A key criterion in the diagnosis of thyroid follicular carcinoma is capsular invasion, but invasion cannot always be demonstrated histologically. Since invasion is likely to evoke reactions in the capsular collagen, we examined the effects of invasion on capsular collagen with the picrosirius orange-red (PSR) staining technique for collagen. Under polarized light, the color of PSR-stained collagen varies as a function of the structural and biochemical properties of the collagen fibers. Capsules of widely invasive carcinomas (n = 10), minimally invasive carcinomas (n = 10), and adenomas (n = 28) were stained with the PSR method. Carcinomas were assessed along the thickened capsule for sites of definite invasion, minimal invasion, and no evidence of invasion. In adenomas, sites of thickened capsules (similar to carcinomas) were compared to sites of thin capsules. All foci were evaluated for the color and color intensity of collagen fibers. We found a significantly higher frequency of yellow-green collagen fibers than of orange-red fibers at sites of invasion, whereas orange-red fibers significantly predominated at non-invaded sites. In a minority of cases both colors occurred but the non-dominant color was of lesser intensity in all but 1 case. There were no significant differences in staining between minimally and widely invasive carcinomas. Thick capsules of adenomas consistently stained with an intense orange-red color, although weakly stained yellow-green fibers were also observed in some of these cases. We conclude that PSR staining can provide diagnostically useful information in capsular samples of carcinomas, when both color and color intensity of PSR staining are evaluated at the same site. Specifically, intense yellow-green birefringence of collagen in a thickened capsule is additional evidence for capsular invasion.
This report describes 28 cases of thyroids which had undergone infarction following fine-needle aspiration (FNA) biopsy procedure. Their cytologic diagnoses included 15 Hürthle cell tumors, eight papillary carcinomas, and five follicular neoplasms. Nine patients also had large-needle biopsy performed within 3 wk of FNA biopsy, three of which showed infarction and one had no surgical follow-up. Surgery was performed on 27 patients within 7 to 90 days. All 27 cases showed partial to total infarction. Histological and cytologic diagnoses were not in accordance in 12 cases, either due to complete fibrosis (two cases), obscuring of histologic details (four cases), or failure to recognize the thin rim of neoplastic tissue at the periphery, by pathologists unaware of the cytologic diagnoses (six cases). Fine-needle biopsy results should be made available to the surgical pathologists handling the surgically excised thyroid specimen. Also, partial or complete infarction must initiate careful examination, so as not to miss any neoplasms.

PURPOSE: Uptake and washout ratios of thallium-201 chloride (201TlCl) were studied to confirm their clinical applicability in the differential diagnosis of benign and malignant follicular lesions of the thyroid. PATIENTS AND METHODS: Sixty-six patients with follicular tumor of the thyroid underwent preoperative thallium scintigram after an intravenous injection of 2 mCi (74 MBq) of 201TlC1. The early accumulation and washout ratios of 201TlCl were obtained by an online data-processing system. All tumors were surgically resected and histopathologically diagnosed as either follicular adenoma (49 patients) or follicular carcinoma (17 patients). Scintigraphic values in terms of the early accumulation and washout ratios were compared between follicular adenoma and follicular carcinoma. RESULTS AND CONCLUSION: Both the early accumulation and washout ratios were significantly higher in follicular carcinoma than in follicular adenoma. It was concluded that dynamic studies on accumulation and washout rates of 201TlCl might be clinically reliable to differentiate between follicular adenoma and follicular carcinoma.

Pseudotumoral granulomatous foreign body reaction to Teflon particles is a rare complication of intracordal Teflon paste injection for unilateral vocal cord paralysis. We report a case of pseudotumor of the thyroid area, which appeared 3 years after intracordal Teflon paste injection and presented as a cold thyroid nodule. At frozen section examination the presence of a giant-cell foreign body granuloma containing numerous refringent particles suggested a 'teflonoma'. The diagnosis was confirmed on permanent sections, after comparison of the intratissular particles with fresh Teflon paste, under conventional and polarized light. The histological and ultrastructural features of the lesion are presented and attention is drawn to the long delay between initial Teflon injection and the clinical manifestation of the lesion.
