The buried horizontal mattress suture.

The buried horizontal mattress suture (BHMS) is a new tension suture technique using synthetic absorbable polyglactin 910 suture. When used in minor surgery of the trunk and extremities, this technique eliminates significant suture marking.

Never say never. Negligible carboxyhemoglobin in the victim of a smoldering mattress fire.

For many years, a high blood carboxyhemoglobin concentration has been used as a criterion for determining that a victim of conflagration was alive in the fire. We report a case in which a man died from the combined effects of burns and hypertensive cardiovascular disease. He had a negligible blood carboxyhemoglobin concentration, but the environmental evidence and autopsy findings are indicative of life in the fire, which arose in a smoldering mattress. We conclude that uncritical, rigid adherence to the requirement that fire victims have high carboxyhemoglobin concentrations can be misleading.

On respirable organic antigens, mattresses and some idiopathic lung diseases.

In surveying the environment of workers in a mattress factory thermophilic actinomycetes as well as phycomycetes and aspergillus species were isolated readily from work room dust, finished bat, sissal and the interior of baled linters. Those persons working with cotton products were shown to have a heavy, daily exposure to organisms known to be implicated in the pathogenesis of hypersensitivity pneumonitis. The authors suggest that further study of the inhaled antigens to which cotton workers are exposed is indicated before discarding immunologic mechanisms to explain the development of byssinosis and other pulmonary disorders to which such workers are subject.

Pressure-relief mattresses and patient comfort.

The ideal pressure-relieving support system is comfortable, relieves pressure and prevents tissue damage. In order to ensure patient compliance with the choice of mattress, patient comfort and quality of sleep should be among the most important factors involved in the decision-making process.

Assessment of alternating air mattresses using a time-based interface pressure threshold technique.

Laboratory evaluation techniques for support surfaces have centered largely around interface pressure (IP) measurement, typically analyzing discrete maximum and minimum levels, or calculating the average pressure. Nowadays, alternating pressure air mattresses (APAMs) are used increasingly for the prevention and treatment of pressure sores. Pressure relief (PR) provided by an APAM is time-varying. A computerized system that measures IP, air pressure, and pressure-time characteristics of dynamic support surfaces has been developed for performance assessment. Using this system, IP was recorded continuously and the durations of pressures below three thresholds (30, 20, and 10 mmHg) were calculated automatically. Fifteen sound volunteers were used to evaluate the pressure-relieving characteristics of four APAMs, including one overlay. Results indicated significant differences (p < 0.001) between products when durations below 20 and 10 mmHg thresholds were analyzed, showing some devices were only capable of momentarily relieving pressure. Maximum contact pressures on the sacrum were significantly lower (p < 0.0001) on devices where inflation pressure was adjusted according to the body mass of the subject. With further clinical validation, this tool could assist in the selection of alternating surfaces of any description.

The tendering process for beds and mattresses.

As increasing numbers of expensive, sophisticated bed and mattress systems become available, more trusts are negotiating contracts for supply, maintenance and training. It is important that tissue viability nurses become involved in such decision-making.

[Hermetic sealing of the cataract incision with intracorneal mattress sutures].

Main factors of hermetic closure of an operative wound in cataract extraction with intracapsular implantation of intraocular lens are discussed. A new method for closure of operation wound in this operation is proposed. Opposition intracorneal mattress sutures are made over the incision plane. Their principal feature is that the thread is not thrown over the external edge of corneal incision, as in traditional non-opposition sutures, and during pulling and suture the thread does not deform the external surface of the cornea in the central zone and hence, causes no postoperative corneal astigmatism. A method combining opposition and interrupted non-opposition sutures is proposed, and various combinations of these sutures are compared. Forty-five patients (45 eyes) were operated on using the proposed technique. Opposition sutures made in accordance with the methods proposed by the authors do not cause high postoperative corneal astigmatism, or it approximates the normal values of corneal astigmatism. A special marker is proposed for preliminary marking of sites for nonperforating incisions on the cornea and the site of cataract incision.
