Musculoskeletal manifestations with panniculitis--a hospital based study on 62 patients in Kuwait.

The study describes 62 patients seen over a period of 4 years mainly presenting with an acute inflammatory ankle arthritis/periarthritis associated with panniculitis. Based upon the clinical characteristics of panniculitis two distinct categories could be defined. These included patients with characteristic lesions of erythema nodosum, seen in 51 (82%), the idiopathic form in 29 (57%), and secondary EN in 22 (43%). Associated conditions included Behçet's disease, oral contraceptive pills, pregnancy and penicillin treatment. Patients clinically not having typical EN was seen in 11 (18%). In this group the histopathology showed erythema induratum in 4, cutaneous-polyarteritis nodosa with minimal panniculitis in 3, Weber-Christian disease, cytophagic histiocytic panniculitis, and 'mixed' lobular and septal variety of panniculitis in 1 each. (In 1 patient the disease was not panniculitis but cutaneous variant of granuloma annulare). It is concluded that in a hospital setting EN associated with acute musculoskeletal symptoms, mainly ankle arthritis/periarthritis or an acute peripheral polyarthritis, was a common rheumatological problem in Kuwait. However, other forms of panniculitides also presented with musculoskeletal manifestations requiring accurate histopathological diagnosis for appropriate treatment.

Birthweight percentiles by gestational age in Kuwait.

Our aim is develop a curve for singleton birthweight based on accurately calculated gestational age. A retrospective analysis of all singleton live births from 22-44 completed weeks of gestation during the period from September 1998 to December 2000 in the two largest birth birth centers in Kuwait was conducted. Neonates with major congenital anomalies and those with unrecorded gestational age were excluded from the study population. Total population and gender-specific birthweight percentiles according to gestational age were developed after smoothening of growth curves. A total of 35768 births were included in the development of the birthweight curve. Percentiles of birthweight for all population and by gender are presented. There was significant difference in birthweight among different ethnic groups in this population. At term, 9.8% of births are smaller than the 10th percentile and 10.0% are larger than the 90th percentile. Plotting birthweight in our population on percentile curves derived from the United States or United Kingdom would generally overestimate small for gestational age newborns and underestimate large for gestational age newborns. We conclude that the diagnosis of clinically significant birthweight abnormalities depends on the fetal growth curve used. A population specific curve of fetal growth dated by ultrasonography would provide a reliable reference for birthweight distribution.

Emergency obstetric hysterectomy in Kuwait: a clinico pathological analysis.

The major objective of this study is to analyze the histological findings in emergency obstetric hysterectomy specimens and correlate them with the clinical diagnosis, epidemiological factors and number of tissue blocks examined. The records of all peripartum hysterectomy specimens examined between 1995 and 2001 in the Department of Pathology, Al-Sabah Hospital Kuwait were analyzed. A minimum of ten blocks from the cervix or lower uterine segment was arbitrarily regarded as adequate. Fifty-eight patients had peripartum hysterectomies (incidence of 0.07% of all deliveries) out of which 65% were aged between 30 and 40 years. The number of hysterectomy increased with parity up to five and then declined. The number of blocks examined varied from 2 to 53. Satisfactory pathology was found in 40 and absent in 18 cases. There were 33 adherent placentas. Amniotic fluid embolism (AFE) was found in seven patients. Adequate tissue blocks (>or=10) were significantly associated with positive pathological findings. P value <0.05.

Pediatric dialysis and renal transplantation in Kuwait over the past 11 years.

Data on end-stage renal disease (ESRD) patients and their renal replacement therapy (RRT) were collected retrospectively from the three dialysis centers, the pediatric urology unit, and the organ transplant center of Kuwait. The study period was from 1 January 1986 to 31 December 1996. A total of 61 children, 50 of whom were Kuwaiti nationals, required RRT for ESRD during those 11 years. This gave an average annual incidence rate of 18 per million Kuwaiti children. Glomerulonephritis was the most-frequent underlying disease and accounted for 44% of total cases, while pyelonephritis (including urinary tract anomalies and dysplastic kidneys) was responsible for 30%. Multisystem disease was responsible for ESRD in 7 patients (14%), 2 of whom had lupus nephritis, 2 vasculitis, 2 Henoch-Schönlein purpura, and 1 hemolytic uremic syndrome. Continuous ambulatory peritoneal dialysis and home intermittent peritoneal dialysis, using cycler machines, were not favored dialysis techniques by most parents, especially for those <6 years old. The actuarial survival on dialysis was 75%+/-7% at 12 months. Of the 8 patients who died, 7 were <6 years old. Thirty-eight patients received 46 kidney transplants, 13 of which were performed on a pre-emptive basis. The actuarial patient survivals at 12 months for those receiving first live and cadaveric kidney transplants were 90%+/-5% and 85%+/-2%, respectively, while those for grafts were 76%+/-8% and 66%+/-2%, respectively. This is the first nationwide long-term study of the incidence and etiology of pediatric ESRD in our area and the RRT in a country with adequate treatment facilities.

Pattern and determinants of dyslipidaemia in type 2 diabetes mellitus patients in Kuwait.

We conducted a clinical study to assess the pattern of dyslipidaemia in type 2 diabetic patients and to examine the demographic and clinical factors associated with dyslipidaemia. The study population comprised 206 consecutive type 2 diabetic patients attending the out-patient clinic at a major hospital in Kuwait. Clinical history and physical examination were done and fasting blood samples were taken to determine HbA1c and lipid levels. American Diabetes Association criteria were applied to define clinical targets for lipid levels and coronary heart disease risk categories. Stepwise multiple linear regression was conducted to identify the demographic and clinical factors associated with lipid levels outside of the clinical target. The large majority of the patients were either over-weight (32%) or obese (57%); the mean BMI was 32.6 kg/m2. Serum total cholesterol, LDL-cholesterol, and triglycerides were above optimal levels in 67%, 86%, and 25% of patients, respectively. For HDL-cholesterol, 63% of men and 71% of women had values below the corresponding optimal level. Only 14 patients (6.8%) had all three lipid values within the respective target level. The percentages of patients with one, two, or all three lipid values outside of target were 31%, 46%, and 16%, respectively. The most frequent (41%) pattern of dyslipidaemia was a combination of LDL-cholesterol level above target with HDL-cholesterol level below target; the second most common pattern was an isolated increase in LDL-cholesterol, observed in 21% of the patients. In the stepwise regression analyses, glycaemic control was strongly associated with dyslipidaemia (i.e. high total- and LDL-cholesterol and triglycerides); female gender were associated with low HDL-cholesterol. Kuwaiti type 2 DM patients have a high prevalence of dyslipidaemia and obesity. Weight reduction, increased physical activity, improved glycaemic control, and increased HDL-cholesterol levels, along with reduced LDL-cholesterol, should be important goals of therapy in these patients to reduce the risk of coronary heart disease.

Chemical compositions and sources of organic matter in fine particles of soils and sands from the vicinity of Kuwait city.

Fine particles in the atmosphere from soil and sand resuspension contain a variety of organic compounds from natural biogenic and anthropogenic matter. Soil and sand samples from various sites near Kuwait city were collected, sieved to retain the fine particles, and extracted with a mixture of dichloromethane and methanol. The extracts were derivatized and analyzed by gas chromatography-mass spectrometry in order to characterize the chemical compositions and sources of the organic components. The major inputs of organic compounds were from both natural biogenic and anthropogenic sources in these samples. Vegetation was the major natural source of organic compounds and included n-alkanols, n-alkanoic acids, n-alkanes, sterols and triterpenoids. Saccharides had high concentrations (31-43%) in the sand dune and seafront samples, indicating sources from decomposed vegation materials and/or the presence of viable microbiota such as bacteria and fungi. Vehicular emission products, leakage of lubricating oils, discarded plastics and emissions from cooking operations were the major anthropogenic inputs in the samples from the urban areas. This input was mainly UCM, n-alkanes, hopanes, plasticizers and cholesterol, respectively.

Reference intervals for some serum biochemical markers of bone metabolism in Kuwait.

Using the Hitachi 717 Selective Multichannel analyser, we established reference intervals for serum albumin (37-9 g/l); measured serum calcium (2.14-2.54 mmol/l); corrected serum calcium (2.25-2.60 mmol/l); serum phosphate (0.74-1.60 mmol/l); and alkaline phosphatase (47.0-289.6 U/l) in the Kuwaiti population. The mean concentrations of calcium and phosphate decreased with increasing age in both sexes except for a sudden increase in women at about menopause. Although there was a skewed distribution of alkaline phosphatase in both sexes there was a significant increase in enzyme activity in women after the age of 50 years. Using corrected serum calcium we found that 15% of women in the reproductive age group in Kuwait had hypocalcaemia.

The outcome of placenta accreta in Kuwait (1981-1993).

OBJECTIVES: To study the demographic characteristics of patients with placenta accreta and to identify the clinical features, maternal and neonatal complications of this condition. METHODS: Sixteen cases of placenta accreta were identified in the Maternity Hospital of Kuwait during the period January 1981 to July 1993. Medical records were reviewed regarding past obstetric history, type of placenta, clinical presentation, maternal and fetal outcome. RESULTS: The rate of placenta accreta was found to be 98 per 1,000,000 deliveries. Emergency hysterectomy was needed in 87.5% of cases. There was one maternal death (6.25%) and three perinatal deaths (18.75%). Hemorrhage was the major presenting symptom either externally and antenatally in previa accreta or postpartum in accreta of the upper segment or internally in the same cases. The major postoperative complications were coagulopathy, urinary injury, pelvic hematoma and abscess, in addition to cardiac arrest. These complications, when considered separately, were not affected by a previous history of uterine scarring. CONCLUSIONS: Placenta accreta is a major cause of obstetric hemorrhage and has an adverse effect on maternal and neonatal outcome. It ought to be considered a possibility in patients with previous uterine scarring, placenta previa or retained placenta. It will remain a growing problem in the developing countries due to the rising incidence of previous multiple cesarean sections. Unfortunately the latter do not deter women in this part of the world from insisting upon having big families and refusing tubal ligation.

A survey of didymozoid trematodes of the barracuda Sphyraena obtusata from Kuwait Bay.

Examination of 120 yellow-finned barracudas, Sphyraena obtusata, from Kuwait Bay revealed 8 species of didymozoid trematodes encapsulated in pairs in the tissues. Four species of the didymozoids, Allodidymozoon operculare, Didymocylindrus sp., Didymocystis sp., and Didymocystoides sp., were found in the head region, 3 species, Didymozoon tetragynae, Lepidodidymozoon sinicum and Platocystoides polyaster, in external surfaces and 1 species, Allodidymozoon sp., in stomach wall. The didymozoid species collected constitute new geographic distribution records and occurrence of D. tetragynae and L. sinicum are new host records.

Epidemiology of multiple sclerosis in Arabs in Kuwait: a comparative study between Kuwaitis and Palestinians.

On December 31, 1988 there were 201 registered multiple sclerosis patients in Kuwait, an overall prevalence rate (PR) of 10.2 per 100,000; among them were 186 Arabs, of whom 72 were Palestinians and 51 Kuwaitis. Comparison of these two subgroups, who had a similar age distribution revealed that the disease was 2 1/2 times more frequent among Palestinians (PR 23.8/100,000) than among Kuwaitis (PR 9.5/100,000). Palestinians also showed significant differences from Kuwaitis in eye color, blood group distribution and HLA-DR and HLA-DQW epitopes frequency. This suggests that genetic rather than environmental factors might be the underlying cause for the high susceptibility to develop MS among Arabs originating from the Eastern Mediterranean basin.

Epidemiology of toxoplasmosis in Kuwait. I. Detection of antibodies to Toxoplasma gondii and percentage distribution among the inhabitants.

Of 246 people living in Kuwait and selected at random, 95.5% gave a positive reaction to the indirect haemagglutination test for antibodies to Toxoplasma gondii, of whom approximately 34% had medium to high titres (1:128 to 1:4096). There was no obvious difference between the proportion of females and males who had been infected. Approximately double the proportion of people in the age groups 20 to 31 and 32 to 43 years had medium to high titres compared with those in other age groups. Even between the former two groups, the percentage with the highest titres (1:512 to 1:4096) declined with age; none was found in the oldest group. When analysed in terms of nationalities, the pattern of results obtained with a small group of Bedouins, where most had medium to high titres, was the opposite of that found with urban Kuwaitis and people of other nationalities. It is suggested that the Bedouins may be particularly prone to the infection.

Hepatitis B virus (HBV) markers among patients with chronic liver disease in Kuwait.

The prevalence of hepatitis B surface antigen (HBsAg), antibody to the hepatitis B core (anti-HBc) and to surface antigen (anti-HBs), was investigated, using sensitive radioimmunoassay (RIA) systems, among patients with different clinical entities of chronic liver disease in Kuwait, and compared to a control blood donor population. 81% of patients and 44% of the controls had at lease one HBV marker. 24% of patients, but non of the controls had both HBsAg and a high titre of anti-HBc in the absence of anti-HBs, suggesting a chronic infection. 31% of our patients with hepatosplenic schistosomiasis, 20% with cryptogenic cirrhosis and chronic active liver disease and 60% with hepatocellular carcinoma had these two markers. HBV antigenaemia was significantly more prevalent among male than among female patients and was particularly high among those less than 35 years old. The high prevalence of the various HBV markers among our patients suggests that HBV is a major factor in the development of chronic liver disease in our area. Furthermore, in view of a high prevalence of antigenaemia in patients with hepato-splenic schistosomiasis, HBV infection must play a concomitant role in the development of more serious form of chronic liver disease among such patients.

Preliminary report on arbovirus-antibody prevalence among patients in Kuwait: evidence of Congo/Crimean virus infection.

From two major hospitals in Kuwait 502 sera were randomly selected from patients during the period December 1979 to October 1982. Serological investigations demonstrated Flavivirus activity in the area and antibody to Congo/Crimean haemorrhagic fever virus was found in 4% of the samples. Clinical data indicate that some cases may have been due to recent Congo/Crimean virus infection.

Induced malaria and antibody titres in acute infections and in blood donors in Kuwait.

Concurrent with the increase in the number of imported cases of malaria into non-endemic Kuwait during the past 5 years, induced infections have been identified for the first time. We report 10 such cases over a 4-year-period. Of 8 transfusion-induced infections, 4 were due to Plasmodium falciparum and 4 to P. vivax. The mean incubation period for P. falciparum patients was 13 d and for P. vivax, 17 d. An accidental syringe-needle transmission and a congenital infection were due to P. falciparum and P. vivax respectively. Malarial antibody levels were assayed on commercially-available cultured P. falciparum schizonts by the indirect fluorescent antibody (IFA) test. To establish a base line, the sera of patients with blood film-confirmed P. falciparum and P. vivax were assayed. 96% of the P. falciparum sera were positive, the geometric mean titre (GMT) being 10,280. However, all sera from P. vivax patients were reactive but the GMT was lower at 505. 28% of sera from Kuwaitis and 45% of sera of a consecutive group of blood donors were also reactive, the respective GMTs being 38 and 51. The risk of transfusion malaria was calculated as 79 per million units drawn, an unacceptably high figure for a non-endemic country. We suggest a revised blood donor policy.

Viral hepatitis and pregnancy in Kuwait.

The frequency and severity of viral hepatitis among pregnant and non-pregnant women in Kuwait was studied from 1980 to 1984. 542 female hepatitis patients were investigated, of whom 52 (9.6%) were pregnant. 35 of the 52 (67.3%) cases of viral hepatitis in pregnancy were due to hepatitis B virus while 11 of 52 (21.2%) and 6 of 52 (11.5%) had acute hepatitis non-A, non-B (NANB) and hepatitis A virus infections, respectively. The frequency and severity of viral hepatitis among the pregnant women was similar to that among non-pregnant women. Hepatitis did not have a deleterious effect on pregnancy and no death was recorded. Fulminant acute NANB hepatitis was seen in only one patient, who recovered completely.

An investigation of patients with pulmonary tuberculosis in Kuwait in preparation for studies of immunotherapy with Mycobacterium vaccae.

Sixty-five patients, many of them immigrant to Kuwait, with bacteriologically proven, adult type, pulmonary tuberculosis were studied by many parameters over the 4 months following diagnosis. Twelve were infected with tubercle bacilli resistant to at least one anti-tuberculosis drug. Preliminary evidence suggested that this was frequently primary resistance in patients infected in their countries of origin. The Kuwaiti environment results in very high skin test and lymphocyte proliferative responses (LTT) to shared and species specific antigens of mycobacteria in healthy persons. In comparison, patients with tuberculosis lacked cellular responses to group i and group ii antigens, but had increased IgG and IgA binding to mycobacterial antigens in general. LTT responses to added interleukin 2, and production of alpha interferon, were normal in our patients, but induction of gamma interferon in response to phytohaemagglutinin was reduced initially, rising towards normal during treatment. Biochemical and haematological abnormalities present at the time of diagnosis rapidly corrected. The disease differed from that reported in most previous studies in that fever was uncommon, the disease was never fatal, and most tuberculin tests were not necrotising. This implied that a detrimental immunopathological component is less pronounced in those exposed to the Kuwaiti environment, and a hypothesis is put forward to explain this.
