Interferon therapy for basal cell carcinoma and squamous cell carcinoma.

Totally 161 basal cell and squamous cell carcinoma (BCC, SCC) patients were treated with human natural leucocytic interferon (HNLI) and recombinant IFN alpha 2c. After HNLI treatment, 61 out of 86 BCC patients and 29 out of 45 SCC patients were cured according to histopathologic and clinical findings. In 13 BCC and 13 SCC patients, the cancer lesion was reduced 25%-90%. After recombinant IFN alpha 2c treatment, 14 of 20 BCC patients and 4 of 10 SCC patients were cured according to histopathologic and clinical findings. In 6 BCC patients and 5 SCC patients the cancer lesion was reduced 25% to 90%. Both types of interferons are effective in the treatment of BCC and SCC patients. Local application of interferon stimulates immune reaction at the site of the tumor. There is a marked difference between the spontaneous macrophage activity and that induced by interferon. The interferon activated macrophages are significantly larger, the number of lysosomes and the density of macrophages is increased. In difficult locations intralesional therapy can be considered to avoid disfigurement of the patients with or without surgery.

Seven different primary cancers in a single patient. A case report and review of multiple primary malignant neoplasia.

A 43-year-old female developed seven different primary carcinomas over a 14-year period. Six were adenocarcinomas and one a squamous carcinoma. Five of the six adenocarcinomas were locally advanced, but only two recurred after excision. There are very few reports in the literature of patients with seven or more primary cancers, but all demonstrated a remarkable absence of metastatic disease and surprisingly prolonged survival. They represent extreme examples of the phenomenon of multiple primary malignant neoplasia (MPMN) and the possibility of their belonging to a cancer family syndrome should always be considered.

[Pleomorphic adenoma of atypical localization].

The authors described 4 cases of multiform adenomata located in other than parotid gland sites. The diagnostic difficulties were stressed as well as the necessity of the histologic verification.

Palisaded encapsulated neuroma: an immunohistochemical study.

Ten palisaded neuromas of the skin were studied immunohistochemically for the presence of S-100 protein, epithelial membrane antigen, neurofilaments, glial fibrillary acidic protein, and positivity with the Leu-7 monoclonal antibody. In all cases, the fascicles of tumor cells were positive for S-100 protein and negative for epithelial membrane antigen; the tumor capsules were negative for the former in all cases but positive for the latter in seven of ten cases. In three lesions, epithelial membrane antigen-positive cells formed sheaths around fascicles of tumor cells. Axons were demonstrated by anti-neurofilament antibody in seven lesions. None of the lesions stained for glial fibrillary acidic protein. All of them showed positivity with the Leu-7 antibody, which stained both tumor spindle cells as well as membranous profiles consistent with myelin sheaths. These results indicate that the tumor is composed of cells of schwannian differentiation whereas the capsule and sheaths surrounding intratumoral fascicles are of perineurial origin. They also indicate the presence of axons, some of which are myelinated. Our findings support the concept of a close relationship between palisaded and traumatic neuroma.

[Parotidectomy as prophylaxis in malignant melanoma of the fronto-temporal and eyelid skin].

Three patients presented with parotid masses which revealed, on frozen section, to be metastatic lesions from malignant melanoma. Their medical history confirmed that the removal of cutaneous lesions of the frontal, temporal or eyelid skin had been performed presumedly without histologic examination. Disease staging was assessed as p-T0N1M0. Parotidectomy was performed in all cases without complications, and DTIC was given in one case. One patient died six months after surgery for metastatic disease, another one died disease-free, the third is available for the follow-up. The Authors recommend parotidectomy for prophylaxis in malignant melanoma more than 0.75 mm in thickness which is within a few centimeters of the parotid gland area. Lymphoscintigram might be useful occasionally.

Plastic surgery of cutaneous head and neck tumors. A different view.

Plastic surgery is really a multispecialty specialty. It encompasses cosmetic surgery, trauma, congenital anomalies, hand surgery, treatment of burns, genital-urinary reconstruction, as well as cancer ablation and reconstruction. This paper reviews three cases of skin cancer (basal cell carcinoma, squamous cell carcinoma, and melanoma) that had a varied presentation and clinical course.

"Benign" metastasizing meningiomas.

Less than one in 1000 meningiomas metastasizes. Although the angioblastic and hemangiopericytic tumors are believed by some authors to metastasize with a greater frequency than the other histologic forms of meningioma, most investigators believe that neither the histologic pattern, local aggressiveness, size, nor location of the tumor can be used accurately to predict which tumors will metastasize. Three new cases are presented, bringing the total reported number of metastasizing meningiomas to 113. If the angioblastic meningiomas and hemangiopericytomas are eliminated from this group, 69 reported cases of "benign" metastasizing meningioma remain.

Cellular reaction to autologous tumor tissue followed by "skin window" technique in orofacial tumors.

In a sample of patients with pseudotumors and benign and malignant tumors of the orofacial region there has repeatedly been investigated, after radical surgery, the cell reaction to autologous tumor tissue applied at indicated time intervals on a small skin lesion. The prognostically favorable picture of a cellular-type hypersensitivity reaction with massive participation of lymphoid cells and basophil granulocytes was only found in some basaliomas and leukoplakias. Conversely, spinocellular carcinomas are, irrespective of localization, characterized by areactivity.
