Agroterrorism, biological crimes, and biowarfare targeting animal agriculture. The clinical, pathologic, diagnostic, and epidemiologic features of some important animal diseases.

In the past 100 years, to our knowledge there have been approximately 12 events involving the intentional introduction of microbiologic agents into livestock and animal populations worldwide, of which three were World War I events in the United States. To the best of the authors' knowledge, there has been no recent intentional introduction of microbiologic agents (viruses or bacteria) into livestock and animal populations in the United States. The criminal or terrorist use of chemicals against animals and agriculture products have been more common. With the political, economic, and military new world order, however, the United States must maintain a vigilant posture. The framework for this vigilance must be an intelligence system sensitive to the needs of agriculture and a first-class animal disease diagnostic surveillance and response system.

FREX: a query interface for biological processes with hierarchical and recursive structures.

An intelligent system for signal transduction pathways and other higher order functional knowledge is presented. Molecular mechanisms of biological processes are typically represented as diagrams ("pathways") that have a graph-analogical network structure. However, due to the diversity of topics that pathways cover, their constituent biological entities are highly diverse and range from metal ion to protein to biological processes in general. In addition, the kinds of interactions that connect biological entities are likewise diverse. Consequently, current knowledge about pathways is highly heterogeneous both in the sense of the types of constituents and the granularity of descriptions. To cope with this problem, the proposed system adopts a recursive and hierarchical representation model that enables the annotation and query of pathways or sub-pathways of arbitral granularity. By combining the use of this hierarchical structure and biological ontologies, literature-based information regarding biological mechanisms becomes accessible by computer.

[Study on the effects of exposure to rare earth elements and health-responses in children aged 7-10 years].

OBJECTIVE: To study the effects of environmental exposure to rare earth elements (REEs) on health in children. METHODS: The level of physical growth and development, function of immune system, intelligence were chosen as the health-response indexes of children. Indicators for physical growth and development were measured. IgA, IgG, IgM and complement C3, C4 in blood serum were determined with turbidimeter measure, IgE were determined by chemiluminescence immunoassay (CLIA), lymphocyle subset CD3, CD4, CD8, CD4/CD8 and NK were determined by flowcytometry. Intelligence quotient (IQ) were examined with Drawing A Man Test in the children aged 7-10 years randomly sampled in the area containing rare earth (RE) ore and the control area in Xunwucounty, Jiangxi. Blood REEs were determined by inductively coupled plasma source mass pectrometry (ICP-MS). RESULTS: All 15 kinds of REEs were detected in each sample, the quantity of blood REEs of the children were lower in exposure group than in control group and in the rare earth area [(2.10 +/- 0.88)ng/g] was 1.73 times as that of the children in the control area [(1.26 +/- 1.35) ng/g]. The difference is significant (P < 0.01). The humoral immunity would be changed in the body of the children exposed to REEs, the quantity of IgM was lower in exposure group than in control group. The measuring result of T lymph subgroup of the children indicates: CD3, CD4, CD4/CD8 changed a lot. The percent of CD3, CD8 were significantly lower and CD4/CD8 was significantly higher in exposure group than in control group. The score of IQ was lower significantly in the exposure group than in the control. The percentage of high IQ decreased, the percentage of low IQ increased. Multiple regression analysis showed the variable of blood REEs in exposure to REEs group or those in the control group were introduced into the Vital capacity, blood pressure and IgM regression model. The distance from home to the RE area and either RE put in home influenced the score of children's IQ. CONCLUSION: The children aged 7-10 years in RE ore area may have higher REEs burden in the body, and exposure to REEs could have adverse influences in children.

Computers in oncology nursing: present use and future potential.

Computers assist oncology nurses in their roles as "care integrators" and caregivers. Computers assist nurses in their care integrator role by supporting communication with ancillary departments and by aiding in the collection, organization, and storage of data. Computers helps nurses in their role as caregivers through automated care planning, discharge planning, and patient monitoring and by tracking patients' educational, therapeutic, comfort, or other needs. Using computers, nurses can document their assessments and interventions and patient outcomes while receiving cues and reminders about policies, procedures, and standards of care. In the future, oncology nurses can expect to see computer technology in more hospitals and a host of new developments, such as more intelligent systems, nursing and medical knowledge on-line, documentation at the bedside, and use of patient data bases in education and research.

Multifunctional instrument for operative laparoscopy: technical, experimental and clinical results in gynaecology.

Whilst endoscopic surgical procedures are getting increasingly more complex, in the various surgical disciplines mono- and bifunctional instruments are only slowly being replaced by multifunctional ones. Therefore a complex, intelligent system was developed, the central part of which is a multifunctional instrument. All basic functions necessary for surgical laparoscopy are integrated and comprise: cutting electrodes (unipolar and bipolar) which can be advanced or retracted pneumatically; coagulation forceps with mechanical control; and irrigation and suction devices. All 5 mm instruments can be used and there is an option for others, such as laser or aqua-dissection. The various functions are controlled via the handle of the multifunctional instrument which is connected to the electronic control unit, the MULTILAP system, which supplies the energy, material, and information flow required. In vivo standardised experiments in pigs were first performed to test the new instrument. Operation time was reduced by more than 20% when compared with the same procedure performed conventionally, during which frequent changing of instruments was necessary. Clinical application, without complications in all 30 patients (uterus preserving procedures or reconstructive tubo-ovarian surgery) confirmed the advantages of a multifunctional device, with optimised cutting and coagulation of vessels more than 1-2 mm in diameter, and reduced duration of operation. Safety and ergonomics were improved. Thus, an electronically controlled instrument with multifunctional working channels for lasers, ultrasound appliances, or mechanical instruments is available for application in all domains of operative laparoscopy.

Evolution of a computer program for classifying protein segments as transmembrane domains using genetic programming.

The recently-developed genetic programming paradigm is used to evolve a computer program to classify a given protein segment as being a transmembrane domain or non-transmembrane area of the protein. Genetic programming starts with a primordial ooze of randomly generated computer programs composed of available programmatic ingredients and then genetically breeds the population of programs using the Darwinian principle of survival of the fittest and an analog of the naturally occurring genetic operation of crossover (sexual recombination). Automatic function definition enables genetic programming to dynamically create subroutines dynamically during the run. Genetic programming is given a training set of differently-sized protein segments and their correct classification (but no biochemical knowledge, such as hydrophobicity values). Correlation is used as the fitness measure to drive the evolutionary process. The best genetically-evolved program achieves an out-of-sample correlation of 0.968 and an out-of-sample error rate of 1.6%. This error rate is better than that reported for four other algorithms reported at the First International Conference on Intelligent Systems for Molecular Biology. Our genetically evolved program is an instance of an algorithm discovered by an automated learning paradigm that is superior to that written by human investigators.

[Relative accuracy and precision of two non-invasive thermometry systems].

The relative accuracy and precision of the two non-invasive thermometry systems, FirstTemp (Intelligent Medical Systems, USA), an infrared tympanic thermometer, and CTM-205 (Terumo, Japan), a newly developed deep body thermometry system, were evaluated in 32 patients undergoing various surgeries under general anesthesia using esophageal temperature as the reference value. The "limits of agreement (mean difference +/- 2SD)" of the "core" temperature measured by FirstTemp and esophageal temperature was 0.4 +/- 0.5 degrees C, and was larger (P < 0.01) than those between rectal temperature and esophageal temperature (0.2 +/- 0.7 degrees C), between tympanic membrane temperature and esophageal temperature (-0.1 +/- 0.4 degrees C), and between "forehead deep body temperature" measured by CTM-205 and esophageal temperature (-0.2 +/- 0.7 degrees C). The "limits of agreement" between "forehead deep body temperature" and esophageal temperature did not differ from that between tympanic membrane temperature and esophageal temperature. The repeatability of the measurement by FirstTemp was good; the difference between paired measurement values was 0 +/- 0.2 degrees C (mean +/- 2SD). We conclude that the relative accuracy and precision of the two systems are still not sufficient for monitoring body temperature during general anesthesia.

[Intelligent peritoneal dialysis system for the treatment of renal failure].

Peritoneal dialysis is an important approach to the treatment of renal failure diseases such as acute renal failure, chronic renal failure and uremia. In this paper are presented the application of the multifunction squirm pump developed by authors, the use of PLC and single-chip to control harmonically, and the instituition of full automatic dialysis therapy. Man-machine interface with integrative touch liquid crystal display is adopted in the system, and Chinese interface is used to display and operate so that user can manipulate it in a guiding way. Hence, the whole process is simple and clear. At the same time, it is very easy to set dialysis parameters suited for medical treatment demands of all kinds of patients. Through squirm pump, electric valve, electronic weighing system and electrical heating system, the accurate control of input and output dialysis volumes, flow velocities, temperature, and other parameters are achieved and automatic alarm function is also implemented. Meanwhile, particular intelligent card system that can access automatically the user's treatment information is syncretized, which is convenient for doctor to make a diagnosis and give treatment or to carry out remote treatment. This system plays an important role in the improvement of peritoneal dialysis technique for renal failure treatment and clinical implementation.

International Federation of Clinical Chemistry. Use of artificial intelligence in analytical systems for the clinical laboratory. IFCC Committee on Analytical Systems.

The incorporation of information-processing technology into analytical systems in the form of standard computing software has recently been advanced by the introduction of artificial intelligence (AI) both as expert systems and as neural networks. This paper considers the role of software in system operation, control and automation and attempts to define intelligence. AI is characterized by its ability to deal with incomplete and imprecise information and to accumulate knowledge. Expert systems, building on standard computing techniques, depend heavily on the domain experts and knowledge engineers that have programmed them to represent the real world. Neural networks are intended to emulate the pattern-recognition and parallel-processing capabilities of the human brain and are taught rather than programmed. The future may lie in a combination of the recognition ability of the neural network and the rationalization capability of the expert system. In the second part of this paper, examples are given of applications of AI in stand-alone systems for knowledge engineering and medical diagnosis and in embedded systems for failure detection, image analysis, user interfacing, natural language processing, robotics and machine learning, as related to clinical laboratories. It is concluded that AI constitutes a collective form of intellectual property and that there is a need for better documentation, evaluation and regulation of the systems already being used widely in clinical laboratories.

Clinical-HINTS: integrated intelligent ICU patient monitoring and information management system.

Clinical-HINTS (Health Intelligence System) is a horizontally integrated decision support system (DSS) designed to meet the requirements for intelligent real-time clinical information management in critical care medical environments and to lay the foundation for the development of the next generation of intelligent medical instrumentation. The system presented was developed to refine and complement the information yielded by clinical laboratory investigations, thereby benefiting the management of the intensive care unit (ICU) patient. More specifically, Clinical-HINTS was developed to provide computer-based assistance with the acquisition, organisation and display, storage and retrieval, communication and generation of real-time patient-specific clinical information in an ICU. Clinical-HINTS is an object-oriented system developed in C+2 to run under Microsoft Windows as an embryo intelligent agent. Current generic reasoning skills include perception and reactive cognition of patient status but exclude therapeutic action. The system monitors the patient by communicating with the available sources of data and uses generic reasoning skills to generate intelligent alarms, or HINTS, on various levels of interpretation of an observed dysfunction, even in the presence of complex disorders. The system's communication and information management capabilities are used to acquire physiological data, and to store them along with their interpretations and any interventions for the dynamic recognition of interrelated pathophysiological states or clinical events.

Multisensor system and artificial intelligence in housing for the elderly.

To improve the safety of a growing proportion of elderly and disabled people in the developed countries, a multisensor system based on Artificial Intelligence (AI), Advanced Telecommunications (AT) and Information Technology (IT) has been devised and fabricated. Thus, the habits and behaviours of these populations will be recorded without disturbing their daily activities. AI will diagnose any abnormal behavior or change and the system will warn the professionals. Gerontology issues are presented together with the multisensor system, the AI-based learning and diagnosis methodology and the main functionalities.

Intelligent surgical instrument system ISIS. Concept and preliminary experimental application of components and prototypes.

The recent progress in endoscopic surgery, which is highlighted by the complexity of fundoplication, stomach surgery and colorectal surgery, has revealed major restrictions with difficult surgical manipulations. Mobilisation, dissection and suturing techniques are hampered mainly by the limited degrees of freedom of the conventional rigid instruments (translation along and rotation around the longitudinal axis and the limited play in the access channel). The frequent interchange of instruments such as coagulation forceps, scissors and suction-irrigation probe is time-consuming. We have established an interdisciplinary development model with the aim of improving surgical technology, instrument systems, the operation theatre and its environment. Concepts of electronic instrument control and sensoric feedback, and the features of the surgical man-machine-interface are described. The first prototypes of an intelligent steerable instrument system, ISIS, and its optional effectors, e.g. semiautomatic sewing device and multifunctional coagulation instrument, were tested in phantom and animal experiments. System analysis will lead to specially designed operating theatres (minimal invasive surgical operating system MINOS).

Medication monitoring in the workplace: toward improving our system of epidemiologic intelligence.

There is a great deal we do not know about the safety of pharmaceutical agents, especially regarding their safe use in the workplace. Economic and scientific imperatives can lead to a new drug's approval and marketing even though testing is limited; therefore, much of the knowledge about drug toxicities must be developed in the postapproval period, through pharmacoepidemiologic methods. The system of epidemiologic intelligence depends on spontaneous, voluntary reports of adverse drug reactions and, as applied to the work force, it is fraught with problems of ascertainment, accountability, and application. Structured epidemiologic studies of these issues have been difficult to perform because of high costs, long time frames, and methodologic problems and biases. Nevertheless, large automated data bases, with the right input, hold great promise for making it easier to accumulate and analyze the data necessary for monitoring drug safety in the workplace.

An intelligent data acquisition system for simultaneous screening of microsomal stability and metabolite profiling by liquid chromatography/mass spectrometry.

This paper describes the development of a mass spectrometer-based, intelligent, programmable, sample-selection data acquisition system with two unique features. One is that the system allows automatic determination of the mass to charge ratio (m/z) of an unknown compound and the utilization of the molecular ion information to perform selective ion monitoring (SIM) experiments for quantitation. The other is its decision-making capability to select intelligently different samples and perform different experiments during data acquisition. These features were demonstrated by the application of the system to simultaneous screening for the microsomal stability and metabolite profiling of adatanserin. In this application, the data acquisition system continuously calculated the peak areas of adatanserin from SIM analyses of a batch of microsomal incubates stopped at various time points. Once the peak area of adatanserin had dropped to an arbitrarily predefined 60% of the initial value, the system made a decision to perform metabolite profiling of the sample. This decision initiated a series of automated operations, such as selecting a sample for re-analysis, changing the data acquisition time and liquid chromatographic gradient and switching the SIM mode to the data-dependent product ion scanning mode. The completed analysis of the batch of samples provided information both on the microsomal stability and on the metabolic profile of adatanserin. This simultaneous approach to investigating microsomal stability and metabolite profiling significantly increases the throughput for drug discovery support.

Manufacturers' approaches in the development of intelligent multilevel safety systems to assist perfusionists during cardiopulmonary bypass.

How safe is safe? The design intent of perfusion safety systems is to offer maximum flexibility, low-cost and failsafe features. While easily said, these requirements are difficult to deliver in a perfusion world where standardization is uncommon. This paper reviews the challenges all manufacturers face as they strive to offer the ultimate in safety, keeping the price affordable for the technology deployed and the usability common sense for the perfusionist using the equipment. As microprocessors are able to assist designers in providing intelligent, multilevel safety systems, care must be taken during the design process to balance technological ability with intuitive use. In the final analysis, however, it is the perfusionist who is the primary safety system, and the technology is his or her backup.
