[Esophagoplasty in caustic esophageal stenosis in children].

Over a period of 30 years, 53 children suffering from caustic stenosis of the oesophagus were treated by oesophagoplasty. The left colon was most often used (49), though on occasions a Gavriliu gastric tube (3) or the right ileo-colon was used (1). Technical failure occurred in 7 cases with left esophagocoloplasty and this was treated with either a gastric tube (4) or right esophagocoloplasty (3). Oesophagectomy to prevent cancer was carried out on 20 occasions after the oesophagoplasty, on 23 occasions at the same time at the oesophagoplasty, and has yet to be carried out in 6 children. Early mortality was zero; 4 uncomplicated cervical fistulae occurred; 12 anastomotic stenoses were cured by dilatation and 5 required further surgery. Death occurred in 3 children as a late event: one at 2 months, due to difficulty with deglutition, one at 2 years, due to meningococcal septicemia, and one at 3 years due to obstruction from adhesions. Peptic reflux was seen in 3/9 Gavriliu plasties and in 6/45 coloplasties. Functional sequelae were absent in 29 cases and 12 cases suffered occasional episodes of obstruction. The best procedure appeared to be posterior mediastinal esophagocoloplasty combined with resection of the damaged oesophagus.

[Esophagoplasty with bypass using an isoperistaltic gastric tube in the treatment of cancer of the thoracic esophagus: study of 13 cases].

In order to relieve complete obstruction of the thoracic esophagus due to spinocellular carcinoma, thirteen patients, all of them in good or at least regular general conditions were submitted to a bypass using a isoperistaltic gastric tube. The disease itself was treated by radiotherapy after surgery. Only one patient died at the hospital due to an error in the radiotherapy schedule. Cervical fistula was observed in six patients seven days after surgery; local treatment was enough for complete healing in five out of six. In one patient surgery was performed in order to repair the anastomotic area. Stenosis was observed in three patients but in all cases it was possible to overcome it by endoscopy. With the progress of the disease other complications were observed such as hemoptysis and respiratory distress. The survival rate was nine months (running between five and sixteen) but it should be noted that swallow capacity was maintained until death. The employed of a bypass of isoperistaltic gastric tube seems to be a desirable alternative in the treatment of esophageal cancer because it allows the reestablishment of deglutition before any treatment such as radiotherapy alone or combined with surgery.

[Esophagoplasty in caustic stenosis in children. Apropos of 28 cases].

28 cases of oesophagectomy for caustic stenosis in children are reported. The authors discuss the technical modalities and the evolution of ideas, especially the importance of the oesophagectomy, in front of the risk of a secondary cancerisation. This severe complication becomes accentuated with the years and prescribes in children more systematic oesophagectomy concomitant or secondary to the oesophagoplasty.

[Value of Tissucol in cervical anastomosis of esophagoplasty].

There is an important fistula rate associated with the cervical anastomosis of oesophagoplasties. The authors report their experience with reinforcement of these anastomoses by adhesion using a fibrinogen based biological tissue glue. They propose that this technique be used in all high risk anastomoses, in particular, for those affecting a non peritonealized section of the digestive tract.

[Vascular microsurgery during esophagoplasty in children].

Clinico-experimental data on using the microvascular technique in plastic operations on the esophagus are described. Experiments in puppies have revealed specific features of the surgical technique, the morphology of intestinal grafts and their taking under conditions of growing organism. The microvascular technique was used in 3 patients. In 2 patients additional nutrition of the grafts was performed, in 1 patient free transplantation of small intestine grafts to the cervical and thoracic parts of the esophagus was fulfilled. Results were good.

[Retrosternal esophagoplasties by gastric upturn. Functional value].

The functional results in 13 cases of retrosternal oesophagoplasty by gastric upturn were compared with those obtained in 20 retrosternal oesophagoplasties by interposed colon and with published data concerning intrathoracic isoperistaltic reconstructions. Nine patients were examined by radiocinematography and 6 by radioisotope scanning to evaluate the speed of passage through the neo-oesophagus and the quality of gastric emptying. It appeared that gastric emptying took place within normal delays. Signs of reflux were observed in 9 of the 13 patients, but the problem did not seem to be worse than with isoperistaltic reconstructions.

[Replacement esophagoplasty by the jejunum].

The authors discuss the problem of oesophagus replacement in children with congenital Type Vogt II atresia. They describe their own experience obtained from replacement, using jejunum.
