri, is described from the stomach of a wild-caught, female saker falcon in Saudi Arabia. This spirurid differs from the nearest genus and species Spiralatus baeri Chabaud, Brygoo & Durette, 1963 in the shape of the pseudolabia, shape of the buccal capsule and absence of a large cephalic vesicle. In addition, third stage spirurid larvae were recovered for the first time from subcutaneous tissues of two houbara bustards. These had died in the Rahim Yar Khan Rehabilitation Center (Houbara Foundation International, Lahore, Pakistan) in Pakistan and were examined at the National Avian Research Center in the United Arab Emirates. The morphology of the larvae and host pathology are described. Comparative studies with the adult spirurids from the saker falcon showed each to have similar cephalic and pharyngeal morphological features to the adults described indicating they are probably the same species. Spirurid nematodes of the suborder Spirurina normally have an arthropod intermediate host. In view of the host, the site from which the larvae were recovered and the fact that this is a rare occurrence, the houbara bustard is considered to be a paratenic host.

[A study of the absorption of NPH insulin administered into subcutaneous tissue: in vitro study of the mechanism of insulin release from NPH insulin crystal].

NPH (Neutral Protamine Hagedorn) insulin has been widely used for the patients with diabetes mellitus. NPH insulin crystal (NPH-C) is an insoluble ionically bonded complex which insulin and protamine formed at neutral pH. It has been thought that NPH-C dissolve in the injected site by splitting of crystals into protamine and insulin. But the mechanism of the dissolution of NPH-C injected into subcutaneous tissue has not been fully clarified. To investigate the mechanism of the release of insulin from NPH-C, we measured the concentrations of immunoreactive insulin (IRI) and arginine, major component of protamine, recovered from NPH-C incubated with human serum. The concentrations of IRI and arginine, recovered from NPH-C incubated with human serum at 37 degrees C, time-dependently increased, but remained unchanged at 4 degrees C. And the concentrations of IRI and arginine recovered from NPH-C incubated with Krebs-Ringer phosphate buffer at 37 degrees C or 4 degrees C did not change. The release of IRI and arginine from NPH-C with human serum was enhanced by the addition of CoCl2 and was inhibited by the addition of CdCl2 or EDTA. Human serum was fractionated using starch block electrophoresis and the NPH-C dissolving activities of each fractions were investigated. NPH-C dissolved in carboxypeptidase N rich-fraction, but not in plasmin-rich fraction. Carboxypeptidase N was purified from human serum and the activity of NPH-C dissolution was investigated. The relative activity of NPH-C dissolution in the purified Carboxypeptidase N was about 250-fold higher than that in the unpurified human serum.(ABSTRACT TRUNCATED AT 250 WORDS)

[A case of paragonimiasis in the abdominal subcutaneous tissue].

A 35-year-old housewife living in Seoul complained of a recurrent palpable abdominal mass. Excisional biopsy was done. The cystic mass showed an immature worm of Paragonimus sp. in the cyst cavity. It measured 7 x 4 mm and showed well-developed oral and ventral sucker, uterus, 5-branched ovary and intestine after acetocarmine staining. But the testes and vitelline duct were not developed fully and there was no egg in the uterus. The patient has eaten raw fish. The case of ectopic paragonimiasis in the abdominal subcutaneous tissue was presented.

On line continuous monitoring of subcutaneous tissue glucose in men by combining portable glucosensor with microdialysis.

For the normalisation of blood glucose levels in diabetic patients by feedback controlled insulin delivery, a self-manageable and reliable method for continuous glucose estimation is still not available. By combining a commercially available needle type dialysis probe (molecular cutoff 20,000 Da) with a sensitive glucose sensor, we obtained a device for continuous glucose measurement in dialysate. This device was tested in healthy volunteers during a 75-g oral glucose tolerance test and in Type 2 (non-insulin-dependent) diabetic patients. Venous glucose and subcutaneous sensor signal were followed for 300 min (ten healthy subjects), 21 h (three healthy subjects) or 9 h (seven Type 2 diabetic patients). The recovery of the microdialysis was interindividually different, but after calibration, glucose levels in the dialysate and subcutaneous glucose sensor signal correlated well (r = 0.84-0.95). Under the assumption of a physiologic and technical delay between intravenous and subcutaneous glucose, correlation coefficient between intravenous glucose and subcutaneous sensor signal ranged from 0.60 to 0.93. We conclude that changes in blood glucose could be monitored in the subcutaneous tissue by the microdialysis technique in a continuous on line manner.

Solitary melanoma confined to the dermal and/or subcutaneous tissue: evidence for revisiting the staging classification.

BACKGROUND: Several patients presented with a single focus of presumed cutaneous metastatic melanoma with an unknown primary tumor based on clinical and histologic staging criteria of the American Joint Committee on Cancer (AJCC). This population is classified as having stage IV disease by the current AJCC staging system, which carries a dismal prognosis (5%-18% 5-year survival). Our clinical observation was that these patients had a higher survival rate than would be expected for stage IV disease. We believe this population represents a subgroup of primary dermal- and or subcutaneously-derived melanoma that simulates cutaneous metastatic melanoma in histologic and clinical presentation but may differ in behavior. OBSERVATIONS: The database records of 1800 patients from the University of Michigan Melanoma Clinic, Ann Arbor, were retrospectively reviewed to identify the prevalence and survival for patients diagnosed with a single focus of presumed metastatic melanoma to the skin based on accepted histologic and clinical parameters. The prevalence of this population was 0.61% (11 of 1800 patients). The Kaplan-Meier 8-year survival estimate was 83% (95% confidence interval, 58%-100%). CONCLUSIONS: By AJCC convention, these cases are classified as stage IV metastatic disease. Our data suggest that these presumed metastatic tumors do not behave like stage IV metastatic disease to the skin via lymphatic or hematogenous spread from an unknown primary site; rather, they are behaving like primary tumors originating in the dermal and/or subcutaneous tissue.

Bone induction in subcutaneous tissue in rats by a newly developed DNA-coated atelocollagen and bone morphogenetic protein.

A unique biomaterial, a mixture of DNA and collagen (DNA/collagen), was developed and its efficacy as a carrier matrix for bone morphogenetic protein (BMP) was evaluated histologically. The material was prepared as a composite of DNA from salmon milt and pepsin-digested type I collagen (atelocollagen) from bovine dermis. Phase-contrast and fluorescence microscopy showed atelocollagen fibres with DNA coating. The dose-response and time-course of bone induction by BMP in DNA/collagen (5 x 10 x 1 mm) in the subcutaneous tissue was investigated in 20 male Wistar rats. The BMP/DNA/collagen induced new bone in a dose-dependent manner (0, 25, 50 or 100 micrograms of BMP). Histological examination in the time-course study showed that the BMP (100 micrograms)/DNA/collagen induced bone formation, while the DNA/collagen alone resulted in the accumulation of fibroblasts. These results indicate that the DNA/collagen is effective as a carrier matrix for BMP. It provides a cell anchorage for differentiation of osteoblasts and is absorbed as bone matures.

Electrolyte concentrations in subcutaneous tissue fluid and lymph.

The electrolyte concentrations of subcutaneous lymph and tissue fluid collected with the wick technique were compared with those of blood serum. The Na+ and K+ concentrations were found to be higher in tissue fluid and lymph than in serum, Cl- concentration was lower in lymph, but in tissue fluid it was about equal to the concentration in blood serum. These relationships do not correspond to the Gibbs-Donnan equilibrium. On the basis of electrolyte concentrations and the equilibration of intravenously injected 131I albumin it is concluded that the analysis of regional lymph gives reliable information about the steady state concentration of electrolytes and small molecules in the interstitial fluid. Acute changes, especially those of macromolecule concentration in tissue fluid can be more readily followed by the analysis of the fluid obtained by the wick technique.

Recombinant factor VIII SQ--stability of VIII: C in homogenates from porcine, monkey and human subcutaneous tissue.

The aim of this paper was to investigate whether a formulation-based approach to understanding and addressing stability could generate a subcutaneous factor VIII preparation for patients as an alternative to the existing intravenous products. The low bioavailability of subcutaneously administered factor VIII could have several causes: proteolytic degradation of the protein in the interstitium; adsorption to tissue, in particular to acidic phospholipids such as L-alpha phosphatidyl-L-serine (phosphatidylserine); the absence of free von Willebrand factor in the interstitium; phagocytosis by macrophages in the interstitium or in the lymph nodes; and coagulation could be initiated upon injection. This study was undertaken to investigate the first three factors in-vitro (i.e., proteolytic degradation, adsorption to tissue and the protective effect of von Willebrand factor). The influence of some other macromolecular stabilisers and protease inhibitors was also investigated. The stability of factor VIII activity (VIII: C) was investigated in homogenates from porcine, monkey and human subcutaneous tissue. Possible coagulation was prevented in these studies by the presence of both citrate and antithrombin. An exploratory in-vivo study was performed in the pig; plasma samples were assayed with a factor VIII:Ag (90kDa) ELISA. The decrease in VIII:C appeared to be more pronounced in homogenates from monkey and human tissues than in porcine homogenate. The results from human tissue homogenate resembled the degradation profile seen in monkey homogenate. Both the von Willebrand factor and phosphatidylserine/phosphatidylcholine (PS/PC) liposomes showed a significant stabilising effect on VIII:C in the tissue homogenates. The qualitative pattern was similar in porcine, monkey and human tissue. A combination of several protease inhibitors seemed to have a protective effect on the stability of VIII: C albeit at high concentrations of inhibitors and the effect was less than that of PS/PC. An exploratory in-vivo study was performed in the pig with phosphatidylserine in two formulations; either in the form of PS/PC liposomes or together with Polysorbate 80 in the form of mixed micelles (phosphatidylserine/P80). Including phosphatidylserine in the formulations appeared to increase the availability, of subcutaneously administered r-VIII SQ in the pig. However, further studies are necessary, preferably in the monkey where in-vitro studies indicate a closer resemblance to the human. In conclusion, a proposed inactivation mechanism for r-VIII SQ in subcutaneous tissue could be adsorption to phospholipid surfaces followed by proteolytic degradation. However, additional studies are required due to the multitude of factors influencing the subcutaneous absorption route. A combination of protease inhibitor(s) together with phosphatidylserine-containing liposomes are suggested for further investigation, preferably in a monkey animal model.

Subcutaneous tissue in the scalp: anatomical, physiological, and clinical study.

The galea aponeurotica has been used by various surgeons either as a supporting sheet or as a blood carrier for bringing flaps to various areas of the face. In this study, we first reviewed the anatomy of the scalp vascularization by means of cadaver injections. The arteries and veins are mainly located in the galea and numerous anastomotic vessels are present over the vertex. The physiology of this vascularization has been studied by percutaneous PO2 registration after occlusion of various vessels by means of a tourniquet placed around the head. The superficial temporal arteries have been found to be the most important vessels for designing flaps over the head. On the basis of these experiments, several patients have been operated upon. The possibility of using a skin island from the retroauricular region based on subcutaneous tissue only is described.

Effect of chronic sympathetic denervation upon local regulation of blood flow in human subcutaneous tissue.

The effect of chronic sympathetic denervation upon the vasoconstrictor response to an increase of vascular transmural pressure in human subcutaneous adipose tissue was investigated in 6 patients suffering from manual hyperhidrosis. Changes in transmural pressure were obtained either by postural changes of a forearm or by venous stasis of 30 mmHg. Blood flow was measured in the distal part of the forearm or crus by means of the local 133Xenon washout technique. 2 patients were studied before and after sympathectomy. When the area under study was lowered about 40 cm below the jugular notch, blood flow decreased about 50% preoperatively, about 30% 24 h after the operation, but remained constant 4 days after or later. Similar results were obtained during venous stasis. Hence about 4 days after sympathectomy, the vasoconstrictor response to an increase in vascular transmural pressure was abolished. In 3 chronically sympathectomized patients blood flow remained constant in the denervated limb, but decreased significantly in the control limb. In another patient studied 580 days after surgery blood flow remained constant during lowering of the denervated forearm as well as during venous stasis. These findings might indicate that the vasoconstrictor response to an increase in vascular transmural pressure in human subcutaneous adipose tissue is due to a local nervous mechanism involving symphathetic adrenergic nerves, but a myogenic mechanism cannot be definitively excluded.
