[The artificial lymph node].

Adsorption of lymphocytes on the sorbents in correction of inflammatory lesions has significant clinical perspectives as the cells and active biological substances they secrete function actively in inflamed tissues acting in the site of its application: every cell adsorbs and inactivates foreign substances, every cytokin affects certain target cells. The products of these cells functioning and dissociation as well as the products of immune system reaction to them remain on the sorbent and are eliminated together with granules during bandaging or by the end of the treatment thus do not entering the organism. Lymphocytic monolayer on the sorbent does not prevent its specific action in pathologic nidus. In this case sorbent does not only drain the tissue duplicating certain primitive non specific functions of regional lymph node (barrier, filtrating, transport, draining and protective functions) but also performs certain highly specialized functions of immunocompetent organs--selective adsorption and inactivation of antigenic substances. It is reasonable to use sorbents for adsorbing lymphocytes in kapron containers which exclude the preparation leak into wounds and cavities and act as a filter preventing fast inactivation of the preparation by large fragments of tissue detritus.

[The use of adaptive-repositioning apparatus for the closing of granulating soft-tissue wounds].

The authors propose a new method of closing granulating wounds by attaching its edges with the help of an adaptation-reposition apparatus. This method was used in treatment of 53 patients with granulating wounds of different localization. Good results were noted in 88.7% and satisfactory ones in 11.3%. The method was shown to be highly efficient as compared with the control group.

[Surgery of some diaphragm diseases].

The authors analyze 127 rare diaphragm diseases. Among them, the rate of true diaphragmatic hernias does not exceed 1%. The congenital through diaphragm defects were encountered three times more often than false hernias, equally frequently on the left and on the right. The majority of the patients (51.97%) had hernia of Larrey's fissure. The rate of true hernia (Morgagni's) was 3 times less than of false hernia (Larrey's). In Bochdalek's hernia (3.15%) false hernias were found three times more often than true hernias. Relaxation of diaphragm was found in 40.94% of the patients. Right-sided complete diaphragm's relaxation occurred 4 times less often than on the left, partial--10 times more often on the right than on the left. Duplication of diaphragm with inserting kapron net, velours or teflon between it layers remains the dominant way of surgical correction of total relaxation of diaphragm's cupula.

[Comparative assesment of methods for the operative treatment of dislocations of the acromial end of the clavicle].

During 20 years the authors observed 861 patients with dislocations of the clavicular acromial process, that made 14% in relation to all other types of dislocations. 252 patients (29.3%) were treated surgically. Capron tapes being used most frequently as an alloplastic material. Surgery was performed after the technic elaborated at the clinic. Late results of the treatment were studied in 202 patients within the terms from 1 to 15 years. Good issues were noted in 76.2%, satisfactory in 37%, poor in 5.5% of patients.

[Experimental study in rabbit on repair of radial bone defect by bioactive degradable polymer].

OBJECTIVE: To investigate the availability of bone defect repair by degradable porous polycaprolactam (PCL) as the carrier of bone morphogenetic protein (BMP). METHODS: Three different kinds of bone substitutes, including decalcified bone matrix, PCL-BMP compounds and simple PCL, were implanted into the radial bone defects in 36 rabbits, and in the other 12 rabbits the bone defects were left untreated as control. X-ray examination, X-ray morphometry, histological and electron microscopic observation were performed at different time after operation. RESULTS: The quantity of new bone formation in PCL-BMP group was prior to that in simple PCL group. The pattern and speed of bone defect repair in PCL-BMP group were similar to those in decalcified bone matrix. Electron microscopic observation showed that the PCL-BMP group degraded faster than simple PCL group, which was more suitable for bone repair. CONCLUSION: PCL is a good carrier of BMP with potential for clinical use.

[Formation of ergosterol by yeasts of Candida genus].

The formation of ergosterol by the Candida yeast was studied using glucose, n-paraffins, a mixture of organic acids and inedible sugars. Yeast of different species and strains showed a similar content of ergosterol which made, as a rule, 0.4--0.5%. The amount of ergosterol remained relatively stable with varying cultivation conditions and decreased 1.5--2 times with a temperature increase.

[A comparative assessment of modern suture materials in stomach resection].

Results of stomach resection by Bilrot-II in 30 experimental animals and 369 patients with gastric and duodenal ulcers were analyzed. Gastroenteroanastomoses were created with various sutures: capron, polysorb, titanium clips. Pneumopression, bacteriologic and morphologic examinations of anastomosis, fibroesophagogastroduodeno- and intestinoscopy, peripheral computed electrogastroenterography were used. It was revealed that mechanical suture with titanium clips was the best. Polysorb thread is inferior in tissue reaction to mechanical suture but superior to capron. Use of polysorb reduced rate of complications in immediate postoperative period 4 times. Taking into consideration high cost of suture devices it is reasonable to introduce polysorb in practice of abdominal surgery.

[Two methods of plastic reconstruction of extensive defects of the chest wall (experimental study)].

The authors examine results from the experimental using two new methods for surgical reconstruction of broad defects of the chest wall. Our study was applied to 18 dogs separated in two series. In the first group of 8 dogs we used bone-muscle lambo for plastic reconstruction formed from the scapula and m.serratus anterior. This autotransplantation tissue was fixed for the neighbouring ribs limiting the defect of the chest wall. A good hermetisation of the pleural cavity and stabilisation of the chest wall was achieved. In the second group of 10 dogs the chest wall defects was closed with special antimicrobal chest prosthesis. It is built up by synthetic net and synthetic ribs. The antimicrobic qualities are being preserved for a continuous period of time. Histological the animals from the first group show the creation of a stabile fibrosis-bone-muscle laver which creates stability and hermetically of the chest cell. The synthetical ribs of the animals from the second group are tightly embraced by cellular tissue that passes through the openings of the ribs. The clinical observations of this methods are an object of another report of ours.

[Closed complete dislocations of the crus and the result of their treatment].

Based on the analysis of treatment of 22 patients with complete closed crural dislocations, it is emphasized that this trauma is associated with extensive and specific injuries to the articular elements. Concomitant injuries of popliteal vessels were found in 4 (of 22) cases. Timely performed operative restoration of the arterial integrity yielded favourable late results in 2 patients. Two patients admitted in later terms were subjected to a high amputation of the leg. In 15 patients the knee joint instability developed due to ligamental rupture in crural dislocation. Eleven patients from this group were operated upon, plastic reconstruction of ruptured lateral and crucial ligaments and the removal of injured meniscuses being accomplished in them. Persistant good and satisfactory functional results were gained in 10 cases. Only one poor issue was noted.

[A new method for cannulation of the common bile duct in an experiment].

The proposed method of common biliary duct cannulation is simple, easily performed and permits studying bile hydrodynamics biligenesis for a necessary time. Tightness of the system is completely preserved after removal a choledochostomical tube. Walls of the invaginated stump of the cystic duct are swiftly abated as a result of hydrostatic pressure and cover its gap. The presence of purse string kapron suture in a stump orifice creates an obstacle to deinvagination. Good reparation properties of serous and subserous membranes, covering the biliary duct, promote rapid closing of stoma. Effectiveness of the proposed method of common biliary duct cannulation by means of the invaginated stump of the cystic duct is confirmed by the smooth postoperative course and absence of a biliary fistula after the tube removal.

Reaction of the human gingival tissue to different suture materials used in periodontal surgery.

The gingival reaction to 4 different suture materials used in periodontal surgery was studied in 36 patients. The gingiva was sutured prior to surgery and biopsies were taken at 3, 7 and 14 days to observe the tissue reaction. The histological examination showed that silk caused the most intense and longest inflammatory response. Polyester and perlon provoked shorter, less intense tissue reactions than silk, and nylon caused the least inflammatory response, with earlier tissue repair.
